The continuous battle between humans and the multitude of pathogenic microorganisms in the environment has sought relief in the form of antimicrobials. But the counter attack by pathogenic organisms in the form of multidrug resistance, acquired by various mechanisms such as transformation, transposition, conjugation and transduction is a major reason for concern. Bacteriophages have contributed in a significant way to dissemination of genes encoding antimicrobial resistance, heavy metal resistance and virulence factors through the phenomenon of transduction. This review aims at compiling information about the different mechanisms by which bacteriophages aid in transferring genes involved in antimicrobial resistance to Escherischia coli in various environments.

In this research the effect of Coconut (CN) fibre on the fresh and thermo gravimetric properties of self-compacting concrete (SCC) at elevated temperatures was investigated experimentally and statistically. The mixtures containing cement, water, fly ash, fine aggregate, coarse aggregate and super plasticizer with the addition of CN fibres (0%, 0.05%, 0.10 %, and 0.15%) by volume of the mixtures were prepared. The fresh and thermo gravimetric properties of the SCC specimens were determined after mixing at elevated temperatures (200℃, 400℃, and 600℃) testing in a laboratory. Three control specimens with 0% CN fibres were used for every mixture of SCC. Regression models were developed to determine the responses. The optimum of the CN fibres was measured.

Background: Tuberculosis is currently the world’s leading cause of death arising from a single infectious condition. While T cell mediated immunity is recognized to have a major contribution to tuberculosis activation, the present investigation confirmed that TB was more prevalent among patients with acute myeloid rather than lymphoid leukemia and such association was frequently overlooked. The primary objective of this study is to estimate the diagnostic delay of tuberculosis among patients with acute myeloid leukemia (AML) and compare it to the general population in Qatar. Secondary objective is to study the clinical and epidemiologic characteristics of tuberculosis in patients with AML. Methods: This is a retrospective study of tuberculosis cases diagnosed in subjects with AML during the period from January 2008 till December 2016. Results: Among 215 subjects with AML identified during the study period, 12 (5.58%) received the diagnosis of tuberculosis. The estimated incidence of tuberculosis among AML cases was 7.14 cases per 1000 per year. The mean delay in diagnosis of tuberculosis was 64.2 days (95% CI: 26.8 - 101.5) and the median was 45 days (interquartile range;Q1 - Q3, 29.5 - 97.5). Prolonged fever was the most common presentation (100% of cases). Parenchymal lung involvement was the most common radiologic abnormality (83.3% of cases). Three patients (25%) died and 8 patients completed 9 to 12 months of anti-tuberculous treatment with clinical and radiological remission. Conclusion: Infections caused by Mycobacterium tuberculosis are not uncommon in patients with AML especially in patients from tuberculosis endemic regions. It constitutes a diagnostic challenge so high index of suspicion is of paramount importance.

Due to poverty and ignorance more than half of the pregnant women in developing country are suffering from iron deficiency anaemia (IDA). Different gastrointestinal side effects cause low compliance to iron supplementation. Amloki (Emblica officinalis) is very popular among pregnant women for its taste and traditional belief of well-being. The aim of this study is to determine the effects of amloki on RBC, WBC and Platelet count. Prospective comparative study was carried out on patients of Outpatient Department of Obstetrics and Gynecology, Dhaka Medical College Hospital from July 2016 to June 2017. A total of 43 pregnant women between the 13th to the 20th weeks of gestation with IDA were selected aged 18 - 36 years in this study. Anemic pregnant women supplemented with oral iron and amloki were considered as study group (A) and control group (B) were with only iron supplementation for 45 days. Total count of RBC, WBC and platelet were done in the Department of Hematology & BMT Unit, Dhaka Medical College Hospital, Dhaka. For statistical analysis, Paired Student’s “t” test and Unpaired Student’s “t” test were considered using SPSS 22.0 version. A significant increase (p < 0.05) of blood RBC count was observed after intervention of iron in both groups. There was also significant increase in RBC count in iron + amloki supplemented group than that of only iron supplemented group. In this study, there was also increase of WBC count in iron + amloki supplemented group and decrease of platelet count in both groups, but these changes were not statistically significant. It can be concluded that oral amloki supplementation causes increased blood RBC count in IDA with pregnancy.

Background: Several circumstances and conditions lead to UGIB. The commonest among them peptic ulcer to an approximation of 50% in cases. The second major cause may be esoghageal varices and third is mallory-weis tears. There are also other causes, which may include erosins, tumors and other malformations like arteriovenous. Methods: This was an observational retrospective study of all patients referred to endoscopy. The study exclusion criteria include all patients who had repeat endoscopies within one month of the previous endoscopy whereas all the patients with UGIB were included in this study. Results: We retrieved data of total 150 patients who had experienced upper gastrointestinal endoscopy during the selected time period covered in the study, March 2015 to February 2016. The mean age of the patients was 43.3 ± 13.80. 80 (53.3%) of the patients were male and 70 (46.7%) were females. Exploring more into UGIB causes the majority of patients were with esophageal varices 68 (45.3%) with male to female share was 2:1. The second most cause was esophagitis (n = 22, (14.67%)), the distribution of the cause was similar in male to female. Conclusion: We may conclude from our findings that the most common cause of UGIB is esophageal varices in our population.

Objective. We investigated gender-specific risks of mother-to-child transmission (MTCT) at birth and at 6 to 8 weeks among infants born to HIV infected African women. Design. Follow-up study of infants enrolled in 2 randomized, phase III, clinical trials to prevent MTCT, conducted in Blantyre, Malawi, in southeast Africa. Methods. Infants were enrolled at birth and monitored postnatally, and their HIV status was assessed at birth and at 6 to 8 weeks (assessment beyond 6-8 weeks is ongoing). Statistical analyses were stratified according to gender, and comparisons were made with descriptive, univariate, and multivariate statistical tests. MTCT was estimated at birth and at 6 to 8 weeks among infants who were not infected at birth. Results. Overall, 966 boys and 998 girls were enrolled. The rate of HIV transmission at birth was 9.5%(187 of 1964 infants). However, at birth significantly more girls (12.6%) than boys (6.3%) were infected with HIV. This association remained significant after controlling for maternal viral load and other factors. Among infants who were uninfected at birth, 8.7%(135 of 1554 infants) acquired HIV by 6 to 8 weeks; of these infants, more girls acquired HIV (10.0%), compared with boys (7.4%). Conclusions. Female infants may be more susceptible to HIV infection before birth and continuing after birth. Alternatively, in utero mortality rates of HIV-infected male infants may be disproportionately higher and thus more HIV-infected female infants are born. In areas of sub-Saharan Africa, where HIV infection rates are high among women of reproductive age, the magnitude of the gender transmission differences observed in this study could have clinical, preventive, and demographic implications.

Background: Hypertension is a common chronic disease and an important public health problem. The prevention of complications depends on hypertension control. Aim: To assess blood pressure control, complication presence, and their influencing factors. Methods: This cross-sectional study was conducted on patients with hypertension attending a private medical facility, a governmental facility, and a primary health care center in Khartoum, Sudan, between September 2022 and February 2023. Statistical Package for Special Sciences version 25 was used for data analysis;frequency and percentage were used to describe the qualitative variables. A chi-square test was used for co-relation analysis;a P-value ≤ 0.05 was considered statistically significant, and the confidence interval was 95%. Results: The study included 250 participants;55.6% were males. Their age was 58.77 ± 6.80 years, and they had hypertension for 10.7 ± 8.51 years. The duration of the disease was 10.7 ± 8.51 years. Systolic blood pressure was 127.1 ± 11.3 mm hg, and diastolic blood pressure was 81.6 ± 7.1 mm hg. Of the participants, 76.4% had controlled hypertension, while 27.6% had complications due to hypertension. Controlled blood pressure was associated with females, age group 40 - 60 years, normal waist circumference, and high adherence level (P ≤ 0.05). The presence of complications was associated with males, participants older than 60 years, and a low adherence level (P ≤ 0.05). Conclusions: In Sudan, the level of control is good;nonetheless, not all patients achieve it. Availability and access to treatment facilities and medications should be improved. Proper patient counseling should be offered, as well as continuous care.
