Surgical Outcomes and Local Recurrence Following Total or Subtotal Gastrectomy for Early Adenocarcinoma of Antrum

Objective： The choice between total gastrectomy （TG） and subtotal gastrectomy （STG） for early adenocarcinoma of the lower third of the stomach is still a matter of debate and controversy amongst surgeons. The aim of this study was to compare the surgical outcomes, quality of life （QoL）, and local recurrence rates as well as other results of curative resection of early distal adenocarcinoma of the stomach using TG or STG performed in Iran. Methods： Hospital records of 66 patients with early distal adenocarcinoma of stomach that underwent even total or subtotal gastrectomy with perigastric （D2） lymphadenectomy between October 2001 and February 2006 in Taleghani Hospital, Iran were reviewed retrospectively. Demographic data as well as clinicopathological factors including number of involved lymph nodes, tumor grading, depth of invation, tumor size, and tumor type were recorded. Post-operative outcomes including mortality and morbidity, tumor recurrence and quality of life were assessed. Univariate analyses using Fisher＇s exact test, the Student t-test, and the Pearson χ^2 test were used. Results： Local recurrence of tumor was found in 8 （23%） TG group patients and in 19 （61%） patients of STG group, the Pearson χ^2 test showed a significant higher recurrence rate in STG （P value=0.002, Relative Risk=2.68, Confidence Interval： 1.37-5.24）.The mean time interval between gastrectomy and tumor recurrence was not different between TG and STG. （19.75±5.1 vs. 18.0±7.8 respectively; P value=0.507） Tumor size 〉5 cm （P value=0.004）, diffuse type （P value=0.034）, poor differentiation （P value=0.000） and serosal invasion （P value=0.012） were found to be significantly related to tumor recurrence in patients undergone gastrectomy. In none of the QoL measures a significant difference was found between TG and STG Conclusion： Our results show that subtotal and total gastrectomy methods with perigastric lymphadenectomy have a similar postoperative complication rate and surgical outcomes a

Obstructive jaundice in Iran:factors affecting early outcome

BACKGROUND:Understanding any factors which influence the morbidity and mortality in patients with obstructive jaundice in each society will better guide appropriate management and lead to improved survival This study aimed to assess baseline etiologies,clinical manifestations,diagnostic results,and morbidity and mortality related to obstructive jaundice in Iranian patients. METHODS:The hospital recorded files of 133 patients with the final diagnosis of obstructive jaundice who had been admitted to the Taleqhani Hospital in Tehran between January 2001 and September 2004 were reviewed. RESULTS:The most common etiologies of obstructive jaundices were neoplasia and then common bile duct stone in the two *** results of ultrasonography were positive in less than half of the ***,the most positive results were related to endoscopic retrograde cholangiopancreatography(ERCP).The in-hospital mortality rate in patients less than 50 years old and elderly patients was 0%and 6.76%,*** in-hospital morbidity rate was 2.25%and 7.51%,respectively in both patients and it was commonly related to infection of ulcer (44.46%),pneumonia(14.75%),myocardial infarction (14.75%),and subdiaphragmatic abscess(11.29%).In patients with a diagnosis of benign obstruction,only one patient died of severe *** malignant group preoperative characteristics,such as weight loss(P=0.015) and serum bilirubin concentration more than 16 mg/dl and postoperative complications,such as sepsis(P<0.001) cardiac arrest(P<0.001),and hepatic coma(P<0.001)were main predictors for the in-hospital mortality ***:Although the mortality and morbidity of obstructive jaundice in our study are less than those in other studies,the determination of preoperative clinical and laboratory indices and postoperative complications of patients is needed for the control of mortality and morbidity rate.

Nowadays,having an effective technique in preparing semi-solid slurries for rheocasting process seems to be an essential *** this study,semi-solid slurry of A356 aluminum alloy was prepared by three-phase annular electromagnetic stirring(A-EMS)technique under different *** effects of stirring current,pouring temperature and stirring time on microstructural evolution,mean particle size,shape factor and solid fraction were *** rheocasting process was carried out by using a drop weight setup and to inject the prepared semi-solid slurry in optimal conditions into the step-die *** filling behavior and mechanical properties of parts were *** evolution showed that the best semi-solid slurry which had fine spherical particles with the average size of~27μm and a shape factor of~0.8 was achieved at the stirring current of 70 A,melt pouring temperature of 670℃,and stirring time of 30 *** these conditions,the step-die cavity was completely filled at die preheating temperature of 470℃.The hardness increases by decreasing step thickness as well as die preheating ***,the tensile properties are improved at lower die preheating *** fracture surface,which consists of a complex topography,indicates a typical ductile fracture.

Modeling and experimental studies of methyl methacrylate polymerization in a tubular reactor

In this study, rheological examination of the mixture of a tubular reactor in which methyl methacrylate was polymerized has been studied. The n(flow behavior index) value of Power Law Model of mixture contained in the reactor has been determined within the span of 0.3492 to 0.9889 by curve fitting. Employing these numerical data for velocity profile, the reactor has been modeled. Moreover, the functions of the reactor have been compared in the three modes of plug, mixed and laminar flow. The results obtained in this research indicate that the polymethyl methacrylate mixture contained in the reactor is pseudo-plastic. Moreover, as the conversion grows, the velocity profile starts as a parabolic profile and approaches the plug mode; although it never reaches the plug. The other conclusions borne in this study indicate that when the reactor's radius is decreased, the conversion rate grows. However, as decreasing the radius would also reduce the productions rate, this procedure is not economical. Finally, in this modeling, the amount of conversion is equal to 56.47% at the end and according to its laboratory proportion which is 55.88%, it has reached the conclusion that the modeling duly undertaken is applicable and valid.

Analytical modeling of static behavior of electrostatically actuatednano/micromirrors considering van der Waals forces

In this paper, the effect of van der Waals （vdW） force on the pull-in behavior of electrostatically actuated nano/micromirrors is investigated. First, the minimum po- tential energy principle is utilized to find the equation gov- erning the static behavior of nano/micromirror under electro- static and vdW forces. Then, the stability of static equilib- rium points is analyzed using the energy method. It is found that when there exist two equilibrium points, the smaller one is stable and the larger one is unstable. The effects of dif- ferent design parameters on the mirror＇s pull-in angle and pull-in voltage are studied and it is found that vdW force can considerably reduce the stability limit of the mirror. At the end, the nonlinear equilibrium equation is solved numer- ically and analytically using homotopy perturbation method （HPM）. It is observed that a sixth order perturbation approx- imation can precisely model the mirror＇s behavior. The re- suits of this paper can be used for stable operation design and safe fabrication of torsional nano/micro actuators.

PREPARATION, CHARACTERIZATION AND ADHESIVE PROPERTIES OF DI-AND TRI-HYDROXY BENZOYL CHITOSAN NANOPARTICLES

Modified chitosans with 3,4-di-hydroxy benzoyl groups （CS-DHBA） and 3,4,5-tri-hydroxy benzoyl groups （CS- THBA） were synthesized and their nanoparticles were prepared via ionic crosslinking by tripolyphosphate （TPP）. The chemical structure and degree of substitution （DS） of di- and tri-hydroxy benzoyl chitosans are determined by FTIR and IH- NMR spectroscopy. The morphology of particles, size distribution and zeta potential of nanoparticles were studied using transmission electron microscopy （TEM） and dynamic light scattering （DLS）, respectively. The mean diameters of particles of CS-DHBA and CS-THBA nanoparticles were 144 nm and 112 nm, respectively. It was found that the particles size decreased slightly with decreasing the degree of substitution and increasing degree of deacetylation （DD）, due to increasing of ionic crosslinking of ammonium ions and polyanions of tripolyphosphate. The TEM photographs of CS-DHBA show that these particles are spherical in shape, but the particles of CS-THBA show some aggregation. In addition, the solubility and the mechanical properties of the prepared modified chitosans and their nanoparticles were evaluated for bio-adhesive and biomedical application. The results of solubility tests indicated that, the CS-DHBA and CS-THBA have higher solubility at pH 〉 7 comparing to CS. Also the CS-DHBA, CS-THBA and their nanoparticles showed a significant adhesive capacity and enhanced tensile strength and tensile modulus.

A Novel Method of Deinterleaving Radar Pulse Sequences Based on a Modified DBSCAN Algorithm

A modified DBSCAN algorithm is presented for deinterleaving of radar pulses in modern EW environments.A main characteristic of the proposed method is that using only time of arrival of pulses,the method can sort the pulses *** PDW information such as rise time,carrier frequency,pulse width,modulation on pulse,fall time and direction of arrival are not *** identify the valid PRIs in a set of interleaved pulses,an innovative modification of the DBSCAN algorithm is introduced which is accurate and easy to *** proposed method determines valid PRIs more accurately and neglects the spurious ones more efficiently as compared to the classical histogram based algorithms such as ***,without specifying any input parameter,the proposed method can deinterleave radar pulses while up to 30%jitter is present in the associated *** accuracy and efficiency of the proposed method are verified by computer simulations and real data *** simulations are based on different real and operational scenarios where the presence of missing and spurious pulses are also ***,the simulation results can be of practical significance.

Improving Software Quality Prediction by Noise Filtering Techniques

Accuracy of machine learners is affected by quality of the data the learners are induced on. In this paper, quality of the training dataset is improved by removing instances detected as noisy by the Partitioning Filter. The fit dataset is first split into subsets, and different base learners are induced on each of these splits. The predictions are combined in such a way that an instance is identified as noisy if it is misclassified by a certain number of base learners. Two versions of the Partitioning Filter are used： Multiple-Partitioning Filter and Iterative-Partitioning Filter. The number of instances removed by the filters is tuned by the voting scheme of the filter and the number of iterations. The primary aim of this study is to compare the predictive performances of the final models built on the filtered and the un-filtered training datasets. A case study of software measurement data of a high assurance software project is performed. It is shown that predictive performances of models built on the filtered fit datasets and evaluated on a noisy test dataset are generally better than those built on the noisy （un-filtered） fit dataset. However, predictive performance based on certain aggressive filters is affected by presence of noise in the evaluation dataset.

Inhibition Behaviour of 2-butinel,4diol and Tartrate Salt,and Their Synergistic Effects on Corrosion of AA3003 Aluminium Alloy in 0.5% NaCl Solution

This work intends to investigate the inhibition behaviour of 2-butine 1, 4diol and potassium sodium tartrate and their synergistic effects on 3003 aluminium alloy corrosion in 0.5% NaCl solution. Experiments were carried out by electrochemical impedance spectroscopy （EIS） and Tafel polarization method in a three-electrode cell. It was concluded that the inhibition efficiencies increased with an increase in the concentrations of inhibitors. For 2-butinel, 4diol and tartrate salt, the optimum in the inhibition efficiency, at room temperature and neutral pH, was observed for concentrations close to 10-3 mol/L and 1.5×10^-3mol/L, respectively. The electrochemical results illustrated that 2-butinel, 4diol and tartrate salt, have significant synergistic inhibition effects on corrosion of 3003 aluminium alloy in 0.5% NaCl solution. The optimum ratio of concentrations for tartrate to alcohol was 2：1.
