The current status of hormone treatment for prostate cancer patients in Korean real-world practice:a multi-institutional observational study

We aimed to evaluate the current nationwide trend,efficacy,safety,and quality of life(QoL)profiles of hormone treatment in real-world practice settings for prostate cancer(PCa)patients in Korea.A total of 292 men with any biopsy-proven PCa(TanyNanyMany)from 12 institutions in Korea were included in this multi-institutional,observational study of prospectively collected data.All luteinizing hormone-releasing hormone(LHRH)agonists were allowed to be investigational drugs.Efficacy was defined as(1)the rate of castration(serum testosterone≤50 ng dl^-1)at 4-week visit and(2)breakthrough(serum testosterone>50 ng dl^-1 after castration).Safety assessments included routine examinations for potential adverse events,laboratory tests,blood pressure,body weight,and bone min era I density(BMD,at baseli ne and at the last follow-up visit).QoL was assessed using the Expa nded Prostate Cancer Index Composite-26(EPIC-26).The most comm on initial therapeutic regimen was LHRH agonist with an ti-androge n(78.0%),and the most commonly used LHRH agonist for combination and mono therapy was leuprolide(64.0%for combi natio n and 58.0%for monotherapy).The castration and breakthrough rates were 78.4%and 6.6%,respectively.The laboratory results related to dyslipidemia worsened after 4 weeks of hormone treatment.In addition,the mean BMD T-score was significantly lower at the last follow-up(mean:-1.950)compared to baseline(mean:-0.195).The mean total EPIC-26 score decreased from 84.8(standard deviation[s.d.]:12.2)to 78.3(s.d.:8.1),with significant deterioration only in the urinary domain(mean:23.5 at baseli ne and 21.9 at the 4-week visit).These findi ngs dem on strate the n ationwide trend of current practice setti ngs in hormone treatment for PCa in Korea.

Impact of adolescent complex regional pain syndrome on the psychopathology of young men ahead of military service: A retrospective cohort analysis of Korean conscription data

Background: The relationship between physical and psychopathological features in complex regional pain syndrome(CRPS) has been a subject of constant interest, but no data are available in adolescents. Therefore, we aimed to identify the factors associated with psychopathology in adolescents with CRPS ahead of military service.Methods: We retrospectively reviewed all conscription examinees who had completed a Military Personality Inventory(MPI) during a period between February 2013 and December 2016. A total of 63 persons with a history of CRPS(19-years of age for all) were enrolled. Basic demographic and pain-related data were analyzed to examine their association with MPI results. The mean FGR score as well as the 8 subdomain scores were compared between those with pain duration at <15 months(n=30) vs. ≥15 months(n=33). Binary MPI results(normal-abnormal) were also compared between the two groups.Results: In multivariate analysis, abnormal MPI was associated with pain duration, with an odds ratio(OR) at 1.05 for every 1-month increase [95% confidence interval(CI) 1.02–1.08;P=0.002]. Subjects with pain duration at ≥15 months have lower faking good response score(P<0.001 vs. those with pain duration at <15 months), and higher abnormal MPI result rate, faking bad response, inconsistency, anxiety, depression, somatization, paranoid, personality disorder cluster A, and personality disorder cluster B scores(P<0.05). Pain duration was significantly associated with the MPI variables.Conclusions: Pain duration is associated with psychopathology in adolescents with CRPS. Psychopathologic features increased as the disease duration increased. A comprehensive understanding of time-dependent psychopathological factors could support the planning of multimodal approaches for managing adolescent CRPS.

Synergetic analysis of springtime air pollution episodes over Gwangju,Korea

The characteristics of springtime aerosols,including their optical and microphysical properties,were analyzed for the months of March to May of 2009 in Gwangju（35.23°N,126.84°E）,Korea.A high Light Detection and Ranging（LIDAR）-derived aerosol depolarization ratio（δ） of 0.25 ± 0.04 was determined on dust particles during the observation period.The？ngstr？m exponent values of the 440–870 nm wavelength pair（？_（440–870）） and single-scattering albedo at 675 nm（Ω_（675）） measured by a CIMEL sun/sky radiometer were 0.77 ± 0.19 and 0.95 ±0.01,respectively.The elevated dust layers reached a maximum elevation of 4 km above sea level.Anthropogenic/smoke particles that originated from highly populated/industrialized regions could be distinguished by their relatively smaller particle size（？_（440–870） ranged between1.33 and 1.36） and higher light-absorbing（Ω_（675） of 0.92 ± 0.01） characteristics.These aerosols are mostly distributed at altitudes 〈 1.2 km.The root-mean-square deviation（RMSD） between the aerosol optical depth（AOD,τ） derived from LIDAR_（（τ_（LIDAR））） and from the CIMEL sun/sky radiometer_（（τ_（CIMEL））） varied with respect to the surface PM10 concentration.The RMSD between τ_（LIDAR） and τ_（CIMEL） was as low as 13% under lower PM_（10） concentration levels（〈 100 μg/m^3）.In contrast,the RMSD between τ_（LIDAR） and τ_（CIMEL） increased three times（~31%） under high surface PM_（10） concentration levels（〉100 μg/m^3）.These results suggest that the accuracy of τ_（LIDAR） is influenced by specific atmospheric conditions,regardless of its uncertainty.

Effects of winter weather on traffic operations and optimization of signalized intersections

Adverse winter weather has always been a cause of traffic congestion and road collisions.To mitigate the negative impacts of winter weather, transportation agencies are under increasing pressure to introduce weather responsive traffic management strategies.Currently, most traffic signal control systems are designed for normal weather conditions and are therefore suboptimal regarding efficiency and safety for controlling traffic during winter snow events due to changes in traffic patterns and driver behaviors. The main objective of this research is to explore how to modify pre-timed traffic signal control parameters under adverse weather conditions to increase traffic efficiency and road safety.This research consists of two main components. First, we examine the impacts of winter weather on three key traffic parameters, i.e., saturation flow rate, start-up lost time, and free flow speed. Secondly, we investigate the potential benefits of implementing weatherspecific signal control plans for uncoordinated intersections as well as coordinated corridors. Two case studies are conducted, each with varying levels of traffic demand and winter event severity, to compare the performance of different signal plans. Evaluation results from both Synchro and VISSIM show that implementing such signal plans is most beneficial for intersection with a medium level of traffic demand. It is also found that the benefit of implementing weather-responsive plans was more compelling at a coordinatedcorridor level than at an uncoordinated-intersection level.

Minimum dose path planning for facility inspection based on the discrete Rao-combined ABC algorithm in radioactive environments with obstacles

Workers who conduct regular facility inspections in radioactive environments will inevitably be affected by radiation.Therefore,it is important to optimize the inspection path to ensure that workers are exposed to the least amount of radiation.This study proposes a discrete Rao-combined artificial bee colony(ABC)algorithm for planning inspection paths with minimum exposure doses in radioactive environments with obstacles.In this algorithm,retaining the framework of the traditional ABC algorithm,we applied the directional solution update rules of Rao algorithms at the employed bee stage and onlooker bee stage to increase the exploitation ability of the algorithm and implement discretion using the swap operator and swap sequence.To increase the randomness of solution generation,the chaos algorithm was used at the initialization stage.The K-opt operation technique was introduced at the scout bee stage to increase the exploration ability of the algorithm.For path planning in an environment with complex structural obstacles,an obstacle detour technique using a recursive algorithm was applied.To evaluate the performance of the proposed algorithm,we performed experimental simulations in three hypothetical environments and compared the results with those of improved particle swarm optimization,chaos particle swarm optimization,improved ant colony optimization,and discrete Rao’s algorithms.The experimental results show the high performance of the proposed discrete Rao-combined ABC algorithm and its obstacle detour capability.

Unsupervised random forest for affinity estimation

This paper presents an unsupervised clustering random-forest-based metric for affinity estimation in large and high-dimensional data.The criterion used for node splitting during forest construction can handle rank-deficiency when measuring cluster compactness.The binary forest-based metric is extended to continuous metrics by exploiting both the common traversal path and the smallest shared parent node.The proposed forest-based metric efficiently estimates affinity by passing down data pairs in the forest using a limited number of decision trees.A pseudo-leaf-splitting(PLS)algorithm is introduced to account for spatial relationships,which regularizes affinity measures and overcomes inconsistent leaf assign-ments.The random-forest-based metric with PLS facilitates the establishment of consistent and point-wise correspondences.The proposed method has been applied to automatic phrase recognition using color and depth videos and point-wise correspondence.Extensive experiments demonstrate the effectiveness of the proposed method in affinity estimation in a comparison with the state-of-the-art.

Multiple flexor tendon ruptures due to osteochondroma of the hamate:A case report

BACKGROUND Closed rupture of the little and ring finger flexor tendons caused by the hamate is mostly associated with a fracture or nonunion of the hamate hook.Only one case of a closed rupture of the finger flexor tendon caused by osteochondroma in the hamate has been reported.Here,we present a case study to highlight the possibility of hamate osteochondroma as a rare cause of finger closed flexor tendon rupture based on our clinical experience and literature review.CASE SUMMARY A 48-year-old man who had been a rice-field farmer for 7–8 h a day for the past 30 years visited our clinic due to the loss of right little finger and ring finger flexion involving both the proximal and distal interphalangeal joints.The patient was diagnosed with a complete rupture of the ring and little finger flexors because of the hamate and was pathologically diagnosed with an osteochondroma.Exploratory surgery was performed,and a complete rupture of the ring and little finger flexors due to an osteophyte-like lesion of the hamate was observed,which was pathologically diagnosed as an osteochondroma.CONCLUSION One should consider that osteochondroma in the hamate may be the cause of closed tendon ruptures.

A taxonomic review of the genus Miaenia Pascoe, 1864(Coleoptera: Cerambycidae) from South Korea with a new record of M. tonsa(Bates)

The cerambycid genus Miaenia Pascoe(Coleoptera: Cerambycoidea) of South Korea is taxonomically reviewed. Two species, Miaenia fasciata fasciata(Matsushita) and Miaenia maritima Tsherepanov have been recorded. In this paper, M. tonsa(Bates) is added to the Korean cerambycid fauna for the first time. A key to Korean Miaenia species, re-description and diagnostic illustrations, including male genitalia for each species, are provided.

Response of ground beetles(Coleoptera:Carabidae)to forest gaps formed by a typhoon in a red pine forest at Gwangneung Forest,Republic of Korea

In Korea, damaging typhoons related to climate change have increased steadily since the 1990s. Red pine （Pinus densiflora） forests in Gwangneung Forest were greatly disturbed by typhoon Kompasu in 2010. A survey was carried out to clarify differences in ground beetle （Coleoptera： Carabidae） communities between forest gaps and undamaged forests. Ground beetles were sampled using pitfall traps from early May to late October 2011. Vegetation changes, litter layer, organic matter layer, and soil conditions were also measured. A total of 1035 ground beetles of 32 species were collected. Contrary to our expectation, species richness, abundance, and community structure of the ground beetles in forest gaps were similar to those in undamaged forests. Species richness and abundance of habitat type were also similar. However, species diversity and estimated species richness in forest gaps were significantly higher than in undamaged forests. These findings suggest that forest gaps formed by a typhoon did not lead to great change in ground beetle communities.

背景:虽然活动衬垫膝关节假体在体外的研究中显示具有更好的关节运动学表现、更低的磨损率,但是否具有更好的体内临床效果仍有争议。目的:对比观察活动衬垫型和固定衬垫型膝关节假体在置换后1年时膝关节活动度和临床效果。设计、时间及地点:回顾性病例对比分析。2003-11/2006-07首尔国立大学盆唐医院关节重建中心。对象:共纳入接受全膝关节置换488例患者669膝。244例患者341膝采用了固定衬垫型假体,244例患者328膝采用活动衬垫型假膝。方法:所有手术皆由一位固定医师(TKKim)完成。采用髌旁内侧入路,术中不翻转髌骨,而将髌骨向外侧脱位进行显露。以间隙测量器作为间隙撑开工具并进行间隙的准确测量,根据测量结果进行软组织松解平衡,直到达到下述标准:屈曲间隙大于伸直间隙不超过3.0～4.0mm或伸直间隙大于屈曲间隙不超过1.0～2.0mm,对于内翻畸形膝关节,外侧间隙大于内侧间隙2.0～3.0mm是可以接受的。常规行髌骨置换,所有假体都采用骨水泥固定。主要观察指标:以角尺测量患者置换前后膝关节屈曲挛缩程度和最大屈曲度。置换前后膝关节的疼痛和功能状态采用AKS评分、髌股关节评分、WOMAC评分和SF-36系统评分进行评价。结果:两组患者在性别、年龄、身高、体质量和体质量指数方面差异没有显著性。置换后两组在PF膝前痛评分、WOMAC疼痛评分和SF-36躯体疼痛评分方面两组差异无显著性(P>0.05);固定衬垫组平均AKS疼痛评分略高于活动衬垫组(48.5±3.2,47.5±4.8,P=0.010)。置换后两组患者的屈曲畸形得到了很好的矫正(P=0.341),两组的膝关节活动度差异没有显著性(P=0.412)。置换后所有反映膝关节功能的评价指标两组间差异均无显著性(P>0.05)。结论:活动衬垫型膝关节假体置换后在膝关节活动度、疼痛缓解以及膝关节功能方面与固定衬垫型膝关节假体相比并没有显著性优势。手术技术是获得置换后良好临床效果的关键。
