AIM: To study the efficacy and the safety of laser lithotripsy without direct visual control by using a balloon catheter in patients with bile duct stones that could not be extracted by standard technique. METHODS: The seventeen patients (7 male and 10 female; mean age 67.8 years) with difficult common bile duct (CBD) stones were not amenable for conventional endoscopic maneuvers such as sphincterotomy and mechanical lithotripsy were included in this study. Laser wavelengths of 532 nm and 1064 nm as a double pulse were applied with pulse energy of 120 mJ. The laser fiber was advanced under fluoroscopic control through the ERCP balloon catheter. Laser lithotripsy was continued until the fragment size seemed to be less than 10 mm. Endoscopic extraction of the stones and fragments was performed with the use of the Dormia basket and balloon catheter. RESULTS: Bile duct clearance was achieved in 15 of 17 patients (88%). The mean number of treatment sessions was 1.7 ± 0.6. Endoscopic stone removal could not be achieved in 2 patients (7%). Adverse effects were noted in three patients (hemobilia, pancreatitis, and cholangitis). CONCLUSION: The Frequency Doubled Double Pulse Nd:YAG (FREDDY) laser may be an effective and safe technique in treatment of difficult bile duct stones.

AIM： To investigate the effects of high glucose levels and anti-vascular endothelial growth factor（VEGF） agents（bevacizumab,ranibizumab and aflibercept） on retinal pigment epithelium（RPE） ***： ARPE-19 cells were cultured at different glucose levels（5.5 mmol/L,25 mmol/L,and 75 mmol/L）.Cell viability was evaluated by MTT assay at 3d after treatment with *** migration ability was measured by wound healing assay at 3d.A cell death detection kit was used to assess apoptosis at 3 and 14 *** proliferation was assessed by EdU assay at *** culture medium was treated with anti-VEGF agents at clinically relevant *** experiment was then repeated at a different glucose ***： The viability and migration of ARPE-19 cells were significantly decreased in the presence of 75 mmol/L as compared to 5.5 mmol/L *** percentage of TUNEL-positive cells was significantly increased and the proliferative potential was decreased with 75 mmol/L compared to 5.5 mmol/L *** were no significant differences in the results between 25 mmol/L and 5.5 mmol/L *** the presence of 75 mmol/L glucose,the groups treated with anti-VEGF showed decreased cell viability and proliferation and increased ***,there were no significant differences between the anti-VEGF ***： High glucose level decreases the viability,wound healing ability,and proliferation of RPE cells,while increasing ***,anti-VEGF agents interfered with the physiological functions of RPE cells under high-glucose conditions,accompanied by decreases in cell viability and proliferation.

In early 2000,large domestic shipyards introduced shipbuilding 3D computer-aided design (CAD) to the hull production design process to define manufacturing and assembly *** production design process accounts for most of the man-hours (M/H) of the entire design process and is closely connected to yard production because designs must take into account the production schedule of the shipyard,the current state of the dock needed to mount the ship’s block,and supply ***,many shipyards are investigating the complete automation of the production design process to reduce the M/H for ***,these problems are still currently unresolved,and a clear direction is needed for research on the automatic design base of manufacturing rules,batches reflecting changed building specifications,batch updates of boundary information for hull members,and management of the hull model change history to automate the production design *** this study,a process was developed to aid production design engineers in designing a new ship’s hull block model from that of a similar ship previously built,based on AVEVA *** automation system that uses the similar ship’s hull block model is proposed to reduce M/H and human errors by the production design ***,scheme files holding important information were constructed in a database to automatically update hull block model ***,for batch updates,the database’s table,including building specifications and the referential integrity of a relational database were *** particular,this study focused on reflecting the frequent modification of building specifications and regeneration of boundary information of the adjacent panel due to changes in a specific ***,the rollback function is proposed in which the database (DB) is used to return to the previously designed panels.

Despite numerous efforts to overcome neuropathic pain,various pharmacological drugs often fail to meet the needs and have many side *** is an aluminosilicate mineral that has been reported to have an anti-inflammatory effect,but the efficacy of muscovite for neuropathic pain has not been ***,we assessed whether muscovite nanoparticles can reduce the symptoms of pain by controlling the inflammatory process observed in neuropathic *** analgesic effects of muscovite nanoparticles were explored using partial sciatic nerve ligation model of neuropathic pain,in which one-third to onehalf of the nerve trifurcation of the sciatic nerve was tightly tied to the dorsal *** nanoparticles(4 mg/100μL)was given intramuscularly to evaluate its effects on neuropathic pain(3 days per week for 4 weeks).The results showed that the muscovite nanoparticle injections significantly alleviated partial sciatic nerve ligation-induced mechanical and cold *** the spinal cord,the muscovite nanoparticle injections exhibited inhibitory effects on astrocyte and microglia activation and reduced the expression of pro-inflammatory cytokines,such as interleukin-1β,tumor necrosis factor-α,interleiukin-6 and monocyte chemoattractant protein-1,which were upregulated in the partial sciatic nerve ligation ***,the muscovite nanoparticle injections resulted in a decrease in activating transcription factor 3,a neuronal injury marker,in the sciatic *** results suggest that the analgesic effects of muscovite nanoparticle on partial sciatic nerve ligation-induced neuropathic pain may result from inhibiting activation of astrocytes and microglia as well as pro-inflammatory *** propose that muscovite nanoparticle is a potential anti-nociceptive candidate for neuropathic *** experimental protocols in this study were approved by the Institutional Animal Ethics Committee(IACUC)at Dongguk University,South Korea(approval No.2017-022-1)

The Korea Water Resources Corporation （K-Water） has encountered four problems during the management of water resources of approximately 1,300 buildings spread throughout the country-degradation, budget constraints, lack of standards, and lack of planning. In this study, we have developed a mechanism for collecting, maintaining, and analyzing ground data for repair and replacement works. Our BMS （building maintenance system） uses the recorded repair and replacement work histories and calculates the life cycle costs of the related items. Further, it enables the performance of repair and replacement tasks through an integrated approach aimed at preventive maintenance. In this manner, the BMS is expected to maximize the efficiency of the formulation of prevention and maintenance work plans, monitoring of work histories, predictions of future tasks from the analyses of repair and replacement histories, and performance of LCC （life cycle cost） calculations. Reliable maintenance of national properties can thus be achieved by using the BMS, which promotes reasonable and efficient repair and replacement activities.

Objective:The effectiveness of adjuvant treatments for resected gallbladder carcinoma(GBC)has remained unclear due to lack of randomized controlled trials;thus,the aim of present study was to evaluate the role of adjuvant treatments,including chemoradiotherapy(CRT)and/or chemotherapy(CTx),in patients with resected ***:A total of 733 GBC patients who received curative-intent surgical resection were identified in a multi-institutional *** 733 patients,372(50.8%)did not receive adjuvant treatment,whereas 215(29.3%)and 146(19.9%)received adjuvant CTx and CRT,*** locoregional recurrence-free survival(LRFS),recurrence-free survival(RFS),and overall survival(OS)of the adjuvant treatment groups were compared according to tumor stage(stage II *** III–IV).Results:In stage II disease(n=381),the 5-year LRFS,RFS,and OS were not significantly different among the no-adjuvant therapy,CTx,and CRT groups,and positive resection margin,presence of perineural invasion,and Nx classification were consistently associated with worse LRFS,RFS,and OS in the multivariate analysis(P<0.05).For stage III–IV(n=352),the CRT group had significantly higher 5-year LRFS,RFS,and OS than the no-adjuvant therapy and CTx groups(67.8%,45.2%,and 56.9%;37.9%,28.8%,and 35.4%;and 45.0%,30.0%,and 45.7%,respectively)(P<0.05).Conclusions:CRT has value as adjuvant treatment for resected GBC with stage III–IV *** study is needed for stage II disease with high-risk features.

Adult intussusception caused by cystic lymphangioma of the colon:A rare case report

We experienced a case of intussusception caused by cystic lymphangioma of the colon in a 32 years old female who was admitted to our hospital for the chief complaint of bloody stool. In the colonoscopic examination, cystic mass with stalk which had smooth mucosal surface was noted at the descending colon. Abdominal ultrasonography and computed tomography revealed left colon intussusception with a multilocular cystic tumor as a leading point. Emergent operation was performed. On the histopathologic examination, the cystically dilated spaces lined by endothelium and septated by fibrous septa were present. The pathological diagnosis was cystic lymphangioma of the colon. Although intussusception due to lymphangioma in an adult are rare, it should be taken into consideration that it is possible diagnosis.
