Trends in colorectal cancer incidence according to an increase in the number of colonoscopy cases in Korea

BACKGROUND The incidence of colorectal cancer(CRC)and preinvasive CRC(e.g.,early colon cancer and advanced adenoma)is gradually increasing in several countries.AIM To evaluate the trend in incidence of CRC and preinvasive CRC according to the increase in the number of colonoscopies performed in Korea.METHODS This retrospective cohort study enrolled Korean patients from 2002 to 2020 to evaluate the incidence of CRC and preinvasive CRC,and assess the numbers of diagnostic colonoscopies and colonoscopic polypectomies.Colonoscopy-related complications by age group were also determined.RESULTS The incidence of CRC showed a rapid increase,then decreased after 2012 in the 50-75 year-age group.During the study period,the rate of incidence of preinvasive CRC increased at a similar level in patients under 50 and 50-75 years of age.Since 2009,the increase has been rapid,showing a pattern similar to the increase in colonoscopies.The rate of colonoscopic polypectomy in patients aged under 50 was similar to the rate in patients over 75 years of age after 2007.The rate of complications after colonoscopy and related deaths within 3 mo was high for those over 75 years of age.CONCLUSION The diagnosis of preinvasive CRC increased with the increase in the number of colonoscopies performed.As the risk of colonoscopy-related hospitalization and death is high in the elderly,if early lesions at risk of developing CRC are diagnosed and treated under or at the age of 75,colonoscopy-related complications can be reduced for those aged 76 years or over.

Tailoring local structures of atomically dispersed copper sites for highly selective CO2 electroreduction

Atomically‐dispersed copper sites coordinated with nitrogen‐doped carbon(Cu–N–C)can provide novel possibilities to enable highly selective and active electrochemical CO_(2) reduction reactions.However,the construction of optimal local electronic structures for nitrogen‐coordinated Cu sites(Cu–N_(4))on carbon remains challenging.Here,we synthesized the Cu–N–C catalysts with atomically‐dispersed edge‐hosted Cu–N_(4) sites(Cu–N_(4)C_(8))located in a micropore between two graphitic sheets via a facile method to control the concentration of metal precursor.Edge‐hosted Cu–N_(4)C_(8) catalysts outperformed the previously reported M–N–C catalysts for CO_(2)‐to‐CO conversion,achieving a maximum CO Faradaic efficiency(FECO)of 96%,a CO current density of–8.97 mA cm–2 at–0.8 V versus reversible hydrogen electrode(RHE),and over FECO of 90%from–0.6 to–1.0 V versus RHE.Computational studies revealed that the micropore of the graphitic layer in edge‐hosted Cu–N_(4)C_(8) sites causes the d‐orbital energy level of the Cu atom to shift upward,which in return decreases the occupancy of antibonding states in the*COOH binding.This research suggests new insights into tailoring the locally coordinated structure of the electrocatalyst at the atomic scale to achieve highly selective electrocatalytic reactions.

Complication after nipple-areolar complex tattooing performed by a non-medical person:A case report

BACKGROUND Compared to earlier,there has been an increase in the tattoo procedures for cosmetic purposes;and there has also been an increase in the tattoo procedures performed by non-medical personnel.In South Korea,only tattoos performed by a doctor are considered legal;however,there is still some debate over whether tattoo procedures performed by non-healthcare providers should be considered legal.CASE SUMMARY A 28-year-old woman visited our hospital with pain in both nipples and heat sensation over the last 4 d.She had a history of a nipple tattoo performed by an unlicensed person.Pinpoint bleeding was noted in both areolar areas,and the exudate mixed with pus and orange color ink was discharged.Oral medication and tulle with foam dressing were performed under the impression of cellulitis and allergic reaction.After 4 wk,nipples remained dark brown in color,resulting in a color mismatch between the nipple and orange-colored areola.The size of the areola was also found to be distinctly asymmetrical after healing.This complication may have been caused by the use of illegal ink or unsanitary procedures,or a problem may have occurred in the post-tattoo management stage.CONCLUSION Doctors use approved ink,aseptic procedure and appropriate postoperative care,and appropriate management can be performed in case of complications.

B cells have an essential immunoregulatory function,which is mediated by the secretion of IL-10 and TGFβand interaction with activated CD4^(+)T cells.This antibody-independent function of B cells contributes to ameliorating autoimmune diseases or infection-induced tissue damage.In this Cellular and Molecular Immunology issue,Fu et al.demonstrated that regulatory B(Breg)cells play a critical role in resolving Pseudomonas aeruginosa-induced pneumonia[1].

Delayed-onset endophthalmitis associated with Achromobacter species developed in acute form several months after cataract surgery:Three case reports

BACKGROUND Achromobacter species-associated endophthalmitis is rare and may present as either acute or chronic postoperative endophthalmitis.Delayed-onset Achromobacter species endophthalmitis appearing in acute presentation that develops more than several months after cataract surgery is very rare.Intraocular lens(IOL)removal is commonly recommended to treat Achromobacter species endophthalmitis,which is based on previous studies.Here,we report the results of surgery without IOL removal when treating patients with delayed-onset postoperative Achromobacter species endophthalmitis that developed in an acute form.CASE SUMMARY Three patients visited our ophthalmology clinic due to visual impairment that began 2-3 d earlier.They had undergone cataract surgery 5-18 mo prior.Bestcorrected visual acuity of the diseased eye was between counting fingers at 30 cm to non-light perception.They showed conjunctival injection,inflammation in the anterior chamber(cell reaction 4+)and hypopyon formation.The patients were diagnosed with infectious endophthalmitis and immediately underwent pars plana vitrectomy,anterior chamber irrigation and intravitreal injection of ceftazidime and vancomycin.Before fluid infusion,a vitreous specimen was obtained. In all cases, the IOLs were not removed. Achromobacter species was detected on vitreousspecimen culture. After surgery, the vitreous opacity decreased gradually and there was littleretinal damage. At 1 mo after treatment, the best-corrected visual acuity had improved to 20/50and 20/40.CONCLUSIONDelayed onset postoperative endophthalmitis caused by Achromobacter species can appear in anacute form. All patients responded well to early vitrectomy and administration of empiricalantibiotics including ceftazidime. There was no need for IOL removal during surgery.

A rational regulation of the solar water splitting reaction pathway by adjusting the surface composition and phase structure of catalysts is a substantial approach to ameliorate the sluggish reaction kinetics and improve the energy conversion efficiency.In this study,we demonstrate a nanocrystalline iron pyrophosphate(Fe_(4)(P_(2)O_(7))_(3),FePy)-regulated hybrid overlayer with amorphous iron phosphate(FePO_(4),FePi)on the surface of metal oxide nanostructure with boosted photoelectrochemical(PEC)water oxidation.By manipulating the facile electrochemical surface treatment followed by the phosphating process,nanocrystalline FePy is localized in the FePi amorphous overlayer to form a heterogeneous hybrid structure.The FePy-regulated hybrid overlayer(FePy@FePi)results in significantly enhanced PEC performance with long-term durability.Compared with the homogeneous FePi amorphous overlayer,FePy@FePi can improve the charge transfer efficiency more significantly,from 60% of FePi to 79%of FePy@FePi.Our density-functional theory calculations reveal that the coexistence of FePi and FePy phases on the surface of metal oxide results in much better oxygen evolution reaction kinetics,where the FePi was found to have a typical down-hill reaction for the conversion from OH*to O_(2),while FePy has a low free energy for the formation of OH*.

Removal of intrahepatic bile duct stone could reduce the risk of cholangiocarcinoma: A single-center retrospective study in South Korea

BACKGROUND Intrahepatic duct(IHD)stones are among the most important risk factors for cholangiocarcinoma(CCC).Approximately 10%of patients with IHD stones develop CCC;however,there are limited studies regarding the effect of IHD stone removal on CCC development.AIM To investigate the association between IHD stone removal and CCC development.METHODS We retrospectively analyzed 397 patients with IHD stones at a tertiary referral center between January 2011 and December 2020.RESULTS CCC occurred in 36 of the 397 enrolled patients.In univariate analysis,chronic hepatitis B infection(11.1%vs 3.0%,P=0.03),carbohydrate antigen 19-9(CA19-9,176.00 vs 11.96 II/mL,P=0.010),stone located in left or both lobes(86.1%vs 70.1%,P=0.042),focal atrophy(52.8%vs 26.9%,P=0.001),duct stricture(47.2%vs 24.9%,P=0.004),and removal status of IHD stone(33.3%vs 63.2%,Pupper normal limit,carcinoembryonic antigen>upper normal limit,stones located in the left or both lobes,focal atrophy,and complete removal of IHD stones without recurrence were independent factors influencing CCC development.However,the type of removal method was not associated with CCC risk.CONCLUSION Complete removal of IHD stones without recurrence could reduce CCC risk.

Immediate secondary rhinoplasty using a folded dermofat graft for resolving complications related to silicone implants: A case report

BACKGROUND Various surgical techniques have been developed to enhance the nose shapes of Asian patients.Silicone implant augmentation rhinoplasty is widely used because it is relatively easy to perform and often yields satisfactory outcomes.However,this technique may lead to complications,including ischemia,necrosis,and over-augmentation.The most appropriate management of these complications,including infection,is immediate implant removal and revision surgery once the accompanying inflammation has healed.Occasionally,the patient may experience distress from nasal deformities during the intervention period.CASE SUMMARY Herein,we describe the case of a patient who underwent a secondary dorsal augmentation,with a folded dermofat graft harvested from the inguinal area and simultaneous implant removal,successfully preventing dimpling of the nasal deformity.CONCLUSION This surgical method can effectively manage implant-related complications following augmentation rhinoplasty using a silicone implant and provide satis-factory patient outcomes.

Ag-GST/HfO_(x)-based unidirectional threshold switching selector with low leakage current and threshold voltage distribution for high-density cross-point arrays

The use of electrochemical-metallization-based volatile threshold switching selectors in cross-point arrays has been widely explored owing to their high on-off ratios and simple structure.However,these devices are unsuitable for cross-point architectures because of the difficulty in controlling the random filament formation that results in large fluctuations in the threshold voltage during operation.In this study,we investigated the unidirectional threshold transition characteristics associated with an Ag/GST/HfO_(x)/Pt-based bilayer selector and demonstrated the occurrence of a low leakage current(<1×10^(-11) A) and low distribution of the threshold voltage(Δ0.11 V).The bilayer structure could control the filament formation in the intermediate state through the insertion of an HfO_(x) tunneling barrier.By stacking a bilayer selector with NiO_(x)based resistive random-access memory,the leakage and programming currents of the device could be significantly decreased.For the crossbar array configuration,we performed equivalent circuit analysis of a one-selector oneresistor(1S1R) devices and estimated the optimal array size to demonstrate the applicability of the proposed structure.The maximum acceptable crossbar array size of the 1S1R device with the Ag/GST/HfO_(x)/Pt/Ti/NiO_(x)/Pt structure was 5.29×10^(14)(N^(2),N=2.3×10^(7)).

Texture development and grain boundary phase formation in Ce-and Ce-La-substituted Nd-Fe-B magnets during hot-deformation process

In this study,the influence of Ce-and Ce-La-substitution for Nd in Nd-Fe-B magnets on their magnetic and microstructural changes in the course of the hot-deformation process was investigated to gain insight into further reducing the Nd content in fine-grained Nd-Fe-B hot-deformed magnets.We found that the[001]texture and microstructure of the RE-rich grain boundary phases,which are the factors that de-termine the remanence and coercivity of hot-deformed magnets,were deteriorated by the Ce-and Ce-La-substitution.This is because the volume fraction of the RE-rich liquid formed in the grain boundaries during the hot-press and hot-deformation decreased due to the Ce-and Ce-La-substitution,thereby in-creasing the friction between the 2:14:1 grains while the c-axes of grains were aligned by grain rotation and decreasing the RE concentration of grain boundary phases.In particular,the Ce-La-co-substitution further prevented the liquid formation within the grain boundaries of the magnets during hot-press and hot-deformation because the wettability of the RE-rich liquid on the 2:14:1 grains became poor when La was substituted for Nd in the 2:14:1 grains.These results indicate that there is room for further im-provement in both the remanence and coercivity of the Ce-and Ce-La-substituted magnets by intro-ducing additional liquid into the grain boundaries prior to hot-press and hot-deformation.By applying the Nd-Cu infiltration process to the melt-spun ribbons(intermediate infiltration,I-infiltration),both the remanence and coercivity of the Nd-saving(Nd 0.7 Ce 0.3)-Fe-B and(Nd 0.7 Ce 0.225 La 0.075)-Fe-B hot-deformed magnets were successfully improved.
