Efficacy of tolvaptan in an infant with syndrome of inappropriate antidiuretic hormone secretion associated with holoprosencephaly:A case report

Quality of life in patients with chronic anal fissure after topical treatment with diltiazem

AIM: To assess the physical and mental health of fissure patients before and after topical treatment with diltiazem. METHODS: Consecutive patients were enrolled prospectively into the study. Quality of life was measured with the short-forum 36 health survey (SF-36) before and after 6-wk treatment with diltiazem. Patients scored symptoms of pain, bleeding, and irritation using numeral rating scales at the initial and follow-up visits. Fissure healing was assessed and side effects were noted. RESULTS: Fissures healed in 21 of 30 (70%) patients. There were significant reductions in the scores of pain, bleeding, and irritation after 1 wk of treatment, respectively. Four patients experienced perianal itching and one patient reported headache. When measured at baseline, pain and irritation showed a negative impact on two of the eight subscales on the SF-36, respectively (bodily pain and social functioning for pain; vitality and mental health for irritation). Repeating the SF-36 showed an improvement in bodily pain (P = 0.001). Patients whose fissures healed reported an improvement in bodily pain, health-perception, vitality, and mental health (P < 0.05). CONCLUSION: Successful treatment of chronic anal fissure with topical diltiazem leads to improvement in health-related quality of life.

错流碟片旋转填充床反应器可有效脱除燃煤烟道气中的二氧化碳和其它有害成分,但液体分布状态对反应速率有很大影响,研究液膜的体积分率可为该类型反应器的设计提供有效的依据参数;实验研究了碟片转速、液体流量、气体流量、碟片间距和碟片开孔率与液膜体积分率的关系,用每张碟片底部对应液体收集器储存并测量碟片表面液膜的体积流量,用高速摄像机记录不同条件下液膜的存在状态和变化规律。研究表明随碟片转速增加液膜分率降低,增强的离心力促使液膜破裂,但液滴的甩出速率增加,促使液滴二次聚并,有不连续液丝产生。增大入口液体流量,液膜分率减小,液膜厚度变化缓慢,空间液滴增大。增大气体流量,液膜分率减小,厚度变薄,碟片表面附着液膜被吹散,促使液体雾化。液膜分率随碟片间距增加减小,提高碟片间距可有效减少液滴二次聚并;碟片开孔率增大液膜分率减小,提高碟片开孔率可有效降低碟片表面液膜的连续性,促使液膜分裂。

Distribution of the P2X2 receptor and chemical coding in ileal enteric neurons of obese male mice(ob/ob)

AIM:To investigate the colocalization,density and profile of neuronal areas of enteric neurons in the ileum of male obese ***:The small intestinal samples of male mice in an obese group(OG)(C57BL/6J ob/ob)and a control group(CG)(+/+)were *** tissues were analyzed using a double immunostaining technique for immunoreactivity(ir)of the P2X2 receptor,nitric oxide synthase(NOS),choline acetyl transferase(ChAT)and calretinin(Calr).Also,we investigated the density and profile of neuronal areas of the NOS-,ChAT-and Calrir neurons in the myenteric *** neurons were labeled using an NADH-diaphorase histochemical staining ***:The analysis demonstrated that the P2X2receptor was expressed in the cytoplasm and in the nuclear and cytoplasmic membranes only in the *** density values(neuron/cm2)decreased 31%(CG:6579±837;OG:4556±407)and 16.5%(CG:7796±528;OG:6513±610)in the NOS-ir and calretininir neurons in the OG,respectively(P<0.05).Density of ChAT-ir(CG:6200±310;OG:8125±749)neurons significantly increased 31%in the OG(P<0.05).Neuron size studies demonstrated that NOS,ChAT,and Calr-ir neurons did not differ significantly between the CG and OG *** examination of NADH-diaphorase-positive myenteric neurons revealed an overall similarity between the OG and ***:Obesity may exert its effects by promoting a decrease in P2X2 receptor expression and modifications in the density of the NOS-ir,ChAT-ir and CalR-ir myenteric neurons.

Influence of Heat Treatment Temperature and of Sb2O3 addition on Photoluminescence Properties of ZnO Ceramics Prepared by Sol-Gel Technique Preparation

To explore thin transparent electroluminescence and electric conductive films by sol-gel technique, Sb2O3 doped n-type ZnO ceramics powders were prepared by sol-gel technique and photoluminescence properties were measured. Then, the influences of composition and heat treatment temperature on photoluminescence properties were investigated in detail. With respect to the dopant concentration, about 1mol% addition of Sb2O3 was effective to increase photoluminescence intensity. With respect to heat treatment temperature, 800℃ was appropriate, and rather higher heat treatment temperature resulted in the formation of Zn7Sb2O12 and decrease the intensity. The excited ultraviolet wavelength of 200nm was proper to intense photoluminescence.

Ethical Dilemma Factor in Regarding Physical Restraints to Elderly of Female Nurses with the Living Together Experience

Purpose: The aim of this study was to illuminate the connotation of “dilemma” regarding the use of physical restraint on elderly patients as represented by female nurses working in general wards at community hospitals who also live with elderly adults at home. Method: The study used the questionnaire method with an independently prepared questionnaire. Participants: The objective of the study was explained to the persons in charge of nursing in the selected 17 hospitals, and 1929. Finally, this study of participants were 524 female nurses working in general 54 wards (excluding the emergency wards of psychiatry, pediatrics, obstetrics, outpatients, operating rooms, and intensive care units) at community hospitals who also live with elderly adults at home. Results and conclusions: Cronbach’s overall coefficient for the 20 items of dilemma was high (0.78) and the factor analysis extracted four dilemma factors as having a characteristic value of 1 (Kaiser-Meyer-Olkin measure value = 0.81) with a cumulative contribution ratio of 64.5%. The high Cronbach’s for these items (0.86, 0.88, 0.87 and 0.81) confirmed the internal consistencies. With respect to the dilemma where nurses working in general wards at community hospitals who also live with elderly adults at home are faced with the physical restraint of elderly persons, four factors in the clarification of the dilemma were extracted: accomplishment of medical-treatment and accident prevention, characteristic features in nursing for elderly patients with dementia, healthcare professional relationship in nursing for elderly patients with dementia, and priorities on the accident prevention for elderly patients with dementia. Therefore, the construction of four systems to cope with these dilemmas is suggested. These systems would enable practising nurses to: (i) institution of policies to encourage discussion between nurses and other medical staff to reach consensus on treatment;(ii) allocating elderly care specialists to w

Inhibition of gastric perception of mild distention by omeprazole in volunteers

AIM： To evaluate the effects of omeprazole on gastric mechanosensitivity in humans. METHODS： A double lumen polyvinyl tube with a plas- tic bag was introduced into the stomach of healthy volunteers under fluorography and connected to a barostat device. Subjects were then positioned so they were sitting comfortably, and the minimal distending pressure （MDP） was determined after a 30-rain adap- tation period. Isobaric distensions were performed in stepwise increments of 2 mmHg （2 min each） starting from the MDR Subjects were instructed to score feel- ings at the end of every step using a graphic rating scale： 0, no perception; 1, weak/vague; 2, weak but significant; 3, moderate/vague; 4, moderate but signifi- cant; 5, severe discomfort; and 6, unbearable pain. Af- ter this first test, subjects received omeprazole （20 mg, after dinner） once daily for 1 wk. A second test was performed on the last day of treatment. RESULTS： No adverse effects were observed. Mean MDP before and after treatment was 6.3 - 0.3 mmHg and 6.2：1：0.5 mmHg, respectively. One subject before and 2 after treatment did not reach a score of 6 at the maximum bag volume of 750 mL. After omeprazole, there was a significant increase in the distension pres- sure required to reach scores of 1 （P = 0.019） and 2 （P = 0.017） as compared to baseline. There were no changes in pressure required to reach the other scores after treatment. Two subjects before and one after omeprazole rated their abdominal feeling 〈 1 at MDP, and mean （±SE） abdominal discomfort scores at MDP were 0.13±0.09 and 0.04±0.04, respectively. Mean scores induced by each MDP ＋ 2, 4, 6, 8, 10, 12, 14, 16, 18 and 20 （mmHg） were 1.1±0.3, 2.0±0.4, 2.9±0.5, 3.3±0.4, 4.6±0.3, 5.2±0.3, 5.5±0.2, 5.5±0.3, 5.7±0.3, and 5.4, respectively. After omepra- zole, abdominal feeling scores for the same incremen- tal pressures over MDP were 0.3±0.1, 0.8±0.1, 2.0±0.4, 2.8±0.4, 3.8±0.4, 4.6±0.4, 4.9±0.3, 5.4±0.4, 5.2±0.6, and 5.0±1.0, respectively. TNF Type-I Receptor Inhibitor, R-7050 Attenuates Acute Kidney Injury in a Mouse Model of Crush Syndrome

Crush syndrome (CS) is caused by severe and extensive muscular skeletal damages, and acute kidney injury (AKI) with hyperkalemia is one of the most lethal factors of this syndrome. Especially under natural disasters of earthquake, many persons die due to AKI and hyperkalemia-induced cardiac arrest, but there has been no pathogenesis-based drugs for preventing CS-induced AKI. Pro-inflammatory cytokines, such as TNF-α and IL-6, play a critical role for induction of AKI during CS development. Glycerol-injected mice are used as an experimental tool for reflecting pathological events of human CS. Using this popular model, we provide evidence to show that TNF type-I receptor (TNFR1) inhibitor, R-7050 significantly attenuates the onset of AKI after the muscular destruction. In this process, R-7050 treatment suppressed the NF-κB activation in the affected kidney, and this was associated with a decrease in blood IL-6, a downstream target of NF-κB. As a result, renal tubular apoptosis became milder in the R-7050-treated CS mice. These findings suggest that induction of IL-6 via sequential events of TNF-α à TNFR1 à NF-κB is contributable for renal tubular apoptosis, a histological hallmark of AKI. Thus, TNFR1-selective inhibition can be a pharmacological strategy to attenuate the onset of AKI immediately after the clinical manifestation of rhabdomyolysis.

CT-maximum intensity projection is a clinically useful modality for the detection of gastric varices

AIM： To evaluate the efficacy of CT-maximum intensity projection （CT-MIP） in the detection of gastric varices and their infiowing and outflowing vessels in patients with gastric varices scheduled to undergo balloonoccluded retrograde transvenous obliteration （B-RTO）. METHODS： Sixteen patients with endoscopically confirmed gastric varices were included in this study. All patients were evaluated with CT-MIP using threedimensional reconstructions, before and after B-RTO. RESULTS： CT-MIP clearly depicted gastric varices in 16 patients （100%）, the left gastric vein in 6 （32.5%）, the posterior gastric vein in 12 （75.0%）, the short gastric veins in 13 （81.3%）, gastrorenal shunts in 16 （100%）, the hemiazygos vein （HAZV） in 4 （25.0%）, the pericardiophrenic vein （PCPV） in 9 （56.3%）, and the left inferior phrenic vein in 9 patients （56.3%）. Although flow direction itself cannot be determined from CT-MIP, this modality provided clear images of the inflowing and the outflowing vessels. Moreover, in one patient, short gastric veins were not seen on conventional angiographic portography images of the spleen, but were clearly revealed on CT-MIP. CONCLUSION： We suggest that CT-MIP should be considered as a routine method for detecting and diagnosing collateral veins in patients with gastric varices scheduled for B-RTO. Furthermore, CT-MIP is more useful than endoscopy in verifying the early therapeutic effects of B-RTO.
