The global,prevalence,and risk factors of postoperative fever after percutaneous nephrolithotomy:A systematic review and meta-analysis

Objective:This study aimed to explore the global,prevalence,and risk factors of fever after percutaneous nephrolithotomy(PCNL)by conducting a systematic review and meta-analysis.Methods:The high-sensitivity searching was conducted without time limitation until December 30,2020 in Web of Sciences,Scopus,and PubMed based on inclusion and exclusion criteria.Results:The prevalence rates of fever and sepsis among patient undergoing PCNL were estimated 9.5%(95%confidence interval[CI]:9.3%-9.7%),and 4.5%(95%CI:4.2%-4.8%),respectively.Nephrostomy tube was used in 9.96%(95%CI:9.94%-9.97%)of patients.The mean preoperative white blood cells of patients were 6.401×109/L;18.3%and 4.55%of patients were considered as the positive urinary culture and pyuria,respectively.About 20.4%of patients suffered from residual stones.The odds ratios(ORs)of fever in patients who suffering from diabetes mellitus,hydronephrosis,staghorn stones,and blood transfusion were 4.62(95%CI:2.95-7.26),1.04(95%CI:0.81-1.34),2.57(95%CI:0.93-7.11),and 2.65(95%CI:1.62-4.35),respectively.Patients who underwent PCNL in prone position were more likely to develop fever(OR:1.23;95%CI:0.75-2.00)than patients in supine position.Conclusion:The current study showed that patients who suffer from diabetes mellitus,hydronephrosis,staghorn stones,nephrostomy tube or double-J stent,blood transfusion,and also patients who underwent PCNL in prone position surgery are more likely to develop a postoperative fever after PCNL.

The aggregation of amyloid-beta peptide and tau protein dysregulation are implicated to play key roles in Alzheimer’s disease pathogenesis and are considered the main pathological hallmarks of this devastating disease. Physiologically, these two proteins are produced and expressed within the normal human body. However, under pathological conditions, abnormal expression, posttranslational modifications, conformational changes, and truncation can make these proteins prone to aggregation, triggering specific disease-related cascades. Recent studies have indicated associations between aberrant behavior of amyloid-beta and tau proteins and various neurological diseases, such as Alzheimer’s disease, Parkinson’s disease, and amyotrophic lateral sclerosis, as well as retinal neurodegenerative diseases like Glaucoma and age-related macular degeneration. Additionally, these proteins have been linked to cardiovascular disease, cancer, traumatic brain injury, and diabetes, which are all leading causes of morbidity and mortality. In this comprehensive review, we provide an overview of the connections between amyloid-beta and tau proteins and a spectrum of disorders.

Diversity of Helicobacter pylori genotypes in Iranian patients with different gastroduodenal disorders

AIM:To investigate the diversity of Helicobacter pylori(H.pylori)genotypes and correlations with disease outcomes in an Iranian population with different gastroduodenal disorders.METHODS:Isolates of H.pylori from patients with different gastroduodenal disorders were analyzed after culture and identification by phenotypic and genotypic methods.Genomic DNA was extracted with the QIAamp DNA mini kit(Qiagen,Germany).After DNA extraction,genotyping was done for cagA,vacA(s and m regions),iceA(iceA1,iceA2)and babA with specific primers for each allele using polymerase chain reaction(PCR).All patients’pathologic and clinical data and their relation with known genotypes were analyzed by using SPSS version 19.0 software.2test and Fisher’s exact test were used to assess relationships between categorical variables.The level of statistical significance was set at P<0.05.RESULTS:A total of 71 isolates from 177 patients with different gastroduodenal disorders were obtained.Based on analysis of the cagA gene(positive or negative),vacA s-region(s1or s2),vacA m-region(m1or m2),iceA allelic type(iceA1and iceA2)and babA gene(positive or negative),twenty different genotypic combinations were recognized.The prevalence of cagA,vacA s1,vacA s2,vacA m1,vacA m2,iceA1,iceA2,iceA1+iceA2and babA were 62%,78.9%,19.7%,21.1%,78.9%,15.5%,22.5%,40.8%and 95.8%,respectively.Interestingly,evaluation of PCR results for cagA in 6 patients showed simultaneous existence of cagA variants according to their size diversities that proposed mixed infection in these patients.The most prevalent genotype in cagA-positive isolates was cagA+/vacAs1m2/iceA1+A2/babA+and in cagA-negative isolates was cagA-/vacAs1m2/iceA-/babA+.There were no relationships between the studied genes and histo-pathological findings(H.pylori density,neutrophil activity,lymphoid aggregation in lamina propria and glandular atrophy).The strains which carry cagA,vacAs1/m1,iceA2and babA genes showed significant associations with severe active chronic gastritis(P=0.011,0.025,0.020 and 0.031,r

Analysis of the bounded and unbounded forms of USBM wettability index

Understanding wettability is important in many processes where porous media and fluids are in contact.The U.S.Bureau of Mines(USBM)is one of the industry standard techniques for measuring wettability.This index is calculated as USBM=lg(A_(1)/A_(2)),where A_(1) and A_(2) are the areas under capillary pressure curves of oil-drive and water-drive processes,respectively.Usually,the USBM is mistakenly assumed to vary over the range of-1 to 1 and compared with other indices.In this study we indicate that the lower and upper bounds of this index are not fully known in practice.As a result,comparison between USBM and other indices may cause erroneous interpretations due to dissimilar ranges of variation.In addition,even during examining the USBM of a sample it may not be possible to accurately interpret its wettability.We highlight the bounded form of the USBM index(denoted as USBM^(*)=A_(1)-A_(2)/A_(1)+A_(2)),which varies over the range of -1 to 1,and suggest that it should replace the traditional form of USBM index.Twenty limestone coreplugs were collected from Asmari and Fahlian formations in two Iranian fields.These samples were used for performing primary imbibition relative permeability measurements,as well as primary imbibition and secondary drainage capillary pressure tests.These experiments are used to show the differences between USBM and USBM^(*)in comparative studies and compare them against other indices of Amott-Harvey,Lak and modified Lak.

Real-world and bottom-up methodology for emission inventory development and scenario design in medium-sized cities

Efficient management of air quality requires a comprehensive emission inventory to support decision-making on air quality improvement.This article presents a comprehensive framework for detailed emission inventory development in cities with low-quality basic data,which examines the emission of primary criteria pollutants(CO,NO_(x),SO_(2),PM_(2.5),PM_(10),and VOC)from mobile sources,residential,commercial,and public services,fuel stations,transport terminals,energy conversion sections,and industries.This research was applied to Tabriz in Northwest Iran,one of the polluted medium-sized cities with a population of 1.77 million.Results show the city daily emission per capita is 569.8 g of CO,68.6 g of NO_(x),38.6 g of VOC,17.6 g of SO_(x),and 3.7 g of PM.Vehicular emissions accounted for 98%of CO,91%of VOCs,61%of NO_(x),and 56%of PM;meaning alternative policy strategies in vehicles would reduce emissions rapidly.Fifteen applicable and effective scenarios in transport and one concerning stationary sourceswere proposed and reduction potential of themwas evaluated.Effectiveness of the public transport improvement and replacement of old passenger cars were founded the key scenarios.These two alternatives decrease 14 and 2 tons of SO_(2) and 6797 and 2394 tons of NO_(x) annually with the cost of$99.5 MM and$366.5 MM,respectively.The findings of this study provides the choice of travel method by each citizen is a function of cost,speed,comfort and safety of travel;therefore,all the requirements of any scenarios must be fully considered in the implementation step.

In the past two decades,various research works have been conducted in the field of flexible electronic devices(FEDs).Researchers have focused their efforts on solving the existing challenges in the electronic,electrochemical,and mechanical behaviors of FEDs.The importance of flexible lithium-ion batteries(FLIBs)in the area of FEDs is evident;however,less attention has been paid to the mechanical behavior of FLIBs in comparison with the material and electrochemical characteristics.The present paper reviewed the research works in the FLIBs,focusing on their mechanical integrity and electrochemical performances.First,an introduction to FLIBs was presented,and the previous review papers published in this field were briefly introduced.Then,a detailed review of the available electrochemical and mechanical research works on FLIBs was presented.Moreover,the mechanical testing methods(tensile,compressive,indentation,fatigue,and adhesion)for the characterization of FLIBs’components,the research works on the simulation and modeling of the mechanical behavior of FLIBs,and a summary of the present situation and the future trend of research in this field were reviewed and presented.

The type of lipid supplement has crucial implications for forage particle size in calf starter diets

Background Forage inclusion in starters of young dairy calves has become an acceptable strategy in the last decade.To compensate for the lower energy provided by forage,concurrent lipid supplementation can be proposed.How-ever,ruminal microbial activity and forage digestibility may be decreased by lipid supplementation.We hypothesized that the composite effect of forage and lipid supplements may be dependent on forage particle size and the type of lipid supplement.Therefore,we evaluated the effect of long(LP;geometric mean,4.97 mm)vs.short alfalfa hay particle sizes(SP;geometric mean,1.26 mm)with either soybean oil(SBO)or palm fatty acids(PLF)as lipid source in a 2×2 factorial design with treatments SP-SBO,SP-PLF,LP-SBO,and LP-PLF.Treatments(n=13 with 6 males and 7 females each)were offered to Holstein calves(3 days old)with equal amounts of lipid(25 g/kg DM)through-out the experimental period.The milk offering scheme(d 1 to 53)was equal for all groups.Data collection continued until 20 d post-weaning.Results Interaction between forage particle size and lipid supplement was significant for the following readouts:the highest and lowest starter intakes during the pre-weaning period occurred in LP-PLF and LP-SBO,respectively.This was associated with similarly contrasting changes in average daily gain(ADG)during the post-weaning period,body weight at the end of experiment,withers height,digestibility of organic matter and neutral detergent fiber,and blood serum concentrations of glucose,beta-hydroxybutyrate,and insulin during the pre-weaning period.Dur-ing both pre-and post-weaning periods,the highest and lowest urinary excretion of allantoin and total purine deriva-tives,representing microbial protein synthesis,were observed in LP-PLF and LP-SBO,respectively,indicating that those diets were most and least favorable for rumen development.Irrespective of forage particle size,supplemental SBO vs.PLF increased serum malondialdehyde as an oxidative stress indicator across periods,increased blood urea nitrogen and feed ef

The demand for green energy is growing these days as a result of the world energy crisis,as well as global warm-ing.Solar cells are in great interest due to the fact that solar energy can be easily converted to electricity,if the photovoltaic cell’s cost can be lowered.One of the methods to make low-cost energy is using Luminescent solar concentrators.They have the advantage of directly integrating solar cells to dense urban areas as well as skyscrapers.Different materials and waveguide sizes have been investigated for use in luminescent solar concen-trators.However,the optimized waveguide geometry and quantum dots concentrators have not been thoroughly studied.In this paper,we have simulated graphene quantum dots using density function theory.A Monte-Carlo ray-tracing simulation was developed to model our device.We have optimized the luminescent solar concentrator geometry by Monte-Carlo simulation.The optimization results show a 99%enhancement in the energy flux gain of the final device.Besides,we have calculated and analyzed the fate of all photons.

The rise in incidence rates of invasive candidiasis warrants an increase in atten-tion and efforts toward preventing and treating this virulent infection.Cardiac involvement is one of the most feared sequelae and has a poor prognosis.Despite the introduction of several novel antifungal agents over the past quarter century,complications and mortality rates due to Candida endocarditis have remained high.Although fungal endocarditis has a mechanism similar to bacterial endocar-ditis,no specific diagnostic criteria or algorithm exists to help guide its manage-ment.Furthermore,recent data has questioned the current guidelines recommen-ding a combined approach of antifungal agents with surgical valve or indwelling prostheses removal.With the emergence of multidrug-resistant Candida auris,a focus on improved prophylactic measures and management strategies is nece-ssary.
