An unprecedented outbreak investigation for nosocomial and community-acquired legionellosis in hong Kong

Background The environmental sources associated with community-acquired or nosocomial legionellosis were not always detectable in the mainland of China and hong Kong, China. The objective of this study was to illustrate the control measures implemented for nosocomial and community outbreaks of legionellosis, and to understand the environmental distribution of legionella in the water system in hong Kong, China. Methods We investigated the environmental sources of two cases of legionellosis acquired in the hospital and the community by extensive outbreak investigation and sampling of the potable water system using culture and genetic testing at the respective premises. Results The diagnosis of nosocomial legionellosis was suspected in a patient presenting with nosocomial pneumonia not responsive to multiple beta-lactam antibiotics with subsequent confirmation by Legionella pneumophila serogroup 1 antigenuria. High counts of Legionella pneumophila were detected in the potable water supply of the 70-year-old hospital building. Another patient on continuous ambulatory peritoneal dialysis presenting with acute community-acquired pneumonia and severe diarrhoea was positive for Legionella pneumophila serogroup 1 by polymerase chain reaction （PCR） testing on both sputum and nasopharyngeal aspirate despite negative antigenuria. Paradoxically the source of the second case was traced to the water system of a newly commissioned office building complex. No further cases were detected after shock hyperchlorination with or without superheating of the water systems. Subsequent legionella counts were drastically reduced. Point-of-care infection control by off-boiled or sterile water for mouth care and installation of water filter for showers in the hospital wards for immunocompromised patients was instituted. Responses of Soil Microbial Community Structure and Activity to Incorporation of Straws and Straw Biochars and Their Effects on Soil Respiration and Soil Organic Carbon Turnover

Like straw, biochar incorporation can influence soil microorganisms and enzyme activities and soil carbon(C) responses;however,few studies have compared the various effects of straw and biochar and the underlying mechanisms. An experiment was performed to study the changes in soil respiration(SR) and soil organic C(SOC) fluxes in response to the incorporation of three kinds of straw(reed, smooth cordgrass, and rice) and their pyrolyzed products(biochars) at Chongming Island, China. In addition, the microbial activity and community structure of some amended soils were also analyzed to clarify the mechanisms of these responses. The results showed that all biochar incorporation(BC) induced lower SR than the corresponding unpyrolyzed straw incorporation(ST), and the average SR in the soils following BC and ST during the experimental periods was 21.69 and 65.32 μmol CO2 m^-2s^-1, ***, the average SOC content was 16.97 g kg-1 following BC, which was higher than that(13.71 g kg-1) following ST,indicating that compared to ST, BC was a low-C strategy, even after accounting for the C loss during biochar production. Among the BC treatments, reed-BC induced the lowest SR(17.04 μmol CO2 m^-2s^-1), whereas smooth cordgrass-BC induced the highest SR(27.02 μmol CO2 m^-2s^-1). Furthermore, in contrast with ST, BC significantly increased the abundance of some bacteria with poorer mineralization or better humification ability, which led to lower SR. The lower easily oxidizable C(EOC) and higher total C contents of biochars induced lower SR and higher SOC in the soil following BC compared to that following ST. Among the BC treatments,the higher total nitrogen content of rice biochar led to significantly higher soil microbial biomass, and the lower EOC content of reed biochar led to lower soil microbial activity and SR.

Sewage sludge(SS)is a residual/semi-solid material produced from industrial and municipal wastewater treatment *** contains a high content of lipids and earth alkaline metals that can be used as catalysts for various chemical applications;however,its valorization has rarely been the focus of *** study demonstrates that SS could be a promising raw material for biodiesel production and a biochar catalyst to promote the reaction kinetics of *** induced transesterification of the SS extract(SSE)was performed in comparison with the conventional homogeneous *** biochar was fabricated via *** highest yield(33.5 wt.%per SSE)of biodiesel production was achieved in 1 min of reaction at 305℃via thermally induced transesterification in the presence of SS biochar,while the yield of biodiesel from(trans)esterification with 5 wt.%H_(2)SO_(4)was less than 1%even after 24 *** reaction kinetics(<1 min)of thermally induced transesterification was extraordinarily faster than that of conventional transesterification(3-24 h).The porous structure and high content of alkaline species in the SS biochar expedited the reaction ***,the integrated/hybridized process for thermally induced transesterification and pyrolysis of the solid residue of SS was experimentally proved for the valorization of SS in this *** that SS is being disposed of as a waste material and generates toxic chemicals in the environment,its valorization into value-added biodiesel and a catalyst could be an environmentally benign and sustainable technique.

Salinity influence on soil microbial respiration rate of wetland in the Yangtze River estuary through changing microbial community

Estuarine wetland, where freshwater mixes with salt water, comprises different regions（rivers and marine ecosystems） with significantly varying tidal salinities. Two sampling areas, ZXS and JS, were selected to investigate the effect of tidal salinity on soil respiration（SR）. ZXS and JS were located in Zhongxia Shoal and Jiangyanan Shoal of Jiuduansha Wetland respectively, with similar elevation and plant species, but significantly different in salinity. The results showed that with almost identical plant biomass, the SR and soil microbial respiration（SMR） of the tidal wetland with lower salinity（JS） were significantly higher than those of the tidal wetland with higher salinity（ZXS）（p 〈 0.05）. however, unlike SMR and SR, the difference in the soil microbial biomass（SMB） was not significant（p 〉 0.05）with the SMB of ZXS a little higher than that of JS. The higher SMR and SR of JS may be closely connected to the soil microbial community structures and amount of dominant bacteria. Abundant β- and γ-Proteobacteria and Actinobacteria in JS soil, which have strong heterotrophic metabolic capabilities, could be the main reason for higher SMR and SR,whereas a high number of ε-Proteobacteria in ZXS, some of which have carbon fixation ability, could be responsible for relatively lower carbon output. Path analysis indicated that soil salinity had the maximum negative total influencing coefficient with SMR among the various soil physical and chemical factors, suggesting that higher soil salinity, restricting highly heterotrophic bacteria, is the principle reason for lower SMR and SR in the ZXS.

Adrenomedullin(ADM) is a peptide hormone widely expressed in different tissues, especially in the vasculature. Apart from its vasodilatatory and hypotensive effect, it plays multiple roles in the regulation of hormonal secretion, glucose metabolism and inflammatory response. ADM regulates insulin balance and may participate in the development of diabetes. The plasma level of ADM is increased in people with diabetes, while in healthy individuals the plasma ADM concentration remains low. Plasma ADM levels are further increased in patients with diabetic complications. In type 1 diabetes, plasma ADM level is correlated with renal failure and retinopathy, while in type 2 diabetes its level is linked with a wider range of complications. The elevation of ADM level in diabetes may be due to hyperinsulinemia, oxidative stress and endothelial injury. At the same time, a rise in plasma ADM level can trigger the onset of diabetes. Strategies to reduce ADM level should be explored so as to reduce diabetic complications.

Survival analysis of breast cancer liver metastasis treated by hepatectomy: A propensity score analysis for Chinese women in hong Kong

Background: Survival of patients with breast cancer liver metastasis is very poor. This study aimed to analyze the survival outcome of hepatectomy for this patient population. Methods: From January 1995 to December 2014, 2522 patients with liver cancer received hepatectomy at our hospital. Twenty-one of them, all female, received the operation for breast cancer liver metastasis. Performance was compared with patients with colorectal liver metastasis treated with hepatectomy after propensity score analysis in a ratio of 1:3. Results: Twenty-one patients received hepatectomy for breast cancer. After propensity score matching, 63 patients who had hepatectomy for colorectal cancer were selected for comparison. There was no significant difference in immediate or short-term outcomes between the two groups of patients in terms of operative time, blood loss and surgical morbidities. All patients with breast cancer had R0 resection. No hospital death occurred. After hepatectomy, the 1-, 3- and 5-year overall survival rates were 100.0%, 58.9% and 58.9% respectively in patients with breast cancer. The 1-, 3- and 5-year overall survival rates were 95.0%, 57.2% and 39.7% respectively in patients with colorectal cancer (P = 0.572). On multivariate analysis, triple negative status was the only independent poor prognostic factor in breast cancer liver metastasis (OR = 6.411;95% CI: 1.351–30.435;P = 0.019). Conclusions: Hepatectomy is a safe and effective way of treating breast cancer liver metastasis at experienced centers where multidisciplinary adjuvant treatments are available. It can be considered more frequently as part of the multidisciplinary care for this patient population.
