BACKGROUND Vaccine-induced immune thrombotic thrombocytopenia(VITT)is a rare and potentially life-threatening condition after receiving coronavirus disease *** is characterized by symptom onset at 5 to 30 d postvaccination,thrombocytopenia,thrombosis,high D-dimer level,and antiplatelet factor 4(anti-PF4)antibody *** can progress very fast,requiring urgent *** few studies have described its detailed clinical course and imaging *** report a typical VITT case in a patient who underwent regular repeated brain imaging *** SUMMARY A young woman presented with headaches at 7 d after the ChAdOx1 nCoV-19vaccine(AZD1222)*** then showed progressive symptoms of left upper limb *** computed tomography revealed venous infarction at the right parietal lobe with a hyperacute thrombus in the cortical *** hours later,brain magnetic resonance imaging revealed hemorrhage at the same *** resonance venography showed an irregular contour of the right transverse *** examination revealed a high D-dimer level,thrombocytopenia,and a high titer for anti-PF4 *** was treated with anticoagulants,intravenous immunoglobulin,and steroids and analgesic agents were administered for pain *** had a marked improvement on headaches and clumsiness after treatment along with radiological thrombus *** follow-up at the outpatient department,her modified Rankin scale at 90 d was *** Clinicians should be alerted whenever patients present with persistent and progressive headaches or focal motor/sensory deficits postvaccination.

Manipulation of spectral amplitude and phase with plasmonic nano-structures for information storage

Optical storage devices, such as compact disk （CD） and digital versatile disc （DVD）, provide us a platform for cheap and compact information storage media. Nowadays, information we obtain every day keeps increasing, and therefore how to increase the storage capacity becomes an important issue. In this paper, we reported a method for the increase of the capacity of optical storage devices using metallic nano-structures. Metallic nano-structures exhibit strong variations in their reflectance and/or transmittance spectra accompanied with dramatic optical phase modulation due to localized surface plasmon polariton resonances. Two samples were fabricated for the demonstration of storage capacity enhancement through amplitude modulation and phase modulation, respectively. This work is promising for high-density optical storage.

Higher-mode effect on the seismic responses of buildings with viscoelastic dampers

In conventional modal analysis procedures,usually only a few dominant modes are required to describe the dynamic behavior of multi-degrees-of-freedom *** number of modes needed in the dynamic analysis depends on the higher-mode contribution to the structural response,which is called the higher-mode *** modal analysis approach, however,may not be directly applied to the dynamic analysis of viscoelastically damped *** is because the dynamic properties of the viscoelastic dampers depend on their vibration ***,the structural stiffness and damping contributed from those dampers would be different for each *** this study,the higher-mode effect is referred to as the response difference induced by the frequency-dependent property of viscoelastic dampers at higher *** analysis procedures for buildings with viscoelastic dampers distributed proportionally and non-proportionally to the stiffness of the buildings are developed to consider the higher-mode *** studies on shear-type viscoelastically damped building models are conducted to examine the accuracy of the proposed procedures and to investigate the significance of the higher-mode effect on their seismic *** damper models are used to estimate the peak damper forces in the proposed procedures. Study results reveal that the higher-mode effect is significant for long-period viscoelastically damped *** higher-mode effect on base shear is less significant than on story acceleration *** difference of the seismic response usually occurs at the top ***,the higher-mode effect may not be reduced by decreasing the damping ratio provided by the viscoelastic *** practical application,it is realized that the linear viscous damping model without considering the higher-mode effect may predict larger damper forces and hence,is on the conservative side.

A novel multisignature scheme for a special verifier group against clerk and rogue-key attacks

The digital signature is a very important subject for network *** multiple signers and multiple verifiers,Xie and Yu(2004) pointed out that the multisignature scheme of Laih and Yen(1996) is vulnerable to a harmful *** attack can occur when a specified group of verifiers cooperate to forge a multisignature by secret key substitution following the leak of a secret key or by group public key adjustment during the process of renewing *** and Yu proposed an improvement of Laih and Yen's multisignature *** this paper,we show that Xie and Yu's scheme is vulnerable to clerk and rogue-key *** propose an improved multisignature scheme to resist such *** the proposed scheme,multiple signers can generate a multisignature for the message with the signers' secret keys,and the specified group of verifiers can cooperate to verify the validity of the multisignature with the signers' public keys and the verifiers' secret *** proposed scheme for a special verifier group not only has the advantages of Xie and Yu's scheme,but also is secure against clerk and rogue-key attacks.

Clinical and pathological correlation of the microdeletion of Y chromosome for the 30 patients with azoospermia and severe oligoasthenospermia

Aim: To review the accumulated 30 patients with different area of Y chromosome microdeletions, focusing on their correlation with the clinical and pathological findings. Methods: A total of 334 consecutive infertile men with azoospermia (218 patients) and severe oligoasthenospermia (116 patients) were screened. Complete physical and endocrinological examinations, general chromosome study and multiplex polymerase chain reaction assay to evaluate the Y chromosome microdeletion were performed. Ten patients received testicular biopsy. Then the clinical and pathological findings were analyzed with reference to the areas of Y chromosome microdeletion. Results: There is a decline of the percentage of sperm appearing in semen in the group that the gene deletion region from AZFc to AZFb. The clinical evidence of the impairment (decreased testicular size and elevated serum FSH) is also relevantly aggravated in this group. However, the pathology of testicular biopsy specimen was poorly correlated with the different deletion areas of the Y chromosome, which may be due to the limited number of specimens. Conclusion: The clinical correlation of spermatogenic impairment to the different AZF deletion regions may provide the information for the infertile couples in pre-treatment counseling.

This study presents a numerical solution to the three-dimensional solute transport in heterogeneous media by using a layer-integrated *** vertical spatial variation of soil and hydraulic properties within each layer,a threedimensional solute transport can be simplified as a quasi-three-dimensional solute transport which couples a horizontal two-dimensional simulation and a vertical onedimensional *** finite analytic numericalmethod was used to discretize the derived two-dimensional governing equation.A quadratic function was used to approximate the vertical one-dimensional concentration distribution in the layer to ensure the continuity of concentration and flux at the interface between the adjacent *** integration over each layer,a set of system of equations can be generated for a single column of vertical cells and solved numerically to give the vertical solute concentration *** solute concentration field was then obtained by solving all columns of vertical cells to achieve convergence with the iterative solution *** proposed model was verified through examples from the published literatures including four verifications in terms of analytical and experimental *** of simulation results indicates that the proposed model satisfies the solute concentration profiles obtained from experiments in time and space.

Favorable clinical outcome of nonalcoholic liver cirrhosis patients with coronary artery disease:A population-based study

AIM To elucidate the prevalence and risk of mortality of nonalcoholic liver cirrhosis(LC) patients with coronary artery disease(CAD).METHODS The study cohort included newly diagnosed nonalcoholic LC patients age ≥ 40 years old without a diagnosis of CAD from 2006 until 2011 from a longitudinal health insurance database. The mean follow-up period for the study cohort was 1152 ± 633 d. The control cohort was matched by sex, age, residence, and index date. Hazard ratios(HRs) were calculated using the Cox proportional hazard model and the Kaplan-Meier method. RESULTS After exclusion, a total of 3409 newly diagnosed nonalcoholic cirrhotic patients were identified from one million samples from the health insurance database. We found that CAD(5.1% vs 17.4%) and hyperlipidemia(20.6% vs 24.1%) were less prevalent in nonalcoholic LC patients than in normal subjects(all P < 0.001), whereas other comorbidities exhibited an increased prevalence. Among the comorbidities, chronic kidney disease exhibited the highest risk for mortality(adjusted HR(AHR) = 1.76; 95%CI: 1.55-2.00, P < 0.001). Ascites or peritonitis exhibited the highest risk of mortality among nonalcoholic cirrhotic patients(AHR = 2.34; 95%CI: 2.06-2.65, P < 0.001). Finally, a total of 170 patients developed CAD after a diagnosis of nonalcoholic LC. The AHR of CAD in nonalcoholic LC patients was 0.56(95%CI: 0.43-0.74, P < 0.001). The six-year survival rates for nonalcoholic LC patients with and without CAD were 52% and 50%, respectively(P = 0.012). CONCLUSION We conclude that CAD was less prevalent and associated with a reduced risk of mortality in nonalcoholic cirrhotic patients.

Terminal ileum gangrene secondary to a type Ⅳ paraesophageal hernia

Type Ⅳ paraesophageal hernia(PEH) is very rare, and is characterized by the intrathoracic herniation of the abdominal viscera other than the stomach into the chest. We describe a 78-year-old woman who presented at our emergency department because of epigastric pain that she had experienced over the past 24 h. On the day after admission, her pain became severe and was accompanied by right chest pain and dyspnea. Chest radiography revealed an intrathoracic intestinal gas bubble occupying the right lower lung field. Emergency explorative laparotomy identified a type Ⅳ PEH with herniation of only the terminal ileum through a hiatal defect into the right thoracic cavity. In this report, we also present a review of similar cases in the literature published between 1980 and 2015 in Pub Med. There were four published cases of small bowel herniation into the thoracic cavity during this period. Our patient represents a rare case of an individual diagnosed with type Ⅳ PEH with incarceration of only the terminal ileum.

p73β inhibits transcriptional activities of enhancer I and X promoter in hepatitis B virus more efficiently than p73α

AIM: p73, as a novel member of a family of p53-related transcription factors, shares redundant functions with p53, such as the abilities of inducing apoptosis and suppressing growth. It is well known that p53 can repress HBV expression and transcription efficiently. The aim of this paper is to investigate the transcriptional effect of p73α and p73β on hepatitis B virus (HBV) and to understand the correlation between HBV and ***: To construct an x-gene inactivated HBV plasmid which was cotransfected with p73α or p73β expression vectors into HepG2 cells. After transiently transfecticn, HBV surface antigen (HBsAg) and HBV e antigen (HBeAg) were detected by ELISA. Viral transcripts synthesized by HBV were evaluated by Northern blotting analysis. The activities of HBV regulatory elements, including enhancer Ⅰ/X promoter (ENI/Xp) and enhancer Ⅱ/core promoter (ENⅡ/Cp) were monitored by luciferase ***: Both p73α and p73β could repress HBsAg and HBeAg expression by downregulating the ENⅠ/Xp and ENⅡ/Cp activities. But p73β exerted stronger inhibition on the activity of ENI/Xp than p73α, resulting in much lower level of viral transcripts and the antigens ***: p73β as a novel member of p53 family can efficiently inhibit HBV transcription mainly through downregulating the activities of the HBV ENI/Xp regulatory elements.
