Evaluation of the Effectiveness of Community Pharmacy Services on Rational Medication Use in Elderly Patients with Hypertension and Diabetes

Objective:To explore the effect of community pharmacy services on rational medication use in elderly patients with hypertension and diabetes.Methods:Between November 2022 and December 2023,80 elderly patients with hypertension and diabetes were selected and randomly divided into a control group(routine medication guidance)and an observation group(community pharmacy services),with 40 subjects each.The medication effect scores,blood pressure,blood sugar levels,and quality of life scores before and after intervention were compared between the two groups.Results:Comparison of pharmaceutical knowledge,medication compliance,and safe medication behavior scores showed that the observation group had higher scores as compared to the control group(P<0.05);blood pressure(systolic blood pressure,diastolic blood pressure,heart rate)and blood sugar(fasting blood glucose,2 h postprandial blood glucose,glycated hemoglobin)index levels were compared,and the observation group’s index levels were lower than those of the control group(P<0.05);the scores of physical health,mental health,social relationships,and environment in the observation group were higher than those in the control group(P<0.05).Conclusion:Community pharmacy services improved the rational medication effect of elderly patients with hypertension and diabetes,and improved their blood pressure,blood sugar control levels,and quality of life.

Analysis of The Correlation Between inhA Gene Mutation and Resistance to Protionamide in Drug-Resistant Mycobacterium Tuberculosis

Objective: To investigate the characteristics of katG and inhA gene mutations in multidrug-resistant tuberculosis (MDR-TB), pre-extensively drug-resistant tuberculosis (preXDR-TB), and their correlation with resistance to protionamide (Pto). Methods: A total of 229 patients with MDR-TB and pre-XDR-TB diagnosed in the Eighth Affiliated Hospital of Xinjiang Medical University from January 2020 to February 2024 were selected to analyze the characteristics of katG and inhA mutations in MTB clinical isolates and their correlation with Pto resistance. Results: The mutation rate of katG (with or without inhA mutation) was 85.2%. The mutation rates in MDR-TB and pre-XDR-TB were 87.4% (125/143) and 81.4% (70/86), respectively. The mutation rate of inhA (including katG mutation) was 14.8% (34/229), which was 12.6% (18/143) and 18.6% (16/86) in MDR-TB and pre-XDR-MTB, respectively. There was no difference in mutation (P > 0.05). Conclusion: The total resistance rate to Pto in 229 strains was 8.7% (20/229), which was 8.4% (12/143) and 9.3% (8/86) in MDR-TB and pre-XDR-TB, respectively. Among the inhA mutant strains, 13 were resistant to the Pto phenotype, and the resistance rate was 65% (13/20). In MDR-TB and pre-XDR-TB strains resistant to Pto, inhA gene mutations occurred in 66.7% (6/9) and 63.6% (7/11), respectively. The resistance rates of MDR-MTB and pre-XDR-TB strains without inhA gene mutation to Pto were 2.4% (3/125) and 5.7% (4/70), respectively.

A structural variation genotyping algorithm enhanced by CNV quantitative transfer

Genotyping of structural variations considering copy number variations(CNVs)is an infancy and challenging problem.CNVs,a prevalent form of critical genetic variations that cause abnormal copy numbers of large genomic regions in cells,often affect transcription and contribute to a variety of diseases.The characteristics of CNVs often lead to the ambiguity and confusion of existing genotyping features and algorithms,which may cause heterozygous variations to be erroneously genotyped as homozygous variations and seriously affect the accuracy of downstream analysis.As the allelic copy number increases,the error rate of genotyping increases sharply.Some instances with different copy numbers play an auxiliary role in the genotyping classification problem,but some will seriously interfere with the accuracy of the model.Motivated by these,we propose a transfer learning-based method to genotype structural variations accurately considering CNVs.The method first divides the instances with different allelic copy numbers and trains the basic machine learning framework with different genotype datasets.It maximizes the weights of the instances that contribute to classification and minimizes the weights of the instances that hinder correct genotyping.By adjusting the weights of the instances with different allelic copy numbers,the contribution of all the instances to genotyping can be maximized,and the genotyping errors of heterozygote variations caused by CNVs can be minimized.We applied the proposed method to both the simulated and real datasets,and compared it to some popular algorithms including GATK,Facets and Gindel.The experimental results demonstrate that the proposed method outperforms the others in terms of accuracy,stability and efficiency.The source codes have been uploaded at github/TrinaZ/CNVtransfer for academic use only.

Revealing the Structure-Property Difference of Natural Rubber Prepared by Different Methods: Protein and Gel Content are Key Factors

Natural rubber (NR) is widely used in various fields including aerospace, military industry and transportation due to its superior elasticity and comprehensive mechanical properties. Nonetheless, the commercial NR prepared by different methods usually exhibits different mechanical properties, primarily due to variations in processing conditions during the conversion from latex to bulk rubber material. Consequently, this poses challenges in scientific research and industrial production of NR. In order to assess the properties of various commercially available NR and identify key structural and compositional components, this study systematically compares and analyzes four representative NR raw materials: air dried sheet (ADS), ribbed smoked sheets (RSS), constant viscidity NR (CV), and whole field latex rubber (WF). The investigation focuses on evaluating their static mechanical behavior, SIC behavior, wear resistance, and fatigue resistance. The findings indicate that protein and gel content exhibit a crucial influence on the NR properties. These constituents contribute to the formation of a high-crosslinking density region, generating a heterogeneous network structure within the rubber. This structure amplifies strains during deformation, leading to earlier and stronger strain-induced crystallization (SIC). Among the four commercial NR brands, RSS demonstrates superior overall mechanical and dynamic properties owing to its high protein and gel content. This study serves as a valuable reference for comprehending the differences in properties among various commercial NR, thereby offering guidance for the actual processing and selection of NR.

An Empirical Study on Google Research Football Multi-agent Scenarios

Few multi-agent reinforcement learning (MARL) researches on Google research football (GRF) focus on the 11-vs-11 multi-agent full-game scenario and to the best of our knowledge, no open benchmark on this scenario has been released to the public. In this work, we fill the gap by providing a population-based MARL training pipeline and hyperparameter settings on multi-agent football scenario that outperforms the bot with difficulty 1.0 from scratch within 2 million steps. Our experiments serve as a reference for the expected performance of independent proximal policy optimization (IPPO), a state-of-the-art multi-agent reinforcement learning algorithm where each agent tries to maximize its own policy independently across various training configurations. Meanwhile, we release our training framework Light-MALib which extends the MALib codebase by distributed and asynchronous implementation with additional analytical tools for football games. Finally, we provide guidance for building strong football AI with population-based training and release diverse pretrained policies for benchmarking. The goal is to provide the community with a head start for whoever experiment their works on GRF and a simple-to-use population-based training framework for further improving their agents through self-play. Dear editor,This letter presents a practical industrial process identification scheme.More specifically,to improve the identification accuracy of practical process,a decoupled identification scheme is developed based on neural fuzzy network and autoregressive exogenous(ARX)model,which is based on multi-signal sources.The multiple signal sources include binary signals and random signals.Experimental results of pH neutralization process show that developed identification scheme can provide accurate identification accuracy.

Influence of blood glucose fluctuations on chemotherapy efficacy and safety in type 2 diabetes mellitus patients complicated with lung carcinoma

BACKGROUND Patients with type 2 diabetes mellitus(T2DM)have large fluctuations in blood glucose(BG),abnormal metabolic function and low immunity to varying degrees,which increases the risk of malignant tumor diseases and affects the efficacy of tumor chemotherapy.Controlling hyperglycemia may have important therapeutic implications for cancer patients.AIM To clarify the influence of BG fluctuations on chemotherapy efficacy and safety in T2DM patients complicated with lung carcinoma(LC).METHODS The clinical data of 60 T2DM+LC patients who presented to the First Affiliated Hospital of Ningbo University between January 2019 and January 2021 were retrospectively analyzed.All patients underwent chemotherapy and were grouped as a control group(CG;normal BG fluctuation with a mean fluctuation<3.9 mmol/L)and an observation group(OG;high BG fluctuation with a mean fluctuation≥3.9 mmol/L)based on their BG fluctuations,with 30 cases each.BGrelated indices,tumor markers,serum inflammatory cytokines and adverse reactions were comparatively analyzed.Pearson correlation analysis was performed to analyze the correlation between BG fluctuations and tumor markers.RESULTS The fasting blood glucose and 2-hour postprandial blood glucose levels in the OG were notably elevated compared with those in the CG,together with markedly higher mean amplitude of glycemic excursions(MAGE),mean of daily differences,largest amplitude of glycemic excursions and standard deviation of blood glucose(P<0.05).In addition,the OG exhibited evidently higher levels of carbohydrate antigen 19-9,carbohydrate antigen 125,carcinoembryonic antigen,neuron-specific enolase,cytokeratin 19,tumor necrosis factor-α,interleukin-6,and highsensitivity C-reactive protein than the CG(P<0.05).Pearson analysis revealed a positive association of MAGE with serum tumor markers.The incidence of adverse reactions was significantly higher in the OG than in the CG(P<0.05).CONCLUSION The greater the BG fluctuation in LC patients after chemotherapy,the more unfavorable the therapeuti

High performance strain sensor based on leather activated by micro-cracking conductive layer

Flexible strain sensors are capable to detect external force induced strain change owing to their unique ability to convert deformation into electrical signals.Generally,micro/nano patterning of conductive layer in strain sensor is an effective method to improve its sensitivity,however the sophisticated manipulation process is limited only in laboratory scale.In this report,a simple and scalable fabrication strategy was used to create micro-cracking conductive layer as an alternative patterning method to achieve high performance of strain sensor.In details,the sensor was fabricated using leather as the substrate to filtrated acidified multi-walled carbon nanotubes(a-MWCNTs)/layered double hydroxides(LDHs)suspension.During stretching process,micro-cracking structure emerged on the percolated a-MWCNTs/LDHs layer,causing a rise up of resistance according to increasing strain and generated a detectable electrical signal.The prepared sensor had a large detecting range(60%),high sensitivity(GF of 7238.92 at strain 30-60%),fast response(tensile response time of 270 ms),good stability and repeatability.The sensor also inherited the advantages of leather,such as biodegradability and good air permeability,and the introduction of a-MWCNTs/LDHs further enhanced its fire retardancy properties.These features ensured the sensor as an eco-friendly,comfortable and safe electronic device for human motion detection.

Precisely manipulation of core composition of core-shell-type cobalt polyoxoniobates and proton conduction study

The development of core-shell nanoclusters with controllable composition is of utmost importance as the material properties depend on their constituent elements.However,precisely tuning their compositions at the atomic scale is not easily achieved because of the difficulty of using limited macroscopic synthetic methods for atomic-level modulation.In this work,we report an interesting example of precisely regulating the core composition of an inorganic core-shell-type cobalt polyoxoniobate[Co_(26)Nb_(36)O_(140)]^(32−)by controlling reaction conditions,in which the inner Co-core composition could be tune while retaining the outer Nb-shell composition of resulting product,leading to a series of isostructural species with a general formula of{Co_(26-n)Nb_(36+n)O_(140)}(n=0–2).These rare species not only can display good powder and single-crystal proton conductivities,but also might provide helpful and atomic-level insights into the syntheses,structures and composition modifications of inorganic amorphous core-shell heterometal oxide nanoparticles.
