Adult to adult living donor liver transplantation (AALDLT) was first preformed in the United States in 1997. The procedure was rapidly integrated into clinical practice, but in 2002, possibly due to the first widely publicized donor death, the number of living liver donors plummeted. The number of donors has since reached a steady plateau far below its initial peak. In this review we evaluate the current climate of AALDLT. Specifically, we focus on several issues key to the success of AALDLT: determining the optimal indications for AALDLT, balancing graft size and donor safety, assuring adequate outflow, minimizing biliary complications, and maintaining ethical practices. We conclude by offering suggestions for the future of AALDLT in United States transplantation centers.

This paper presents a comparison among different hydrodynamic models for the analysis of the unsteady loads delivered by a marine propeller working in an axial, non-uniform inflow. Specifically, for a propeller subjected to a wake-field dominated by local high-frequency changes in space, the unsteady hydroloads predicted by the Nakatake formulation are compared with those given by the Theodorsen and Sears theories, respectively. Drawbacks and potentialities of these approaches are highlighted to assess a computationally efficient hydrodynamic solver for the analysis of operating conditions where propeller blades are significantly perturbed by a multi-harmonic onset-flow. Guidelines coming from this investigation may drive the choice of a fast and reliable unsteady propeller modeling that represents a good trade-off between accuracy of simulation and cost of computation within implementation in Computational Fluid Dynamics （CFD） solvers. The hydrodynamic formulations herein proposed are validated through numerical comparisons with the （accurate but computationally expensive） propeller loads predicted by a fully 3-D panel-method Boundary Element Method （BEM） solver, suited for the analysis of propellers operating in a complex hydrodynamic environment.

A Boundary Element Method （BEM） hydrodynamics combined with a flow-alignment technique to evaluate blades shed vorticity is presented and applied to a marine propeller in open water. Potentialities and drawbacks of this approach in capturing propeller performance, slipstream velocities, blade pressure distribution and pressure disturbance in the flow-field are highlighted by comparisons with available experiments and RANSE results. In particular, correlations between the shape of the convected vortex- sheet and the accuracy of BEM results are discussed throughout the paper. To this aim, the analysis of propeller thrust and torque is the starting point towards a detailed discussion on the capability of a 3-D free-wake BEM hydrodynamic approach to describe the local features of the flow-field behind the propeller disk, in view of applications to propulsive configurations where the shed wake plays a dominant role.

The high rate of sustained viral response(SVR)to boceprevir or telaprevir-based triple therapy in hepatitis C(HCV)-related,non-cirrhotic na ve patients or relapsers to previous antiviral treatment leads clinicians to believe that the impact of metabolic host factors on SVR is minimal when triple therapy is used,unlike what is observed with the peginterferon and ribavirin *** concept is strongly expressed by some opinion leaders on the basis of the data derived from subanalyses of registrative trials as well as from a post-hoc analysis of the phaseⅡC208 clinical *** perception of unrestrainable therapeutic success with the use of newer,more powerful antivirals is now reinforced by the brilliant results obtained with sofosbuvir,an HCV NS5B polymerase inhibitor,as well as by the data from the phaseⅡandⅢstudies on the various combinations of second-generation NS3/4A inhibitors and NS5A and/or NS5B ***,a great deal of concern has emerged from the real world scenario in which patients are often older and have more comorbidities than patients in the"world of trials".Furthermore,many of them have advanced fibrosis and previous failure with peginterferon and ribavirin *** data from the recent literature suggest that the host metabolic factors may play a minor but non-negligible role in these difficult-to-treat patients,an issue that will hopefully be investigated in further *** editorial aims to provide a detailed analysis of the role that host metabolic factors played in the past and what role they may play in the era of direct antiviral agents.

Thank you for inviting us to comment the review article entitled"Use of robotics in liver donor right hepatectomy"by Chen PD et al.(1),published on HBSN *** their interesting paper,the authors stated that despite the fact that minimally invasive surgery(MIS)has been documented with safety and feasibility in complex liver surgery the progress has been slow in regard to the application of MIS in donor graft *** surgeons have devoted themselves to develop pure MIS approach for donor hepatectomy,but the steep learning curve remains barrier for its wide *** authors suggest that the introduction or robotic assisted surgery may contribute to a wider acceptance of MIS as the technique of choice for live donor *** a great attention to donor selection and continuous improvement in donor care the risks of donor hepatectomy remain real and the complication rate not insignificant(2).Traditional open liver resection results in post-operative pain and in at least 30%of the cases these pains were attributed to abdominal wall trauma(1,2).For these reasons and with the aim of facilitating donor recovery,transplant surgeons have been exploring the possibility of routinely applying MIS to live donor *** bridge the gap between open and pure MIS donor procurement,hand-assisted laparoscopic surgery was initially introduced in the 90'.It was not until 2002 that Soubrane O presented the first two cases of pure laparoscopic left lateral living donor hepatectomy,followed in the years to come by several successful attempts to extend the procedure to the procurements of full left and right hepatic lobes(1-6).

We report on a 74-year-old patient with recurrent cholangitis and a large juxtapapillary duodenal diverticulum. Despite drainage of the common bile duct by an endoscopically placed stent, the elevated liver enzymes normalized only partially. To rule out other possible causes of liver injury, a percutaneous liver biopsy was done. After the liver biopsy the patient developed fulminant septic shock and died within 24 h. We discuss the possible causes of the septic shock following percutaneous liver biopsy in our patient and give a concise overview of the literature.

Post hepatectomy liver failure(PHLF)remains the most dreaded complication in major *** future remnant liver(FRL)plays a pivotal role in prevention of ***-operative portal vein embolization(PVE)has become standard of care for increasing the FRL in preparation for major *** liver partition and portal vein ligation for staged hepatectomy(ALPPS)has also been used,though has demonstrated a substantial risk of morbidity and ***,there are many situations in which PVE achieves an inadequate extent of hypertrophy,potential increasing the risk of *** and colleagues explore their data with a prospective review of preoperative PVE versus liver venous deprivation(LVD)regarding intra and post-operative complications,as well as,histologic findings(1).This study adds to a growing body of literature assessing the benefit of LVD over PVE and ALPPS both,in regards to increased rate of hypertrophy,improvement in FRL,in the face of similar morbidity/mortality rates compared to PVE.

Angiogenesis is a word that refers to new blood vessel formation,and this process is of fundamental importance for physiological development and tissue homeostasis,as well as the genesis of several diseases,including ***,studies carried out in the last years have shown that angiogenesis is essential for the growth of many solid *** is also important for the growth of many hematological malignancies,including acute myeloid leukemia(AML).Endothelial cells are essential constituents of the bone marrow vascular niches,structures essential for the survival and maintenance of normal hematopoietic stem/progenitor *** marrow endothelial cells play an essential role in leukemia development and there is growing evidence that a targeting of both leukemic and endothelial cells of the leukemic vascular niche may improve the efficacy of antileukemic *** marrow angiogenesis is frequently increased in AML,is morphologically evidenced as increased microvascular density,and is typically associated with some AML *** molecular mechanisms underlying the increased angiogenesis in some AML subtypes have been *** conclusion,a better understanding of angiogenesis as well as the fundamental interactions between bone marrow endothelial cells and leukemic stem cells may contribute to improve antileukemia treatments.
