bACKGROUND Cronkhite-Canada syndrome(CCS)is a rare,noninherited disease characterized by gastrointestinal polyposis with diarrhea and ectodermal abnormalities.CCS polyps are distributed through the whole digestive tract,and they are common in the stomach and colon but very uncommon in the esophagus.CASE SUMMARY Here,we present a case of a 63-year-old man with skin hyperpigmentation accompanied by diarrhea,alopecia,and loss of his fingernails.Laboratory data indicated anemia,hypoalbuminemia,hypocalcemia,hypokalemia,and positive fecal occult blood.Endoscopy showed numerous polyps scattered throughout the digestive tract,including the esophagus.He was treated with nutritional support and glucocorticoids with remission of his symptoms.CONCLUSION Comprehensive treatment led by hormonal therapy can result in partial or full remission of clinical symptoms.Treatment should be individualized for each patient according to their therapy response.Surveillance endoscopy is necessary for assessing mucosal disease activity and detecting malignant transformation.

This paper is devoted to the solvability of Markovian quadratic backward stochastic differential equations(bSDEs for short) with bounded terminal conditions. The generator is allowed to have an unbounded sub-quadratic growth in the second unknown variable z. The existence and uniqueness results are given to these bSDEs. As an application, an existence result is given to a system of coupled forward-backward stochastic differential equations with measurable coefficients.

In situ measurement of multiple pollutants coexisting in sediment porewater is an essential step in comprehensively assessing the bioavailability and risk of pollutants,but to date,this needs to be better developed.In this study,a passive sampler,consisting of an“I-shaped”supporting frame and inorganic/organic sampling units,incorporating equilibrium dialysis theory and kinetic/equilibrium sorption principle,was developed for the synchronous measurement of inorganic(e.g.,phosphorus and metal(loid)s)and organic pollutants(e.g.,parent and substituted PAHs).The equilibrium time and sampling rates were explored in laboratory tests to support in situ application.Profiles of pollutants in porewater within a vertical resolution of centimeters,i.e.,1 cm and 2 cm for inorganic and organic pollutants,respectively,were obtained by field deployment of the sampler for further estimation of diffusive fluxes across the sediment-water interface.The results suggested that the role of sediments for a specific pollutant may change(e.g.,from“sink”to“source”)during the sampling time.This study demonstrated the feasibility of synchronous measurement of inorganic and organic pollutants in sediment porewater by the passive sampler.In addition,it provided new insight for further investigation into the combined pollution effects of various pollutants in sediments.

According to the report from the Chinese Center for Disease Control and Prevention,the prevalence of human immunodeficiency virus(HIV)infection exceeded 1.2 million individuals by the year 2022,with an annual increase of about 80000 cases.The overall prevalence of hepatitis b surface antigen among individuals co-infected with HIV reached 13.7%,almost twice the rate of the general population in China.In addition to the well-documented susceptibility to opportunistic infections and new malignancies,HIV infected patients frequently experience liver-related organ damage,with the liver and kidneys being the most commonly affected.This often leads to the development of end-stage liver and kidney diseases.Therefore,organ transplantation has emerged as an important part of active treatment for HIV infected patients.However,the curative effect is not satisfactory.HIV infection has been considered a contraindication for organ transplantation.Until the emergence of highly active anti-retroviral therapy in 1996,the once intractable replication of retrovirus was effectively inhibited.With prolonged survival,the failure of important organs has become the main cause of death among HIV patients.Therefore,transplant centers worldwide have resu-med exploration of organ transplantation for HIV-infected individuals and reached a positive conclusion.This study provides an overview of the current landscape of HIV-positive patients receiving liver transplantation(LT)in main-land China.To date,our transplant center has conducted LT for eight end-stage liver disease patients co-infected with HIV,and all but one,who died two months postoperatively due to sepsis and progressive multi-organ failure,have survived.Comparative analysis with hepatitis b virus-infected patients during the same period revealed no statistically significant differences in acute rejection reactions,cytomegalovirus infection,bacteremia,pulmonary infections,acute kidney injury,new-onset cancers,or vascular and biliary complications.

Alzheimer's disease is a neurodegenerative disease with complex etiology.Gut microbiota influences the gutbrain axis,which may affect pathways related to the pathogenesis of Alzheimer's disease.Additionally,diet and physical activity are likely to affect the pathology of Alzheimer's disease as well as the gut microbiota.This demonstrates that it may be possible to prevent or halt the progression of Alzheimer's disease by regulating the gut microbiota using diet and physical activity strategies.Therefore,the present study reviews the association between these two interventions and gut microbiota in the human body.It also summarizes how these two interventions benefit Alzheimer's disease.Furthermore,the primary limitations of these two interventions are discussed and promising strategies are proposed,which may be beneficial to further study and develop the intervening measure for the progression of Alzheimer's disease.

The stability of the microstructure and mechanical properties of the pre-hardened sheets during the pre-hardening forming(PHF)process directly determines the quality of the formed components.The microstructure stability of the pre-hardened sheets was in-vestigated by differential scanning calorimetry(DSC),transmission electron microscopy(TEM),and small angle X-ray scattering(SAXS),while the mechanical properties and formability were analyzed through uniaxial tensile tests and formability tests.The results in-dicate that the mechanical properties of the pre-hardened alloys exhibited negligible changes after experiencing 1-month natural aging(NA).The deviations of ultimate tensile strength(UTS),yield strength(YS),and sheet formability(Erichsen value)are all less than 2%.Also,after different NA time(from 48 h to 1 month)is applied to alloys before pre-hardening treatment,the pre-hardened alloys possess stable microstructure and mechanical properties as well.Interestingly,with the extension of NA time before pre-hardening treatment from 48 h to 1 month,the contribution of NA to the pre-hardening treatment is limited.Only a yield strength increment of 20 MPa is achieved,with no loss in elongation.The limited enhancement is mainly attributed to the fact that only a limited number of clusters are transformed into Guinier-Preston(GP)zones at the early stage of pre-hardening treatment,and the formation ofθ''phase inhibits the nucleation and growth of GP zones as the precipitated phase evolves.

In situ direct reprogramming technology can directly convert endogenous glial cells into functional neurons in vivo for central nervous system repair. Polypyrimidine tract-binding protein 1(PTb) knockdown has been shown to reprogram astrocytes to functional neurons in situ. In this study, we used AAV-PHP.e b-GFAP-sh PTb to knockdown PTb in a mouse model of ischemic stroke induced by endothelin-1, and investigated the effects of GFAP-sh PTb-mediated direct reprogramming to neurons. Our results showed that in the mouse model of ischemic stroke, PTb knockdown effectively reprogrammed GFAP-positive cells to neurons in ischemic foci, restored neural tissue structure, reduced inflammatory response, and improved behavioral function. These findings validate the effectiveness of in situ transdifferentiation of astrocytes, and suggest that the approach may be a promising strategy for stroke treatment.

Efficacy and safety of Chinese herbal medicine as adjunctive therapy in sepsis patients with bloodstream infection:a propensity-matched analysis

ObJECTIVE:To investigate the efficacy and safety of Chinese herbal medicine in treating sepsis patients with bloodstream infection.METHODS:A 6-year retrospective study was carried out at a university hospital in China.Adult sepsis patients with bloodstream infection were included.The primary outcome was 28-day mortality after admission.Propensity score method was used to adjust for possible confounding.28-day mortality was estimated by KaplanMeier analysis and compared using the log-rank test.Cox regression analysis was carried out to identify factors impacting in-hospital mortality outcomes.RESULTS:Following the application of the propensity score method,a total of 176 patients were included.The all-cause 28-day mortality in the control group and Chinese herbal medicine group was 21.6%and 14.8%,respectively.Kaplan-Meier survival analysis showed that Chinese herbal medicine was associated with a lower hazard ratio(HR)in all-cause 28-day death compared with the control group[HR=0.44,95%CI(0.22,0.90),Pbetween the two groups(P>0.05).blood-activating and stasis-eliminating herb administration was associated with reduced inhospital mortality among sepsis patients with bloodstream infection[HR=0.54,95%CI(0.34,0.94),Pbal medicine,especially the blood-activating and stasis-eliminating herb,might have certain efficacy and safety in treating sepsis patients with bloodstream infection.Clinicians should prescribe blood-activating and stasis-eliminating herb in treating these two coalescent critical diseases as long as no contraindications exist.However,further studies are needed to validate our results.

Constructions of optimal binary locally repairable codes via intersection subspaces

Locally repairable codes（LRCs）, which can recover any symbol of a codeword by reading only a small number of other symbols, have been widely used in real-world distributed storage systems, such as Microsoft Azure Storage and Ceph Storage Cluster. Since binary linear LRCs can significantly reduce coding and decoding complexity, constructions of binary LRCs are of particular interest. The aim of this paper is to construct dimensional optimal binary LRCs with disjoint local repair groups. We introduce a method to connect intersection subspaces with binary LRCs and construct dimensional optimal binary linear LRCs with locality 2b（b 3） and minimum distance d 6 by employing intersection subspaces deduced from the direct sum. This method will sufficiently increase the number of possible repair groups of dimensional optimal LRCs, thus efficiently expanding the range of the construction parameters while keeping the largest code rates compared with all known binary linear LRCs with minimum distance d 6 and locality 2b.
