two-proton relative momentum distributions from the break-up channels 23Al→p+p+21Na and 22Mg→p+p+20Ne at an energy of 60-70 A MeV have been measured together with two-proton opening angles at the projectile fragment separator beamline (RIPS) in the RIKEN Ring Cyclotron Facility. the results demonstrate the existence of diproton emission component from single-step 2He for highly excited 23Al and 22Mg.

the title compound 4’,9’,4”,9”-tetra-tert-butyl-1’,6’,1”,6”-tetramethoxy-2,5-dithia[3.3] metabiphenylophane was synthesized as a calixarene like large-sized metacyclophane from the corresponding 3,3’-bis(chloromethyl)-5,5’-bis(1,1-dimethylethyl)-2,2’-dimethoxy-1,1’-biphenyl and 3,3’-bis (mercaptomethyl)-5,5’-bis(1,1-dimethylethyl)-2,2’-dimethoxy-1,1’-biphenyl via an intermolecular condensation reaction. the compound was characterized using 1H NMR spectroscopy and mass spectrometry. Its exact conformational structure was determined via single crystal X-ray diffraction analysis. the compound has quite strained 1,2-alternate like structure because of its two high dihedral angled and rigid biphenyl parts in the crystal. the crystal includes the two isomers as two different 1,2-alternate like conformers, and the ratio of the two isomers is 78:22.

Background: Isolated left venticular noncompaction(IVNC) is a rare congenital heart disease charactrized by a pattern of an excessively prominent trabecular meshwork with deep intertrabecular recesses. Heart rate variability(HRV) has been reported to be impaired in various heart diseases, though little is known regarding HRV in adult patients with IVNC. Methods: We measured spectral components of HRV using fast Fourier transformation of 24h Holter recordings in 10 adult patients with IVNC, 40 patients with myocardial infarction(MI), 40 patients with hypertrophic cardiomyopathy(HCM) and 40 healthy controls. Results: the low frequency component and the high frequency component of HRV were lower in IVNC patients tahn those in controls(265±213 ms2 vs. 469±195 ms2, pthermore, 3 IVNC patients with a previous history of heart failure exhibited more decreased HRV(low frequency, 75±56 ms2; high frequency, 39±18 ms2). Contrary, the ratio of low frequency to high frequency component was higher in patients with IVNC than controls(3.5±0.5 vs. 3.2±0.3, pthe degree of impaired HRV was severest in MI patients, intermediate in IVNC patients and mildest in HCM patients compared with controls. Conclusions: HRV is impaired in adult patients with IVCN, especially in patients with a previous history of heart failure, suggesting vagal withdrawal or sympathetic enhancement. HRV in IVNC adults is less impaired than in MI patients, and more impaired than in HCM patients of our cohort.

Omeprazole is mainly metabolized in the liver by CYP2C19, a genetically determined enzyme, while rabeprazole is mainly nonenzymatically degraded with a minor involvement by CYP2C19. We investigated the gastric ulcer healing effect of omeprazole versus rabeprazole evaluated endoscopically with reference to the different CYP2C19 genotypes. Eighty patients with active gastric ulcer were treated with a daily dose of 20 mg of omeprazole or 10 mg of rabeprazole. the endoscopic evaluation was performed at the baseline and 2- and 8- week posttreatment periods. the endoscopic improvement of gastric ulcer size and ulcer healing rates using a thin rubber disc with a diameter of 6 mm, were evaluated in relation to the CYP2C19 genotypic status. the mean 2- week posttreatment ulcer size value by rabeprazole did not significantly differ among the different CYP2C19 genotypes, whereas the mean value in the homozygous extensive metabolizer patients treated with omeprazole was significantly (P = 0.0057) greater than in those with rabeprazole. However, after the 8- week treatment, omeprazole and rabeprazole showed the similarly high healing rates of 87.8% (31/37) and 88.9% (32/36), respectively. Although both omeprazole and rabeprazole showed a high healing rate of gastric ulcer after the 8- week treatment period, the healing effect of rabeprazole appears to be relatively independent of the CYP2C19 status, resulting in an earlier repair of gastric mucosal damage evaluated endoscopically compared to that of omeprazole.

PURPOSE: to report a case of human t-cell leukemia virus type1 (HtLV-1)-associated adult t-cell leukemia/lymphoma (AtLL) whose initial sign was exophthalmos. DESIGN: Case report. MEtHODS: A 64-year-old Japanese man presented with exophthalmos and choroidal folds of the right eye without general symptoms and received ophthalmologic and laboratory examination, magnetic resonance imaging (MRI), and orbital biopsy. MEtHODS: Antibodies against HtLV-1 were extremely high but atypical lymphocytes were not present in the peripheral blood. MRI showed multiple tumorous lesions in both orbits. Orbital biopsy revealed an orbital mass that consisted of atypical lymphocytes originally from t cells and established the diagnosis of lymphoadenopathy type of AtLL. CONCLUSION: Although ocular involvement by AtLL is extremely rare, AtLL can first present in the orbit, and only the results of a biopsy can provide definitive information for its diagnosis.

Sinus cycle length has been reported to fluctuate after a ventricular premature beat(VPB). the purpose of this study was to assess the short-term fluctuations of sinus cycle length in patients with hypertrophic cardiomyopathy(HCM) and prior myocardial infarction(MI). Methods: the relative deviation of RR intervals from the mean of the last two RR intervals preceding a VPB were calculated during the 20 subsequent beats following the VPB from Holter recordings in 92 patients with non-obstructive HCM, 57 patients with prior MI and 54 healthy controls. Results: In controls, the deviations of the RR intervals were negative for several beats after a VPB and subsequently changed to positive before returning to the baseline. Similar changes in RR intervals following a VPB were exhibited in HCM patients; however, the late positive deviations of RR intervals were more marked than in controls. By contrast, in patients with prior MI, the early negative deviations of RR intervals were smaller compared with controls, and the deviations returned to the baseline without incidence of the positive changes. Conclusions: Short-term fluctuations in sinus cycle length after a VPB differed exclusively among HCM patients, prior MI patients, and healthy controls.
