Multi-institutional retrospective analysis of FOLFIRI in patients with advanced biliary tract cancers

BACKGROUND Gemcitabine plus platinum is the standard of care first-line treatment for advanced biliary tract cancers(BtC).there is no established second-line therapy,and retrospective reviews report median progression-free survival(PFS)less than 3 mo on second-line therapy.5-Fluorouracil plus irinotecan(FOLFIRI)is a commonly used regimen in patients with BtC who have progressed on gemcitabine plus platinum,though there is a paucity of data regarding its efficacy in this *** to assess the efficacy of FOLFIRI in patients with biliary tract *** We retrospectively identified patients with advanced BtC who were treated with FOLFIRI at MD Anderson,University of Michigan and Mayo Clinic in *** were collected on patient demographics,BtC subtype,response per RECISt v1.1,progression and *** Ninety-eight patients were included of which 74(75%)had metastatic and 24(25%)had locally advanced disease at the time of treatment with *** median age was 60(range,22-86)*** number of patients with extrahepatic cholangiocarcinoma,gall bladder cancer and intrahepatic cholangiocarcinoma were 10,17 and 71,*** was used as 1st,2nd,3rd or 4th–Nth lines in 8,50,36 and 4 patients,*** duration on FOLFIRI in the entire cohort was 2.2(range,0.5-8.4)*** median PFS and overall survival were 2.4(95%confidence interval(CI):1.7-3.1)and 6.6(95%CI:4.7-8.4)mo,*** PFS for patients treated with FOLFIRI in 1st,2nd,3rd or 4th–Nth lines were 3.1,2.5,2.3 and 1.5 mo,*** patients received concurrent bevacizumab(n=13)or EGFR-targeted therapy(n=5)with FOLFIRI,with a median PFS of 2.7 mo(95%CI:1.7-5.1).CONCLUSION In this largest multi-institution retrospective review of 98 patients with BtC treated with FOLFIRI,efficacy appears to be modest with outcomes similar to other cytotoxic chemotherapy regimens.

Linusorbs(LOs),particularly the Methionine(Met)-containing yet tryptophan(trp)-free LO(CLB and CLP),play a significant role in shaping the quality and sensory attributes of flaxseed *** investigate the stability of these LOs and their oxidized forms during accelerated oxidation,the polar fractions of crude flaxseed oil were selectively removed using silica absorption and then reintroduced separately into the oil matrix along with LOs andγ-tocopherol(γ-t).through comprehensive analysis of oxidative stability indexes,LOs content changes,oxidation kinetics,and the correlation between LOs and oxidative stability indexes,we observed that the LOs fraction alone exhibited a moderate antioxidant effect on the peroxide value(PV)but a weaker effect on the panisidine value(p-AV)of flaxseed ***,upon addition ofγ-t,we observed a weak synergistic effect betweenγ-t and LOs in suppressing PV,while a stronger effect in suppressing ***,a comparison of the oxidation kinetics of different LOs indicated thatγ-t preferentially protected the oxidation of CLP over *** protective trends ofγ-t towards the methionine sulfoxide(MetO)and methionine sulfoxide(MetO2)form of LOs were also observed,albeit to a lesser *** analysis revealed that changes in Metcontaining LOs,particularly CLP,showed a high correlation with PV in the tested matrices,while MetOcontaining LOs better depicted changes in ***,CLP demonstrated higher susceptibility to the influence of antioxidants,making it more suitable for evaluating the peroxide value in flaxseed oil samples.

In recent years, great progress has been made regarding the treatment of inflammatory bowel disease (IBD), particularly in the field of biological therapies. Nevertheless, the ultimate treatment is not in sight. With the development of new medication, it has become clear that we need a new understanding of IBD. therapy needs to fit the different subtypes of IBD; e.g. mild disease in comparison to severe chronic active disease or Crohn's disease with or without fistulation or stenosis. the following article gives a practical overview of actual treatments for IBD. the intention of this article is not to provide a complete review of all new scientific developments, but to give a practical guideline for therapy of IBD.

Aims the direct effects of atmospheric and climatic change factors—atmospheric[CO_(2)],air temperature and changes in precipitation—can shape plant community composition and alter ecosystem *** is essential to understand how these factors interact to make better predictions about how ecosystems may respond to *** investigated the direct and interactive effects of[CO_(2)],warming and altered soil moisture in open-top chambers(OtCs)enclosing a constructed old-field community to test how these factors shape plant *** and methods the experimental facility in Oak Ridge,tN,USA,made use of 4-m diameter OtCs and rain shelters to manipulate[CO_(2)](ambient,ambient+300 ppm),air temperature(ambient,ambient+3.5C)and soil moisture(wet,dry).the plant communities within the chambers comprised seven common old-field species,including grasses,forbs and *** tracked foliar cover for each species and calculated community richness,evenness and diversity from 2003 to *** findings this work resulted in three main findings:(1)warming had speciesspecific effects on foliar cover that varied through time and were altered by soil moisture treatments;(2)[CO_(2)]had little effect on individual species or the community;(3)diversity,evenness and richness were influenced most by soil moisture,primarily reflecting the response of one dominant *** conclude that individualistic species responses to atmospheric and climatic change can alter community composition and that plant populations and communities should be considered as part of analyses of terrestrial ecosystem response to climate ***,prediction of plant community responses may be difficult given interactions between factors and changes in response through time.

the major flow physics of the unsteady condensation in the subsonic flows induced by the unsteady expansion waves in shock tube was studied in this paper. the unsteady condensation phenomenon was analyzed by using the two-dimensional, unsteady, Navier-Stokes equations, which were fully coupled with a droplet growth equation. the third-order tVD MUSCL scheme was applied to solve the governing equation systems. the computational results were compared with the previous experimental data. the time-dependent behavior of unsteady condensation of moist air in shock tube was investigated in details. the results show that the major characteristics of the unsteady condensation phenomenon in shock tube are very different from those in the supersonic wind tunnels.

Enhanced dielectric and optical properties of nanoscale barium hexaferrites for optoelectronics and high frequency application

M-type barium hexaferrites with chemical composition Ba1-xDyxFe12-yCryO19(x = 0.0, 0.1, 0.2, and y =0.0, 0.4, 0.5) were synthesized via sol–gel auto-combustion method. the samples were pre-sintered at 400℃ for 3 h and sintered at 950℃ for 5 h. the changes in the structural, dielectric, and optical properties were studied after the substitution of Dy3+and Cr3+ions. X-ray diffraction(XRD) analysis confirms the formation of single phase hexaferrites with the absence of secondary phase. FtIR analysis gives an idea of the formation of hexaferrites with the appearance of two peaks at 438 cm-1and 589 cm-1. the field emission scanning electron micrographs(FESEM) show a combination of crystallites with large shapes close to hexagonal platelet-like shape and others with rice or rod-like shapes, whereas EDX and elemental analysis confirm the stoichiometry of prepared samples. the calculated band gap from UV-vis NIR spectroscopy spectra was found to decreases with increase in Dy3+–Cr3+substitution. the dielectric properties were explained on the basis of Maxwell–Wagner model. Enhancement of dielectric constant at higher frequencies was observed in all the samples. Low dielectric loss is also observed in all the samples and Cole–Cole plot shows that grain boundary resistance(Rgb) contribute most to the dielectric properties. the prepared samples exhibit properties that could be useful for optoelectronics and high frequency application.

Abrasive resistance of metastable V–Cr–Mn–Ni spheroidal carbide cast irons using the factorial design method

Full factorial design was used to evaluate the two-body abrasive resistance of 3wt%C–4wt%Mn–1.5wt%Ni spheroidal carbide cast irons with varying vanadium（5.0wt%–10.0wt%） and chromium（up to 9.0wt%） contents. the alloys were quenched at 920℃. the regression equation of wear rate as a function of V and Cr contents was proposed. this regression equation shows that the wear rate decreases with increasing V content because of the growth of spheroidal VC carbide amount. Cr influences the overall response in a complex manner both by reducing the wear rate owing to eutectic carbides（M7C3） and by increasing the wear rate though stabilizing austenite to deformation-induced martensite transformation. this transformation is recognized as an important factor in increasing the abrasive response of the alloys. By analyzing the regression equation, the optimal content ranges are found to be 7.5wt%–10.0wt% for V and 2.5wt%–4.5wt% for Cr, which corresponds to the alloys containing 9vol%–15vol% spheroidal VC carbides, 8vol%–16vol% M7C3, and a metastable austenite/martensite matrix. the wear resistance is 1.9–2.3 times that of the traditional 12wt% V–13wt% Mn spheroidal carbide cast iron.

Folded patch antennas were investigated for the development of low-cost and wireless smart-skin sensors that monitor the strain in metallic *** the patch antenna is under strain/deformation,its resonance frequency varies *** variation can be easily interrogated and recorded by a wireless *** patch antenna adopts a specially chosen substrate material with low dielectric attenuation,as well as an inexpensive off-the-shelf radiofrequency identification(RFID)chip for signal *** the RFID chip harvests electromagnetic power from the interrogation signal emitted by the reader,the patch antenna itself does not require other(internal)power sources and,thus,serves as a battery-less(passive)and wireless strain *** this preliminary investigation,a prototype folded patch antenna has been designed and *** testing results show strong linearity between the interrogated resonance frequency and the strain experienced by the *** experiments,the strain sensing resolution is demonstrated to be under 50με,and the wireless interrogation distance is shown to be over a few feet for this preliminary prototype.

the distance from an approaching threat at which animals initiate flight - flight-initiation distance （FID） -- is a sensitive metric of variation in risk, but the effects on FID associated with the risk of possessing highly detectable external coloration are unknown. We tested whether variation in the degree of plumage vividness in birds explained variation in flight-initiation distance. After controlling for body mass, the distance at which the experimental approach began, and phylogenetic relatedness, plumage vividness was not a predictor of FID. Contrary to the expectation that vividness affects risk, and therefore risk assess- ment, these results suggest that birds do not compensate for greater visual conspicuousness by fleeing sooner from approaching threats [Current Zoology 61 （4）： 773-780, 2015].
