Risk scores for allograft failure: Are they still useful in liver recipients from donation after circulatory death?

BACKGROUND Liver grafts from donation after circulatory death(DCD)are associated with a higher risk of early graft dysfunction,determined by the warm ischemia and cold ischemia *** is essential to have precise criteria to identify this complication in order to guide therapeutic *** to validate different graft and recipient survival scores in patients undergoing liver transplantation(Lt)with DCD *** A retrospective and observational unicentric study was conducted on 65 Lt patients with grafts obtained from controlled DCD donors from November 2013 to November *** United Kingdom(UK)risk score,early allograft dysfunction(EAD)Olthoff score,and model for early allograft function(mEAF)score were used to evaluate the risk of graft and recipient survival *** survival analysis purposes,we used the Kaplan-meier method,and the differences between subgroups were compared using the log-rank(mantel-Cox)*** Sixty-five patients were included in the *** UK risk score did not demonstrate predictive capacity for recipient or graft ***,in donors aged over 70 years old(18.4%),it significantly predicted graft survival(pto Kaplan-meier survival curves,graft survival rates at 6 months,2 years,and 5 years in the futility group dramatically decreased to 50%compared to the other groups(log-rank 8.806,pthe EAD Olthoff and mEAF scores did not demonstrate predictive capacity for recipient or graft *** on Kaplan-meier survival curves,patients with a mEAF score≥7 had a lower graft survival rate at 6 months,2 years,and 5 years compared to patients with a lower mEAF score(log-rank 4.667,pth UK DCD risk score and mEAF score showed predictive capability for graft survival.

BACKGROUND Among diverse profound impacts on patients’quality of life(QoL),end-stage renal disease(ESRD)frequently results in increased levels of depression,anxiety,and *** replacement therapies such as hemodialysis(HD)and transplantation(tX)are intended to enhance QoL,although their ability to alleviate psychological distress remains *** research posits the existence of a significant correlation between negative emotional states and QoL among ESRD patients,with varying effects observed in HD and tX *** to examine the relationship between QoL and negative emotional states(depression,anxiety,and stress)and predicted QoL in various end-stage renal replacement therapy patients with *** this cross-sectional study included HD or tX patients in the Eastern Region of Saudi *** 36-item Short Form Survey and Depression Anxiety Stress Scale(DASS)was used for data collection,and correlation and regression analyses were *** the HD and tX transplantation groups showed statistically significant inverse relationships between QoL and DASS *** patients with high anxiety levels and less education scored low on the physical component summary(pCS).In addition,the results of the mental component summary(mCS)were associated with reduced *** with older transplant patients,tX patients’pCS scores were lower,and depression,stress,and negative working conditions were highly correlated with mCS *** the findings of this study revealed notable connections between well-being and mental turmoil experienced by individuals undergoing HD and *** pCS of HD patients is affected by heightened levels of anxiety and lower educational attainment,while the mCS of transplant patients is influenced by advancing age and elevated stress *** insights will contribute to a more comprehensive understanding of patient support.

Superplasticity of fine-grained mg-10Li alloy prepared by severe plastic deformation and understanding its deformation mechanisms

the superplastic behavior and associated deformation mechanisms of a fine-grained mg-10.1 Li-0.8Al-0.6Zn alloy(LAZ1011)with a grain size of 3.2μm,primarily composed of the BCCβphase and a small amount of the HCpαphase,were examined in a temperature range of 473 K to 623 *** microstructural refinement of this alloy was achieved by employing high-ratio differential speed *** best superplasticity was achieved at 523 K and at strain rates of 10^(-4)-5×10^(-4)s^(-1),where tensile elongations of 550±600%were *** the heating and holding stage of the tensile samples prior to tensile loading,a significant increase in grain size was observed at temperatures above 573 ***,it was important to consider this effect when analyzing and understanding the superplastic deformation behavior and *** the investigated strain rate range,the superplastic flow at low strain rates was governed by lattice diffusion-controlled grain boundary sliding,while at high strain rates,lattice diffusion-controlled dislocation climb creep was the rate-controlling deformation *** was concluded that solute drag creep is unlikely to *** the late stages of deformation at 523 K,it was observed that grain boundary sliding led to the agglomeration of theαphase,resulting in significant strain *** mechanism maps were constructed forβ-mg-Li alloys in the form of 2D and 3D formats as a function of strain rate,stress,temperature,and grain size,using the constitutive equations for various deformation mechanisms derived based on the data of the current tests.

BACKGROUND prevalence of hepatocellular carcinoma(HCC)is increasing,especially in patients with metabolic dysfunctionassociated steatotic liver disease(mASLD).AIm to investigate rifaximin(RIF)effects on epigenetic/autophagy markers in *** Adult Sprague-Dawley rats were randomly assigned(n=8,each)and treated from 5-16 wk:Control[standard diet,water plus gavage with vehicle(Veh)],HCC[high-fat choline deficient diet(HFCD),diethylnitrosamine(DEN)in drinking water and Veh gavage],and RIF[HFCD,DEN and RIF(50 mg/kg/d)gavage].Gene expression of epigenetic/autophagy markers and circulating miRNAs were *** All HCC and RIF animals developed metabolic-dysfunction associated steatohepatitis fibrosis,and cirrhosis,but three RIF-group did not develop *** animals who developed HCC with those who did not,miR-122,miR-34a,tubulin alpha-1c(tuba-1c),metalloproteinases-2(mmp2),and metalloproteinases-9(mmp9)were significantly higher in the *** opposite occurred with Becn1,coactivator associated arginine methyltransferase-1(Carm1),enhancer of zeste homolog-2(Ezh2),autophagy-related factor LC3A/B(map1 Lc3b),and p62/sequestosome-1(p62/SQStm1)-*** with controls,map1 Lc3b,Becn1 and Ezh2 were lower in HCC and RIF-groups(pm1 was lower in HCC compared to RIF(ppatic expression of mmp9 was higher in HCC in relation to the control;the opposite was observed for p62/Sqstm1(ppression of p62/SQStm1 protein was lower in the RIF-group compared to the control(p=0.024).there was no difference among groups for tuba-1c,Aldolase-B,alpha-fetoprotein,and mmp2(p>0.05).miR-122 was higher in HCC,and miR-34a in RIF compared to controls(pmiR-26b was lower in HCC compared to RIF,and the inverse was observed for miR-224(pthere was no difference among groups regarding miR-33a,miR-143,miR-155,miR-375 and miR-21(p>0.05).CONCLUSION RIF might have a possible beneficial effect on preventing/delaying liver carcinogenesis throu

BACKGROUND the concept of positive health(pH)supports an integrated approach for patients by taking into account six dimensions of *** approach is especially relevant for patients with chronic *** gastrointestinal and hepatopancreatico-biliary(GI-HpB)disorders are among the top-6 of the most prevalent chronically affected organ *** impact of chronic GI-HpB disorders on individuals may be disproportionally high because:(1)the affected organ system frequently contributes to a malnourished state;and(2)persons with chronic GIHpB disorders are often younger than persons with chronic diseases in other organ *** to describe and quantify the dimensions of pH in patients with chronic GI-HpB *** prospective,observational questionnaire study performed between 2019 and 2021 in 235 patients with a chronic GIHpB disorder attending the Outpatient Department of the maastricht University medical *** questionnaires and data from patient files were used to quantify the six dimensions of *** consistency was tested with mcDonald’s ***-order pearson correlations and t-tests were used to assess associations and differences.A p valuethe GI-HpB patients scored significantly worse in all dimensions of pH compared to control data or norm scores from the general *** quality of life,participation and daily functioning,GI-HpB patients scored in the same range as patients with chronic disorders in other organ systems,but depressive symptoms(in 35%)and malnutrition(in 45%)were more frequent in patients with chronic GI-HpB *** scores between the six dimensions were only very weak to weak,forcing us to quantify each domain *** All six dimensions of pH are impaired in the GI-HpB *** and depressive symptoms are more prevalent compared to patients with chronic disorders in other organ systems.

Getting the most Out of Every training Day: the Influence of Instructors on Self-Regulated Learning During Firefighter Leadership Courses

Leadership courses in the fire services are highly challenging, and they can seriously exhaust trainees and hamper their selfregulated learning efforts(for example, setting goals, focusing attention, seeking feedback). We theorize that experiences of failure or overload can curtail trainees' available energy resources on subsequent training days, which, in turn, should affect trainees' learning efforts. Given instructors' central role in leadership courses, we hypothesize that supportive and humble instructor behaviors decrease experiences of failure and overload and, thus, increase self-regulated learning. moreover, we argue that supportive instructor behavior may amplify the positive effects of high energy resources, while humble behavior may alleviate the negative impact of low resource levels. We tested preregistered hypotheses with 118 firefighters participating in two-week leadership courses at a German fire academy. the participants completed short web-based questionnaires before and after classes each day. multilevel analyses confirmed that perceived daily supportive and humble instructor behavior predicted trainees' reports of daily self-regulated learning activity. Notably, this effect was independent from positive effects of trainees' reported energy resources in the morning. Supportive and humble behavior did not moderate the eff ect of energy resources. Our findings suggest that instructors play a crucial role in facilitating effective learning under challenging training conditions. Furthermore, we offer implications for leaders in fire services, who often conduct trainings with their subordinates.
