BACKGROUND Single-organ vasculitis(SOV)is characterized by inflammation of a blood vessel,affecting one organ,such as the skin,genitourinary system,or the aorta without systemic *** SOV is rare,with hepatic artery involvement reported only in two prior published ***,we presented a case of isolated hepatic artery vasculitis presenting after Pfizer-BioNtech mRNA corona virus disease 2019(COVID-19)*** SUMMARY A 50-year-old woman with hypertension presented to our Emergency Department with recurrent diffuse abdominal pain that localized to the epigastrium and emesis without diarrhea that began eight days after the second dose of the PfizerBioNtech COVID-19 *** work revealed an elevated C-reactive protein(CRP)of 19 mg/L(normaltase 150 U/L(normal 25-105 U/L),gamma-glutamyl transferase(GGt)45 U/L(normalted immunoglobulins(Ig)G 18.4 g/L(normal 7-16 g/L)and IgA 4.4 g/L(normal 0.7-4 g/L).An abdominal computed tomography revealed findings in keeping with hepatic artery vasculitis.A detailed review of her history and examination did not reveal infectious or systemic autoimmune causes of her *** extensive autoimmune panel was ***-19polymerase chain reaction nasopharyngeal swab,human immunodeficiency virus,viral hepatitis and Heliobacter pylori serology were *** six months,the patient’s symptoms,and blood work spontaneously *** High clinical suspicion of SOV is required for diagnosis in patients with acute abdominal pain and dyspepsia.

Resistance Measured by Airflow Perturbation Compared with Standard Pulmonary Function Measures

Background: Routine lung function testing requires expensive equipment, or requires maximum expiratory effort. the airflow perturbation device (APD) is a light handheld device, allowing for serial measures of respiratory resistance noninvasively and effortlessly. Methods: In a convenience sample of 398 patients undergoing pulmonary function testing, we compared routine spirometric indices (forced expired volume in 1 second (FEV1), peak expiratory flow (PEF)), and airways resistance (Raw-272 patients), to measures of respiratory resistance measured with the APD including inspiratory (IR), expiratory (ER) and averaged (AR) resistance. Results: Measures of lung function were significantly correlated (p 0.001). On regression analysis, between 7% - 17% of the variance (R2) for FEV1, PEF, and Raw was explained by APD measurements. Approximately 2/3 of the variance in FEV1 was explained by PEF measurements. Conclusions: APD measurements of lung function correlate with conventional measures. Future studies should be directed at exploring the use of the APD device in serial measures of lung function in patients with lung disease.

the service life of refractories in ladle walls is limited by corrosion,abrasion,thermal shock and structural spalling mechanisms. When the ladle is lined with bricks they typically need to be completely removed after a certain number of heats to be replaced by new bricks of the same size as the original bricks. Not so for monolithic ladle linings. At the end of their service life the remains of the castable can almost completely be recycled as they can in fact stay in place. Only the worn-out part of the lining has to be replaced by a new castable. 20%-50% of the initial ladle lining can be recycled ＂insitu＂. the installation can efficiently be done by shotcreting technics. But it requires a castable that resists slag penetration very well. Castables based on a calcium magnesium aluminate bond provide the required penetration resistance. Pumping and shotcreting is very well adapted for repairs of blast furnace shafts,torpedo cars,hot metal and steel ladle linings[1]. However,very little is published about how a good pumping and shotcreting performance can be achieved when the installation has to be done under extreme weather conditions. At high ambient temperature the challenge is to ensure a good pumping result without early castable stiffening. Blockage of the pump would be the consequence. At low ambient temperature the difficulty is to de-activate the highly efficient deflocculant fast enough with a gelling agent added intothe wet mix at the nozzle to prevent the gunned material slipping off the wall. this paper studies strategies how to achieve good installed properties even at extreme ambient temperatures. the interactions between deflocculants,retarders,gelling agents,and calcium magnesium aluminate binder as a function of temperature are studied for an alumina- spinel ladle castable. A black phosphorus(BP) functionalized optical fiber sensor based on a microfiber coil resonator(MCR) for Pb^2+ ion detection in an aquatic environment is presented and experimentally demonstrated. the MCR-BP sensor is manufactured by winding a tapered microfiber on a hollow rod composed of a low-refractive-index polycarbonate(PC) resin with the BP deposited on the internal wall of the rod. Based on the propagation properties of the MCR, the chemical interaction between the Pb^2+ ions and the BP alters the refractive index of the ambient environment and thus results in a detectable shift in the transmission spectrum. the resonance wavelength moves towards longer wavelengths with an increasing concentration of Pb^2+ ions, and the sensor has an ultra-high detection resolution of 0.0285 ppb(parts per billion). the temperature dependence is 106.95 pm/°C due to the strong thermo-optic and thermal-expansion effect of the low-refractive-index PC resin. In addition, the sensor shows good stability over a period of 15 days. the local pH also influences the sensor, with the resonance wavelength shift increasing as pH approaches a value of 7 but then decreasing as the pH value increases further due to the effect of the BP layer by H^+ and OH-ions. the sensor shows the potential for high-resolution detection of Pb^2+ ions in a liquid environment with the particular advantages of having a simple structure, ease of fabrication,low cost, low loss, and simple interrogation.

Systemic Inflammatory Markers and Disease Severity in Chronic Obstructive Pulmonary Disease—the Effect of Acute Exercise and Pulmonary Rehabilitation

Background: Decreased physical capacity and increased systemic inflammatory response are frequently observed in patients with chronic obstructive pulmonary disease (COPD). the relationship between the inflammatory response and disease severity and the immunological response to exercise were addressed in COPD. Objective: the first objective was to identify systemic biomarkers and their relationship with COPD severity. the second objective was to examine the effect of both acute exercise and pulmonary rehabilitation on these biomarkers. Methods: Forty subjects participated in the study. thirty-two patients with moderate or severe COPD and 8 healthy non-smokers completed the study. Spirometry was preformed. Physical capacity was determined by a progressive symptom-limited cycle ergo meter (incremental) test. Blood samples were analyzed for C-reactive protein (CRP), pro-inflammatory cytokines (IL-6, tNF-α), pro-fibrotic cytokines (tGF-β) and oxidative burst in circulating leukocytes before and after exercise, and before and after pulmonary rehabilitation. Results: IL-6, CRP, WCC and tGF-β were higher in COPD (p α, CRP and tGF-β were negatively related to forced expiratory volume in 1 s (FEV1) (r = 0.4054, 0.3221, 0.1528, 0.1846 and 0.1187, respectively). Acute exercise increased circulating leucocytes and oxidative stress in both groups (p = 0.000, 0.0049 respectively), while IL-6 was increased in COPD group ((p = 0.0115) and circulating tNF-α in healthy control (p = 0.0369). Pulmonary rehabilitation didn’t modify the levels of inflammatory mediators. Conclusions: Reduced lung function is associated with increased levels of systemic inflammatory markers and acute exercise can further increase this inflammatory response. However pulmonary rehabilitation is unlikely to exacerbate systemic inflammation in COPD.
