Epidemiological, Clinical, Paraclinical, Etiological and therapeutic Aspects of Liver Cirrhosis in the Hepato-Gastroenterology Department of the Hospital Aristide Le Dantec in Dakar

Introduction: Liver cirrhosis is a global public health issue. Our aim was to determine the epidemiological, clinical, paraclinical and therapeutic aspects of liver cirrhosis in the hepato-gastroenterology department of the hospital Aristide Le Dantec in Dakar. Patients and Methods: We conducted a retrospective study with a descriptive focus covering the period from January 1st, 2010 to December 31, 2020. We included the medical records of patients which presented body of clinical and paraclinical arguments which supported the diagnosis of cirrhosis. the data collected were related to age, gender, clinical, paraclinical, therapeutic and prognosis aspects of cirrhosis and were analyzed with the software Sphinx Plus. Results: Prevalence of cirrhosis was 6.2%. Sex ratio was 2.1%. the average age was 38 years. Asthenia (60.9%), altered performance status (60.1%), abdominal pain (37.2%), gastrointestinal bleeding (29.6%) and abdominal swelling (27.8%) were the most common motives of consultation. Physical examination revealed primarily portal hypertension (74.9%), liver failure (2.4%), hepatomegaly (28.2%) and anemia (13.7%). Viral hepatitis B was the most common etiology (81.9%). Abdominal screening (ultrasound/Ct scan) showed hepatomegaly (80.6%), liver dysmorphia (87.7%), portal hypertension signs (85.3%) and portal vein thrombosis (18.2%). 55.1% Patients were classified as Child Pugh A, 33.8% were Child Pugh B and 11.1% were Child Pugh C. tenofovir Disoproxil Fumarate (tDF) was prescribed to all patients affected by hepatitis B virus only. Evolution was characterized by clinical improvement and prognosis reclassification. Conclusion: Liver cirrhosis is a frequent pathology in the hepato-gastroenterology department of the hospital Aristide Le Dantec in Dakar. It affects young male adult. Hepatitis B virus is the leading cause. tDF is an effective treatment.

Acute Pancreatitis during SARS CoV 2 Infection

Introduction: Covid-19 is a systemic disease that can spread to all systems. Among the gastrointestinal manifestations, pancreatic involvement can have a major prognostic impact. We report 3 acute pancreatitis cases occurring during Covid-19, in Dakar. Case 1: 65-year-old woman who presented with intense atypical epigastric pain. Physical examination revealed obesity, high blood pressure and abdominal tenderness. Biological tests found increase CRP (134 mg/l) and lipasemia (312 UI/l). Abdominal Ct scan showed findings of Balthazar grade C acute pancreatitis. Rt-PCR for SARS CoV 2 Rt-PCR was positive. the outcome was favorable. Case 2: 56-year-old woman patient with history of nephroangiosclerosis who presented with dyspnea, cough, fever and moderate epigastric pain. Physical examination revealed epigastric tenderness, high blood pressure, anuria. Biological testing, noted increase CRP (96 mg/l), lipasemia (793 UI/l), creatinine (227 mg/l) and urea (3.84 g/l). Abdominal Ct scan showed acute edematous pancreatitis findings. SARS CoV 2 Rt PCR was positive. the outcome was favorable. Case 3: 27-year-old man who presented with physical asthenia, headache, and epigastric pain. Physical examination found epigastric tenderness. Elevated CRP level was of 102 mg/l and lipasemia level was of 427 UI/l (7n). Abdominal Ct scan showed acute edematous pancreatitis findings. SARS CoV 2 Rt PCR was positive. the outcome was favorable. Conclusion: Acute pancreatitis can occur during Covid-19 infection. However, the imputability to the Covid-19 disease necessitates to rule out the most common causes.

Experimental Study and thermal Modelling of Cocoa Shell Convective Drying in an Indirect Solar Dryer

the concern of the present work is the convective drying of empty cocoa shells in an indirect solar dryer. Some drying experiments, using one sample, were carried out. During the experiments, the sample is introduced in the drying chamber. then at steady time intervals, the sample is withdrawn from the drying chamber, for a rapid weighing. After each weighing, the sample is reintroduced in the dryer. At each time interval, the ambient temperature of the drying chamber and its relative humidity γ are measured by a thermo-hygrometer. From the experimental data, a theoretical determination of the moisture evaporated from the product was performed and a good agreement was found between the theoretical and experimental values, confirmed by the value of the RMSE. those calculations used the constants in the nusselt number found in literature. then those constants were evaluated again, to get new values more suitable with the experimental data. the dimensionless numbers of nusselt, Grashof and Prandtl were calculated. that allowed the calculation of the average value of the nusselt number. the average convective heat transfer coefficient was determined.

Evaluation of slope stability through rock mass classification and kinematic analysis of some major slopes along nH-1A from Ramban to Banihal, north Western Himalayas

the network of Himalayan roadways and highways connects some remote regions of valleys or hill slopes,which is vital for India’s socio-economic *** to natural and artificial factors,frequency of slope instabilities along the networks has been increasing over last few *** of stability of natural and artificial slopes due to construction of these connecting road networks is significant in safely executing these roads throughout the *** rock mass classification methods are generally used to assess the strength and deformability of rock *** study assesses slope stability along the nH-1A of Ramban district of north Western *** structurally and non-structurally controlled rock mass classification systems have been applied to assess the stability conditions of 14 *** evaluating the stability of these slopes,kinematic analysis was performed along with geological strength index(GSI),rock mass rating(RMR),continuous slope mass rating(CoSMR),slope mass rating(SMR),and Q-slope in the present *** SMR gives three slopes as completely unstable while CoSMR suggests four slopes as completely *** stability of all slopes was also analyzed using a design chart under dynamic and static conditions by slope stability rating(SSR)for the factor of safety(FoS)of 1.2 and 1 respectively.Q-slope with probability of failure(PoF)1%gives two slopes as stable *** slope angle has been determined based on the Q-slope safe angle equation and SSR design chart based on the *** value ranges given by different empirical classifications were RMR(37-74),GSI(27.3-58.5),SMR(11-59),and CoSMR(3.39-74.56).Good relationship was found among RMR&SSR and RMR&GSI with correlation coefficient(R 2)value of 0.815 and 0.6866,***,a comparative stability of all these slopes based on the above classification has been performed to identify the most critical slope along this road.

Primary total Hip Arthroplasty on Complex Hips Conditions in a Low-Resource Setting

Introduction: total hip arthroplasty of complex morphology is a challenge for the orthopaedic surgeon. Careful analysis of the hip’s anatomy helps to unravel the difficulties and anticipate the procedures to be performed and the implants to be planned. the aim was to identify the types of hip that make first-line tHR difficult, specify the technical procedures to be used on these sites, and assess the functional results of the series. Material and Methods: this was a retrospective study that concerned patients operated on for total hip arthroplasty between January 2015 and December 2022 at the medical center “La Grâce” in Bobo-Dioulasso, Burkina Faso. Patients with coxarthrosis (on hip dysplasia, acetabular protrusio, acetabular malunion or neurological hip) and those with ankylosis of the hip, osteonecrosis secondary to neglected dislocation of the hip or hemoglobinopathy were included. Results: A total of 31 total hip replacements were performed in 30 patients. the mean age of patients at the surgery time was 36.2 years with extremes of 17 and 61 years. the male-to-female sex ratio was 1. the main indications for tHA were the dysplasic hip osteoarthritis (11 cases) and the neglected hip dislocations (7 cases). In situ femoral neck osteotomy before hip dislocation was performed in seven cases. the acetabulum reconstruction techniques varied from the structural iliac bone graft (n = 3) and cancellous bone graft (n = 4) to the Kerboull plate (n = 1). After 45 months of mean follow-up, all hips were evaluated. the mean PMA score increased from 7.1 [4 - 8] before the surgery to 13.2 [13 - 17]. Conclusion: the large spectrum of challenges in complex hip management requires effective preoperative planning. Preoperative planning minimizes complications and ensures a better outcome.
