Spectrum of Diseases, Disease Related Mortality and Overall Financial Burden of Hospitalized Patients in Medical Wards of Tertiary Care Public Hospital

Background: Detailed assessment of pattern of diseases and trend of mortalities tends to aid in formulating pivotal aspects of in-hospital policies, standards of care and so consequently promoting efficient work system. This also reserves limited resources keeping within the limit of financial grasp. The study aims to reimburse the colossal paucity of data covering epidemiological burden of diseases, impact on mortality as well as financial burden in socioeconomically challenged public health system. Methods: This was a retrospective cross-sectional hospital based study conducted in department of Medicine, Civil Hospital Karachi, from February 2016 to July 2016 using questionnaire based data collection system. Results: Eight thousand and fifty three (8053) cases with almost equal gender proportion were analyzed. Chronic liver disease (CLD) was the most rampant disease, followed by cerebrovascular accident (CVA), tuberculosis and diabetes. Overall 51% cases were attributed to infectious diseases. Overall the average percentage mortality was found to be 20% of all cases. Mean duration of stay was 7.56 ± 7 days. Estimated average total in-hospital expenditure per patient on each visit or stay was 159.15 ± 41.2 $USD (16670.96 ± 4315.7 PKR). Average total diagnostic and medications costs per visit or stay were 89.74 ± 31.2 $USD (9400.26 ± 3268.2 PKR) and 48.68 ± 18.0 $USD (5099.23 ± 1885.5 PKR) respectively. Conclusion: Chronic liver disease, cerebrovascular accident and diabetes were most frequent reasons of hospital admission and mortality. Recognition of most common and fatal diseases with their expenditure is going to lead to formulation of targeted policies both of which would be effective and efficient.

Objective:To evaluate the antioxidant potential and pancreatic lipase inhibitory action of optimized hydroethanolic extracts of Solanum ***:Optimized extraction for maximum recovery of metabolites was performed using a combination of freeze-drying and ultrasonication followed by determination of antioxidant and antiobesity *** ultra-high performance liquid chromatography equipped with mass spectrometry was used to analyze metabolite profiling of Solanum *** studies were performed using molecular docking and electrostatic potential analysis for individual *** hypolipidemic potential of the most potent extract was assessed in the obese mice fed on fat rich ***:The 80%hydroethanolic extract exhibited the highest extract yield,total phenolic contents,total flavonoid contents along with the strongest 2,2-diphenyl-1-picrylhydrazyl scavenging activity,total antioxidant power,and pancreatic lipase inhibitory *** 80%hydroethanolic extract not only regulated the lipid profile of obese mice but also restricted the weight gain in the liver,kidney,and *** 80%hydroethanolic extract also reduced alanine transaminase and aspartate transaminase concentrations in *** effects of plant extract at 300 mg/kg body weight were quite comparable with the standard drug ***:Solanum nigrum is proved as an excellent and potent source of secondary metabolites that might be responsible for obesity mitigation.

All CABG Patients Who Have No Contraindications: Do They Get Perioperative Beta Blockers?

New-onset postoperative atrial fibrillation (POAF) following Coronary artery bypass graft (CABG) surgery has been described in up to 15% to 40% of patients in the initial postoperative period. POAF is related with higher mortality, increased hospital resource utilization, postoperative extra ITU hours and hospital days, consequently increasing hospital-related budgets. Beta blocker administration decreases the rate of POAF from 30% - 40% to 12% - 16% after CABG. According to the EACTS (European Association of Cardiothoracic Surgery) guideline December 2006, β-Blockers should routinely be used as the first choice for the prophylaxis of atrial fibrillation (AF) in all patients undergoing cardiac surgery, if not contraindicated (IB). To compare the contemporary practice with the recommended standard retrospective data of consecutive 400 patients treated with isolated CABG between July 2015 and June 2017 were collected. Those patients who received β-blockers on the day of surgery or the following morning (Continued and Restarted on 1st POD) met the standard guidelines. Thus, according to the data (12% + 20%) 32% of the patients met the standard. To compare the rate of Postoperative Atrial fibrillation, we divided the patients into two groups. Group A, who followed the guideline (128 patients) and Group B, who resumed β-Blockers 48 hours onwards (272 Patients). In group A, only 8 patients developed postoperative AF whereas in group B 88 patients developed postoperative AF which is also statistically significant (P β-blockers use at perioperative period.

Ti_(3)CN,as a typical hetero-MXene,has attracted tremendous attention for its unique ***,its ultrafast photonics applications are still ***,the few-layer Ti_(3)CN MXene was successfully prepared by selective etching and molecular delamination *** nonlinear optical response of few-layer Ti_(3)CN MXene at 640 nm was studied using the open-aperture Z-scan *** asprepared Ti_(3)CN MXene sample exhibited excellent nonlinear saturable absorption characteristics,resulting in the nonlinear absorption coefficient b of4.05×10^(-2)cm/GW,which was one order of magnitude larger than that of black phosphorus(BP)and molybdenum disulfide(MoS_(2)).For the optical modulation applications of few-layer Ti_(3)CN MXene,passively Q-switched(PQS)solid-state visible lasers based on Ti_(3)CN saturable absorber(SA)at 522 nm,607 nm,639 nm,and 721 nm were successfully ***,a Ti_(3)CN-based stable passively mode-locked Pr:YLF red laser was also successfully achieved with a pulse duration of 30 ps,and the corresponding repetition rate was 73.1 *** optical modulation device based on few-layer Ti_(3)CN MXene shows good *** work demonstrates that the tremendous prospects of the few-layer Ti_(3)CN MXene as a visible optical modulation device in ultrafast photonics applications.

Computation is spanning from PC to Mobile devices. The Mobile Ad hoc Networks (MANETs) are optimal choice to accommodate this growing trend but there is a problem, security is the core issue. MANETs rely on wireless links for communication. Wireless networks are considered more exposed to security attacks as compared to wired networks, especially;MANETs are the soft target due to vulnerable in nature. Lack of infrastructure, open peer to peer connectivity, shared wireless medium, dynamic topology and scalability are the key characteristics of MANETs which make them ideal for security attacks. In this paper, we shall discuss in detail, what does security mean, why MANETs are more susceptible to security attacks than wired networks, taxonomy of network attacks and layer wise analysis of network attacks. Finally, we shall propose solutions to meet the security challenges, according to our framed security criteria.

The minimal invasiveness and precision of endoscopicultrasound(EUS) has lead to both its widespread use as a diagnostic and staging modality for gastrointestinal and pancreaticobiliary malignancies, and to its expand-ing role as a therapeutic modality. EUS-guided celiac plexus neurolysis is now a well-accepted modality for palliation of pain in patients with pancreatic cancer. EUS-guided ablation, brachytherapy, fiducial marker placement, and antitumor agent injection have been described as methods of performing minimally invasive oncological therapy. EUS-fine needle injection may be performed as adjunctive, alternative, or palliative treatment. This review summarizes the studies to date that have described these methods. A literature search using the Pub Med/MEDLINE databases was performed. While most published studies to date are limited with disappointing outcomes, the concept of a role of EUS in oncological therapy seems promising.

Rocket “Eruca sativa”:A salad herb with potential gastric anti-ulcer activity

AIM:To validate gastric anti-ulcer properties of Rocket "Eruca sativa" on experimentally-induced gastric secretion and ulceration in albino ***: Gastric acid secretion studies were undertaken using pylorus-ligated rats. Gastric lesions in the rats were induced by noxious chemicals including ethanol, strong alkalis, indomethacin and hypothermic restraint stress. The levels of gastric wall mucus (GWM), nonprotein sulfhydryls (NP-SH) and malondialdehyde (MDA) were also measured in the glandular stomach of rats following ethanol administration. The gastric tissue was also examined histologically. The extract was used in two doses (250 and 500 mg/kg body weight) in all experiments. RESULTS: In pylorus-ligated Shay rats, the ethanolic extract of Rocket "Eruca sativa L." (EER) signifcantly and dose-dependently reduced the basal gastric acid secretion, titratable acidity and ruminal ulceration. Rocket extract significantly attenuated gastric ulceration induced by necrotizing agents (80% ethanol, 0.2 mol/L NaOH, 25% NaCl), indomethacin and hypothermic restraint stress. The anti-ulcer effect was further confirmed histologically. On the other hand, the extract signifi cantly replenished GWM and NP-SH levels, as well as the MDA level signifi cantly reduced by extract ***: Rocket extract possesses anti-secretory, cytoprotective, and anti-ulcer activities against experimentally-induced gastric lesions. The anti-ulcer effect is possibly through prostaglandin-mediated activity and/or through its anti-secretory and antioxidant properties.
