针对传统肺结节检测中存在灵敏度低、假阳性高、小结节难检测的问题,提出一种基于深度卷积神经网络的两阶段肺结节检测框架。第一阶段使用特征金字塔子网提取肺部影像的多层次特征,引入多尺度区域建议子网用于在高灵敏度下检测出所有的候选结节;第二阶段设计级联卷积神经网络模型减少假阳性,通过保留分类错误样本用于重新训练模型,将多个模型结果进行投票选出最终分类结果。LUNA16数据集上的实验结果表明,所提框架灵敏度达到95.9%,检测效果优于其它算法,能够有效实现肺结节的准确检测。

The minimal invasiveness and precision of endoscopicultrasound(EUS) has lead to both its widespread use as a diagnostic and staging modality for gastrointestinal and pancreaticobiliary malignancies, and to its expand-ing role as a therapeutic modality. EUS-guided celiac plexus neurolysis is now a well-accepted modality for palliation of pain in patients with pancreatic cancer. EUS-guided ablation, brachytherapy, fiducial marker placement, and antitumor agent injection have been described as methods of performing minimally invasive oncological therapy. EUS-fine needle injection may be performed as adjunctive, alternative, or palliative treatment. This review summarizes the studies to date that have described these methods. A literature search using the Pub Med/MEDLINE databases was performed. While most published studies to date are limited with disappointing outcomes, the concept of a role of EUS in oncological therapy seems promising.

Background: Long segment transpedicular screw fixation in thoracolumbar spine injury has gained popularity in the last decades as it provides immediate stability, prevents further risk of complications and allows early mobilization of patient. Objectives: Evaluation of clinical and functional outcome after long segment transpedicular screw fixation in unstable thoracolumbar spine injury with incomplete neurological deficit patients. Methods: This quasi experimental study was conducted at Dhaka Medical College and Hospital (DMCH), Dhaka, Bangladesh from January 2016 to December 2017. A total of twenty four (24) patients with traumatic unstable fracture of the thoracolumbar spine with incomplete neurological deficit underwent long segment transpedicular screw fixation were included in this study. Clinical and radiologic outcomes were assessed pre-operatively and post-operatively. Results: A total of 24 patients aged between 20 - 60 years of both sexes (Male-21, Female-03) were included in this study. Their mean (±SD) age was 33.12 (±8.57) years. Most of them were farmer [09 (37.5%)] and day labour [08 (33.33%)]. Motor vehicle accidents were the most common [16 (66.67%)] cause of injury and then fall from height [08 (33.33%)]. The most common level of injury involved was L1-10 (41.67%) patients, followed by L2-07 (29.17%), T12-05 (20.83%) and T11-02 (8.33%) of the study patients. Burst fracture was the commonest [22 (91.67%)] type of fracture followed by Chance fracture [02 (8.33%)]. Pre-operative mean (±SD) Cobb angle, Kyphotic deformation of vertebral body, Beck index and vertebral compression deformity were 21.83 ± 4.5 degrees, 22.09 ± 4.40 degrees, 0.78 ± 2.65 and 0.18 ± 0.93 mm respectively, after procedure which were significantly (p Conclusions: Long segment transpedicular screw fixation is an effective method of treatment in patients having thoracolumbar spine injury with incomplete neurological deficit.

Engineering Geological and Geotechnical Investigations for Design of Oxygen Plant

The most important aspect of every civil engineering project is acquiring reliable information on the ground on which the project will be constructed. This research includes a site investigation, which is seen as a primary stage in gathering geological, geotechnical, and other essential engineering data for structures’ safe and cost-effective design. Five boreholes at well-spaced spots were drilled for subsurface investigation at a maximum depth of 15 m to 30 m. The standard penetration tests (SPT) were performed at different depths, soil samples were taken at various intervals, and lithological changes were observed. The friction angle was between 19.6ºand 33.03º, whereas the cohesion ranges between 0.25 kg/cm2 and 0.42 kg/cm2, indicating a strong resistance to shearing and a high capacity to sustain the load. Furthermore, the soil samples’ maximum dry density ranges from 1.63 g/cm3 to 1.80 g/cm3. In addition, water table depths were recorded from 6.0 m to 7.0 m. The net bearing capacity for isolated/pad foundation at a depth of 1.5 m to 2.5 m below the ground level has been calculated as 95.0 to 120.0 kPa and 120.0 to 180.0 kPa for raft foundation. The net allowable pressure settlement limits for isolated/pad and raft foundations are 25 mm (1-inch) and 50 mm (2-inches), respectively. The investigation has found no severe geological flaws on the proposed construction site, and therefore it is appropriate for the construction of an Air Separation Unit (ASU) Oxygen Plant.

Wound healing is a dynamic process that involves a series of molecular and cellular events aimed at replacing devitalized and missing cellular components and/or tissue ***,extracellular vesicles(EVs),naturally cell-secreted lipid membrane-bound vesicles laden with biological cargos including proteins,lipids,and nucleic acids,have drawn wide attention due to their ability to promote wound healing and tissue ***,current exploitation of EVs as therapeutic agents is limited by their low isolation yields and tedious isolation *** circumvent these challenges,bioinspired cell-derived nanovesicles(CDNs)that mimic EVs were obtained by shearing mesenchymal stem cells(MSCs)through membranes with different pore *** characterisations and highthroughput proteomics confirmed that MSC-CDNs mimicked ***,these MSC-CDNs were efficiently uptaken by human dermal fibroblasts and demonstrated a dose-dependent activation of MAPK signalling pathway,resulting in enhancement of cell proliferation,cell migration,secretion of growth factors and extracellular matrix proteins,which all promoted tissue *** note,MSC-CDNs enhanced angiogenesis in human dermal microvascular endothelial cells in a 3D PEGfibrin scaffold and animal model,accelerating wound healing in vitro and in *** findings suggest that MSC-CDNs could replace both whole cells and EVs in promoting wound healing and tissue regeneration.

Spectrum of Upper GI Endoscopy in Pediatric Population at a Tertiary Care Centre in Pakistan

Although upper GI endoscopy is considered to be a gold standard in diagnosis of many pediatric gastrointestinal disorders, there is limited data about its utility from developing countries. We carried out this retrospective study at Aga khan University Hospital, Karachi, Pakistan from 2008-2012. During the study period, a total of 200 procedures were performed. Mean age of patients was 8.5 years. 66% of patients received general anesthesia for the procedure. Failure to thrive with suspected coeliac disease was the most common indication for the procedure, seen in 31% of patients. Gastritis was the most common abnormal endoscopic finding, seen in 14.5% of patients. Gastritis was the most common histopathological finding on biopsy, seen in 31% of the patients (n = 62). Findings?were consistent with coeliac disease in 18% (n = 36), duodenitis, 10.5% (n = 21) and esophagitis, 4%?(n = 8) of patients. No immediate post procedure complication was noted in our study.
