Partial Splenic Artery Embolization in Cirrhosis Is a Safe and Useful Procedure

Background: Portal Hypertension is a common complication of cirrhosis. It leads to splenomegaly which manifests with features of hypersplenism. This results in leucopenia which increases the likelihood of sepsis and prevents treatment with interferon. Thrombocytopenia increases the risk of bleeding including variceal bleeds which make the anemia worse. This study was done to determine the usefulness and safety of partial splenic artery embolization (PSAE) in portal hypertension due to cirrhosis. Methods: Patients with PSAE were identified by using International Classification of Diseases (ICD)-10 coding from medical records and their charts were reviewed retrospectively. 25 patients underwent splenic artery embolization at The Aga Khan University Hospital Karachi from November 2000 to December 2016. 18 patients who underwent PSAE for disabling hypersplenism caused by cirrhosis were included. Patients who were under 18 year of age, or in whom PSAE were performed for reasons other than cirrhosis and those with missing records/incomplete data were excluded (n = 7). Information was collected regarding demographic details, procedure indications, nature, technique, clinical efficacy, repeat embolization and complications along with laboratory and radiological investigations. Results: Eighteen patients of cirrhosis with a mean age of 43.47 ± 10.926 years, of which 14 were males, underwent PSAE (19 procedures). Indications were severe hypersplenism which precluded treatment with interferon and ribavirin (n = 8) and recurrent gastro-oesophageal variceal (gOV) bleeds due to advanced Child-Pugh grade and thrombocytopenia (n = 10). Hematological parameters improved significantly following PSAE. Three out of eight patients successfully completed interferon + ribavirin treatment for hepatitis C (HCV) infection post PSAE, and gOV bleeds stopped in eight out of 10 patients. Meta-Analysis on Efficacy and Safety of Modified guizhi Shaoyao Zhimu Tang with the Western Medicines in Treating Rheumatoid Arthritis

Objective: To assess the efficacy and safety of modified guizhi Shaoyao Zhimu Tang (gSZT) with the Western medicines in treating rheumatoid arthritis. Methods: All the relevant studies were collected by searching the Pubmed, Web of science, Embase, Cochrane Library, SpringerLink, China National Knowledge Infrastructure (CNKI), VIP Journals database, and Wanfang database from their inception to October 2017. After the assessment, the eligible ones were subject to Jadad score. The meta-analysis was conducted by RevMan 5.3 software. Results: Totally 10 randomized controlled trials (RCTs) involving 962 patients were enrolled. The clinical efficacy of analysis showed that modified gSZT with the Western medicines can significantly improve the clinical total effective rate [OR = 4.64, 95%CI (3.03, 7.12), P −14.58, 95%CI (−16.49, −12.68), P −12.62 95%CI (−18.69, −6.54), P −49.88, 95%CI (−77.6, −22.09), P −5.34, 95%CI (−5.86, −4.82), P groups, however, the clinical symptoms of the group on modified gSZT with the Western medicines were obviously reduced than the other one [OR = 0.25, 95%CI (0.10, 0.61), Z = 3.06, P gSZT with the Western medicines improved the clinical efficacy, symptoms and the blood biochemical indicators were improved significantly than that of Western medicines alone on rheumatoid arthritis. At the same time, it can reduce adverse effects and enhance safety.

A signal is the entity that carries information. In the field of communication signal is the time varying quantity or functions of time and they are interrelated by a set of different equations, but some times processing of signal is corrupted due to adding some noise in the information signal and the information signal become noisy. It is very important to get the information from corrupted signal as we use filters. In this paper, Butterworth filter is designed for the signal analysis and also compared with other filters. It has maximally flat response in the pass band otherwise no ripples in the pass band. To meet the specification, 6th order Butterworth filter was chosen because it is flat in the pass band and has no amount of ripples in the stop band.

The activity of antioxidative enzymes system is affected by salt stress, chlorophyll content (CHL), leaf relative water content (RWC), Na+ and K+ contents, their ratio and some oxidative stress indices were studied in leaves of ten bread wheat cultivarsSehar-06, Lu-26, NARC-09, BARC-09 and Pirsbak-09’(salt-tolerant) and Kaghan-94, Rohtas-90, Soughat-90, shaheen-94 and Zardana-89’(salt-sensitive), grown under salinity treatments carried out in five levels (1 dS·m-1 as control, 2, 4, 8, 16 dS·m-1) via sodium chloride. Under high salt potency significant increase for activities of antioxidant enzymes such as ascorbate peroxidase (APX) and guiacol peroxidase (gPX), occured in salt tolerant varieties. Meanwhile, under salinity condition the activities of antioxidant enzymes such as superoxide dismutase (SOD), catalase (CAT) and (gPX) in sensitive cultivar were lower than control. Regarding (APX) activity there was no significant difference between salinity and control situation. Under salt stress membrane stability index (MSI) of both cultivars were negatively influenced. Hydrogen peroxide (H2O2) content of salinity sensitive cultivars was higher than control. Salt tolerant varieties had more amounts of K+ content, K+ and Na+ ratio, relative water content, yield and chlorophyll under salt conditions, and sensitive ones recorded higher Na+ content at tillering stage. The mechanism of salt stress might be achieved due to low lipid peroxidation, assumingly lower changes in membrane stability index and evasion of Na+ combination and amplified activity of antioxidant enzymes.

Secure exchange of information is the basic need of modern digital world of e-communication which is achieved either by encrypting information or by hiding information in other information called cover media. Concealing information requires a well designed technique of Stegnography. This work presents a technique, variable tone variable bits (VTVB) Stegnography, to hide information in a cover image. The VTVB Stegnography hides variable data in discrete cosine transform (DCT) coefficients of the cover image. VTVB Stegnography provides variable data hiding capacity and variable distortion. Additional large data hiding this technique provide extra security due to the large key size making VTVB Stegnography technique much more immune to steganalysis. The hiding makes the existence of information imperceptible for steganalysis and the key of keeping a secret makes the recovering of information difficult for an intruder. The key size is depending on cover image and numbers of bits of discrete cosine transform (DCT) coefficients used for information embedding. This is a very flexible technique and can be used for low payload applications, e.g. watermarking to high payload applications, e.g. network Stegnography.

Objective: To investigate the incidence of retinopathy of prematurity (ROP), analyze the risk factors of ROP, and put forward effective screening strategies, to reduce its incidence. Background: Retinopathy of prematurity (ROP) is an eye disease that can happen in new born premature (born early) babies or have weigh less than 3 pounds at birth. ROP happens when abnormal blood vessels grow in the retina. There are multiple risk factors, which are causing the ROP. In our study we will analyses the risk factors of ROP. Methods: From February 2016 to August 2021, 190 premature infants in the neonatal intensive care unit (NICU) who received inpatient care and ophthalmic screening was selected as study subjects. ROP group (n = 32) and non-ROP group (n = 158) were selected, and the clinical data of the two groups were compared. Including oxygen concentration, mechanical ventilation, broncho pulmonic dysplasia, delivery mode (cesarean section, vaginal delivery), blood transfusion, anemia, gestational diabetes, gestational hypertension, fetal distress, preterm birth weight, gestation age, etc. Women were divided into two groups according to whether they had gestational diabetes mellitus (gDM) or not: gestational diabetic (n = 38) and non-gestational diabetics (n = 152). Age, pregnancy times, birth times, oxygen inhalation, birth weight, and gestational age were compared between the two groups. The X2 test for counting data and the t-test for measuring data are then conducted according to the distribution characteristics of the data, The correlation analysis between ROP and a single risk factor was performed by chi-square test, and the analysis of the correlation between many risk factors and ROP was conducted by Logistic regression analysis. Effect of Foliar Application of Micronutrients on Fruit Quality of Peach

This experiment was conducted to study the effect of foliar spray of micronutrients on quality of peach fruits at Horticulture Farm, University of Agriculture Peshawar during 2010. The experiment was conducted in randomized complete design with six treatments and four replications on peach orchards (early grand). Our pre-treatment soil analysis showed silt loam soil class, alkaline and calcareous in nature and adequate in organic matter. The soil was deficient in P, Zn, Fe and B, whereas adequate in Mn and Cu. The fruit quality was evaluated and maximum fruit length, diameter and yield were noted in T6 (Zn + Cu + Fe + Mn + B). The juice pH decreased and the juice acidity increased in a linear fashion after foliar spray of micronutrients. The total soluble solids of fresh fruit juice ranged 7.01%-8.88% and vitamin C ranged from 4.80%-7.90% after foliar spray. So the foliar spray of micronutrients significantly (P ≤ 0.05) affected the quality of peach fruit.

Polycystic ovarian syndrome (PCOS) is the most common endocrine disorder in women of reproductive age, associated with chronic anovulation and hyperandrogenism. The cause of PCOS is multi-factorial and the mechanism is still unclear. A number of complications are closely related with PCOS such as infertility, hairy and obesity. Approximately up to one-third of females with PCOs have issues with infertility. Obesity is an independent factor which exacerbates infertility in PCOS, reduces the efficacy of infertility treatment and provokes a greater risk of miscarriage. Preconception counseling and guidelines play an initial major role in infertility treatment. The option of the most appropriate treatment depends upon various factors such as age, tubal patency, quality of sperm, experience, and duration of previous treatments and the level of anxiety of the couple. Tubal patency test and semen analysis are mandatory to decide the suitable treatment. Treatment modalities have two measures—non-pharmacological and the pharmacological. Oral clomiphene citrate remains the first line treatment to trigger ovulation. gonadotrophin and laparoscopic ovarian drilling (LOD) surgery are the other treatment option. In vitro fertilization (IVF) and embryo transfer are the third line treatment and IVF remains the first option in the patient with diagnosed and established bilateral tubal occlusion and/or concentration of recovered motile sperm less than or equal to 5 million. Here, etiology, pathophysiology and molecular mechanisms of PCOS are reviewed, and perspectives are given for further research. The recent development, treatment, and diagnosis of PCOS are also highlighted. The summary will be of guiding significance in the prevention and treatment of PCOS.
