在过去的两个多世纪,学术界有大量学者研究影响国际贸易的因素。这些因素基本集中在竞争优势、汇率、工资、相对价格、税率等。但是其他一些对国际贸易也会产生深远影响的因素却没有得到足够的重视,比如文化因素。本文的目的即在于提供有力支持以论证文化冲击对与中国进行国际贸易的影响。毕竟中国文化同世界其他国家文化之间差异巨大。本文选取教育领域作为研究对象,探索文化因素是如何影响教育出口的。研究从三个方面展开,第一,搜集并整理近年留学生来华留学数据,了解该领域整体情况;第二,对在中国的外国留学生展开问卷调查,了解他们的文化冲击程度;最后,将问卷调查结果与参与调查的学生的不同情况和中国教育产业状况进行对比,得到有效结论,找到中国教育出口与外国留学生在华感受到的文化冲击之间的联系。如此,本文为这一有待开发的学术领域提供了新的学术成果,指出了中国仍有很多潜力无限的新市场,并为更进一步的学术研究开辟了道路。

Page Kidney is a relatively rare cause of Acute Renal Failure (ARF) presenting as accelerated and uncontrolled hypertension secondary direct compression of the renal parenchyma by an extrinsic source. This case report describes a 44-year-old male with advanced acute renal failure requiring hemodialysis, hypertension, and initial suspicion for thrombotic thrombocytopenic purpura who developed a case of Page Kidney following retroperitoneal hematoma following a renal biopsy. The patient was medically managed with intravenous nifedipine until blood pressure stabilized after improvement of the hematoma. Usually hematomas are self-resolving, however rarely they can result in the Page phenomenon—extrinsic compression of the affected kidney by the hematoma resulting in a picture that is similar to acute renal failure (ARF). This case highlights the importance of early medical management of blood pressure control after renal compression has been identified.

Major depression during pregnancy is a common psychiatric disorder that arises from a complex and mul- tifactorial etiology. Psychosocial stress, sex, hormones, and genetic vulnerability increase the risk for triggering mood disorders. Microglia and toll-like receptor 4 play a crucial role in triggering wide and varied stress-induced responses mediated through activation of the inflammasome; this leads to the secretion of inflammatory cytokines, increased serotonin metabolism, and reduction of neurotransmitter availability along with hypothalamic-pituitary-adrenal axis hyperactivity. Dysregulation of this intricate neu- roimmune communication network during pregnancy modifies the maternal milieu, enhancing the emergence of depressive symptoms and negative obstetric and neu- ropsychiatric outcomes. Although several studies have clearly demonstrated the role of the innate immune system in major depression, it is still unclear how the placenta, the brain, and the monoaminergic and neuroendocrine systems interact during perinatal depression. Thus, in the present review we describe the cellular and molecular interactions between these systems in major depression during preg- nancy, proposing that the same stress-related mechanisms involved in the activation of the NLRP3 inflammasome in microglia and peripheral myeloid cells in depressed patients operate in a similar fashion in the neuroimmune placenta during perinatal depression. Thus, activation of Toll-like receptor 2 and 4 signaling and the NLRP3 inflammasome in placental immune cells may promote a shift of the Thl/Th2 bias towards a predominant Thl/Thl7 inflammatory response, associated with increased secretion of pro-inflammatory cytokines, among other secreted autocrine and paracrine mediators, which play a crucial role in triggering and/or exacerbating depressive symptoms during pregnancy.

如今,世界范围内有关废物管理的情况正变得越来越重要,从家庭一代开始,到目前为止,以致力于货币活动的经济部门对废物的管理而闻名。就巴拿马的实际情况而言,巴拿马是一个人口和经济不断增长的国家。值得注意的是,巴拿马的固体废物回收、再利用和再循环政策自2015年以来一直在改善。根据环境绩效指数,巴拿马在全球排名70位,在废物管理方面排名78位[1],获得这样的参考解释了作为一个发展中国家,它必须继续改变其法规和战略决策,本研究提供了一个说明性案例,其中巴拿马城的废物产生被引用。就巴拿马而言,一些研究强调了在物流领域为该国创造增值服务的重要性,从而支持中轴线,即保持巴拿马作为美洲物流中心的地位[2、][3]上述可以应用于一种增值服务,如逆向物流,即产品的回收和再利用或再制造,甚至回收。本文将利用逆向物流网络对家庭产生的废弃物进行管理,称之为普通家庭,作为一个以可回收废弃物为出发点,其中心点在未来的分拣中心位置,并将其分配到回收中心进行处理。目前,巴拿马还没有一个综合的可回收废物分类网络,供以后的回收利用是市议会由民众自愿开展的活动。这项研究的目的是捕捉一种工具的使用,这种工具可以支持利益相关者在战略规划领域的决策,从而在长期内获得更好的结果。因此,将实施从运筹学领域导出的数学模型,称为离散位置,而网络由客户区域构成,作为家庭可回收废物产生的供应点,设施的候选位置点,以适应特定任务,需求点是这些可回收废物的最终目的地。同时,该模型将被塑造成一个处理离散量即非连续值和连续值的混合整数规划模型。它将在名为Pul P[4]的优化问题工具下解决,这是一个开源编程包,包含在名为Python的计算资源库中。该模型有助于确定可回收废物网络中设施和产品流的最佳位置。利用Python计算工具分析了容量的伸缩性。本文以巴拿马市为例,以可回收废弃物分类回收网络为例,提出一个优化模型。

Plasma Citrulline Concentrations in Neonates and Infants with or without Gastrointestinal Disease and Bowel Resection

Various studies have shown a role for citrulline as a gut mass biomarker in patients with short bowel syndrome. Our hypothesis is that plasma citrulline is both a gastrointestinal (GI) function and a gut mass marker. Our objective was to validate previous observations, by prospectively analyzing plasma citrulline concentrations in patients with GI disease with or without bowel resection, compared to patients without GI disease. Plasma from blood samples of parenteral nutrition fed neonates and infants was obtained. Samples were analyzed by ion-exchange chromatography. Data collected included age, diagnoses and surgical documentation of bowel resection. Patients were classified into 3 main groups: those without GI disease nor resection (Group 1), those with GI disease but no resection (Group 2), and those with GI disease and resection (Group 3). Group medians were compared using Kruskal-Wallis ANOVA. Seventeen samples were evaluated. Patients in Group 3 were older compared to patients in Groups 1 and 2;median age (in days) 156 vs. 12 vs. 57 respectively. Median (range) plasma citrulline concentrations were 20.9 (14.9 - 29.0) μmol/L, 8.7 (0.5 - 20.0) μmol/L and 9.6 (5.9 - 13.2) μmol/L for Groups 1, 2 and 3 respectively. There were significant differences among medians and sample distributions between Groups 1 and 2 and between 1 and 3 (p < 0.05). No differences were observed between Groups 2 and 3. Patients without GI disease and no resection had significantly higher plasma citrulline concentrations than patients with GI disease with or without resection at the time of assessment.

Applications of finite element simulation in orthopedic and trauma surgery

Research in different areas of orthopedic and trauma surgery requires a methodology that allows both a more economic approach and the ability to reproduce different situations in an easy way. Simulation models have been introduced recently in bioengineering and could become an essential tool in the study of any physiological unity, regardless of its complexity. The main problem in modeling with finite elements simulation is to achieve an accurate reproduction of the anatomy and a perfect correlation of the different structures, in any region of the human body. Authors have developed a mixed technique, joining the use of a three-dimensional laser scanner Roland Picza captured together with computed tomography(CT) and 3D CT images, to achieve a perfect reproduction of the anatomy. Finite element(FE) simulation lets us know the biomechanical changes that take place after hipprostheses or osteosynthesis implantation and biological responses of bone to biomechanical changes. The simulation models are able to predict changes in bone stress distribution around the implant, so allowing preventing future pathologies. The development of a FE model of lumbar spine is another interesting application of the simulation. The model allows research on the lumbar spine, not only in physiological conditions but also simulating different load conditions, to assess the impact on biomechanics. Different degrees of disc degeneration can also be simulated to determine the impact on adjacent anatomical elements. Finally, FE models may be useful to test different fixation systems, i.e., pedicular screws, interbody devices or rigid fixations compared with the dynamic ones. We have also developed models of lumbar spine and hip joint to predict the occurrence of osteoporotic fractures, based on densitometric determinations and specific biomechanical models, including approaches from damage and fracture mechanics. Microarray technology is a powerful tool to investigate whole genome expression profiles to study the crosstalk between pathogens and associated hosts that cause illness. Microarrays have been used in several comparative genome hybridization studies of pathogens. In addition to detection and identification of pathogens, microarrays are ideal for characterizing genetic differences between bacteria isolates of the same species to the strain level. Furthermore, the use of microarrays has been gaining importance in the detection of viral agents. Here we explore the use of microarrays for simultaneous detection of viruses and modifications made over these techniques. Microarray technology has also been incorporated to compensate for time-consuming sequencing. The procedure of fungi identification based on sequences and studies reported the use of microarrays to identify pathogenic yeasts and molds by targeting the internal transcribed spacer regions in fungal rRNA genes. The remarkable advancement in genomics over the last decade has made it possible for microarray technology to evolve towards being the best option for clinical diagnostics because they have several advantages.

BACKGROUND Meckel's diverticulum(MD) occurs predominantly in children and adolescents. It is rarely diagnosed in adults. Preoperative diagnosis is difficult due to low sensitivity of the radiological imaging studies. The role of video capsule endoscopy(VCE) in the diagnosis of MD is unknown, and the endoscopic patterns are not defined. We will describe four of our cases of MD evaluated with VCE and make a review of the literature focusing on the endoscopic *** SUMMARY We present four cases of MD confirmed by surgery. They were all adult males with ages going from 18 to 50 years, referred to our service from 2004 to 2018, due to obscure gastrointestinal bleeding(OGIB). Theyhad a history of 1 mo to 10 years of overt and occult bleeding episodes. Laboratory blood test showed an iron-deficiency anemia from 4 to 9 g/dL of hemoglobin that required multiple hospitalizations and blood transfusions in all cases. Repeated upper digestive endoscopies and colonoscopies were negative. Small bowel was examined with VCE, which revealed double lumen images in all cases, one with polyps and three with circumferential ulcers in the diverticulum. However, based on VCE findings, preoperative diagnosis of MD was suggested only in two patients. Capsule was retained in one patient, which was recovered with surgery. The anatomopathological report revealed ulcerated ectopic gastric mucosa in all *** VCE is useful for the diagnosis of MD. However, endoscopic characteristics must be recognized in order to establish preoperative diagnosis.
