Introduction: RRD (Rhegmatogenous Retinal Detachment) is a separation between the neuroepithelium and the pigment epithelium due to the passage of fluid through a retinal dehiscence. It constitutes a major ophthalmologic emergency. Its management is primarily surgical, either through external or internal approaches, with tamponade using gas or silicone oil. The purpose of this study was to report the various complications associated with the use of silicone oil in vitreoretinal surgery. Patients et methods: We conducted a retrospective, descriptive, and analytical study from October 1, 2020, to October 31, 2023, which included all patients who underwent surgery for RRD using three-port vitrectomy (3PV) with tamponade using 1000 centistoke silicone oil (Group 1) and 5000 centistoke silicone oil (Group 2). All patients underwent a complete ophthalmologic examination and were operated on by the same surgeon. Data analysis was performed using Excel software. Results: Overall, 31 patient files representing 33 eyes were collected, with a mean age of 48.83 years and a sex ratio of 4.16. Group 1 consisted of 16 eyes (48.48%), and Group 2 consisted of 17 eyes (51.51%). The different complications observed were cataracts in all phakic subjects, accounting for 57.57%;ocular hypertonia in 69.69% (27.27% in Group 1;42.42% in Group 2);anterior chamber silicone oil migration in 24.24% (9.09% in Group 1;15.15% in Group 2);recurrence of retinal detachment in 21.21% (6.06% in Group 1;15.15% in Group 2);and silicone oil emulsification in 24.24% (15.15% in Group 1;9.09% in Group 2). Additionally, there was one case of corneal degeneration in Group 1. Conclusion: Silicone oil is an effective tamponade agent used in the treatment of retinal detachments. Close patient follow-up is necessary due to the complications associated with its use, which can occur either early or late after surgery.

Introduction Acute angle closure crisis (AACC) is a major ophthalmic emergency. It is a sight-threatening pathology that is very common in women but rarely described in melanodermas. The aim of this work is to investigate the epidemiological and clinical aspects of acute angle closure crisis. Results We collected 24 patient records, totaling 25 eyes. The average age of our patients was 52.04 years, with a male-to-female ratio of 0.6. A family history of glaucoma occurred in 34% of patients. Patients consulted for a painful red eye associated or not with decreased visual acuity (DVA). The condition was bilateral in one patient. The average visual acuity wax 0.34/10e. The cornea was cloudy and the pupil in areflexic mydriasis in all sick eyes. Hypothalamia was present in 72% of eyes and cataracts in 54.16% of cases. The average intraocular press was 44.38 mmHg and the average cup/disc (C/D) was 0.46. Gonioscopy was performed in 7 patients. The fellow eye did not present any abnormalities in 92% of cases. Conclusion The acute angle closure crisis is a rare condition in melanodermas and common in woman. His diagnostic is essentially clinical and completed by gonioscopy which plays a fundamental role. .

Regional Climate Models in the Simulation of the Drought of the 1970’s and 1980’s Years in Senegal (In West Africa)

West Africa was hit by an unprecedented drought in the 1970’s and 1980’s years, with dramatic consequences for surface and groundwater resources. In the context of climate change, there are many studies for the prediction of the increase in the occurrence of these droughts. To predict this situation in the Senegalese region, it is necessary to use regional climate models, which carrying out the study. This work deals with the interest to examine the capacity of the RCMs (regional climate models) in order to reproduce the deficit on the 1970’s year rainfall in Senegal. In this work, we used daily precipitation data from five (5) regional climate models to characterize the droughts in Senegal by using the SPI (Standardized Precipitation Index) on different time scales (3, 6, 12 and 24 months). For this purpose, the index was calculated over two distinct periods: 1951-1969 and 1970-1990. The results show that the period 1970-1990 was drier than the period 1951-1969. For the zonal average, the results show that the North of Senegal was more affected by this deficit rainfall than the South part. The analysis of the interannual variability of rainfall for some stations in Senegal shows that the drought did not start at the same time throughout the zone.

Eyelid Wounds: Epidemiological, Clinical and Etiological Aspects

Purpose: The aim is to show epidemiological, clinical and etiological characteristics of palpebral wounds. Methodology: This was a retrospective study focusing on patients with an eyelid wound, over a 10-year period from 2012 to 2021. We collected and analyzed the data using Excel. Results: The frequency of eyelid wounds was 0.1%. The average age of our patients was 19.38 years with a range of 1 and 62 years. The sex ratio was 3.7. Eighty-one percent of patients lived in Dakar. Fifty-seven percent (57%) of patients consulted less than 24 hours after the trauma and 7% of patients on D1. The circumstances were brawls (11%), domestic accidents (9%), road accidents (6%), and work accidents (6%). We noted 1 case of dog bite. Thirteen patients presented with post-traumatic decreased visual acuity. Involvement of the isolated upper eyelid was noted in 40% of cases and both eyelids in 15% of cases. Involvement of the lacrimal ducts was noted in 17% of cases, and that of the free edge in 21% of cases. Eyelid wounds were associated with eyeball damage in 21% of cases. Various associated lesions were observed. Ninety-one percent of patients received surgical treatment. Three cases of superinfections, 1 case of conjunctival granuloma and 1 case of phthysis of the eyeball with postoperative retinal detachment were noted. Conclusion: Eyelid sores are relatively common in our context. They require rapid surgical treatment in order to preserve the functional and aesthetic prognosis. .

Totally Occluded Coarctation of the Aorta in a Young Adult

We report the case of a 20-year-old female patient presenting with a totally occluded coarctation of the aorta. The patient was admitted for the evaluation of resistant grade III high blood pressure. The physical examination revealed a blood pressure gradient between the upper and lower limbs, absent femoral pulses, a diffuse continuous murmur over the chest, and hyperpulsatility of the carotid arteries and sternal notch. A transthoracic echocardiography outlined a narrowing in the caliber of the isthmic aorta without acceleration of flow or gradient. Thoracic CT angiography revealed a complete aortic coarctation with interruption of continuity between segment 3 and the descending aorta.

Introduction: The acronym SAPHO (synovitis, Acne, Pustulosis, Hyperostosis and Osteitis) is a syndrome combining osteoarticular and cutaneous manifestations. It occurs mainly between the ages of 30 and 50. Sternocostoclavicular hyperostosis is one of the main distinguishing features. We report a case of SAPHO in Dakar diagnosed by bone scintigraphy. Observation: 28-year-old Senegalese women presented with left shoulder pain and relative functional impotence for over 2 years. Examination revealed right sternoclavicular hyperostosis and left shoulder pain on palpation. Questioning revealed a history of acne and hyperostosis of the right first toe. Bone scintigraphy, performed after injection of 630 MBq of 99mTc-HMDP, revealed: hyperfixation of the bilateral (right++) manubrio-sternal and sternoclavicular junction, producing the classic bull’s horn image;hyperfixation of the left shoulder with an inflammatory appearance;hyperfixation of the sacroiliac joints suggestive of bilateral sacroiliitis;hyperfixation of the right first toe;two mandibular hyper fixations probably related to dental damage. This scintigraphic appearance in one was strongly suggestive of SAPHO syndrome. Conclusion: SAPHO syndrome, related to spondyloarthropathy, associates cutaneous and osteoarticular signs. It is characterized by frequent delays in diagnosis due to poor recognition. Soy is an invaluable diagnostic tool, enabling us to assess the extent of the disease and its evolution.

Introduction: Sinonasal polyposis (SNP) is a chronic inflammatory disease of the mucosa of the nasal cavities and facial sinuses. It is characterized by an oedematous, multifocal and bilateral degeneration of the nasosinus mucosa, which originates in the lateral masses of the ethmoid, where it causes the formation of smooth, gelatinous, translucent and pyriform polyp lesions. The objective of this study was to review epidemiological, clinical, paraclinical data and evaluate the results of endoscopic surgical treatment. Patients and Methods: This is a retrospective study on 60 patients followed at the ENT department of the Fann National University Hospital Center, from January 2010 to December 2015. All patients with sinonasal polyposis were included in the study. Results: The average age of our patients was 38 years and the sex ratio (M/F) was 0.8. In the patients’ histories, we found 18% asthma and 10% Widal’s disease. The average consultation time was 8.5 years. All patients had consulted for nasal obstruction;rhinorrhea was bilateral and found in 67.7% of cases, with olfactory disorders accounting for 50%. The CT scan performed in 58% of cases made it possible to specify the extent of the lesions;the involvement of the ethmoidal sinus was constant and extended to the other sinuses except in 2 cases. All patients had received medical treatment with local corticosteroids. Endoscopic surgical treatment was initiated in 43% of cases after failure of corticosteroid-based medical treatment. The evolution under treatment marked by the reappearance of symptoms that increased each month. At one month postoperatively, all clinical symptomatology had improved with the exception of olfactory disorders, which persisted in 3 patients. At 12 months we noted 12 cases of reappearance of nasal obstruction. Conclusion: SNP is a disease of little known etiology. The diagnosis is almost always clinical. Introduction: Graves’ disease is the most common cause of hyperthyroidism. Its treatment uses synthetic antithyroid drugs but the use of aggressive radical therapy such as surgery or non-aggressive therapy such as iodine-131 is not uncommon. Treatment of Graves’ disease with radioactive iodine or iratherapy is a simple, inexpensive, well-tolerated treatment. It was introduced in Senegal in 2016. We report through this work the preliminary assessment of the only nuclear medicine service in Senegal in the management of Graves’ disease by iodine-131. Patients and Methods: Retrospective study of the first cases of Graves’ disease treated with iratherapy in Senegal. Socio-demographic, clinical, paraclinical, therapeutic and evolutionary aspects were studied. Radiation protection rules have been implemented and contraception has been effective for six months in women of childbearing age. Results: 25 patients were collected with a mean age of 45 years, twenty women (80%), a family goiter in 24% and a psycho-affective context in 64% of cases. Thyrotoxicosis syndrome was associated with goiter in 68% of patients and exophthalmos in 64%. Thyroid ultrasound performed in 20 patients showed vascular goiter in 80% and thyroid scintigraphy in 3 patients, homogeneous and diffuse hyperfixation. TRAK dosed in 8 patients was still positive. All patients had received first-line medical treatment. The average duration of this treatment was more than 18 months in 92%. The empirically used iodine-131 activity averaged 15.35 mCi. Oral corticosteroid therapy was prescribed in 7 patients for the prevention of malignant orbitopathy. No early side effects were noted. The remission rate at 3 months was 52% and at 6 months was 88% to 92%. Conclusion: The effectiveness of radioactive iodine, in particular ablative doses in the treatment of hyperthyroidism, is no longer to be demonstrated. Taking into account our socioeconomic context, iratherapy should be a treatment of choice for hyperthyroidism

Comparative Evaluation of the Chemical Composition and Physical Properties of Reinforced Concrete Steel Bars Used in Construction in Senegal

This article presents, the study of a comparative evaluation of the chemical composition and physical properties, linear mass deviations, of four (04) types of steel used in the construction sector in Senegal. Type 1 (E1), Type 2 (E2) and Type 3 (E3) steels are produced by locally established companies and Type 4 (E4) witness bars are imported from the France. The chemical analyses of the different types of steel were carried out by combustion, infrared (IR) detection for carbon and sulfur, by reducing fusion for nitrogen and by optical emission spectrometer (SEO) for the rest of the elements. The composition was determined on bars with a diameter of 10 mm. Linear mass deviations were evaluated for steels with a diameter of 8 mm, 10 mm and 12 mm. The results of the chemical analyses showed that the limit value for the percentage of carbon was exceeded by 29.16% for the steel, type 3. For the other types (1, 2 and 4), the limit values set out in the French standard NF EN 10,080 are not exceeded. As regards the relative differences in mass, the results showed that for steels of local manufacture, all the samples of bars with diameters 10 and 12 mm and 33% of steels with diameters 8 mm do not comply with the standard. The results also indicate that the chemical composition and relative linear mass deviations of the steels, type 4 comply with the standard. Thus, locally manufactured steels are not always suitable for use in reinforced concrete constructions.
