Effectiveness of overuse injury prevention programs on upper extremity performance in overhead youth athletes:A systematic review

Several targeted upper extremity injury prevention programs have been developed to mitigate the risk of upper extremity overuse injuries among youth athletes in overhead sports;however,their effectiveness on performance outcome measures has not been investigated.This systematic review evaluated the effectiveness of existing upper extremity injury prevention programs that focused on modifying intrinsic risk factors,and performance outcome measures in overhead youth athletes.The secondary aim was to identify the training components of these programs.PubMed,Physiotherapy Evidence Database(PEDro),SPORTDiscus(via EBSCOhost),and Web of Science were searched from January 2000 to November 2020 for studies that implemented training programs or exercises for upper extremity injury prevention among youth athletes in overhead throwing or striking sports.An updated search was conducted from December 2020 to October 2022.A program was deemed effective for a performance outcome measure if significant improvements were observed in the intervention group as compared to the control group.Of the 1394 studies identified,five studies met the inclusion criteria.The effectiveness of the injury prevention programs on the identified performance outcome measures of strength,mobility,and sport-specific measures were 30.4%,28.6%,and 22.2%,respectively.The training components targeted were strength,mobility,and plyometrics.Strength was the most common training component and was also the most widely investigated performance outcome measure.Overall,current upper extremity injury prevention programs seem effective at improving performance outcome measures of strength,mobility,and sport-specific outcomes with training components of strength,mobility and plyometrics.Standardized protocols are required for the measurement and reporting of performance outcomes measures,and the reporting of training components.

Seismic fragility analysis of bridges by relevance vector machine based demand prediction model

A relevance vector machine(RVM)based demand prediction model is explored for efficient seismic fragility analysis(SFA)of a bridge structure.The proposed RVM model integrates both record-to-record variations of ground motions and uncertainties of parameters characterizing the bridge model.For efficient fragility computation,ground motion intensity is included as an added dimension to the demand prediction model.To incorporate different sources of uncertainty,random realizations of different structural parameters are generated using Latin hypercube sampling technique.Mean fragility,along with its dispersions,is estimated based on the log-normal fragility model for different critical components of a bridge.The effectiveness of the proposed RVM model-based SFA of a bridge structure is elucidated numerically by comparing it with fragility results obtained by the commonly used SFA approaches,while considering the most accurate direct Monte Carlo simulation-based fragility estimates as the benchmark.The proposed RVM model provides a more accurate estimate of fragility than conventional approaches,with significantly less computational effort.In addition,the proposed model provides a measure of uncertainty in fragility estimates by constructing confidence intervals for the fragility curves.

Effect of non-steroidal anti-inflammatory drugs on fracture healing in children:A systematic review

BACKGROUND Non-steroidal anti-inflammatory drugs(NSAIDs)are among the most commonly prescribed medications in the United States.Although they are safe and effective means of analgesia for children with broken bones,there is considerable variation in their clinical use due to persistent concerns about their potentially adverse effect on fracture healing.AIM To assess whether NSAID exposure is a risk factor for fracture nonunion in children.METHODS We systematically reviewed the literature reporting the effect of NSAIDs on bone healing.We included all clinical studies that reported on adverse bone healing complications in children with respect to NSAID exposure.The outcomes of interest were delayed union or nonunion.Study quality was assessed using the Newcastle-Ottawa scale for non-randomized studies.A final table was constructed summarizing the available evidence.RESULTS A total of 120 articles were identified and screened,of which 6 articles were included for final review.Nonunion in children is extremely rare;among the studies included,there were 2011 nonunions among 238822 fractures(0.84%).None of the included studies documented an increased risk of nonunion or delayed bone healing in those children who are treated with NSAIDs in the immediate post-injury or peri-operative time period.Additionally,children are likely to take these medications for only a few days after injury or surgery,further decreasing their risk of adverse side-effects.CONCLUSION This systematic review suggests that NSAIDS can be safely prescribed to pediatric orthopaedic patients absent other contraindications without concern for increased risk of fracture non-union or delayed bone healing.Additional prospective studies are needed focusing on higher risk fractures and elective orthopaedic procedures such as osteotomies and spinal fusion.

Factors associated with subsequent surgery after septic arthritis of the knee in children

BACKGROUND Septic arthritis of the knee in children is a challenging problem.Surgical debridement is an established treatment,but there is a paucity of literature on long-term prognosis.AIM To determine the rates and factors associated with return to surgery(RTS)and readmission after index surgical debridement for septic arthritis of the knee in children.METHODS This is a retrospective cohort study that utilizes data from the Healthcare Cost and Utilization Project(HCUP).We included patients between ages 0 to 18 years that underwent surgical debridement for septic arthritis of the knee between 2005 and 2017.Demographic data included age,gender,race,hospital type and insurance type.Clinical data including index admission length of stay(LOS)and Charlson Comorbidity Index(CCI)were available from the HCUP database.Descriptive statistics were used to summarize the data and univariate and multivariate analyses were performed.RESULTS Nine-hundred thirty-two cases of pediatric septic knee were included.This cohort was 62.3%male,with mean age of 9.0(±6.1)years.Approximately 46%of patients were white and approximately half had Medicaid insurance.Thirty-six patients(3.6%)required RTS at a minimum of 2 year after index surgery,and 172 patients(18.5%)were readmitted at any point.The mean readmission LOS was 11.6(±11.3)d.Higher CCI was associated with RTS(P=0.041).There were no significant associations in age,gender,race,insurance type,or type of hospital to which patients presented.Multivariate analysis showed that both increased CCI(P=0.008)and shorter LOS(P=0.019)were predictive of RTS.CONCLUSION Septic arthritis of the knee is an important condition in children.The CCI was associated with RTS at a minimum of 2 years after index procedure.No association was found with age,gender,race,insurance type,or hospital type.Shorter LOS and CCI were associated with RTS in multivariate analysis.Overall,risk of subsequent surgery and readmission after pediatric septic knee arthritis is low,and CCI and shorter LOS are predictive of RTS.

Evaluation of Energy Efficiency Performance of Heated Windows

Fenestration systems are widely used across the world.There is expansive research on window configurations,frames,and glazing technology,but not enough research has been published on reducing window heat loss through heat application to a pane.The presented study attempted to evaluate the performance of heated windows by developing an experimental setup to test a window at various temperatures by varying the power input to thewindow.Heated double pane window was installed in an insulated box.Atemperature gradient was developed across the window by cooling one side of the window using gel-based ice packs.The other face of the window was heated by enabling power at different wattages through the window.The temperature of the inside and outside panes,current and voltage input,and temperature of the room and box were recorded.The data was used to calculate the apparent effective resistance of the window when not being heated vs.when being heated.The study concluded that,when window temperature was maintained close to the room temperature,the heated double pane window is effective in reducing heat loss by as much as 50%as compared to a non-heated double pane window.When temperature of the window was much higher than the room temperature,the heat loss through the window increased beyond that of a non-heated window.The issues encountered during the current stages of experiments are noted,and recommendations are provided for future studies.

Cardio Vascular disease(CVD),involving the heart and blood vessels is one of the most leading causes of death throughout the world.There are several risk factors for causing heart diseases like sedentary lifestyle,unhealthy diet,obesity,diabetes,hypertension,smoking and consumption of alcohol,stress,hereditary factory etc.Predicting cardiovascular disease and improving and treating the risk factors at an early stage are of paramount importance to save the precious life of a human being.At present,the highly stressful life with bad lifestyle activities causes heart disease at a very young age.The main aim of this research is to predict the premature heart disease based on machine learning algorithms.This paper deals with a novel approach using the machine learning algorithm for predicting the cardiovascular disease at the premature stage itself.Support Vector Machine(SVM)is used for segregating the CVD patients based on their symptoms and medical observation.The experimentation results by using the proposed method will facilitate the medical practitioners to provide suitable treatment for the patients on time.A sophisticated model has been developed with the current approach to examine the various stages of CVD and the performance metrics used have given effective and fruitful results as compared to other machine learning techniques.

As far as the present state is concerned in detecting the behavioral pattern of humans(subject)using morphological image processing,a considerable portion of the study has been conducted utilizing frontal vision data of human faces.The present research work had used a side vision of human-face data to develop a theoretical framework via a hybrid analytical model approach.In this example,hybridization includes an artificial neural network(ANN)with a genetic algorithm(GA).We researched the geometrical properties extracted from side-vision human-face data.An additional study was conducted to determine the ideal number of geometrical characteristics to pick while clustering.The close vicinity ofminimum distance measurements is done for these clusters,mapped for proper classification and decision process of behavioral pattern.To identify the data acquired,support vector machines and artificial neural networks are utilized.A method known as an adaptiveunidirectional associative memory(AUTAM)was used to map one side of a human face to the other side of the same subject.The behavioral pattern has been detected based on two-class problem classification,and the decision process has been done using a genetic algorithm with best-fit measurements.The developed algorithm in the present work has been tested by considering a dataset of 100 subjects and tested using standard databases like FERET,Multi-PIE,Yale Face database,RTR,CASIA,etc.The complexity measures have also been calculated under worst-case and best-case situations.

Effect of PAIP1 on the metastatic potential and prognostic significance in oral squamous cell carcinoma

Poly Adenylate Binding Protein Interacting protein 1(PAIP1) plays a critical role in translation initiation and is associated with the several cancer types. However, its function and clinical significance have not yet been described in oral squamous cell carcinoma(OSCC) and its associated features like lymph node metastasis(LNM). Here, we used the data available from Gene Expression Omnibus(GEO), The Cancer Genome Atlas(TCGA), and Clinical Proteomic Tumor Analysis Consortium(CPTAC) to analyze PAIP1 expression in oral cancer. The publicly available data suggests that PAIP1 mRNA and protein levels were increased in OSCC. The high PAIP1 expression was more evident in samples with advanced stage, LNM, and worse pattern of invasion. Moreover, the in vitro experiments revealed that PAIP1 knockdown attenuated colony forming, the aggressiveness of OSCC cell lines, decreasing MMP9 activity and SRC phosphorylation. Importantly, we found a correlation between PAIP1 and pSRC through the analysis of the IHC scores and CPTAC data in patient samples. Our findings suggest that PAIP1 could be an independent prognostic factor in OSCC with LNM and a suitable therapeutic target to improve OSCC patient outcomes.
