Nearly 80% of patients with pancreatic ductal adenocarcinoma(PDAC)develop cachexia along their disease *** is characterized by progressive weight loss,muscle wasting,and systemic inflammation and has been linked to poorer outcomes and impairments in quality of *** of PDAC cachexia has historically involved a multidisciplinary effort comprised of nutritional support,pancreatic enzyme replacement therapy,and/or pharmacologic *** current interventions to mitigate PDAC cachexia,a significant proportion of patients continue to die from complications associated with cachexia underscoring the need for novel insights and treatments for this *** highlight the feasibility and effectiveness of a recent enteral feeding prospective trial at our institution to improve cachexia outcomes in patients with advanced ***,we were among the first to characterize the stool microbiome composition in patients with advanced PDAC receiving enteral feeding for the treatment of *** insights into the relationship between enteral nutritional support,cachexia,and the gut microbiome are *** promising results are discussed in the context of a potential ability to modulate the stool microbiome as a new interventional strategy to mitigate PDAC cachexia.

Do Community-Based Programs Help to Improve HIV Treatment and Health Outcomes? A Review of the Literature

Increased availability of antiretroviral treatment for HIV makes the goal of universal access attainable. However, in most resource constrained settings where existing health systems are?largely dysfunctional, major barriers to achieving this goal remain. While treatments with antiretroviral drugs (ART) are the focal point of HIV management, it is increasingly recognized that ART alone will not be sufficient to adequately deal with the lifelong consequences of the disease. In addition, the current global economic downturn will continue to impact on funding for HIV care and support, making the search for sustainable solutions more urgent. This article reviews the current evidence base on the impact of community-based programs on HIV/AIDS treatment and general health outcomes, as well as their contribution to ensuring sustained care for HIV-positive people. Our findings suggest that these programs overall have a positive impact on various dimensions of HIV treatment and care, and make a significant contribution to health and HIV outcomes. The authors argue that better use of community platforms for HIV treatment and care programs could be critical for attaining desired goals and should be prioritized at all levels of program design, implementation, and monitoring and evaluation.

Botanic Garden Collections—An Under-Utilised Resource

Botanic gardens and arboreta around the world are repositories of diverse collections of useful plants in their gardens and seed banks. However, the crop and forestry communities often overlook these collections, and so they are an underutilised resource. For example, in analysis of the ex situ conservation status of 6,941 socio-economically important plant taxa using data from forestry and crop collections, but omitted collections in botanic gardens and arboreta. In this paper, we compared the socio-economically important taxa identified by Khoury et al. from GRIN-Global World Economic Plants (WEP) with data on living and seed collections held in botanic garden and arboreta, as recorded in BGCI’s global PlantSearch and ThreatSearch datasets. This analysis produced an assessment of the proportion of these taxa that are found in botanic gardens and arboreta, the number of gardens or arboreta they are found in and what potential they have to contribute to research, conservation and sustainable use. We also compared the species conservation comprehensiveness assessments carried out by Khoury et al. with the threatened status of those species, according to the IUCN Red List and other threat assessment methodologies in order to ascertain whether threatened, useful species are well-conserved in botanic gardens. At least 6017 of the 6941 socio-economically important WEP taxa (86.7%) were currently found in the living and seed collections of botanic gardens and arboreta with 1456 taxa (21%) held in >40 collections. Khoury et al. suggested that 6748 of the WEP taxa are of either medium or high conservation priority. However, our analysis suggested that just 1153 taxa have been assessed as threatened at a national or international level. To understand and engineer plant metabolism, we need a comprehensive and accurate annotation of all metabolic information across plant species. As a step towards this goal, we generated genome-scale metabolic pathway databases of 126 algal and plant genomes, ranging from model organisms to crops to medicinal plants(https://***).Of these, 104 have not been reported *** systematically evaluated the quality of the databases, which revealed that our semi-automated validation pipeline dramatically improves the quality. We then compared the metabolic content across the 126 organisms using multiple correspondence analysis and found that Brassicaceae,Poaceae, and Chlorophyta appeared as metabolically distinct groups. To demonstrate the utility of this resource, we used recently published sorghum transcriptomics data to discover previously unreported trends of metabolism underlying drought tolerance. We also used single-cell transcriptomics data from the Arabidopsis root to infer cell typespecific metabolic pathways. This work shows the quality and quantity of our resource and demonstrates its wide-ranging utility in integrating metabolism with other areas of plant biology.

Quantitative Evaluation of Commercially Available Test Kit for Ciguatera in Fish

Reliability of the commercially available Cigua-Check? test kits to identify ciguateric fish was evaluated by assessing the uniformity of conclusions by multiple readers examining identical sticks. One hundred and eighty-eight samples of two types of reef fish were tested for ciguatera using Cigua-Check? test strips. Paired subsamples of all specimens were also analyzed via the more rigorous N2a neuroblastoma mouse bioassay that is specific for aberration of Na+-channel activity that is presumptive for ciguatera fish poisoning. In a double blind trial, four individuals visually examined identical Cigua-Check? strips to conclude whether the samples were positive or negative for ciguatera. Of the 121 samples that were shown to be positively ciguatoxic via bioassay, the four strip readers were in agreement in concluding positive ciguateric status in 9.9% of the samples;these four readers concluded these positive samples were negative for ciguatera in 26.2% of these bioassay-positive samples. Of the 67 samples that were shown in N2a bioassay to be negatively ciguatoxic, the four readers were in agreement in concluding a negative ciguateric status in 26.9% of the samples;these four strip readers concluded these same samples were positive for ciguatera in 14.9% of these negative samples. The low levels of uniform conclusions among readers examining identical test strips, and the low frequency of agreement with the correct ciguateric status as shown by separate N2a neuroblastoma analyses indicate severe short-comings in the reliability of these test kits to accurately reflect the ciguateric status of samples. The level of uncertainty associated with Cigua-Check? test kits indicate a continuing need for improvement of a simple, rapid, and reliable means to identify ciguateric fish.

AIM: To identify potential risk indicators for emerging adults transitioning to adult health care. BACKGROUND: Without maturity, independence, knowledge and motivation to manage their diabetes and successfully transition to adult healthcare, a proportion of emerging adults will struggle, leaving themselves vulnerable to diabetes-related complications. METHODS: Fifty-three emerging adults (aged 17 - 19 years) recently transitioned from pediatric to adult healthcare were recruited. Data included demographic, glycated haemoglobin, Body Mass Index, base-line and 12-month data from four psychosocial measures: Problem Areas in Diabetes, Diabetes Empowerment Scale, Hospital Anxiety and Depression Scale and dichotomous questions from the Eating Attitude Test. Missed appointments were obtained from hospital records. RESULTS: No significant differences in age, gender, BMI and individual survey scales between three appointment groups. Median HbA1c in the Sat-1 group was 7% (53 mmol/mol), compared with 8.6% (70 mmol/mol) and 8.5% (69 mmol/mol) respectively in the Sat-2 and Unsatisfactory groups. A ROC curve analysis and classification tree analysis identified optimal threshold values for the survey scales and their linear combinations. These values were used to make comparisons across two appointment and three HbA1c status groups. The unsatisfactory appointment group was characterised by statistically higher proportions of participants with (1) a low score for empowerment alone or in combination (less) with perceived problems and (2) statistically higher perceived problems and anxiety scores. CONCLUSIONS: Findings suggest that diabetes self-care confidence, less perception of diabetes as a problem, could be a useful indicator of future appointment attendance. Similarly, depression levels could be a useful predictor of better metabolic control following transition.
