The Role of Ultrasound and Air Leak Measurement in Assessing Lung Expansion after Thoracic Surgery

Objective: To determine if lung ultrasound semiology is applicable to the post-surgical hemithorax, and its value in the evaluation of lung expansion in the postoperative period when paired with the use of digital chest drain systems. Methods: Prospective observational study including all patients undergoing thoracic surgery from June 2012 to March 2013. Patients undergoing pleurodesis or hemodynamically unstable were not considered candidates. final inclusion in the study was based on the availability of digital chest drain system. A transthoracic lung ultrasound evaluation of the anterior and anterolateral windows was performed 20 minutes after chest drain placement. Presence or not of lung sliding and air leak values taken from the digital chest drain system were recorded. Data were submitted to a binomial classification test for analysis. Results: forty-nine patients were included, yielding a total of 64 hemithoraces. Lung sliding was seen in 53 cases (82.8%), and an air leak value of 20 ml/min or less in 56 cases (87.5%). Sensitivity was 92.8% (95% CI: 82.6% to 97.9%) and specificity 87.5% (95% CI: 47.3% to 97.9%). Positive predictive value was 98.1%, and negative predictive value was 63.6%. Conclusions: Transthoracic lung ultrasound is a useful technique that can complement the use of digital chest drain systems in the evaluation of post-surgical lung expansion. The incorporation of lung ultrasound can greatly reduce the need for chest radiographs in thoracic surgery departments.

A low-grade gold ore from Ilesha-Itagunmodi, south western Nigeria was panned, dried and ball-milled before sieving into fractions. The morphological, mineralogical and chemical composition was studied by optical micro-scopy (Reflected and Transmission), X-ray diffraction (XRD) and Energy Dispersive X-ray fluorescence (XRf), respectively. The sieved fractions were subjected to chemical analysis (AAS). The +106 μm sieve had the highest concentration of the mineral and was then selected for the upgrade through froth flotation using standard reagents. In this paper a report on a 2k factorial experiment that provides an understanding of the impact of operational variables on the quantity of gold mineral obtained from the ore during froth flotation is presented. Analysis results showed that Ilesha-Itagunmodi gold ore is non-refractory with fine grain particles, amenable to froth flotation and contained about 20 other associated minerals, gold had a concentration of about 0.0024%. A com-bination of P-Xanthate and amine glycol collectors at a pH of 9.2 only produced a considerable increase in gold yield. This translated to about 87.13% increase in recovery of gold from the ore. Analysis of variance (ANOVA) was carried out and the model equation obtained was subsequently optimised to obtain a model equation that could be used in predicting the recoverable quantity of gold, indicating that f11,1-values for Collector concentration, frother concentration, pH and Conditioning time were 156.86, 6.96, 43.81, and 56.77 respectively. A model with an f value of 88.41 was obtained which indicated that the model was significant. The model equation obtained was subsequently optimised to be able to predict the recoverable quantity of gold. A “Pred R-Squared” value of 0.9365 (93.65%) was also obtained and is in reasonable agreement with the “Adj R-Squared” value of 0.9534 (95.34%). It was established that Ilesha placer gold ore is amenable to froth flotation using standard reagents.

Meplazumab,a humanized CD147 antibody,has shown favourable safety and efficacy in our previous clinical *** DEfLECT(NCT04586153),167 patients with severe COVID-19 were enroled and randomized to receive three dosages of meplazumab and a *** at 0.12 mg/kg,compared to the placebo group,showed clinical benefits in significantly reducing mortality by 83.6%(2.4%vs.14.6%,p=0.0150),increasing the proportion of patients alive and discharged without supplemental oxygen(82.9%vs.70.7%,p=0.0337)and increasing the proportion of patients who achieved sustained clinical improvement(41.5%vs.31.7%).The response rate in the 0.2 mg/kg group was relatively increased by 16.0%compared with the placebo group(53.7%vs.46.3%).Meplazumab also reduced the viral loads and multiple cytokine *** with the placebo group,the 0.3 mg/kg significantly increased the virus negative rate by 40.6%(p=0.0363)and reduced IL-8 level(p=0.0460);the 0.2 mg/kg increased the negative conversion rate by 36.9%,and reduced IL-4(p=0.0365)and IL-8 levels(p=0.0484).In this study,the adverse events occurred at a comparable rate across the four groups,with no unexpected safety findings *** conclusion,meplazumab promoted COVID-19 convalescence and reduced mortality,viral load,and cytokine levels in severe COVID-19 population with good safety profile.

Purpose： The present study investigated the effects of 16 weeks of small-volume, small-sided soccer training soccer group （SG, n = 13） and oscillating whole-body vibration training vibration group （VG, n = 17） on body composition, aerobic fitness, and muscle PCr kinetics in healthy inactive premenopausal women in comparison with an inactive control group （CO, n = 14）. Methods： Training for SG and VG consisted of twice-weekly 15-min sessions with average heart rates （HRs） of ～ 155 and 90 bpm respectively. Pre- and post-measurements of body composition （DXA）, phosphocreatine （PCr） on- and off-kinetics, and HR measurements during stand- ardised submaximal exercise were performed. Results： After 16 weeks of training in SG, fat percentage was lowered （p = 0.03） by 1.7%±2.4% from 37.5% ± 6.9% to 35.8% ± 6.2% and the PCr decrease in the quadriceps during knee-extension ramp exercise was attenuated （4% ± 8%, p = 0.04）, with no changes in VG or CO （time- group effect： p=0.03 and p = 0.03）. Submaximal exercise HR was also reduced in SG after 16 weeks of training （6%± 5% of HRmax, p = 0.01）. Conclusion： Short duration soccer training for 16 weeks appears to be sufficient to induce favourable changes in body composition and indicators of aerobic fitness and muscle oxidative capacity in untrained premenopausal women.
