Serial changes in expression of functionally clustered genes in progression of liver fibrosis in hepatitis C patients

AIM: To investigate the relationship of changes in expression of marker genes in functional categories or molecular networks comprising one functional category or multiple categories in progression of hepatic fibrosis in hepatitis C (HCV) patients. METHODS: Marker genes were initially identified using DNA microarray data from a rat liver fibrosis model. The expression level of each fibrosis associated marker gene was analyzed using reverse transcription-polymerase chain reaction (RT-PCR) in clinical biopsy specimens from HCV-positive patients (n = 61). Analysis of changes in expression patterns and interactions of marker genes in functional categories was used to assess the biological mechanism of fibrosis. RESULTS: The profile data showed several biological changes associated with progression of hepatic fibrosis. Clustered genes in functional categories showed sequential changes in expression. Several sets of clustered genes, including those related to the extracellular matrix (ECM), inflammation, lipid metabolism, steroid metabolism, and some transcription factors important for hepatic biology showed expression changes in the immediate early phase (F1/F2) of fibrosis. Genes associated with aromatic amino acid (AA) metabolism, sulfur-containing AA metabolism and insulin/ Wnt signaling showed expression changes in the middle phase (F2/F3), and some genes related to glucose metabolism showed altered expression in the late phase of fibrosis (F3/F4). Therefore, molecular networks showing serial changes in gene expression are present in liver fibrosis progression in hepatitis C patients. CONCLUSION: Analysis of gene expression profiles from a perspective of functional categories or molecular networks provides an understanding of disease and suggests new diagnostic methods. Selected marker genes have potential utility for biological identification of advanced fibrosis.

Vicissitude of Curetted Adenoid Vegetations

Weights of curetted adenoid were measured and were compared with both weights of tonsils and the rate of adenoidectomy among the tonsillectomized cases. This study included 603 patients whose adenoids were curetted during the 11-year period. 90% of patients were 2 to 9 years old. The rate of curetted adenoid vegetation among the tonsillec-tomized cases was 80% among patients from 1 to 6 years old and 70% among patients of 7 and 8 years old. The rate remarkably decreased from 9 years of age. The average weight of the curetted adenoids in each age group ranged from 0.7 g to 1.9 g. There was no statistical correlation in the distribution of the average weight of the curetted adenoids between males and females as well as between the weight of the tonsils and the weight of the curetted adenoids. A hypothesis on the cause of adenoid hypertrophy was presented in this study.

Body mass index correlated with forced expiratory volume in 1 second/forced vital capacity in a population with a relatively low prevalence of obesity

Background Obesity is the most common metabolic disease in the world. However, the relationship between obesity and lung function is not fully understood. Although several longitudinal studies have shown that increases in body weight can lead to reductions in pulmonary function, whether this is the case with the Japanese population and whether high body mass index （BMI） status alone represents an appropriate predictor of obstructive lung dysfunction remains *** purpose of present study was to estimate the effect of BMI on lung function measured by spirometry of Japanese patients in general clinics. We measured BMI and performed spirometry on screening patients who had consulted general *** Subjects comprised 1231 patients ≥40 years of age （mean age （65.0±12.0） years, 525 men, 706 women） who had consulted clinics in Nagasaki Prefecture, Japan, for non-respiratory disease. BMI was calculated and lung function was measured by *** BMI was found to be positively correlated with forced expiratory volume in 1 second （FEV1）/forced vital capacity （FVC） in men and with maximum mid-expiratory flow （MMF） in all subjects. Following adjustment for relevant factors, a significant positive correlation between BMI and FEV1/FVC was identified for all subjects. Comparison between subjects with normal BMI （18.5-25.0） and higher BMI （25.1-30.0） also demonstrated that FEV1/FVC and percentage of predicted maximum mid-expiratory flow （%MMF） were significantly higher in the latter *** In a population without marked respiratory disease, higher BMI subjects showed less obstructive pulmonary dysfunction compared to normal BMI subjects. High BMI status alone may be inappropriate as a predictor of obstructive lung dysfunction, particularly in populations with a low prevalence of obesity.

Tonsils Increase in Weight during Growth

Purpose: During my 40 years of performing tonsillectomy, I have noticed that tonsils seem to increase in size with aging. The purpose was to study the relationship between the weight of tonsils and patients’ age. Subjects and Methods: Tonsillectomies were performed in 1246 patients diagnosed with sleep disorders with hypertrophied tonsils. The excised tonsils were weighed. Results: 1) The number of male tonsillectomy cases was 2.3 times higher than the number of female cases;2) Most tonsillectomies were performed in children three to six years of age, and in adults who were around 30 years old;3) The maximum age of the male patients was 57 years and that of the female patients was 63 years;4) The weight of tonsils increased with age in the males, but the maximum weight of female tonsils was reached at around nine years of age;5) Tonsils of males were heavier and larger than those of females of the same age;6) A high degree of correlation was observed between the weights of the right and left tonsils of a subject. Conclusions: 1) The weight of tonsils increased with growth;2) Tonsils in males were more hypertrophic than those in females;3) The age distribution of the cases suggested that the lifespan of these patients is shorter than the norm;4) The ideal age at which to perform tonsillectomy is thought in two to three years;5) A high degree of corelation eixists between the weight of the right and left tonsils of a subject.

Association between colonic polyps and diverticular disease

AIM: To evaluate the association between colonic polyps and diverticular disease in Japan. METHODS: We retrospectively reviewed the medical records of 672 consecutive patients who underwent total colonoscopy between August 2006 and April 2007 at Nishinjo Hospital, Okinawa, Japan. Patients with a history of any of the following were excluded from the study: previous polypectomy, colonic resection, and inflammatory bowel diseases. The association between colonic polyps and diverticular disease was analyzed by logistic regression analysis, adjusted for age and sex. RESULTS: Prevalence of colonic polyps in all patients with diverticular disease was significantly higher than that in those without diverticular disease (adjusted odds ratio 1.7). CONCLUSION: Our data showed that patients with diverticular disease have a higher risk of colonic polyps compared to those without.
