Machine perfusion versus cold storage of livers： a meta-analysis

Abstract Different organ preservation methods are key factors influencing the results of fiver transplantation. In this study, the outcomes of experimental models receiving donation after cardiac death （DCD） livers preserved through machine perfusion （MP） or static cold storage （CS） were compared by conducting a meta-analysis. Standardized mean difference （SMD） and 95% confidence interval （CI） were calculated to compare pooled data from two animal species. Twenty-four studies involving MP preservation were included in the meta-analysis. Compared with CS preservation, MP can reduce the levels of aspartate aminotransferase （AST）, alanine aminotransferase （ALT）, lactate dehydrogenase （LDH）, and hyaluronic acid （HA） and the changes in liver weight. By contrast, MP can enhance bile production and portal vein flow （PVF）. Alkaline phosphatase （ALP） levels and histological changes significantly differed between the two preservation methods. In conclusion, MP of DCD livers is superior to CS in experimental animals.

Context.There are no reports of the incidence of mercury poisoning in a large population in China.This study investigated the epidemiology,clinical manifestations,and treatment outcomes of Chinese patients with mercury poisoning.Methods.Data for 288 mercury poisoning patients at our hospital were collected from July 2012 to September 2017,including sex,age,admission time,blood mercury content,24-h urinary protein levels,electromyography（EMG） findings,renal biopsy,and follow-up.Patient characteristics were evaluated by statistical and correlation analyses.Results.Mercury poisoning due to occupational exposure was more common in males in the 20-to 50 year-old age group,while female patients aged 31-50 years were more likely to experience cosmetics-related poisoning.Mercury poisoning due to inappropriate use of Chinese folk remedies（CFRs） was observed at all ages regardless of gender.The most common symptoms were neurological（50.3%）,renal（16.4%）,and respiratory（8.0%）.Of the 136 patients without complications for whom follow-up data were available,100 patients showed normal serum mercury concentration upon reexamination range 6 to 12 months after discharge.Of the 13 patients with EMG-confirmed neurological injury,10 showed full recovery after 38.5 ±8.03 months.Of the 18 patients with nephrotic syndrome that had successful follow-up visits,15 had normal urine protein and serum albumin levels and renal function after 22.67±10.26 months.Conclusion.Follow-up visits revealed that various complications of mercury poisoning（nephrotic syndrome and peripheral nerve injury） can be alleviated by sodium dimercaptosulfonate treatment.Regulation of skin-lightening cosmetic products,CFP surveillance,and prevention and control of occupational contact must be improved to decrease the incidence of mercury poisoning in China.

目的探讨后腹腔镜微创手术对局限性原发性肾癌患者免疫与神经内分泌功能的影响。方法选择82例局限性原发性肾癌患者为研究对象,采用随机数字表法将患者分为观察组和对照组,每组41例。观察组患者接受后腹腔镜微创手术治疗,对照组患者接受传统开放式肾癌根治术治疗。记录并比较两组患者的手术时间、术中出血量、术后下床时间和住院时间,分析手术前后两组患者T淋巴细胞亚群指标和神经内分泌激素水平的变化情况。结果观察组患者的术中出血量少于对照组患者,手术时间、术后下床时间和出院时间均短于对照组患者,差异均有统计学意义(P﹤0.05)。术前,两组患者的T淋巴细胞亚群指标与神经内分泌激素水平比较,差异均无统计学意义(P﹥0.05)。术后第1天,两组患者的CD3+、CD4+和胰岛素水平均低于本组术前,皮质醇、促肾上腺皮质激素(ACTH)水平均高于本组术前,且观察组患者的CD3+、皮质醇和ACTH水平均高于对照组,胰岛素水平低于对照组,差异均有统计学意义(P﹤0.05)。术后第5天,观察组患者的CD3+、CD8+水平均低于本组术前,CD4+/CD8+比值高于本组术前,对照组患者的CD3+水平低于本组术前,且观察组患者的CD3+水平、CD4+/CD8+比值和胰岛素水平均高于对照组患者,CD8+水平、皮质醇和ACTH水平均低于对照组,差异均有统计学意义(P﹤0.05)。结论后腹腔镜微创手术治疗局限性原发性肾癌具有出血少、手术创伤小等优点,且对患者免疫功能和神经内分泌功能的影响较小。
