Thioredoxin (Trx) is a small ubiquitous dithiol protein which together with the FADcontaining enzyme thioredoxin reductase (TR) and NADPH (the Trx system) is a hydrogen donor for ribonucleotide reductase essential for DNA synthesis and a general protein disulfide reductase involved in redox regulation. Selenite, selenodiglutathione (GS-Se-SG) and selenocystine are efficiently reduced by thioredoxins and also directly by NADPH and mammalian TR but not by the E. coli enzyme. Incubation of selenite or GS-Se-SG with the Trx system or with mammalian TR results in a rapid formation of selenide, which by redox cycling with oxygen may cause a large non-stoichiometric oxidation of NADPH. Selenocystine is efficiently reduced into two molecules of the selenol amino acid selenocysteine by mammalian TR with a Km-value (6μmol·L-1 ) and a high turnover number (kcat, 3200 min-1) almost identical to the natural substrate Trx-S2. TR also directly reduces lipid hydroperoxides and this peroxidase reaction is strongly stimulated by the presence of catalytic amounts of free selenocysteine. Glutaredoxin (Grx) which catalyzes GSH dependent disulfide reduction also via a redox-active disulfide and Trx are both efficient electron donors to the hut-nan plasrna glutathione peroxidase providing a mechanism by which human plasma glutathione peroxidase may reduce hydroperoxides in an environment almost free from glutathione. Selenate is reduced by Grx and Trx in the presence of GSH. The DNA-binding of the transcription factor AP-1 is strongly inhibited by GS-Se-SG and selenite. Furtherrnore, selenide formed by TR-mediated reduction of selenite and GS-Se-SG inhibits lipoxygenase and changes the electron spin resonance spectrum of the active site iron. Purpose： Subtrochanteric fractures of the femur are being managed successfully with various intramedullary and extramedulary implants with reasonable success. However, these implants require precise placement under image intensifier guidance, which exposes the surgeon to substantial amount of radiation. It also restricts the management of these fractures at peripheral centers where facility of image intensifiers is not available. Keeping this in mind we designed this study to identify if contralateral reversed distal femoral locking plate can be used successfully without the use of image intensifier. Methods： Twenty-four consecutive patients （18 men and 6 women） with a mean age of 28 years （range 19-47 years） suffering subtrochanteric fractures of the femur underwent open reduction and internal fixation with reversed contralateral distal femoral locking plate. The outcome was assessed at the mean follow-up period of 3.2 years （range 2-4.6 years） using the Harris hip score. Results： Twenty-one fractures united with the primary procedure, with a mean time of consolidation being 11 weeks （range, 9-16 weeks）. One patient developed superficial suture line infection, which resolved with oral antibiotics. Another patient had a fall 3 weeks after surgery and broke the plate. Repeat surgery with reversed distal femoral locking compression plate was performed along with bone grafting and the fracture united. Two cases had nonunion, which went in for union after bone grafting. The mean Harris hip score at the time of final follow-up was 90.63 （range 82-97）. Conclusion： The reversed contralateral distal femoral plate is a biomechanically sound implant, which when used for fixation of the subtrochanteric fractures with minimal soft tissue stripping shows results comparable to those achieved by using other extramedullary implants as well as intramedullary devices. The added advantage of this implant is its usability in the absence of an image intensifier.

目的研究慢性鼻-鼻窦炎(CRS)患者行克拉霉素治疗的临床效果.方法选择2019年1月～2019年12月间襄阳市第一人民医院收治的84例CRS患者,采用随机数表法分为观察组(克拉霉素)与对照组(鼻用激素布地奈德)各42例,比较两组白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、血清总IgE(TIgE)、嗜酸粒细胞阳离子蛋白(ECP)水平及有效率.结果与对照组比较,观察组IL-6、TNF-α、TIgE及ECP值较低;观察组有效率高于对照组(P<0.05).结论克拉霉素治疗CRS患者能提高整体疗效、稳定病情,缓解不适,值得临床应用.

Aqueous binary dopant (ZrOCl2/AgI) is used in different ratio such as 1:1, 1:2 and 2:1 (w/w) for chemical doping to enhance the conductivity of synthesized Poly (o-toluidine) (POT). The doping of Poly (o-toluidine) is carried out using tetrahydrofuran as solvent. Doped samples are characterized using various techniques such as I-V characteristics, UV-Visible spectroscopy, X-ray diffractometry (XRD), FTIR and Photoluminescence (PL) studies. A significant enhancement in DC conductivity has been observed with the introduction of binary dopant. UV-Visible study shows that optical parameters change considerably after doping. Interestingly, both direct and indirect band gaps are observed in the doped samples. XRD patterns show the semi-crystalline nature of doped Poly (o-toluidine). FTIR study shows structural modifications in functional groups with doping in POT. A Photolyminescence spectrum exhibits the emission properties of the samples.

Prevalence of Dry Eye and Its Association with Various Risk Factors in Rural Setup of Western Uttar Pradesh in a Tertiary Care Hospital

Purpose: To study the prevalence of dry eye in a hospital based population of rural setup and to evaluate its association with various risk factors. Material and Methods: In this cross sectional study, patients above 20 years of age were screened randomly for dry eye. An 8 points questionnaires, slit lamp examination of meibomian glands, tear film breakup time, fluorescein staining of cornea, schirmer test were used to diagnose dry eye. The diagnosis was made when three of the five parameters were positive. The role of various occupations as well as role of different exposure factors like sunlight, excessive wind, smoking, drugs, and air pollution as dry eye risk factors was accessed. Result: Out of 445, 45.39% patients had dry eye. Dry eye prevalence was higher in those above 70 years of age (74%). It was higher in male population (51.82%) compare to female population (37.37%), nearly equal in rural (46.04%) and urban population (44.31%) and highest among factory workers (90%). Correlation of dry eye with drugs (P = 0.0002), sunlight/high temperature (P = 0.0003) and smoking (P = 0.03) were significant. Conclusion: This is a hospital based study which provides prevalence of dry eye in rural region of western Uttar Pradesh. It is more common in old age male population and significantly higher in factory workers. Out of different modifiable risk factors most important are drugs, sunlight/high temperature and smoking.
