Background: Visceral adiposity correlates strongly with insulin resistance and components of metabolic syndrome. Visfatin is preferentially produced by human visceral adipose tissue. Objective: The objective of this study was to evaluate the relationship between circulating levels of visfatin and other adipocytokines in patients with type 2 diabetes (DM) compared to the age-matched control subjects without DM. Methods: Anthropometric parameters, lipid profiles, high-sensitivity CRP (hs-CRP), insulin resistance [HOMA-IR], and levels of visfatin, TNF-α, and resistin were evaluated in 55 subjects with diabetes and 56 age matched control subjects without diabetes. Results: The visfatin levels were significantly lower in subjects with diabetes (p < 0.0001). Visfatin levels correlated strongly with resistin and CRP. We noted negative correlation of visfatin levels with BMI, triglycerides and glucose. Conclusion: These data suggest that visfatin may be a marker of subclinical inflammatory state.

Outcome of patients with traumatic cranial nerve palsy admitted to a university hospital in Nepal

Background Cranial nerve palsy (CNP) is a common complication of traumatic brain injury (TBI). Despite a high incidence of TBI in Nepal (382 per 100,000), literature on the specific management and outcome of CNP is lacking. This study aimed to examine the outcomes of TBI patients involving single versus multiple *** A retrospective chart review of 170 consecutive TBI patients admitted to the tertiary neurosurgical center in Nepal between April 2020 and April 2022 was conducted. Demographic, clinical, and etiological characteristics; imaging findings; and management strategies were recorded, compared, and analyzed using descriptive statistics. The Glasgow Outcome Scale Extended (GOSE) was used to measure the outcomes in two groups of patients (single and multiple CNP) at 3 *** Out of 250 eligible patients, 80 were excluded and CNP was noted in 29 (17.1%) of the remaining 170. The median age was 34.9 years, and falls (60.6%) were the most common cause of trauma. TBI severity was categorized based on GCS: mild (82.4%), moderate (15.9%), and severe (1.8%). Cranial nerve involvement was seen in 29 (17.05%) patients: single cranial nerve involvement in 26 (89.65%) and multiple nerve involvement in 3 (10.34%). The most common isolated cranial nerve involved was the oculomotor nerve (37.9%). CT findings revealed a maximum of skull fractures with no significant association between CNP and CT *** CNP is a common consequence of TBI with the most common etiology being falls followed by RTA. Single CNP was more common than multiple CNP with no significant difference in the outcome in the 3-month GOSE score. Further research is needed to determine the burden of traumatic CNP and establish specific management guidelines for different types of CNP.

Tuning of Optical Properties via Annealing of Bismuth Ferrite (BiFeO3) Thin Films

In this study we are reporting annealing induced optical properties of bismuth ferrite (BiFeO3) thin films deposited on glass substrate via spin coating at 5000 rpm. The structural, optical and surface morphology of BiFeO3 (BFO) thin films have been studied via X-ray diffraction (XRD), Fourier transform infrared (FT-IR), Optical absorption (UV-Vis) and Photoluminescence (PL) spectroscopy. XRD spectra confirm annealing induced phase formation of BiFeO3 possessing a rhombohedral R3c structure. The films are dense and without cracks, although the presence of porosity in BFO/glass was observed. Moreover, optical absorption spectra indicate annealing induced effect on the energy band structure in comparison to pristine BiFeO3. It is observed that annealing effect shows an intense shift in the UV-Vis spectra as diffuse absorption together with the variation in the optical band gap. The evaluated optical band gap values are approximately equal to the bulk band gap value of BiFeO3.

Aqueous binary dopant (ZrOCl2/AgI) is used in different ratio such as 1:1, 1:2 and 2:1 (w/w) for chemical doping to enhance the conductivity of synthesized Poly (o-toluidine) (POT). The doping of Poly (o-toluidine) is carried out using tetrahydrofuran as solvent. Doped samples are characterized using various techniques such as I-V characteristics, UV-Visible spectroscopy, X-ray diffractometry (XRD), FTIR and Photoluminescence (PL) studies. A significant enhancement in DC conductivity has been observed with the introduction of binary dopant. UV-Visible study shows that optical parameters change considerably after doping. Interestingly, both direct and indirect band gaps are observed in the doped samples. XRD patterns show the semi-crystalline nature of doped Poly (o-toluidine). FTIR study shows structural modifications in functional groups with doping in POT. A Photolyminescence spectrum exhibits the emission properties of the samples.

Pediatric vs adult pulmonary tuberculosis:A retrospective computed tomography study

AIM: To compare the manifestations of chest tuberculosis(TB) in pediatric and adult patients based on contrast enhanced computed tomography of ***: This was a retrospective study consisting of 152 patients of chest TB including 48 children and 104 adults who had undergone contrast enhanced computed tomography of chest prior to treatment. The patterns and severity of parenchymal, mediastinal and pleural manifestations were analyzed and compared among different age ***: Parenchymal changes observed include consolidation, air space nodules, miliary TB, cavitation, bronchiectasis and fibrosis and these were noted in 60% of children, 71% of adolescents and 76.9% of adults. These changes were more common in right upper lobe in all age groups. There was no significant difference in the frequency of these changes(except nodules) in different age groups. Centrilobular nodules were seen less commonly in children less than 10 years(P = 0.028). Pleural effusion was noted in 28(18.42%) patients and pericardial effusion in 8(5.3%) patients. No significant difference in the serosal involvement is seen among children and adults. Mediastinal adenopathy was seen 70% of children, 76.3% adolescents and 76.9% of adults and paratracheal nodes were seen most frequently. Nodes had similar features(except matting) among all age groups. Matting of nodes was seen more commonly in children(P = 0.014). CONCLUSION: Pediatric chest tuberculosis can have severe parenchymal lesions and nodal involvement similar to adults. The destructive lung changes observed in children needs immediate attention in view of the longer life span they have and hence in formulating optimal treatment strategies.

Association of Perceived Stress and Gastrointestinal Symptoms in College Students: A Systematic Review

Stress and anxiety are common entities in college students, and only a handful of students can handle these stressors appropriately, while others may show physical signs and symptoms. The main purpose of this systematic review was to examine the association between perceived stress and gastrointestinal symptoms. A detailed search was conducted using five databases: PubMed, Research Gate, Science Direct, American Journal of Gastroenterology, and Google scholar. Ten papers were finalized after the application of various inclusion and exclusion criteria. The Newcastle Ottawa scale was used to evaluate the quality of the included articles as all the included articles were cross-sectional studies. The systematic review of these finalized articles found that most studies revealed the prevalence of gastrointestinal symptoms in college students and were linked to perceived stress.

AIM To evaluate the role of oral curcumin in inducing clinical remission in patients with mild to moderate ulcerative colitis(UC).METHODS A prospective randomized double-blind placebo-controlled trial comparing the remission inducing effect of oral curcumin and mesalamine 2.4 g with placebo and mesalamine 2.4 g in patients of ulcerative colitis with mild to moderate severity was conducted from January 2003 to March 2005. The included patients received 1 capsule thrice a day of placebo or curcumin(150 mg) for 8 wk. Patients were evaluated clinically and endoscopically at 0,4 and 8 wk. The primary outcome was clinical remission at 8 wk and secondary outcomes were clinical response, mucosal healing and treatment failure at 8 wk. The primary analysis was intention to treat worst case scenario(ITT-WCS).RESULTS Of 300 patients with UC, 62 patients(curcumin: 29, placebo: 33) fulfilled the inclusion criteria and were randomized at baseline. Of these, 21 patients did not complete the trial, 41 patients(curcumin: 16, placebo: 25) finally completed 8 wk. There was no significant difference in rates of clinical remission(31.3% vs 27.3%, P = 0.75), clinical response(20.7% vs 36.4%, P = 0.18), mucosal healing(34.5% vs 30.3%, P = 0.72), and treatment failure(25% vs 18.5%, P = 0.59) between curcumin and placebo at 8 *** Low dose oral curcumin at a dose of 450 mg/d was ineffective in inducing remission in mild to moderate cases of UC.
