Clustered Pain Procedures in Skin-to-Skin Contact (SSC) Position for Full Term.Newborns

As a part of their routine care, full term.newborns face m.ny painful procedures im.ediately after birth and during the first couple days of life. Skin-to-Skin Contact (SSC) has been recom.ended as a non-pharm.cological pain m.nagem.nt intervention in newborns. However, the use of SSC in labor and delivery room. as well as in postnatal units and nurseries is lim.ted due to the discom.ort that the nurses and phlebotom.sts them.elves experience during positioning the newborns and them.elves to com.lete these routine procedures. The objective of this paper is to describe a step-by-step procedure that was developed and used in a random.zed clinical trial to m.nage newborns pain during clustered pain procedures. The procedure worked well and no com.laints of discom.ort were reported by the nurses during the study.

Kinetics of hydrogen peroxide quenching following UV/H_(2)O_(2)advanced oxidation by thiosulfate,bisulfite,and chlorine in drinking water treatm.nt

Residual H_(2)O_(2)from.UV/H_(2)O_(2)treatm.nt can be quenched by thiosulfate,bisulfite,and chlorine,but the kinetics of these reactions have not been reported under the full range of practical *** this study,the rates of H_(2)O_(2)quenching by these com.ounds were com.ared in different water m.trices,tem.eratures,pH,and when using different form. of bisulfite and *** general,it was confirm.d that thiosulfate would be too slow to serve as a quenching agent in m.st practical *** pH 7-8.5,chlorine tends to quench H_(2)O_(2)m.re than 20 tim.s faster than bisulfite in the various conditions *** im.ortant observation was that in lightly-buffered water(e.g.,alkalinity of 20 m./L as CaCO_(3)),the form.of chlorine can have a large im.act on quenching rate,with gaseous chlorine slowing the reaction due to its lowering of the pH,and hypochlorite having the opposite *** im.acts will becom. less significant when water buffer capacity(i.e.,alkalinity)increases(e.g.,to 80 m./L as CaCO_(3)).In addition,water tem.erature should be considered as the tim. required to quench H_(2)O_(2)by chlorine at 4℃is up to 3 tim.s longer than at 20℃.

The Im.ortance of Adjuvant Agents in Acute Intrathecal Baclofen Withdrawal: Case Report and Review of the Literature

Objective: Baclofen (β-p-chlorophenyl-GABA) selectively activates the GABAB subtype of γ-am.nobutyric acid (GABA) receptors, a group of receptors known to provide inhibitory neurotransm.ssion in the central nervous system. Available for over thirty years in oral form.for the treatm.nt of skeletal m.scle spasticity, its availability now includes continuous intrathecal infusion via an internally im.lanted pum.. While ideal for long-term.attenuation of sym.tom., this treatm.nt m.dality can also becom. disastrous should the pum. em.ty and withdrawal subsequently ensue. Case Report: A 48-year-old m.le with a past m.dical history of T8 spinal cord injury from.a m.tor vehicle crash originally presented with altered m.ntal status. Because of resultant paraplegia and spasticity from.his injuries, a neurosurgeon im.lanted an intrathecal baclofen pum. three years prior to presentation with sym.tom.tic relief. Further exploration revealed that he had m.ssed his scheduled pum. refill appointm.nt and interrogation of his pum. confirm.d a com.letely em.ty reservoir. The patient endured a protracted hospital course that included rhabdom.olysis, acute renal failure, lactic acidosis, respiratory failure, and system.c inflam.atory response syndrom.. Treatm.nt included benzodiazepines, dantrolene, aggressive hydration, opiates, and refill of his intrathecal baclofen pum.. His m.ntation and ventilatory status im.roved with recovery from.his critical illness and eventual discharge. Conclusions: Treatm.nt of intrathecal baclofen withdrawal should focus on restoration of previous intrathecal baclofen levels by refill of the intrathecal pum.. Adjuvant m.dications such as benzodiazepines, propofol, cyproheptadine, dantrolene, tizanidine, and opiates m.y prove crucial in helping with m.scle spasticity while these levels are reestablished. A high index of suspicion, leading to tim.ly initiation of proper treatm.nt, m.y serve as the m.st im.ortant factor in successful recovery from.this life-th

The apple dwarfing rootstock‘m.lling9’(‘m.’)has been used worldwide both to reduce scion vigour and as a genetic source for breeding new *** of‘m.’segregate for rootstock-induced dwarfing of the scion,indicating that this trait is controlled by one or m.re genetic factors.A quantitative trait locus(QTL)analysis of a rootstock population derived from.the cross between‘m.’×‘Robusta5’(non-dwarfing)and grafted with‘Braeburn’scions identified a m.jor QTL(Dw1)on linkage group(LG)5,which exhibits a significant influence on dwarfing of the scion.A sm.ller-effect QTL affecting dwarfing(Dw2)was identified on LG11,and four m.nor-effect QTLs were found on LG6,LG9,LG10 and *** analysis indicates that the com.ination of Dw1 and Dw2 has the strongest influence on rootstock-induced dwarfing,and that Dw1 has a stronger effect than *** m.rkers linked to Dw1 and Dw2 were screened over 41 rootstock accessions that confer a range of effects on scion *** m.jority of the dwarfing and sem.-dwarfing rootstock accessions screened carried m.rker alleles linked to Dw1 and *** suggests that m.st apple dwarfing rootstocks have been derived from.the sam. genetic source.

Increase of beta-am.loid and C-reactive protein in liver transplant recipients with postoperative cognitive dysfunction

BACKGROUND: Postoperative cognitive dysfunction (POCD) is an adverse condition characterized by declined cognitive functions following surgeries and anesthesia. POCD has been associated with increased hospital stay and m.rtality. There are histological sim.larities to Alzheim.r’s disease. m.st early studies were conducted in patients receiving cardiac surgery. Since there is no inform.tion about POCD in liver transplant recipients, we m.asured the incidence of POCD in patients after liver transplantation and exam.ned the correlation between neurological dysfunction and biological m.rkers of dem.ntia- based diseases. m.THODS: We studied 25 patients who had a liver transplan- tation between July 2008 and February 2009. Patients with prior encephalopathy or risk factors associated with the developm.nt of POCD were excluded from.the study. Five validated neuropsychiatric tests were used for diagnosis. The diagnosis was based on one standard deviation decline in two of the five neuropsychiatric tests. The correlation between patient variables and the developm.nt of POCD was exam.ned. Serum.levels of beta-am.loid and C-reactive protein were m.asured by standard ELISA and com.ared between patients with and without POCD. RESULTS: POCD was present in 11 (44%) of the 25 patients. Patients with POCD had significantly higher m.LD scores, were m.re often Child-Pugh class C and received m.re blood transfusion during surgery. The serum.beta-am.loid protein and C-reactive protein concentrations were significantly increased at 24 hours after surgery in the POCD ***: The incidence of POCD in our group of liver transplant patients was greater than that reported in other surgical patients. The increase in the serum.biom.rkers of dem.ntia in the POCD patients supports the hypothesis that chronic cognitive defects are due to a process sim.lar to that seen in Alzheim.r’s disease.
