Performance of Non-invasive Blood Parameters for Ruling Out Significant Liver Fibrosis in Patients with Chronic Hepatitis B

Background and Aims:Evaluation of significant liver fibro-sis is important for treatment decision and treatment re-sponse evaluation in patients with chronic hepatitis B. Since liver biopsy is invasive and transient elastography (TE) has limited availability, various non-invasive blood parameters need evaluation for their capabilities for detection of signifi-cant fibrosis. Methods:In this retrospective study, records of patients who had undergone liver biopsy for treatment-na?ve chronic hepatitis B were evaluated to obtain various non-in-vasive blood parameters (aspartate aminotransferase-to-pla-telet ratio index [referred to as APRI], Fibrosis-4 score [referred to as FIB-4], gamma-glutamyl transpeptidase-to-platelet ratio [referred to as GPR], and gamma-glutamyl transpeptidase-to-albumin ratio [referred to as GAR]), in ad-dition to TE, to assess significant liver fibrosis and compare these to fibrosis stage in liver biopsy. Results:A total of 113 patients were included in the study (median age 33 [inter-quartile range: 11-82 years], 74% males). Most (75%) pa-tients were HBeAg-negative. The liver biopsy revealed significant fibrosis (Ishak ≥3) in 13% of the patients and nil or mild fibrosis (Ishak<3) in 87%of the patients. TE findings were available for 85 patients, APRI and FIB-4 for 95 patients, GPR for 79 patients, and GAR for 78 patients. The median values of all the parameters were significantly higher in pa-tients with significant fibrosis, as compared to patients with non-significant fibrosis, and all the blood parameters as well as TE were able to identify patients with significant fibrosis significantly well (p<0.05). All non-invasive parameters had low positive predictive value but negative predictive value above 92%. Compared to TE, all the non-invasive blood pa-rameters had similar area under the curve for detecting sig-nificant fibrosis, with excellent negative predictive value (≥93%). Gastrointestinal tuberculosis accounts for 3%of the extrapulmonary tuberculosis with ileocaecal region being the common site of involvement up to 75%.Primary involvement of appendix is very rare and accounts for only 0.6%to 2.9%of gastrointestinal tuberculosis in the absence tubercular focus *** pre-operative investigations usually give non-specific results. The diagnosis in most instances made only after *** we report a case of primary appendicular tuberculosis in a patient presented with caecal perforation.

Anticancer potential of Alternanthera sessilis extract on HT-29 human colon cancer cells

Objective: To identify the bioactive extracts from Alternanthera sessilis and investigate its cytotoxicity potential against colon cancer cells, HT-29. Methods: This study examined the effects of three parts(aerial, leaf, stem) of whole plant on HT-29 colon cancer cell lines. Three different extracts from the plant parts were prepared by maceration technique using 80% ethanol. The anticancer activities were determined using MTT, clonogenic, cell motility and AOPI assay. The chemical composition profiling was analyzed by GC-MS. Results: Among three plant part extracts, leaf extract greatly suppressed the growth of colon cancer cells in time and dosage-dependent manner, followed by aerial and stem. The cytotoxicity results were rationalized with clonogenic, cell motility and AO/PI assay, where extract showed the most active activity compared to aerial and stem extracts. GC-MS analysis of leaf extract showed there were various recognized anti-cancer, anti-oxidant and anti-inflammatory compounds. Conclusions: Amid the screened extracts, the leaf extract exhibits the credible cytotoxic, anti-proliferative and apoptotic activity and hence, our findings call for additional research to conclude the active compounds and their mechanisms determining the apoptotic activity.

The aim of the present work is to develop rifampicin loaded phospholipid lipospheres containing sulfphobutyl etherβ-cyclodextrin and Vitamin C for inhalation to test their potential for deep lung *** findings of the solid state characterization revealed the amorphous nature of the *** exhibited a better flowability,an aerodynamic diameter in the range of 1.76 to 3.99μ***,the fine particle fraction and emitted dose was found in the range of 68.84–83.73% and 80–93%,***,lipospheres exhibited enhanced/equivalent efficacy in vitro in H37Rv ***,the results show the potential of lipospheres for pulmonary delivery of rifampicin.

Palladium-catalyzed carboxylative Suzuki coupling reactions of benzyl chlorides with allyl pinacol-borate were successfully conducted in the absence of any extra ligand to produce β,γ-unsaturated esters in satisfactory to good yields. The carboxylative Suzuki coupling reaction proceeded smooth-ly under mild conditions in the presence of palladium nanoparticles generated in situ through the formation of a π-benzylpalladium chloride intermediate.

Metathesis synthesis, characterization, spectral and photoactivity studies of Ln_(2/3)MoO_4(Ln=La, Pr, Nd, Sm, Eu, Gd, Tb, Dy, Er and Y)

Rare earth ortho-oxomolybdates of composition Ln2/aMoO4 （Ln=La, Pr, Nd, Sm, Eu, Gd, Tb, Dy, Er and Y） were prepared by metathesis reaction. All these materials were characterized by powder X-ray diffraction, Fourier transform-infrared spectroscopy （FT-IR） and UV-Vis diffuse reflectance spectra. The Rietveld analysis of all the samples was carried out to obtain the unit cell parameters and reliability factors. All the materials were crystallized in tetragonal lattice with I41/a space group. The infrared spectra of all the samples were characterized by bands due to MoO42 units. The bandgap energies of all the samples were obtained from UV-Vis DRS spectra. The luminescence spectra of Ln2/3MoO4 （Ln=Pr, Eu and Dy） showed characteristic transitions of the rare earth ions. The visible light photocatalytic activity of all the samples was studied against degradation of methylene blue.

Comparative quantitative analysis of fruit oil from Hippophae rhamnoides(seabuckthorn) by qNMR, FTIR and GC–MS

Objective: To develop a qNMR method for quantitative analysis of triacylglycerols in fruit oil of Hippophae rhamnoides(seabuckthorn, SBT) and analyze commercial samples of SBT oils using GC–MS and ***: SBT fruit oil(IPHRFH) was extracted with hexane and the triglyceride(TAG) was isolated by vacuum liquid chromatography. Six different branded SBT oils purchased from e-commerce suppliers(Amazon) and in-house prepared SBT oil was analyzed by qNMR and fatty acyl composition of TAGs determined by using NMR. In-house oil was also analysed by GC–MS and FTIR ***: The qNMR results showed that the oil contained 80.3% of triacylglycerol(TAG). The SBT oil TAGs comprised of linolenate 6.6%, palmitoleate/oleate 65.4%, and total saturated fatty acyl chain including palmitate 28% as determined by qNMR. GC–MS analysis revealed that the major acyl functionalities present in the TAG were palmitoleic acid 36.5%, oleic acid 12.9%, palmitic acid 21.2%, and linoleic acid 18%. Of the six commercial samples analyzed, samples from only one supplier(SW) were fruit oil;All others were the seed oils or mix of fruit oil and seed oil. The labels for samples except for the SW did not indicate whether it was fruit oil or seed ***: The results suggest that SBT oil should be analyzed by combination of GC–MS, FTIR and qNMR for factual content of free fatty acid or TAGs, which are chemically different in nature and affect the quality of oil. GC–MS showed the content of omega free fatty acids after hydrolysis, while qNMR and FTIR showed the content of TAGs. The major acyl functionalities found in SBT fruit oil TAGs are palmitoleate/palmitate/oleate, while linoleate and linonelate make up a minor fraction. Furthermore,analysis of commercial samples showed discrepancies between label claims and actual content.

This work describes the development of a capillary electrophoresis （CE） method for the simultaneous separation of acetophenone （AP）, 2-hydroxyacetophenone （2-HAP）, 3-hydroxyacetophenone （3-HAP） and 4-hydroxyacetophenone （4-HAP） in synthetic mixtures using 10 mmol/L of sodium tetraborate buffer （pH 9.5）. The aim of this work is to demonstrate the effectiveness of CE to separate AP and its monohydroxy isomers and to define how the separations are affected by buffers, buffer pH, sample matrices and separation voltage. This method was successfully used for the trace level separation and determination of 2-HAP, 3-HAP and 4-HAP in synthetic mixture and 4-HAP in spiked plasma samples.

Screening and antibacterial efficacy of selected Indian medicinal plants

Objective: To evaluate the antibacterial efficacy of five Indian medicinal plants such as Acalypha indica L.(A. indica), Aerva lanata(L.) Juss. ex Schult.(A. lanata), Clerodendrum inerme(L.) Gaertn., Pergularia daemia(Forsk.) Chiov. and Solanum surattense Burm. f. against opportunistic bacterial pathogens isolated from HIV infected patients for the potential phytoconstituents in plant ***: The opportunistic bacterial pathogens such as Escherichia coli(E. coli),Pseudomonas aeruginosa, Salmonella typhi and Serratia marcescens from Gramnegative group and Staphylococcus aureus from Gram-positive group were isolated from HIV infected patients. The antibacterial efficacy of ethanolic extracts of selected medicinal plants was carried out by disc diffusion method. The potential phytoconstituents of medicinal plant extracts were identified by gas chromatography and mass spectrometry(GC–MS) ***: Among the five medicinal plants tested, A. indica and A. lanata showed the significant antibacterial activity. A. indica showed potential activity against Staphylococcus aureus and E. coli. A. lanata significantly exhibited antibacterial activity against E. coli, Salmonella typhi and Pseudomonas aeruginosa. A total of 19 phytoconstituents were identified in the ethanolic extract of A. indica and A. lanata by GC–MS analysis ***: The results of the present investigation revealed that A. indica and A. lanata, possessed significant antibacterial activity when compared with the other plant extracts tested. The presence of 3-O-methyl-D-glucose by GC–MS analysis in both A. indica and A. lanata extracts has not been reported elsewhere in the literature and the findings in this study could be the first one to report.

Institutional performance and participatory paradigms: Comparing two groups of watersheds in semi-arid region of India

Watershed development programmes carried out in different agroclimatic conditions in India resulted in beneficial impact in terms of productivity enhancement and natural resources conservation, but less attention paid to institutional and participatory aspects. This paper explored the performance of various institutions regarding execution of watershed development programmes in semi-arid region of India. Recorded observations from documents maintained at watershed level and information collected through primary survey as well as focus group discussion with different types of stakeholders were used for analysis. The results indicated lacunae in participatory aspects during programme implementation process like monitoring activities, management of common property resources and equity. Gap in linkages and differential level of performance of various watershed level functionaries indicates the necessity for corrections in the structures and linkages pattern for sustainability of the infrastructure and institutions. The study also showed unequal priorities by the implementing agencies towards in-stitutions, land and water resources development, production enhancement activities and fodder re-sources development as well as rationalities of technical, economic, financial, political and social aspects among the watersheds.
