Evaluation of an alternative dosing regimen with tadalafil, three times per week, for men with erectile dysfunction： sure study in Italy

Aim： To examine the preference for two dosing regimens of 20 mg of tadalafil, on demand or three times per week, in men affected with erectile dysfunction （ED） in Italy. Methods： Scheduled Use versus on demand Regimen Evaluation （sure） is a multicenter, crossover and open-label study, involving 94 urology centers in Italy. Patients aged 18 years or older affected with ED for at least 3 months were enrolled and randomized to 20 mg of tadalafil treatment on demand or three times per week for 5-6 weeks. After a 1-week washout, patients were crossed over to the alternate regimen for 5-6 weeks. A treatment preference question was used to determine the preferred treatment regimen. International Index of Erectile Function （IIEF） and Sexual Encounter Profile （SEP） questionnaire were used as efficacy measures. Results： A total of 1 058 men （mean age 54.8 years）, were randomized to treatment. Overall, 59.1% of patients preferred the on-demand regimen and 41.9% preferred the three times per week dosing. Both regimens were efficacious and well tolerated. Although a statistically higher improvement of the IIEF erectile function （IIEF-EF） domain score and the SEP questionnaire was reported for the three times per week compared to the ondemand treatment regimen, this difference was numerically minimal and lacking in clinical significance. Conclusion： Tadalafil is effective and well tolerated whether used on demand or three times per week. Patients should be given the option to choose the best treatment regimen according to personal needs and preferences.

目的探讨28例经菱形窝入路切除的脑干海绵状血管瘤的适应证、手术技巧及术中注意点。方法术中采取坐位,枕下正中开颅,广泛打开小脑延髓裂;在脑干表面有色素沉淀或膨隆处,或在B超、导航引导、术中颅神经监护下切开脑干,病变暴露后清除血肿,然后切除病变,保留含铁血黄素沉淀的胶质组织。结果28例中有21例位于桥脑;术中发现脑干表面有含铁血黄素沉淀21例,其中脑干表面膨隆9例;所有病变位于脑干背侧;随访期间:症状和体征较术前改善的19例,保持术前水平的5例,加重、出现新的颅神经症状、或其他神经功能障碍(共济失调、运动障碍、感觉障碍)共4例。MRI随访未见病变残留或复发。结论经菱形窝入路主要适于桥脑部、且位于背侧的海绵状血管瘤;脑干表面的含铁血黄素沉淀和(或)膨隆是脑干的安全进入区,术中结合神经电生理监测、B超以及导航是手术成功的关键;术中尽可能全切病变,但保留含铁血黄素沉淀的胶质组织。

Recently,many researchers have tried to develop a robust,fast,and accurate *** algorithm is for eye-tracking and detecting pupil position in many applications such as head-mounted eye tracking,gaze-based human-computer interaction,medical applications(such as deaf and diabetes patients),and attention *** real-world conditions challenge the eye appearance,such as illumination,reflections,and *** the other hand,individual differences in eye physiology and other sources of noise,such as contact lenses or *** present work introduces a robust pupil detection algorithm with and higher accuracy than the previous attempts for real-time analytics *** proposed circular hough transform with morphing canny edge detection for Pupillometery(ChMCEP)algorithm can detect even the blurred or noisy images by using different filtering methods in the pre-processing or start phase to remove the blur and noise and finally the second filtering process before the circular hough transform for the center fitting to make sure better *** performance of the proposed ChMCEP algorithm was tested against recent pupil detection *** and results show that the proposed ChMCEP algorithm achieved detection rates of 87.11,78.54,58,and 78 according to´Swirski,ExCuSe,Else,and labeled pupils in the wild(LPW)data sets,*** results show that the proposed approach performs better than the other pupil detection methods by a large margin by providing exact and robust pupil positions on challenging ordinary eye pictures.

In this paper we consider a generalize dynamic entropy measure and prove that this mea- sure characterizes the distribution function uniquely. Also we propose cumulative resi- dual R^nyi entropy of order statistics and prove that it also determines the distribution function uniquely. Applications of entropy concepts to DNA sequence analysis, the ulti- mate support for the biological systems, have been widely explored by researchers. The entropy measures discussed here can be applied for analysis of ordered DNA sequences.

The peritoneal parietal and visceral surfaces of the abdomen and pelvis are an important anatomic site for the dissemination of epithelial ovarian cancer(EOC). The transcoelomic spread of cancer cells gives rise to peritoneal carcinomatosis(PC) which, without special treatments, is a fatal manifestation of EOC. In order to control PC cytoreductive surgery to remove macroscopic disease is combined with perioperative intraperitoneal(IP) and perioperative intravenous chemotherapy to eradicate microscopic residual disease. Chemotherapy agents are selected to be administered by the IP or intravenous route based on their pharmacologic properties. A peritoneal-plasma barrier which retards the clearance of high molecular weight chemotherapy from the peritoneal cavity results in a large exposure of small cancer nodules on abdominal and pelvic surfaces. Tissue penetration is facilitated by moderate hyperthermia(41-42 ℃) of the IP chemotherapy solution. Timing of the chemotherapy as a planned part of the surgical procedure to maximize expo-sure of all peritoneal surfaces is crucial to success.
