Role of prebiotics, probiotics, and synbiotics in management of inflammatory bowel disease: Current perspectives

Experimental evidence supports the fact that changes in the bowel microflora due to environmental or dietary factors have been investigated as implicating factors in the etiopathogenesis of inflammatory bowel disease(IBD).The amassing knowledge that the inhabited microbiome regulates the gut physiology and immune functions in IBD,has led researchers to explore the effectiveness of prebiotics,probiotics,and synbiotics in treating *** therapeutic approach focuses on restoring the dynamic balance between the microflora and host defense mechanisms in the intestinal mucosa to prevent the onset and persistence of intestinal *** microbial strains and carbohydrate blends,along with their combinations have been examined in experimental colitis models and clinical trials,and the results indicated that it can be an attractive therapeutic strategy for the suppression of inflammation,remission induction,and relapse prevention in IBD with minimal side *** mechanisms of action of probiotics(for e.g.,Lactobacillus species,and Bifidobacterium species)have been reported such as suppression of pathogen growth by releasing certain antimicrobial mediators(lactic and hydrogen peroxide,acetic acid,and bacteriocins),immunomodulation and initiation of an immune response,enhancement of barrier activity,and suppression of human T-cell *** such as lactulose,lactosucrose,oligofructose,and inulin have been found to induce the growth of certain types of host microflora,resulting in an enriched enteric *** non-digestible food dietary components have been reported to exert anti-inflammatory effects by inhibiting the expression of tumor necrosis factor-α-related cytokines while augmenting interleukin-10 *** proand prebiotics has established their efficacy in healthy subjects,a better understanding of the luminal ecosystem is required to determine which specific bacterial strain or combination of probiotics and prebiotics

BACKGROUND Internal hernia(IH)is a rare culprit of small bowel obstruction(SBO)with an incidence ofvent misdiagnosis,improper treatment,and subsequent morbidity and *** To determine the clinico-demographic profile,radiological and operative findings,and postoperative course of patients with IH and its association with *** Medical records of 586 patients with features of SBO presenting at a tertiary care centre at Lucknow,India between September 2010 and August 2023 were *** Out of 586 patients,7(1.2%)were diagnosed with *** these,4 had congenital IH and 3 had acquired *** male-to-female ratio was 4:*** median age at presentation was 32 ***-enhanced computed tomography(CECT)was the most reliable investigation for preoperative identification,demonstrating mesenteric whirling and clumped-up bowel *** paraduodenal hernia and transmesenteric hernia occurred with an equal frequency(approximately 43%each).Intraoperatively,one patient was found to have bowel ischemia and one had associated malrotation of *** follow-up,no recurrences were *** IH,being a rare cause,must be considered as a differential diagnosis for SBO,especially in young patients in their 30s or with unexplained abdominal pain or discomfort post-surgery.A rapid imaging evaluation,preferably with CECT,is necessary to aid in an early diagnosis and prompt intervention,thereby reducing financial burden related to unnecessary investigations and preventing the morbidity and mortality associated with closed-loop obstruction and strangulation of the bowel.

The effects of dietary supplementation with mushroom or selenium enriched mushroom powders on the growth performance and intestinal health of post-weaned pigs

Background There is an urgent need to identify natural bioactive compounds that can enhance gastrointestinal health and promote pig growth performance in the absence of pharmacological levels of zinc oxide(ZnO).The objectives of this study were to:1)compare the effects of mushroom powder supplemented with inorganic selenium(inSeMP)to mushroom powder enriched with organic selenium(orgSeMP)to pharmacological levels of ZnO on growth performance and faecal scores(FS)for the first 21 d post-weaning(Period 1);and 2)compare the molecular and microbial effects of inSeMP and orgSeMP in these pigs on d 39 post-weaning(Period 2).Methods In Period 1,pigs(3 pigs/pen;8 pens/treatment)were assigned to:(1)basal diet(control);(2)basal diet+zinc oxide(ZnO)(3100 mg/kg d 1–14,1550 mg/kg d 15–21);(3)basal diet+mushroom powder supplemented with inorganic selenium(inSeMP)containing selenium(selenite)content of 0.3 mg/kg feed;(4)basal diet+mushroom powder enriched with organic selenium(orgSeMP)containing selenium(selenocysteine)content of 0.3 mg/kg *** powders were included at 6.5 g/kg of *** In Period 1,there was no effect of diets on average daily gain(ADG)and gain:feed(G:F)ratio(P>0.05).The orgSeMP supplemented pigs had a lower average daily feed intake(ADFI)compared to all other groups(P0.05).The orgSeMP supplementation increased the caecal abundance of bacterial members of the Firmicutes and Bacteroidetes phylum,including Lactobacillus,Agathobacter,Roseburia,and Prevotella and decreased the abundance of Sporobacter compared to the basal group,while inSeMP increased the caecal abundance of Prevotella and decreased the caecal abundance of Sporobacter compared to the basal group(P<0.05).Dietary

Internet of Things(IoT)and blockchain receive significant interest owing to their applicability in different application areas such as healthcare,finance,transportation,*** image security and privacy become a critical part of the healthcare sector where digital images and related patient details are communicated over the public *** paper presents a new wind driven optimization algorithm based medical image encryption(WDOA-MIE)technique for blockchain enabled IoT *** WDOA-MIE model involves three major processes namely data collection,image encryption,optimal key generation,and data ***,the medical images were captured from the patient using IoT ***,the captured images are encrypted using signcryption *** addition,for improving the performance of the signcryption technique,the optimal key generation procedure was applied by WDOA *** goal of the WDOA-MIE algorithm is to derive a fitness function dependent upon peak signal to noise ratio(PSNR).Upon successful encryption of images,the IoT devices transmit to the closest server for storing it in the blockchain *** performance of the presented method was analyzed utilizing the benchmark medical image *** security and the performance analysis determine that the presented technique offers better security with maximum PSNR of 60.7036 dB.

Primary pediatric mid-brain lymphoma:Report of a rare pediatric tumor in a rare location

Primary central nervous system lymphoma(PCNSL) is a rare disease in pediatric age group. A thirteen-yearold male child presented with complaints of headache for six months, vomiting and diplopia for three days. Magnetic resonance imaging of the brain showed a single lesion of 1.7 cm × 1.6 cm × 1.6 cm in the mid brain and tectum. He underwent a gross total resection of the tumor. The histopathological evaluation revealed B cell high grade non Hodgkin lymphoma. The patient was treated with High dose methotrexate and cranio spinal radiation. The patient was alive without disease 12 mo after completion of treatment. This case highlights importance of keeping PCNSL as differential in brain stem lesions of pediatric patients also. Radiation and chemotherapy remains the most important treatment for such patients.

Timing of chemotherapy and survival in patients with resectable gastric adenocarcinoma

AIM: To evaluate the timing of chemotherapy in gastric cancer by comparing survival outcomes in treatment groups. METHODS: Patients with surgically resected gastric adenocarcinoma from 1988 to 2006 were identified from the Los Angeles County Cancer Surveillance Program. To evaluate the population most likely to receive and/or benefit from adjunct chemotherapy, inclusion criteria consisted of Stage Ⅱ or Ⅲ gastric cancer patients > 18 years of age who underwent curative-intentsurgical resection. Patients were categorized into three groups according to the receipt of chemotherapy:(1) no chemotherapy;(2) preoperative chemotherapy; or(3) postoperative chemotherapy. Clinical and pathologic characteristics were compared across the different treatment ***: Of 1518 patients with surgically resected gastric cancer, 327(21.5%) received perioperative chemotherapy. The majority of these 327 patients were male(68%) with a mean age of 61.5 years; and they were significantly younger than non-chemotherapy patients(mean age, 70.7; P ve chemotherapy was administered to 11.3% of patients(n = 37) and postoperative therapy to 88.7% of patients(n = 290). An overall survival benefit according to timing of chemotherapy was not observed on univariate or multivariate analysis. Similar results were observed with stagespecific survival analyses(5-year overall survival: Stage Ⅱ, 25% vs 30%, respectively; Stage Ⅲ, 14% vs 11%, respectively). Therefore, our results do not identify a survival advantage for specific timing of chemotherapy in locally advanced gastric cancer. CONCLUSION: This study supports the implementation of a randomized trial comparing the timing of perioperative therapy in patients with locally advanced gastric cancer.

目的建立同时测定复方水杨酸硫乳膏中水杨酸和醋酸氯己定含量的高效液相色谱法,以提升其质量标准。方法色谱柱采用AtlantisC18柱(250mm×4.6mm,5μm),流动相为水(乙酸铵26g和三乙胺37mL,加水至1000mL,并用冰醋酸调pH至4.0)-甲醇(50∶50,v/v),流速为1.0mL/min,柱温为35℃,检测波长为257nm,进样量为10μL。结果在该测定条件下阴性对照无干扰,水杨酸、醋酸氯己定的质量浓度分别在0.0999~0.5996g/L和0.0050~0.0300g/L范围内与峰面积线性关系良好(r均为0.9999),平均回收率分别为100.80%和99.41%,RSD分别为0.78%和0.99%(n=9)。结论该方法简便快捷,准确可靠,可同时测定复方水杨酸硫乳膏中水杨酸和醋酸氯己定的含量。
