Cancer, Malnutrition and Cachexia: We Must Break the Triad

Many factors can modify nutritional status in cancer patients, including cachexia, nausea and vomiting, decreased caloric intake or oncologic treatments causing malabsorption. The cachexia-anorexia syndrome is a complex metabolic syndrome associated with cancer and some other palliative conditions characterized by involuntary weight loss involving fat and muscle, anorexia, early satiety, fatigue and weakness due to shifts in metabolism caused by tumour by-products and cytokines. Cachexia is a distressing and debilitating condition, affecting significant numbers of patients with advanced disease and is the primary cause of death in about 20% of all patients with cancer. Though cachexia is most commonly associated with particular tumours, such as head and neck, gastrointestinal tract, pancreas, central nervous system and lung, it may affect any patient with any tumour at any site;no patient and no tumour are excluded. Current treatment for principally depends on its prevention rather than reversing the present disease state, and the clinical results are far from being satisfactory. A careful decision based on good clinical judgement is necessary before deciding to start either enteral or parenteral nutrition, to avoid a useless, costly and difficult treatment. Treatment should be directed toward improvement in the quality of life of the patient and should often include nutritional counseling. It should take into consideration both disease and treatment related factors as well as the cachexia syndrome itself.

A 10 Years Comparative Study to Assess Trends in Seroprevalence of Transfusion Transmitted Infections among Blood Donors at Gwalior, India

Introduction: Transfusion Transmitted Infections (TTIs) threaten safety of the recipients and the community as a whole and are the subject of real concern worldwide. Aims and Objectives: To know the prevalence of transfusion transmitted infections amongst the blood donors, to evaluate the changing trends of TTIs and to compare these observations within the study as well as with the other relevant studies. Place and Duration of Study: This study was carried out at Blood Bank, Department of Pathology, Gajra Raja Medical College, Gwalior, India, from January 2004 to December 2013 (ten years). Materials and Methods: In this study 122,006 voluntary and replacement donations were screened for TTIs;HIV, HBV, HCV, Syphilis, Malaria and their seroprevalence was calculated. Further study was divided in Group “A” (from 2004 to 2008) and Group “B” (from 2009 to 2013) to compare the results. Results: Out of total 122,006 blood units collected, 79,750 (65.3%) were voluntary and 42,256 (34.7%) were replacement donors. The seropositivity of TTIs in the entire study, in group “A” and in Group “B” was 3.26% (3985/122,006) (p = 0.000005), 2.25% (1238/54,874) (p = 0.000005) and 4.09% (2747/67,123) (p = 0.000005) respectively. In Group “A” and “B” seroprevalence of HIV, HBV, HCV, Syphilis and Malaria was 0.29%, 1.16%, 0.61%, 0.06%, 0.11% and 0.13%, 3.15%, 0.24%, 0.17%, 0.03% respectively. Conclusion: Our study concluded that there was significant increase in seroprevalence of HBV and syphilis whereas decreasing pattern in HIV, HCV and Malaria was observed in last five years as compared to previous five years among the blood donors.

Antimicrobial activity of extracts of local plants was evaluated, with two strain i.e E. coil and S. aurri （gram positive and gram negative） microorganisms. The in vitro anti bacterial activity was performed by agar disc diffusion method. The most active antibacterial plants were S. trifiliatus and A. mexicana. The significant antibacterial activity of active extracts was compared with the standered, antimicrobics, streptomycin （10 μg/disc）. The results obtained in the present study suggest that S. trifoliatus and A. mexicana can be used in treating diseases caused by the test organism.

Variation in Senescence Pattern of Different Classes of Rice Tillers and Its Effect on Panicle Biomass Growth and Grain Yield

In rice, the initiation of tillers is staggered and temporally spaced, but maturity is synchronous. Duration of growth in a later-initiated tiller on a higher culm node is shorter and it contributes less biomass and grain yield. The present investigation attempts to discover the manner in which ordered pattern of senescence in basipetal succession impacts source capacity of tillers in two contrasting rice cultivars, namely Lalat (high tillering) and MGD-106 (medium tillering) during the dry season of 2009 and ascertain how tiller production capacity influences dry matter partitioning and tiller dynamics of the plant. In both the rice cultivars, the progress of senescence among different types of tillers was indicated by gradual decline of photosynthetic pigments, total nitrogen and protein concentrations and increase of lipid peroxidation and peroxidase activity of the flag leaf, which affected photosynthetic efficiency. The effects were more pernicious on the newer tillers compared to older tillers. It was observed that metabolic dominance of the older tillers over newer tillers could be accrued due to higher photosynthetic source capacity of the former than that of the latter. It was concluded that flag leaf of a later-initiated tiller is less tolerant to senescence induced photo-oxidative stress, which decreases both source and sink activities. Increase of tiller number and order in rice increases vulnerability of the later-initiated tillers for oxidative stress and grain filling.

目的:检测泪液功能试验(SchirmerⅠtest,SⅠt)及泪膜破裂时间(tear filmbreak-up time,TBUT)在翼状胬肉患者中的改变。方法:筛选50例患者(100眼,50例翼状胬肉眼),行SⅠt及TBUT试验。结果:SⅠt试验在翼状胬肉眼的平均值为19.6±11.6 (1 ～40) mm,对照眼为17.2±10.6(1 ～35)mm,翼状胬肉眼中20例(40%)SⅠt试验结果异常,对照眼为21例(42%),其差异无统计学意义(P=075)。两组间差异无统计学意义(t=1.453,P=0.15)。TBUT在翼状胬肉眼及对照眼中分别为7.4±5.1 ( 2 ～20)s, 13.4±6.1 (2 ～25)s,两组间的差异有显著统计学意义(t=8.029,P<0.01)。翼状胬肉眼中TBUT异常的为39眼(78%),对照眼中为16眼(32%);差异有显著统计学意义(P<0.01)。结论:与对照眼相比较,SⅠt试验在翼状胬肉眼中差异无统计学意义,TBUT在翼状胬肉眼中有所下降。

A rapid, sensitive and accurate liquid chroma-tographic tandem mass spectrometric method is described for the determination of Mimosine in Mimosa pudica Linn. whole plant powder. Mi-mosine was extracted from the plant using 1.0% HCl in water. The chromatographic separation was achieved using a Thermo Hypurity C18 (50 x 4.6 mm) 5.0 μ column interfaced with a triple quadrapole mass spectrometer. The mobile phase consisted of a mixture of Methanol: 10 mM ammonium formate buffer whose pH was ad-justed to 3.00 ± 0.05 with formic acid (80:20, v/v) and was delivered at a flow rate of 1.0 mL min-1. Electrospray ionization (ESI) source operated in the negative ion mode was used for the quantitation. Detection was performed using an Applied Biosystems Sciex API 3200 Mass spec-trometer. The method was found to be simple, precise, accurate, fast, specific and sensitive and can be used for routine quality control analysis of Mimosine in Mimosa pudica Linn.

高温处理对诱导果实增强对日灼的抗性有一定的作用 ,可降低日灼率。苹果果实在 4 5℃下处理 2h ,在3d后将处理果和对照果同时暴露在阳光下 ,结果发现 :热处理能够在一定程度上增强果实对日灼的抗性。在 4 5℃下处理 2、4和 6h后立即取样或室温下放置 2 4h后取样 ,对热激蛋白的分析表明 ,大分子量热激蛋白量变化不大 ,而小分子量热激蛋白可能在诱导果实抗热性方面起重要作用。随着处理时间延长 ,在一定时间范围内 ,热激蛋白抗热性水平增加。经测定 ,树冠外围充分曝光果和树冠内膛遮荫果对比 ,二者热激蛋白含量有显著差异 ,这可能是遮荫果对日灼更为敏感的原因。

Aim: This study was aimed to review and establish the practice of exchange transfusion (ET) with whole blood reconstituted (WBR) in hemolytic disease of newborn (HDN). Objectives: To observe fall in indirect serum bilirubin, correction of anemia and comparison with related studies. Background: Hemolytic disease of the Newborn is characterized by presence of IgG antibodies in maternal circulation, which causes hemolysis in the fetus by crossing the placenta and sensitizing red cells for destruction by macrophages in the fetal spleen with consequent hyperbilirubinemia. Exchange transfusion with or without phototherapy is the method of choice for treating the newborn with on going hemolysis Methods/Materials: Sample size consisted of 110 neonates in whom 119 exchange transfusions were carried out with WBR. WBR was prepared by suspending O Rhesus-D (RhD) positive/negative cells (compatible with neonate’s/ mother’s serum) in AB plasma. Double volume exchange transfusion(s) were carried out through umbilical vein by push-pull technique. Results: Out of 110 cases, 61 (55.5%) were of RhD HDN whereas ABO and other group HDN cases were 30 (27.3%) and 19 (17.3%) respectively. An average post-ET fall in indirect serum bilirubin by 54.6% and correction of anemia by3.7 gm/dl were reported in the study. Conclusion: An average post-ET fall in indirect serum bilirubin and correction of anemia was found to be more significant when compared to other studies. Hence we recommend exchange transfusion in HDN with WBR to obtain reasonable fall in indirect serum bilirubin and high average rate of correction of anemia.
