Gastric cancer(GC)remains a substantial health burden worldwide,ranking fifth in incidence and third in mortality among all cancer *** have persistently attempted to address this growing burden through surgical management of GC encompassing various aspects of surgery,including advances in surgical techniques and tools for minimally invasive surgery,novel technology for real-time image-guided surgery,and function-preserving and oncometabolic surgeries,aimed at improving patients’quality of *** current perspective discusses the five most critical dimensions of the recent technical improvements and conceptual changes in GC *** recommend further exploration of long-term benefits of these advancements,identification of breakthrough solutions to address current challenges,and delivery of the best quality of care.

Surfaces with hydrophilic and antimicrobial properties are very attractive for cardio-vascular device-associated *** aim of this study was to prepare and coat a hydrophilic polymer containing a functional group capable of forming triazole functionality onto the surface of polyurethane(PU).The modified surfaces were assessed with cell adhesion,bacterial adhesion and bacterial *** fibroblast cells(NIH-3T3)and three bacterial species were used for *** results showed that the modified surface not only exhibited a significant reduction in cell adhesion with a 25%-59%decrease to mouse fibroblast but also showed a sig-nificant reduction in bacterial attachment with 26%-67%,24%-61%and 23%-57%decrease to Staphylococcus aureus,Escherichia coli and Pseudomonas aeruginosa,respectively,as compared with original ***,the polymer-modified surface exhibited a significant antibacterial function by inhibiting bacterial growth with reduction of 49%-84%,44%-79%and 53%-79%to ***,*** and ***-ginosa,respectively,as compared with original *** results indicate that covalent polymer attachment enhanced the antibacterial and antifouling properties of the PU surface.

Hydrophilic polymers are very useful in biomedical *** this study,biocom-patible polyethylene glycol(PEG)and polyvinylpyrrolidone(PVP)polymers end-capped with succinimidyl groups were either modified or synthesised and attached to poly-vinylchloride *** modified surfaces were evaluated with cell adhesion and bacterial adhesion.3T3 mouse fibroblast cells and three bacteria species were used to evaluate surface adhesion *** showed that the modified surface exhibited significantly reduced 3T3 cell adhesion with a 50%-69%decrease for PEG and a 64%-81%for PVP,as compared to unmodified *** modified surface also showed significantly reduced bacterial attachment with 22%-78%,18%-76%and 20%-75%decrease for PEG and 22%-76%,18%-76%and 20%-73%for PVP to Staphy-lococcus aureus,Escherichia coli and Pseudomonas aeruginosa,respectively,as compared to unmodified *** seems that an appropriate chain length or molecular weight(neither the longest nor the shortest chain length)determines the lowest cell and bacterial adhesion in terms of *** the other hand,a mixture of polymers with different chain lengths exhibited the lowest cell and bacterial adhesion in terms of PVP.

OBJECTIVE: To investigate the clinical effect of Qingxue Dan(QXD) on obesity and metabolic biomarker related to ***: A randomized, double blinded, placebo-controlled trial with a paralleled study design was conducted. Twenty-six obese volunteers aged between 30 and 60 with obesity and more than 2 metabolic risk factors were recruited at the department of oriental rehabilitation medicine, Kyunghee university oriental medical hospital, Seoul, Korea. Subjects were randomly assigned to an intervention(QXD) group or a placebo group, and treated with 900 mg/d of QXD or placebo medicine for 8 weeks. Primary endpoint was the change of body mass index(BMI) at 8 week from baseline. Secondary outcomes included the change of body composition, abdominal fat mass measured with Dual energy X-ray absorptiometry(DXA), blood pressure,lipid profiles and the homeostasis model assessment for insulin resistance(HOMA-IR).RESULTS: BMI was decreased in the QXD group sig-nificantly. Total body fat, abdominal fat mass measured with DXA Region of Interest and waist circumference(WC) showed a trend toward decreasing in the QXD group, but fat free mass was decreased in all groups. Triglyceride(TG) was decreased in QXD group significantly, but WC, total cholesterol and high-density lipoprotein cholesterol were increased in both group. BP didn't change during the study period. HOMA-IR is decreased in both groups without group ***: 8-weeks of oral administrations of QXD(900 mg/d) reduces BMI, with a tendency of lose of total body fat mass, especially abdominal fat. It also significantly reduced serum TG *** results suggest QXD could be used to treat obesity and metabolic risk factors. Further study is needed to confirm our pilot findings.

In order to obtain all the properties of fermented ginseng, we fermented ginseng using Bacillus subtilis (B. subtilis) isolated from Cheonggukjang. A sterilized ginseng medium was made with 4-year-old ginseng powder and distilled water (300% ginseng powder [w/w]), and the ginseng was fermented by B. subtilis (1% ginseng medium) followed by incubation at 37°C for 3, 5, 7, and 10 days. The growth of B. subtilis in the ginseng medium significantly increased up to 9 log CFU/g, but no significant difference was observed after 3 days. As the fermentation progressed, the ginsenoside Rd and Rg+Rh1 contents increased by 255.3% - 322.5% and 165.6% - 228.6%, respectively, whereas the Rc, Re, and Rg1 contents decreased by 30.7% - 39.6%, 10.5% - 12.8%, and 16.2% - 16.6%, respectively. After 3 days of fermentation, a 6.25% - 7.12% viscous substance was produced;thereafter, the viscous substance was gradually reduced until it disappeared. The viscosity of the medium significantly decreased with a longer fermentation time. Fibrinolytic activity increased during 3 - 10 days of fermentation, indicating a relative activity of 85.0% - 100.0%.

AIM To investigate the efficacy and safety of endoscopic stent insertion in patients with delayed gastric emptying after *** In this study, we prospectively collected data from patients who underwent stent placement for delayed gastric emptying(DGE) after distal gastrectomy between June 2010 and April 2017, at a tertiary referral academic center. Clinical improvement, complications, and consequences after stent insertion were *** Technical success was achieved in all patients(100%). Early symptom improvement was observed in 15 of 20 patients(75%) and clinical success was achieved in all patients. Mean follow-up period was 1178.3 ± 844.1 d and median stent maintenance period was 51 d(range 6-2114 d). During the follow-up period, inserted stents were passed spontaneously per rectum without any complications in 14 of 20 patients(70%). Symptom improvement was maintained after stent placement without the requirement of any additional intervention in 19 of 20 patients(95%).CONCLUSION Endoscopic stent placement provides prompt relief of obstructive symptoms. Thus, it can be considered an effective and safe salvage technique for post-operative DGE.

Osteocytes are mechanosensitive bone cells, but little is known about their effects on tumor cells in response to mechanical stimulation. We treated breast cancer cells with osteocyte-derived conditioned medium(CM) and fluid flow-treated conditioned medium(FFCM) with 0.25 Pa and 1 Pa shear stress. Notably, CM and FFCM at 0.25 Pa induced the mesenchymal-to-epithelial transition(MET), but FFCM at 1 Pa induced the epithelial-to-mesenchymal transition(EMT). This suggested that the effects of fluid flow on conditioned media depend on flow intensity. Fluorescence resonance energy transfer(FRET)-based evaluation of Src activity and vinculin molecular force showed that osteopontin was involved in EMT and MET switching. A mouse model of tumorinduced osteolysis was tested using dynamic tibia loadings of 1, 2, and 5 N. The low 1 N loading suppressed tumor-induced osteolysis, but this beneficial effect was lost and reversed with loads at 2 and 5 N, respectively. Changing the loading intensities in vivo also led to changes in serum TGFβ levels and the composition of tumor-associated volatile organic compounds in the ***, this study demonstrated the critical role of intensity-dependent mechanotransduction and osteopontin in tumorosteocyte communication, indicating that a biophysical factor can tangibly alter the behaviors of tumor cells in the bone microenvironment.
