Background: Leptomeningeal seeding （LMS） is a devastating complication of malignancy. Among several pathogenesis, surgical spillage has been one of the iatrogenic factors for the development of LMS. We retrospectively analyzed various surgical factors affecting the development of LMS. Patients and Methods: We identified 294 patients with brain metastasis who had been treated using surgery between 2001 and 2009. We classified the tumor location into subcortical, pial, epipial, juxtaventricular and ventricular. The description of surgical resection was divided into piecemeal and en-bloc and the risk of using cavitron ultrasonic aspirator （CUSA） were investigated. Results: A total 44 of the 294 patients （15%） developed LMS after undergoing index procedure. The median time to LMS development was 6months （range, 1.0-42.0）. Among the 294 surgical patients, seven out of 101 en-bloc removal patients （6.9%） developed LMS, whereas 37 out of 193 piecemeal removal patients （19.2%） developed LMS, and the incidence of LMS was significantly different between these two surgical resection modes （p = 0.005）.The adjusted hazard ratio （HR） for patients whose tumors were resected by piecemeal compared to en-bloc was 3.12 （95% CI, 1.4-7.0;p = 0.006）. The incidence of LMS was significantly higher （p ＜ 0.001） in patients whose tumors were resected by CUSA, and the HR of using CUSA was 4.18 （95% CI, 1.9-9.4;p = 0.001）. The location of tumor at high-risk of CSF exposure （epipial and ventricular） conferred increased risk of LMS after the treatment compared to low-risk group with the HR 2.52 （95% CI, 1.4-4.7;p = 0.004）. On the other hand, the location of tumor at low-risk of CSF exposure （subcortical） showed significantly reduced risk of LSM compared to the other locations （HR=0.27;95% CI 0.12-0.65;p-value = 0.003）. Background There are limited data comparing long-term clinical outcomes between first-generation (1G) and second-generation (2G) drug-eluting stents (DESs) in patients who underwent successful percutaneous coronary intervention (PCI) for coronary chronic total occlusion (CTO) lesion. Methods A total of 840 consecutive patients who underwent PCI with DESs for CTO lesion from January 2004 to November 2015 were enrolled. Finally, a total of 324 eligible CTO patients received 1G-DES (Paclitaxel-eluting stent or Sirolimus-eluting stent, n = 157) or 2G-DES (Zotarolimus-eluting stent or Everolimus-eluting stent, n = 167) were enrolled. The clinical endpoint was the occurrence of major adverse cardiac events (MACE) defined as all-cause death, recurrent myocardial infarction (re-MI), total repeat revascularization [target lesion revascularization (TLR), target vessel revascularization (TVR), and non-TVR]. We investigated the 5-year major clinical outcomes between 1G-DES and 2G-DES in patient who underwent successful CTO PCI. Results After propensity score matched (PSM) analysis, two well-balanced groups (111 pairs, n = 222, C-statistic = 0.718) were generated. Up to the 5-year follow-up period, the cumulative incidence of all-cause death, re-MI, TLR, TVR and non-TVR were not significantly different between the two groups. Finally, MACE was also similar between the two groups (HR = 1.557, 95% CI: 0.820–2.959, P = 0.176) after PSM. Conclusions In this study, 2G-DES was not associated with reduced long-term MACE compared with 1G-DES following successful CTO revascularization up to five years.

This paper presents the light trapping effect by various front and back ZnO layers in silicon thin-film solar cells. The haze ratio in textured front ZnO layer does not have direct correlation with jsc. The μc-Si:H and a-Si:H/μc-Si:H tandem solar cells prepared on textured ZnO:Al with small feature size (low haze ratio) give higher spectral response throughout whole wavelength range. The a-Si:H top cells act as a buffering layer between front textured ZnO and μc-Si:H layer. By this buffering effect, the carrier recombination by defects, observed in μC-Si:H single pin cells, is suppressed in a-Si:H/μc-Si:H tandem cells. As back reflection materials, doped and undoped ZnO layers with various thickness are incorporated with Ag back metal. In order to prevent reverse diode formation, thickness of undoped ZnO should be thin. However, this thin ZnO is not effective to improve light trapping by internal reflection of red/IR light. In case of doped ZnO, thinner layer gives higer red/IR response due to reduced free carrier absorption.

Background Danshen （Radix Salvia miltiorrhizae） has been used as a traditional medicine in Asia for treatment of various microcirculatory disturbance related diseases. Tanshinones are mainly hydrophobic active components, which have been isolated from Danshen and show various biological functions. In this study, we observed the neuroprotective effect of tanshinone I （Tsl） against ischemic damage in the gerbil hippocampal CA1 region （CA1） after transient cerebral ischemia and examined its neuroprotective mechanism. Methods The gerbils were divided into vehicle-treated-sham-group, vehicle-treated-ischemia-group, Tsl-treated-sham- group, and Tsl-treated-ischemia-group. Tsl was administrated intraperitoneally three times （once a day for three days） before ischemia-reperfusion. The neuroprotective affect of Tsl was examined using H＆E staining, neuronal nuclei （NeuN） immunohistochemistry and Fluoro-Jade B staining. To investigate the neuroprotective mechanism of Tsl after ischemia- reperfusion, immunohistochemical （IHC） and Western blotting analyses for Cu, Zn-superoxide dismutase （SOD1）, Mn- superoxide dismutase （SOD2）, brain-derived neurotrophic factor （BDNF） and insulin-like growth factor-I （IGF-I） were *** Treatment with Tsl protected pyramidal neurons from ischemia-induced neuronal death in the CA1 after ischemia-reperfusion. In addition, treatment with Tsl maintained the levels of SOD1 and SOD2 as determined by IHC and Western blotting in the CA1 after ischemia- reperfusion compared with the vehicle-ischemia-group. In addition, treatment with Tsl increased the levels of BDNF and IGF-I determined by IHC and Westem blotting in the Tsl-treated-sham-group compared with the vehicle-treated- sham-group, and their levels were maintained in the stratum pyramidale of the ischemic CA1 in the Tsl-treated- ischemia-group. Factors associated with significant liver fibrosis assessed using transient elastography in general population

AIM: To investigate the prevalence of significant liver fibrosis assessed using transient elastography(TE) and its predictors in asymptomatic general ***: A total of 159 subjects without chronic viral hepatitis who underwent comprehensive medical health check-up between January 2012 and July 2012 were prospectively recruited. Significant liver fibrosis was defined as liver stiffness value > 7.0 k ***: The mean age and body mass index(BMI) of the study population(men 54.7%) was 56.0 years and 24.3 kg/m2. Among the study subjects, 11(6.9%) showed significant liver fibrosis. On univariate analysis, BMI, alanine aminotransferase(ALT), homeostasis model assessment of insulin resistance, carotid intimal media thickness(IMT), number of calcified plaques on carotid ultrasound, and visceral fat area on computed tomography were significantly higher in subjects with significant liver fibrosis than in those without(all P < 0.05). However, on multivariate analysis, BMI [odds ratio(OR) =1.487; P = 0.045], ALT(OR = 1.078; P = 0.014), carotid IMT(OR = 3.244; P = 0.027), and the number of calcified carotid plaques(OR = 1.787; P = 0.031) were independent predictors of significant liver fibrosis. CONCLUSION: The prevalence of significant liver fibrosis assessed using TE was 6.9% in apparently healthy subjects. High BMI, high ALT, thicker carotid IMT, and higher numbers of calcified carotid plaqueswere independently associated with the presence of significant liver fibrosis.

福利国家体制的可持续发展需要遵循以下两个原则:第一,现金供给运行模式的合理化和确保健全的财政体系及适合市场的机制管理是必要的;第二,便于确保政治方面的支持,同时具有生产性意义的人力资源投资、社会充分就业等社会服务领域向普遍化方向转变。所以,未来韩国福利国家的核心是充分就业为主导的社会服务性国家。这种经济、政治上可持续发展的韩国福利体制转变可以概括为现金供给的合理化改革和社会服务业的扩大这两个方面。另外,为了确保国民整合和对社会政策的支持,还必须坚持向为全体国民提供便利的普遍主义的福利国家方向发展。
