AIM To evaluate the prognostic value of the neutrophil-tolymphocyte ratio(NLR) and platelet-to-lymphocyte ratio(PLR) in patients with colorectal cancer(CRC).METHODS Between April 1996 and December 2010, medical records from a total of 1868 patients with CRC were retrospectively reviewed. The values of simple inflammatory markers including NLR and PLR in predicting the long-term outcomes of these patients were evaluated using Kaplan-Meier curves and Cox regression models.RESULTS The median follow-up duration was 46 mo(interquartile range, 22-73). The estimation of NLR and PLR was based on the time of diagnosis. In multivariate Cox regression analysis, high NLR (≥ 3.0) and high PLR(≥ 160) were independent risk factors predicting poor long-term outcomes in patients with stage Ⅲ and Ⅳ CRC. However, high NLR and high PLR were not prognostic factors in patients with stage Ⅰ and Ⅱ CRC.CONCLUSION In this study, we identified that high NLR (≥ 3.0) and high PLR (≥ 160) are useful prognostic factors to predict long-term outcomes in patients with stage Ⅲ and Ⅳ CRC.

AIM:To compare the macular structure including foveal thickness among patients with optic neuritis(ON)according to the etiology and to investigate the possible correlation between structural and visual outcomes METHODS:In this retrospective cross-sectional study,the clinical data of patients with aquaporin-4 immunoglobulin G-related ON(AQP4 group,40 eyes),myelin oligodendrocyte glycoprotein IgG-related ON(MOG group,31 eyes),and multiple sclerosis-related ON(MS group,24 eyes)were obtained.The retinal thickness of the foveal,parafoveal and perifoveal regions were measured.Visual acuity(VA),visual field index and mean deviation were measured as visual outcomes.RESULTS:The AQP4 group showed a significantly thinner fovea(226.4±13.4μm)relative to the MOG(236.8±14.0μm,P=0.015)and MS(238.9±14.3μm,P=0.007)groups.The thickness in the parafoveal area also was thinner in the AQP4 group,though the difference in perifoveal retinal thickness was not significant.Foveal thickness was correlated with VA in the AQP4 group(coefficientρ=-0.418,P=0.014),but not in the MOG and MS groups(P=0.218 and P=0.138,respectively).There was no significant correlation between foveal thickness and visual field test in all three groups.CONCLUSION:The significant thinning in the fovea and parafoveal areas in the AQP4 group compared to the MOG and MS groups are found.Additionally,macular changes in AQP4-ON show a significant correlation with VA.The results provide the possibility that retinal structural damage could reflect functional damage in AQP4-ON,distinct from MOGON and MS-ON.

The research for the Intelligent Reflecting Surface(IRS)which has the advantages of cost and energy efficiency has been studied.Channel capacity can be effectively increased by appropriately setting the phase value of IRS elements according to the channel conditions.However,the problem of obtaining an appropriate phase value of IRs is difficult to solve due to the non-convex problem.This paper proposes an iterative algorithm for the alternating optimal solution in the Single User Multiple-Input-Multiple-Output(SU-MIMO)systems.The proposed iterative algorithm finds an alternating optimal solution that is the phase value of IRS one by one.The results show that the proposed method has better performance than that of the randomized IRS systems.The number of iterations for maximizing the performance of the proposed algorithm depends on the channel state between the IRS and the receiver.

BACKGROUND Inflammatory bowel disease(IBD)was previously regarded as a Western disease;however,its incidence is increasing in the East.The epidemiology of IBD in Asia differs significantly from the patterns in the West.AIM To comprehensively investigate the epidemiology of IBD in South Korea,inclu-ding its incidence,prevalence,medication trends,and outcomes.METHODS We analyzed claims data from the Health Insurance Review and Assessment Service and Rare and Intractable Diseases(RIDs),operated by the National Health Insurance Service of South Korea.Patients with IBD were identified based on the International Classification of Diseases,Tenth Revision,and RID diagnostic codes for Crohn’s disease(CD)and ulcerative colitis(UC)from 2010 to 2018.RESULTS In total,14498 and 31409 patients were newly diagnosed with CD and UC,respectively,between 2010 and 2018.The annual average incidence of CD was 3.11 cases per 105 person-years,and that of UC was 6.74 cases per 10^(5) person-years.Since 2014,the incidence rate of CD has been stable,while that of UC has steadily increased,shifting the peak age group from 50-year-olds in 2010 to 20-year-olds in 2018.The CD and UC prevalence increased consistently over the study period;the use of 5-aminosali-cylates and corticosteroids gradually decreased,while that of immunomodulators and biologics steadily increased in both CD and UC.The clinical outcomes of IBD,such as hospitalization and surgery,decreased during the study period.CONCLUSION The CD incidence has been stable since 2014,but that of UC has increased with a shift to a younger age at peak incidence between 2010 and 2018.IBD clinical outcomes improved over time,with increased use of immunomodu-lators and biologics.

Carbazole moiety-based 2PACz([2-(9H-carbazol-9-yl)ethyl]phosphonic acid)self-assembled monolayers(SAMs)are excellent hole-selective contact(HSC)materials with abilities to excel the charge-transferdynamics of perovskite solar cells(PSCs).Herein,we report a facile but powerful method to functionalize the surface of 2PACz-SAM,by which reproducible,highly stable,high-efficiency wide-bandgap PSCs can be obtained.The 2PACz surface treatment with various donor number solvents improves assembly of 2PACz-SAM and leave residual surface-bound solvent molecules on 2PACz-SAM,which increases perovskite grain size,retards halide segregation,and accelerates hole extraction.The surface functionalization achieves a high power conversion efficiency(PCE)of 17.62%for a single-junction wide-bandgap(~1.77 e V)PSC.We also demonstrate a monolithic all-perovskite tandem solar cell using surfaceengineered HSC,showing high PCE of 24.66%with large open-circuit voltage of 2.008 V and high fillfactor of 81.45%.Our results suggest this simple approach can further improve the tandem device,when coupled with a high-performance narrow-bandgap sub-cell.

Portal vein thrombosis(PVT)is no longer a definitive contraindication in liver transplants(LTs)[1].Complex vascular reconstructions such as cavoportal hemitransposition(CPHT)[2–5],renoportal anastomosis(RPA)[6,7],and use of sizable collaterals(pericholedochal varix[8,9],coronary vein,peripancreatic or perigastroesophageal varices[10],right superior colic vein[11],ileocolic vein[12],and left gastric vein[13]),or combined liverpancreas-small bowel transplant[14]are required for portal inflow in patients with total portosplenomesenteric thrombosis.

The safety of patients and the quality of medical care provided to them are vital for their wellbeing.This study establishes a set of RFID(Radio Fre-quency Identiﬁcation)-based systems of patient care based on physiological sig-nals in the pursuit of a remote medical care system.The RFID-based positioning system allows medical staff to continuously observe the patient's health and location.The staff can thus respond to medical emergencies in time and appropriately care for the patient.When the COVID-19 pandemic broke out,the proposed system was used to provide timely information on the location and body temperature of patients who had been screened for the disease.The results of experiments and comparative analyses show that the proposed system is superior to competing systems in use.The use of remote monitoring technology makes user interface easier to provide high-quality medical services to remote areas with sparse populations,and enables better care of the elderly and patients with mobility issues.It can be found from the experiments of this research that the accuracy of the position sensor and the ability of package delivery are the best among the other related studies.The presentation of the graphical interface is also the most cordial among human-computer interaction and the operation is simple and clear.

With technological and social development in recent decades,people have begun pursuing more comfortable lives that frequently feature household pets that are treated like members of the family.On average,one out of every three households has a pet.This has also led to the creation and growth of many businesses in the pet industry.A few companies have developed a system that allows busy office workers to remotely care for pets at home based on the Internet of Things and an intelligent adjustment function.As owners of two dogs,the authors of this study observed their pets’living habits and recorded environmental conditions that appear suitable for them.These data were then used to develop an automatic control system to care for pets.The observational data on the pets’habits and environment were written in a program in Arduino by using the ESP8266 Wi-Fi module.The module and booster module control is the switch and setting of various household appliances.According to the loop setting of the program,the system does not need to manually switch or adjust the electrical settings of the environment.Instead,the pet's living environment is assessed by using various sensors.The use of Arduino programs helps develop a system that can automatically adjust the environment to one that is most suitable for the pet's comfort.

Speech plays an extremely important role in social activities.Many individuals suffer from a“speech barrier,”which limits their communication with others.In this study,an improved speech recognitionmethod is proposed that addresses the needs of speech-impaired and deaf individuals.A basic improved connectionist temporal classification convolutional neural network(CTC-CNN)architecture acoustic model was constructed by combining a speech database with a deep neural network.Acoustic sensors were used to convert the collected voice signals into text or corresponding voice signals to improve communication.The method can be extended to modern artificial intelligence techniques,with multiple applications such as meeting minutes,medical reports,and verbatim records for cars,sales,etc.For experiments,a modified CTC-CNN was used to train an acoustic model,which showed better performance than the earlier common algorithms.Thus a CTC-CNN baseline acoustic model was constructed and optimized,which reduced the error rate to about 18%and improved the accuracy rate.
