A rational regulation of the solar water splitting reaction pathway by adjusting the surface composition and phase structure of catalysts is a substantial approach to ameliorate the sluggish reaction kinetics and improve the energy conversion efficiency.In this study,we demonstrate a nanocrystalline iron pyrophosphate(Fe_(4)(P_(2)O_(7))_(3),FePy)-regulated hybrid overlayer with amorphous iron phosphate(FePO_(4),FePi)on the surface of metal oxide nanostructure with boosted photoelectrochemical(PEC)water oxidation.By manipulating the facile electrochemical surface treatment followed by the phosphating process,nanocrystalline FePy is localized in the FePi amorphous overlayer to form a heterogeneous hybrid structure.The FePy-regulated hybrid overlayer(FePy@FePi)results in significantly enhanced PEC performance with long-term durability.Compared with the homogeneous FePi amorphous overlayer,FePy@FePi can improve the charge transfer efficiency more significantly,from 60% of FePi to 79%of FePy@FePi.Our density-functional theory calculations reveal that the coexistence of FePi and FePy phases on the surface of metal oxide results in much better oxygen evolution reaction kinetics,where the FePi was found to have a typical down-hill reaction for the conversion from OH*to O_(2),while FePy has a low free energy for the formation of OH*.

Flexible logic circuits and memory with ultra-low static power consumption are in great demand for battery-powered flexible electronic systems. Here, we show that a flexible nonvolatile logic-in-memory circuit enabling normally-off computing can be implemented using a poly（1,3,5-trivinyl-l,3,5-trimethyl cyclotrisiloxane）（pV3D3）-based memristor array. Although memristive logic-in-memory circuits have been previously reported, the requirements of additional components and the large variation of memristors have limited demonstrations to simple gates within a few operation cycles on rigid substrates only. Using memristor-aided logic （MAGIC） architecture requiring only memristors and pV3D3-memristor with good uniformity on a flexible substrate, for the first time, we experimentally demonstrated our implementation of MAGIC-NOT and -NOR gates during multiple cycles and even under bent conditions. Other functions, such as OR, AND, NAND, and a half adder, are also realized by combinations of NOT and NOR gates within a crossbar array. This research advances the development of novel computing architecture with zero static power consumption for battery- powered flexible electronic systems.

Early extrahepatic recurrence as a pivotal factor for survival after hepatocellular carcinoma resection:A 15-year observational study

BACKGROUND Surgical resection is one of the most widely used modalities for the treatment of hepatocellular carcinoma(HCC).Early extrahepatic recurrence(EHR)of HCC after surgical resection is considered to be closely associated with poor prognosis.However,data regarding risk factors and survival outcomes of early EHR after surgical resection remain scarce.AIM To investigate the clinical features and risk factors of early EHR and elucidate its association with survival outcomes.METHODS From January 2004 to December 2019,we enrolled treatment-naïve patients who were≥18 years and underwent surgical resection for HCC in two tertiary academic centers.After excluding patients with tumor types other than HCC and/or ineligible data,this retrospective study finally included 779 patients.Surgical resection of HCC was performed according to the physicians’decisions and the EHR was diagnosed based on contrast-enhanced computed tomography or magnetic resonance imaging,and pathologic confirmation was performed in selected patients.Multivariate Cox regression analysis was performed to identify the variables associated with EHR.RESULTS Early EHR within 2 years after surgery was diagnosed in 9.5%of patients during a median followup period of 4.4 years.The recurrence-free survival period was 5.2 mo,and the median time to EHR was 8.8 mo in patients with early EHR.In 52.7%of patients with early EHR,EHR occurred as the first recurrence of HCC after surgical resection.On multivariate analysis,serum albumin100 U/L,surgical margin involvement,venous and/or lymphatic involvement,satellite nodules,tumor necrosis detected by pathology,tumor size≥7 cm,and macrovascular invasion were determined as risk factors associated with early EHR.After sub-categorizing the patients according to the number of risk factors,the rates of both EHR and survival showed a significant correlation with the risk of early EHR.Furthermore,multivariate analysis revealed that early EHR was associated with substantially worse survi

BACKGROUND Hepatocellular carcinoma(HCC)is one of the most common cancers worldwide and has a high mortality.However,the treatment options for advanced HCC are limited to tyrosine kinase inhibitors,such as sorafenib and lenvatinib.Since previous regimens have an insufficient efficacy,the combination therapy of atezolizumab and bevacizumab(Ate/Bev)has been investigated,which showed an improvement in progression-free and overall survival.However,the adverse events of this combination therapy in advanced HCC have not been established.Herein,we report a novel case of an unresectable HCC and acute respiratory distress syndrome(ARDS)after a combination therapy of Ate/Bev.CASE SUMMARY An 82-year-old male visited our outpatient clinic for an incidentally detected liver mass.Liver magnetic resonance imaging and enhanced chest computed tomography(CT)were performed,which showed arterial hyperenhancement with washout in delayed phase suggesting HCC,and a well-defined metastatic solid nodule,respectively.F-18 fluorodeoxyglucose positron emission tomography(PET)-CT exhibited multiple hypermetabolic lesions in the iliac bone,lumbar vertebrae,and femur.Because of the high burden of the intrahepatic tumor,transarterial radioembolization was initially performed;after 37 d,a combination therapy of Ate/Bev was administered.The patient visited the emergency department three days after Ate/Bev treatment complaining of dyspnea.He was diagnosed with severe pneumonitis based on CT.Despite administering oxygen via a high-flow nasal cannula,the P/F ratio was only 74;therefore,the patient was diagnosed with ARDS based on the overall examination results.Low tidal volume with high positive end-expiratory pressure,sedative agents combined with a neuromuscular blocker,and a systemic steroid were promptly applied to manage the ARDS.However,the patient did not recover from the hypoxia and expired 31 h after being admitted.CONCLUSION Clinicians should be aware of severe pneumonitis due to the immune-related adverse events of this combination thera

Underwater endoscopic mucosal resection for neoplasms in the pyloric ring of the stomach: Four case reports

BACKGROUND Tumors located in the pylorus are technically more complex to resect by endoscopic resection,as the anatomical characteristics of this region can affect the adequate assessment of margins and performance of the procedure.We reported the results of underwater endoscopic mucosal resection(UEMR)of benign mucosal neoplasms located in the pyloric ring.CASE SUMMARY This case series describes 4 patients with 4 mucosal neoplasms located in the pyloric ring.The diameter of each neoplasm was less than 15 mm.We performed UEMR for the lesions.Water immersion enabled slight floating of the lesions,resulting in easy identification.We achieved en bloc resection with a snare and electrosurgical unit.All procedure were performed within 3 min without adverse events.Pathologic examination showed low-grade dysplasia with clear resection margins in one case and hyperplastic polyps in three cases.CONCLUSION UEMR can be an effective and safe treatment method for neoplasms in the gastric pyloric ring.

In this study, we report Monopis crocicapitella （Clemens, 1859）（Tineidae）, which was collected from bat guano in a cave in the southern region of Korea, for the first time from East Asia. We briefly redescribe and illustrate the external morphology and genital structures of both sexes. Also, we discuss the local habitat characteristics and some of the ecological information that was observed during our field investi-gation.

Three-dimensional label-free morphology of CD8+T cells as a sepsis biomarker

Sepsis is a dysregulated immune response to infection that leads to organ dysfunction and is associated with a high incidence and mortality rate.The lack of reliable biomarkers for diagnosing and prognosis of sepsis is a major challenge in its management.We aimed to investigate the potential of three-dimensional label-free CD8+T cell morphology as a biomarker for sepsis.This study included three-time points in the sepsis recovery cohort(N=8)and healthy controls(N=20).Morphological features and spatial distribution within cells were compared among the patients'statuses.We developed a deep learning model to predict the diagnosis and prognosis of sepsis using the internal cell morphology.Correlation between the morphological features and clinical indices were analysed.Cell morphological features and spatial distribution differed significantly between patients with sepsis and healthy controls and between the survival and non-survival groups.The model for predicting the diagnosis and prognosis of sepsis showed an area under the receiver operating characteristic curve of nearly 100%with only a few cells,and a strong correlation between the morphological features and clinical indices was observed.Our study highlights the potential of three-dimensional label-free CD8+T cell morphology as a promising biomarker for sepsis.This approach is rapid,requires a minimum amount of blood samples,and has the potential to provide valuable information for the early diagnosis and prognosis of sepsis.

Dear Editor,Recently,there has been a rise in interest in the roles of cytokines and cytokine receptors in malignant human tumors.Cytokine receptors,especially typeⅡinterleukin-4 receptor(IL-4R),have recently been examined as novel therapeutic targets of human cancers[1-3].The typeⅡIL-4R complex is composed of IL-4Rαand interleukin-13 receptorα1(IL-13Rα1),and the expression of IL-4Rαand IL-13Rα1 was higher in cancer tissues compared with normal counterpart tissues[3,4].

Clinical characteristics and prognosis of patients with ulcerative colitis that shows rectal sparing at initial diagnosis

BACKGROUND Ulcerative colitis(UC)is characterised by mucosal inflammation from the rectum to its proximal area in a symmetric and continuous fashion.However,although uncommon,we encounter cases of UC with rectal sparing in the initial stage.AIM To evaluate the clinical characteristics and clinical course for rectal sparing UC compared with typical UC.METHODS We looked at records from 2004 to 2015,and selected patients who were newly diagnosed with UC,and who could be followed up for at least 5 years in our hospital.We then retrospectively analysed the medical records and endoscopic findings of those patients.To compare the clinical course and prognosis,we matched each patient with rectal sparing UC 1:3 with controls by age,sex,and disease extent.RESULTS Of 619 UC patients,24(3.9%)showed rectal sparing at diagnosis.During the follow-up period(median 8 years),in two(8.3%)of the 24 patients,rectal sparing remained through follow-up inspections;but for the other 22(91.7%)patients,obvious rectal inflammation was found at follow-up endoscopy.Of the 24 patients,8(33.3%)were initially misdiagnosed with infectious colitis.No diagnosis was changed to Crohn’s disease.The uses of corticosteroid or biologic agents,hospitalisation rate,and colectomy rates were not different between the rectal sparing UC group and typical UC group.CONCLUSION Some patients with UC can reveal atypical patterns of disease distribution,such as rectal sparing in its initial stage;but despite this,the clinical course and prognosis may not differ from those of typical UC patients.

Photoelectrochemical(PEC)H_(2)O_(2)production through water oxidation reaction(WOR)is a promising strategy,however,designing highly efficient and selective photoanode materials remains challenging due to competitive reaction pathways.Here,for highly enhanced PEC H_(2)O_(2)production,we present a conformal amorphous titanyl phosphate(a-TP)overlayer on nanoparticulate TiO_(2)surfaces,achieved via lysozyme-molded in-situ surface reforming.The a-TP overlayer modulates surface adsorption energies for reaction intermediates,favoring WOR for H_(2)O_(2)production over the competing O_(2)evolution reaction.Our density functional theory calculations reveal that a-TP/TiO_(2)exhibits a substantial energy uphill for the O·*formation pathway,which disfavors O_(2)evolution but promotes H_(2)O_(2)production.Additionally,the a-TP overlayer strengthens the built-in electric field,resulting in favorable kinetics.Consequently,a-TP/TiO_(2)exhibits 3.7-fold higher Faraday efficiency(FE)of 63%at 1.76 V vs.reversible hydrogen electrode(RHE)under 1 sun illumination,compared to bare TiO_(2)(17%),representing the highest FE among TiO_(2)-based WOR H_(2)O_(2)production systems.Employing the a-TP overlayer constitutes a promising strategy for controlling reaction pathways and achieving efficient solar-to-chemical energy conversion.
