Simultaneous laparoscopic and arthroscopic excision of a huge juxta-articular ganglionic cyst compressing the sciatic nerve:A case report

BACKGROUND A large ganglionic cyst extending from the hip joint to the intrapelvic cavity through the sciatic notch is a rare space-occupying lesion associated with compressive lower-extremity neuropathy.A cyst in the pelvic cavity compressing the intrapelvic-sciatic nerve is easily missed in the diagnostic process because it usually presents as atypical symptoms of an extraperitoneal-intrapelvic *** present a case of a huge ganglionic cyst that was successfully excised laparoscopically and endoscopically by a gynecologist and an orthopedic *** SUMMARY A 52-year-old woman visited our hospital complaining of pain and numbness in her left buttock while *** pain began 3 years ago and worsened,while the numbness in the left lower extremity lasted 1 *** was diagnosed and unsuccessfully treated at several tertiary referral centers many years *** resonance imaging revealed a suspected paralabral cyst(5 cm×5 cm×4.6 cm)in the left hip joint,extending to the pelvic cavity through the greater sciatic *** CA-125 and CA19-9 tumor marker levels were within normal ***,the cyst was compressing the sciatic ***,endoscopic and laparoscopic neural decompression and mass excision were performed simultaneously.A laparoscopic examination revealed a tennis-ball-sized cyst filled with gelatinous liquid,stretching deep into the hip *** excisional biopsy performed in the pelvic cavity and deep gluteal space confirmed the accumulation of ganglionic cysts from the hip joint into the extrapelvic intraperitoneal *** Intra-or extra-sciatic nerve-compressing lesion should be considered in cases of sitting pain radiating down the ipsilateral lower *** large juxta-articular ganglionic cyst was successfully treated simultaneously using laparoscopy and arthroscopy.

Anti-listerial synergism of leaf essential oil of Metasequoia glyptostroboides with nisin in whole,low and skim milks

Objective:To examine the individual and synergistic anti-listcrial effect of nisin and leaf essential oil of Metasequoia glyptostroboides(***) against one of the leading foodhorne pathogens Listeria monocytogenes(***) ATCC 19116 in milk ***:The whole(8%),low(1%) and skim(no fat content) milk samples were inoculated with *** ATCC 19116 along with leaf essential oil of *** or nisin alone as well in ***:In this study,the leaf essential oil at the concentrations of 2% and 5% revealed strong anti-listerial effect against *** ATCC 19116 in all categories of milk *** at the concentrations of 250 and 500 IU/mL displayed a strong inhibitory effect against ATCC 19116 as compared to the control ***,synergistic combinations of leaf essential oil(1%) and nisin(62.5,125.250 and 500 IU/mL) also had a remarkable anti-listerial synergism in all the tested milk samples including whole,low and skim milk after 14 ***:As a major finding,the leaf essential oil of *** might he a useful candidate for using in food industry to control the growth of foodhorne pathogenic bacteria as confirmed by its potent anti-listerial Synergistic effect with nisin against *** ATCC 19116 in different milk samples.

Reinfection rate and endoscopic changes after successful eradication of Helicobacter pylori

AIM:To determine the long-term outcomes regarding reinfection with Helicobacter pylori(***)and endoscopic changes after successful *** eradication. METHODS:From June 1994 to January 2007,186 patients(M:F=98:88;mean age 50.0±11.4 years), in whom *** had been successfully eradicated, were *** mean duration of follow up was 41.2±24.0 mo. Combination of Korean Red Ginseng Extract and Hydrogen-Rich Water Improves Spermatogenesis and Sperm Motility in Male Mice

Objective: To investigate the effect of hydrogen-rich Korean Red Ginseng(KRG) water(HRGW) mixture on the spermatogenesis and sperm motility of mice of different ages. Methods: Eighty young(3 month-old) and aged(12 month-old) male mice were randomly assigned to 4 groups(n=10 per group) including control group, hydrogen-rich water(HRW) group(10 m L/kg daily), KRG group(50 mg/kg daily) and HRGW group(10 m L/kg HRW and 50 mg/kg KRG daily) by an oral zoned needle for 4 weeks. Sperm count and motility were measured using sperm suspension released from cauda epididymis. Serum follicle stimulating hormone(FSH), luteinizing hormone(LH), testosterone, and reactive oxygen species(ROS) in serum have also been estimated. Tubular changes were examined through histological hematoxylin and eosin staining. Expression of antioxidation(PPx3, PPx4, GSTm5 and GPx4), spermatogenesis(inhibin-α, neptin-2 and CREM), antiaging(SIRT1 and SIRT2), and angiogenesis [visfatin and vascular endothelial growth factor(VEGF)] related genes were examined through realtime polymerase chain reaction. Results: HRW and KRG treatment stimulated spermatogenesis followed by increasing sperm production and sperm motility(P<0.05). These effects were strengthened synergistically by a HRGW mixture(P<0.05 or P<0.01). HRGW greatly increased the expressions of antioxidation, antiaging, spermatogenesis related genes and VEGF especially in aged mice(P<0.05). Serum testosterone and FSH levels also increased, while serum ROS level decreased(all P<0.05). Conclusion: HRGW increases sperm production and motility by enhancing antioxidation and stimulating spermatogenesis and sex hormone production, particularly in aged mice.

Cholecystectomy is independently associated with nonalcoholic fatty liver disease in an Asian population

AIM: To investigate the relationship between gallstone disease and nonalcoholic fatty liver disease(NAFLD) in a large Asian population. METHODS: A cross-sectional study including 17612 subjects recruited from general health check-ups at the Seoul National University Hospital, Healthcare System Gangnam Center between January 2010 and December 2010 was conducted. NAFLD and gallstone disease were diagnosed based on typical ultrasonographic findings. Subjects who were positive for hepatitis B or C, or who had a history of heavy alcohol consumption(> 30 g/d for men and > 20 g/d for women) or another type of hepatitis were excluded. Gallstone disease was defined as either the presence of gallstones or previous cholecystectomy, and these two entities(gallstones and cholecystectomy) were analyzed separately. Clinical parameters including body mass index, waist circumference, hypertension, diabetes, smoking status, and regular physical activity were reviewed. Laboratory parameters, including serum levels of gamma-glutamyl transpeptidase, alanine aminotransferase, aspartate aminotransferase, fasting glucose, fasting insulin, total cholesterol, triglycerides, and high-density lipoprotein, were also ***: The mean age of the subjects was 48.5± 11.3 years, and 49.3% were male. Approximately 30.3% and 6.1% of the subjects had NAFLD and gallstone disease, respectively. The prevalence of gallstone disease(8.3% vs 5.1%, P < 0.001), including both the presence of gallstones(5.5% vs 3.4%, P < 0.001) and a history of cholecystectomy(2.8% vs 1.7%, P < 0.001), was significantly increased in the NAFLD group. In the same manner, the prevalence of NAFLD increased with the presence of gallstone disease(41.3% vs 29.6%, P < 0.001). Multivariate regression analysis showed that cholecystectomy was associated with NAFLD(OR = 1.35, 95%CI: 1.03-1.77, P = 0.028). Preparation and characterization of Li_(4)Ti_(5)O_(12) synthesized using hydrogen titanate nanowire for hybrid super capacitor

The electrical characteristics of hybrid super capacitor were evaluated by synthesizing LTO(Li_(4)Ti_(5)O_(12))using TiO_(2) having a hydrogen titanate nanowire *** of the hydrogen titanate nanowire was implemented by using TiO_(2) having size of 60 nm and NaOH,and performing synthesis at 70℃for 6 h with a sonochemical *** compound was synthesized at 150℃for 36 h and at 180℃for 36 h respectively by using the hydrogen titanate nanowire and LiOH·H2O as starting materials with a hydrothermal *** final LTO compound was synthesized at 700℃for 6 h using a solid-state *** a result of manufacturing the hybrid super capacitor using LTO synthesized at 180℃for 36 h with the hydrothermal method,a capacity of 198 mA·h/g has been achieved compared to a theoretical capacity of 172 mA·h/g of existing LTO,and thus,the capacity has been increased by about 13%.Further,such excellent cycle performance has ensured its possibility as a high-capacity capacitor.

Development of practical postprocessor for 5-axis machine tool with non-orthogonal rotary axes

In order to develop a practical postprocessor for 5-axis machine tool,the general equations of numerically controlled(NC) data for 5-axis configurations with non-orthogonal rotary axes were exactly expressed by the inverse kinematics,and a windows-based postprocessor written with Visual Basic was developed according to the proposed *** developed postprocessor is a general system suitable for all kinds of 5-axis machines with orthogonal and non-orthogonal rotary *** implementation of the developed postprocessor and verification by a cutting simulation and machining experiment,the effectiveness of the proposed algorithm is *** is improved by allowing exchange of data formats such as rotational total center position(RTCP) controlled NC data,vector post NC data,and program object file(POF) cutter location(CL) data,and convenience is increased by adding the function of work-piece origin ***,a practical post-processor for 5-axis machining is developed.

Continuity control method of cutter posture vector for efficient five-axis machining

During five-axis machining of impeller, the excessive local interference avoidance leads to inconsistency of cutter posture, low quality of machined surface and increase of processing time. Therefore, in order to improve the efficiency of five-axis machining of impellers, it is necessary to minimize the cutter posture changes and create a continuous tool path while avoiding interference. By using an MC-space algorithm for interference avoidance, an MB-spline algorithm for continuous control was intended to create a five-axis machining tool path with excellent surface quality and economic feasibility. A five-axis cutting experiment was performed to verify the effectiveness of the continuity control. The result shows that the surface shape with continuous method is greatly improved, and the surface roughness is generally favorable. Consequently, the effectiveness of the suggested method is verified by identifying the improvement of efficiency of five-axis machining of an impeller in aspects of surface quality and machining time.

Throughput-oriented associated transaction assignment in sharding blockchains for IoT social data storage

Blockchain is a viable solution to provide data integrity for the enormous volume of 5G IoT social data, while we need to break through the throughput bottleneck of blockchain. Sharding is a promising technology to solve the problem of low throughput in blockchains. However, cross-shard communication hinders the effective improvement of blockchain throughput. Therefore, it is critical to reasonably allocate transactions to different shards to improve blockchain throughput. Existing research on blockchain sharding mainly focuses on shards formation, configuration, and consensus, while ignoring the negative impact of cross-shard communication on blockchain throughput. Aiming to maximize the throughput of transaction processing, we study how to allocate blockchain transactions to shards in this paper. We propose an Associated Transaction assignment algorithm based on Closest Fit (ATCF). ATCF classifies associated transactions into transaction groups which are then assigned to different shards in the non-ascending order of transaction group sizes periodically. Within each epoch, ATCF tries to select a shard that can handle all the transactions for each transaction group. If there are multiple such shards, ATCF selects the shard with the remaining processing capacity closest to the number of transactions in the transaction group. When no such shard exists, ATCF chooses the shard with the largest remaining processing capacity for the transaction group. The transaction groups that cannot be completely processed within the current epoch will be allocated in the subsequent epochs. We prove that ATCF is a 2-approximation algorithm for the associated transaction assignment problem. Simulation results show that ATCF can effectively improve the blockchain throughput and reduce the number of cross-shard transactions.
