Pancreatic ductal adenocarcinoma(PDAC)is one of the most aggressive solid malignancies.A specific mechanism of its metastasis has not been established.In this study,we investigated whether Neural Wiskott-Aldrich syndrome protein(N-WASP)plays a role in distant metastasis of PDAC.We found that N-WASP is markedly expressed in clinical patients with PDAC.Clinical analysis showed a notably more distant metastatic pattern in the N-WASP-high group compared to the N-WASP-low group.N-WASP was noted to be a novel mediator of epithelialmesenchymal transition(EMT)via gene expression profile studies.Knockdown of N-WASP in pancreatic cancer cells significantly inhibited cell invasion,migration,and EMT.We also observed positive association of lysyl oxidase-like 2(LOXL2)and focal adhesion kinase(FAK)with the N-WASP-mediated response,wherein EMT and invadopodia function were modulated.Both N-WASP and LOXL2 depletion significantly reduced the incidence of liver and lung metastatic lesions in orthotopic mouse models of pancreatic cancer.These results elucidate a novel role for N-WASP signaling associated with LOXL2 in EMT and invadopodia function,with respect to regulation of intercellular communication in tumor cells for promoting pancreatic cancer metastasis.These findings may aid in the development of therapeutic strategies against pancreatic cancer.

Recently,with the normalization of non-face-to-face online environments in response to the COVID-19 pandemic,the possibility of cyberattacks through endpoints has increased.Numerous endpoint devices are managed meticulously to prevent cyberattacks and ensure timely responses to potential security threats.In particular,because telecommuting,telemedicine,and teleeducation are implemented in uncontrolled environments,attackers typically target vulnerable endpoints to acquire administrator rights or steal authentication information,and reports of endpoint attacks have been increasing considerably.Advanced persistent threats(APTs)using various novel variant malicious codes are a form of a sophisticated attack.However,conventional commercial antivirus and anti-malware systems that use signature-based attack detectionmethods cannot satisfactorily respond to such attacks.In this paper,we propose a method that expands the detection coverage inAPT attack environments.In this model,an open-source threat detector and log collector are used synergistically to improve threat detection performance.Extending the scope of attack log collection through interworking between highly accessible open-source tools can efficiently increase the detection coverage of tactics and techniques used to deal with APT attacks,as defined by MITRE Adversarial Tactics,Techniques,and Common Knowledge(ATT&CK).We implemented an attack environment using an APT attack scenario emulator called Carbanak and analyzed the detection coverage of Google Rapid Response(GRR),an open-source threat detection tool,and Graylog,an open-source log collector.The proposed method expanded the detection coverage against MITRE ATT&CK by approximately 11%compared with that conventional methods.

Cognitive development theories differ on how young students can meaningfully process new information and retain that information for future knowledge-building through scaffolding within their zone of proximal development.More traditional theories like the cognitive load theory adhere to the rote memorization approach by categorizing students as passive learners and the teachers as initiators who provide information in a structured,often rigid format,to be stored and retrieved for future application using their working memory.In contrast,the more progressive theories,like constructivism,are premised on the belief that students should proactively initiate their own learning while teachers act more as facilitators.The current trend in government policy under ESSA is to embrace the latter approach in the classroom,which is also more inclusive of all types of students,especially neurodiverse students.Moreover,teachers can utilize the wider range of assistive technology tools to accommodate and support their students’unique learning styles.

With the development of the 5th generation of mobile communi-cation(5G)networks and artificial intelligence(AI)technologies,the use of the Internet of Things(IoT)has expanded throughout industry.Although IoT networks have improved industrial productivity and convenience,they are highly dependent on nonstandard protocol stacks and open-source-based,poorly validated software,resulting in several security vulnerabilities.How-ever,conventional AI-based software vulnerability discovery technologies cannot be applied to IoT because they require excessive memory and com-puting power.This study developed a technique for optimizing training data size to detect software vulnerabilities rapidly while maintaining learning accuracy.Experimental results using a software vulnerability classification dataset showed that different optimal data sizes did not affect the learning performance of the learning models.Moreover,the minimal data size required to train a model without performance degradation could be determined in advance.For example,the random forest model saved 85.18%of memory and improved latency by 97.82%while maintaining a learning accuracy similar to that achieved when using 100%of data,despite using only 1%.

We demonstrate a new type of multifocal and extended depth of focus(EDOF)intraocular lenses(IOLs)embeddingμm-thin geometric phase(GP)lens layers.As an emerging approach for lens phase design,the GP modulated IOLs outperform conventional diffractive IOLs in multifocality while completely avoiding the clinically undesirable demand for additional surface patterns to standard monofocal IOL designs.The number of foci and light splitting ratio of the GP IOLs are adjusted by changing the number of stacked GP layers and the thickness of each layer.Bifocal and trifocal GP IOLs are fabricated by radial alignment of anisotropic orientation in UV-curable liquid crystal polymers.After characterizing the defocus image and modulation transfer function of the GP IOLs,it is expected that GP IOLs will alleviate the most common problems associated with multifocal and EDOF IOLs,blurred vision and photic phenomena caused by light scattering and posterior capsule opacification.

BACKGROUND Although intestinal obstruction is one of the most common surgical emergencies in an infant,it is difficult to diagnose neonatal enteric duplication cysts(EDC)preoperatively owing to their rarity as a cause of intestinal obstruction.We describe a case report of a neonatal EDC presenting intestinal obstruction and shock.CASE SUMMARY A 32-d-old male infant with a prenatal sonographic finding of bladder distension was admitted to our hospital for a severely distended abdomen,fever,and oliguria.The first diagnostic hypothesis was septic shock and intestinal obstruction.The patient’s symptoms worsened;following an emergency surgical exploratory laparotomy and histopathological findings,the final diagnosis of cecal duplication cyst was confirmed.The patient’s postoperative course was uneventful,and on the fifth postoperative day,oral feeding restarted.Twenty days later,the patient was discharged from the hospital.CONCLUSION Although EDC located in the cecum is exceptional,it should be considered when evaluating suspected intestinal obstruction and shock.

OBJECTIVE:Chemotherapeutic agents such as docetaxel(DTX)can trigger chemotherapy-induced peripheral neuropathy(CIPN),which is characterized by unbearable pain.This study was designed to investigate the analgesic effect and related neuronal mechanism of low-frequency median nerve stimulation(LFMNS)on DTX-induced tactile hypersensitivity in mice.METHODS:To produce CIPN,DTX was administered intraperitoneally 4 times,once every 2 d,to male ICR mice.LFMNS was performed on the wrist area,and the pain response was measured using von Frey filaments on both hind paws.Western blot and immunofluorescence staining were performed using dorsal root ganglion and spinal cord samples to measure the expression of brainderived neurotrophic factor(BDNF).RESULTS:Repeated LFMNS significantly attenuated the DTX-induced abnormal sensory response and suppressed the enhanced expression of BDNF in the DRG neurons and spinal dorsal area.CONCLUSIONS:LFMNS might be an effective nonpharmaceutical option for treating patients suffering from CIPN via regulating the expression of peripheral and central BDNF.

Background: Electroacupuncture(EA) may reduce the severity of acute pancreatitis(AP) and provide additional pain relief in patients with chronic pancreatitis. However, the ability of EA to relieve pain in patients with AP has not been well documented.Objective: This study was undertaken to compare the pain-relieving effects of EA and conventional treatment in patients with AP.Design, setting, participants and interventions: This study was conducted using a randomized, controlled,three-arm, parallel-group and multi-center design. Patients diagnosed with AP were randomly and equally assigned to EA1, EA2 or control groups. All participants received conventional standard-of-care therapy for AP. Local EA alone was administered in EA1, and local plus distal EA was given in EA2.Local EA included two abdominal acupoints, while distal EA included twelve peripheral acupoints. EA groups underwent one session of EA daily for 4 days(days 1–4), or until pain was resolved or discharged.Main outcome measures: The primary outcome measure was the change in the visual analogue scale(VAS;0–100) pain score between baseline and day 5.Results: Eighty-nine participants were randomized into EA1, EA2 and control groups, and 88(EA1, 30;EA2, 29;control, 29) were included in the full-analysis set. VAS score change(median [interquartile range]) on day 5 was(12.3 ± 22.5) in the EA1 group,(10.3 ± 21.5) in the EA2 group, and(8.9 ± 15.2) in the control group. There were not significant differences in the change in VAS score among treatments(P = 0.983). However, time to food intake was significantly shorter in the EA group(EA1 + EA2) than in the control group(median 2.0 days vs 3.0 days), with a hazard ratio of 0.581(P = 0.022;95% CI,0.366–0.924). No significant adverse events occurred.Conclusion: EA treatment did not significantly reduce pain after 4 days of treatment in patients with AP-associated abdominal pain but significantly reduced time to first food intake.

Effectiveness and safety of auriculotherapy for breastfeeding: a systematic review

OBJECTIVE:To summarize and critically evaluate the evidence pertaining to the effectiveness and safety of auriculotherapy for breastfeeding.METHODS:We performed a systematic search of10 databases from their inception dates to May 15,2017.Randomized controlled trials evaluating the effectiveness and safety of auriculotherapy for breastfeeding were included.Risk of bias was assessed using the Cochrane risk of bias tool.RESULTS:This review included 31 randomized controlled trials with 5389 participants.Owing to the obvious clinical or statistical heterogeneity,a Meta-analysis was not conducted.Twenty-nine studies compared auriculotherapy plus routine care with routine care alone.No consistent adjunctive effect of auriculotherapy was observed in increasing the volume of milk production(9 studies showed significant results in favor of auriculotherapy and 5 did not)and serum prolactin level(7 studies showed significant results in favor of auriculotherapy and 2 did not),and in facilitating the initiation of milk secretion(10 studies showed significant results in favor of auriculotherapy and 7 did not).Two studies compared auriculotherapy with no treatment.Auriculotherapy as a monotherapy showed significant effects in increasing serum prolactin level and facilitating the onset of milk secretion;however,it was limited and inconclusive because only two studies were included in this comparison.The subgroup analysis of women who underwent cesarean delivery showed inconsistent results with respect to all outcomes.No serious adverse events were reported with the use of auriculotherapy.The methodological quality of the included studies was generally poor.CONCLUSION:Although some studies showed positive results,no definite conclusion about the effectiveness and safety of auriculotherapy for breastfeeding could be drawn,owing to the low methodological quality of the included studies and the heterogeneity among trials.Large-scale,well-designed studies are warranted on this topic.
