AIM: To examine if the rate of decrease in serum bilirubin after preoperative biliary drainagecan be used as a predicting factor for surgical complications and postoperative recovery after pancreaticoduodenectomy in patients with distal common bile duct ***: A retrospective study was performed in 49 consecutive patients who underwent pancrea-ticoduodenectomy for distal common bile duct cancer. Potential risk factors were compared between the complicated and uncomplicated groups. Also, the rates of decrease in serum bilirubin were compared pre- and postoperatively. RESULTS: Preoperative biliary drainage (PBD) was performed in 40 patients (81.6%). Postoperative morbidity and mortality rates were 46.9% (23/49) and 6.1% (3/49), respectively. The presence or absence of PBD was not different between the complicated and uncomplicated groups. In patients with PBD, neither the absolute level nor the rate of decrease in serum bilirubin was significantly different. Patients with rapid decrease preoperatively showed faster decrease during the first postoperative week (5.5 ± 4.4 μmol/L vs -1.7 ± 9.9 μmol/L, P = 0.004).CONCLUSION: PBD does not affect the surgical outcome of pancreaticoduodenectomy in patients with distal common bile duct cancer. There is a certain group of patients with a compromised hepatic excretory function, which is represented by the slow rate of decrease in serum bilirubin after PBD.

We demonstrate a femtosecond Cr：YAG laser mode-locked by a carbon nanotube saturable absorber mirror（CNT-SAM） at a repetition rate of 550 MHz. By employing the CNT-SAM, which exhibits a modulation depth of 0.51% and a saturation fluence of 28 μJ∕cm^2 at 1.5 μm, we achieved a compact bulk Cr：YAG laser with selfstarting mode-locked operation near 1.5 μm, delivering an average output power of up to 147 m W and a pulse duration of 110 fs. To our knowledge, this system provides the highest repetition rate among reported CNT-SAM mode-locked Cr：YAG lasers and the shortest pulse duration among saturable absorber mode-locked Cr：YAG lasers with repetition rates above 500 MHz.

Hemorrhagic hemangioma in the liver:A case report

Hemangioma is the most common type of benign tumor that arises in the liver. Although rupture and hemorrhage of hepatic hemangioma are rare complications, they can be the cause of mortality. The authors report a case of hemorrhagic hepatic hemangioma: in a 54-yearold woman who was admitted with epigastric *** had taken oral contraceptives several weeks *** results of a blood examination were normal. An abdominal computed tomography scan revealed a tumor in hepatic segment 4, and a hemorrhage inside the cystic mass was suspected. The mass was removed laparoscopically to confirm the tumor properties and control the hemorrhage. The pathologic findings of the resected mass were consistent with hepatic hemangioma with intratumoral hemorrhage. The patient was discharged 8 d after the surgery, without further complications or complaints, and the patient's condition was found to have improved during follow-up.

BACKGROUND A large ganglionic cyst extending from the hip joint to the intrapelvic cavity through the sciatic notch is a rare space-occupying lesion associated with compressive lower-extremity neuropathy.A cyst in the pelvic cavity compressing the intrapelvic-sciatic nerve is easily missed in the diagnostic process because it usually presents as atypical symptoms of an extraperitoneal-intrapelvic *** present a case of a huge ganglionic cyst that was successfully excised laparoscopically and endoscopically by a gynecologist and an orthopedic *** SUMMARY A 52-year-old woman visited our hospital complaining of pain and numbness in her left buttock while *** pain began 3 years ago and worsened,while the numbness in the left lower extremity lasted 1 *** was diagnosed and unsuccessfully treated at several tertiary referral centers many years *** resonance imaging revealed a suspected paralabral cyst(5 cm×5 cm×4.6 cm)in the left hip joint,extending to the pelvic cavity through the greater sciatic *** CA-125 and CA19-9 tumor marker levels were within normal ***,the cyst was compressing the sciatic ***,endoscopic and laparoscopic neural decompression and mass excision were performed simultaneously.A laparoscopic examination revealed a tennis-ball-sized cyst filled with gelatinous liquid,stretching deep into the hip *** excisional biopsy performed in the pelvic cavity and deep gluteal space confirmed the accumulation of ganglionic cysts from the hip joint into the extrapelvic intraperitoneal *** Intra-or extra-sciatic nerve-compressing lesion should be considered in cases of sitting pain radiating down the ipsilateral lower *** large juxta-articular ganglionic cyst was successfully treated simultaneously using laparoscopy and arthroscopy.

AIM: To analyze the risk factors of pancreatic leakage after ***: We retrospectively reviewed 172 consecutive patients who had undergone pancreatico-duodenectomy at Inha University Hospital between April 1996 and March 2006. We analyzed the pancreatic fistula rate according to the clinical characteristics, the pathologic and laboratory findings, and the anastomotic ***: The incidence of developing pancreatic fistulas in patients older than 60 years of age was 21.7% (25/115), while the incidence was 8.8% (5/57) for younger patients; the difference was significant (P = 0.03). Patients with a dilated pancreatic duct had a lower rate of post-operative pancreatic fistulas than patients with a non-dilated duct (P = 0.001). Other factors, including clinical features, anastomotic methods, and pathologic diagnosis, did not show any statistical difference. CONCLUSION: Our study demonstrated that pancreatic fistulas are related to age and a dilated pancreatic duct. The surgeon must take these risk factors into consideration when performing a pancre-aticoduodenectomy.

AIM: To evaluate the eff icacy of a new nuclear imaging Infecton (Tc-99m ciprofloxacin) for the diagnosis of acute cholecystitis. METHODS: Sixteen patients thought to have acute cholecystitis were included in this study. The diagnosis of acute cholecystitis was made based on clinical symptoms and ultrasonographic and pathologic f indings. RESULTS: The 16 patients were composed of 12 acute and 4 chronic cholecystitis patients. Twelve patients with acute cholecystitis were image-positive, including one false-positive. Four patients with chronic cholecystitis were image-negative, of whom three were true-negative. This nuclear imaging had a sensitivity of 91.7%, a specificity of 75%, a positive-predictive value of 91.7%, and a negative-predictive value of 75%. CONCLUSION: Tc-99m ciprofloxacin imaging is easy to perform and applicable for the diagnosis of acute cholecystitis.

We report on the microstructure and the strengthening mechanisms of additively manufactured parts fabricated by directed energy deposition of Ti-6Al-4V(Ti64)powders blended with yttria-stabilized zirconia(YSZ)*** specimens showed refined microstructures as compared to bare as-deposited Ti64,where theαand columnar priorβgrain sizes decreased with increasing YSZ *** YSZ nanoparticles decomposed during the deposition process and led to the formation of yttrium oxide and some excess oxygen in the Ti64 *** decrease in the sizes of the priorβgrains could be attributed to the increasing amount of dissolved oxygen and yttrium,which promoted constitutional ***,the reduction in the size of theαgrains could be ascribed to a shift of the onset of theβ→α+βtransformation to a higher temperature and shorter time with increasing concentration of dissolved ***,the contributions of the underlying strengthening mechanisms for the as-deposited specimens were quantitatively determined.

Existing segmentation and augmentation techniques on convolutional neural network(CNN)has produced remarkable progress in object ***,the nominal accuracy and performance might be downturned with the photometric variation of images that are directly ignored in the training process,along with the context of the individual CNN *** this paper,we investigate the effect of a photometric variation like brightness and sharpness on different *** observe that random augmentation of images weakens the performance unless the augmentation combines the weak limits of photometric *** approach has been justified by the experimental result obtained from the PASCAL VOC 2007 dataset,with object detection CNN algorithms such as YOLOv3(You Only Look Once),Faster R-CNN(Region-based CNN),and SSD(Single Shot Multibox Detector).Each CNN model shows performance loss for varying sharpness and brightness,ranging between−80%to 80%.It was further shown that compared to random augmentation,the augmented dataset with weak photometric changes delivered high performance,but the photometric augmentation range differs for each ***,we discuss some research questions that benefit the direction of the *** results prove the importance of adaptive augmentation for individual CNN model,subjecting towards the robustness of object detection.

Sonodynamic therapy(SDT)has attracted great interest in the field of cancer therapy because of its non-invasiveness,deep penetration,spatiotemporal ***,the sonodynamic effect is severely hindered by hypoxia and a high glutathione(GSH)level in tumor *** this work,a new type of nanohybrid sonosensitizer is designed,which incorporates several prominent advantages of organic,inorganic,natural sonosensitizers for imaging-guided SDT and ferroptosis *** an endogenous transporter,hemoglobin(Hb)has dual functions of oxygen and iron ***,Hb can transport iron to the tumor site for Fe-dependent ferroptosis,yet this oxygen carrier is able to enhance the sensitivity of tumor cells to oxygen-driven *** innovatively hybridized Hb,sinoporphyrin sodium(DVDMS),titanium dioxide(TiO_(2))to prepare an integrated nano-oxygen carrier(designated as HDT)for multifarious cancer theranostics,which not only attained satisfactory sonosensitization of DVDMS/TiO_(2)but also achieved oxygen-boosted SDT and potent ferroptosis via *** ultrasound irradiation,the oxygenation of HDT markedly amplified the generation of reactive oxygen species(ROS)in hypoxic tumors,promoted the valence transition of Hb to accelerate Fenton reaction,led to *** strategy virtually eradicated tumors in situ and exhibited immunoregulatory potential for augmenting anti-tumor effects in vivo and in vitro through ROS formation,O_(2)self-supplement,GSH depletion,lipid ***,HDT demonstrated excellent dual-modal imaging performance,providing a valuable platform for photoacoustic/fluorescence-guided SDT and ferroptosis induction in tumor cells.
