Background:Lymphatic filariasis(LF)remains one of the world’s most debilitating parasitic infections and is a major contributor to poor health in many endemic countries.The provision of continuing care for all those affected by LF and its consequences is an important component of the United Nations’Sustainable Development Goals.The aim of this study is to integrate lymphedema care into the primary health care system of the State by developing lymphedema clinics at each district,through training of health personnel to fulfill WHO recommendation for morbidity management and disability prevention.Methods:Selected health care providers from all the districts in Kerala State of India participated in intensive training sessions endorsed by the State’s health administration.The six training sessions(from 5 June 2017 to 25 May 2018)included appropriate self-care information and development of individual plans for each participating institution to provide instruction and care for their lymphoedema patients.The learning achieved by attendees was assessed by pre-and post-training tests.The number of lymphoedema patients receiving care and instruction from the post-training activities of each participating institution was assessed from local records,6 months after the conclusion of the training sessions.Results:One hundred and eighty-four medical personnel(91 doctors and 93 nurses)from 82 medical institutions were trained which quickly led to the establishment of active lymphoedema clinics providing the essential package of care(EPC)for lymphoedema patients at all the participating institutions.Six months after the training sessions the number of previously unidentified lymphoedema patients registered and receiving care at these clinics ranged from 296 to almost 400 per clinic,with a total of 3,477 new patients receiving training in EPC.Conclusions:Generalist health personnel,when appropriately trained,can provide quality lymphoedema care in public health settings and patients when provided services close to their home,are w

Worldwide Interoperability for Microwave Access(WiMAX)trusts Multiple Input Multiple Output-Orthogonal Frequency Division Multiplexing(MIMO-OFDM)combination for the deployment of physical layer functions and for connecting the medium access control to the wireless media.Even though Orthogonal Frequency Division Multiplexing(OFDM)facilitates reliable digital broadband transmission in the fading wireless channels,the presence of synchro-nization errors in the form of Carrier Frequency Offset(CFO)and Time Offset(TO)adversely affect the performance of OFDM based physical layers.The objective of this work is to improve the accuracy of the frequency and the time offset estimation in the WiMAX physical layer.A method to enhance the synchro-nization accuracy byﬁne-tuning the merit factor of the preamble sequence is sug-gested in this paper.Also,a new preamble with improved synchronization accuracy is proposed for the WiMAX system.The performance of the proposed preamble is evaluated in a Rayleigh fading channel and the results of simulations show that the Mean Square Error(MSE)in offset estimation is signiﬁcantly reduced and it outperforms the standard WiMAX preamble.

Peer-to-Peer(P2P)electricity trading is a significant research area that offers maximum fulfilment for both prosumer and consumer.It also decreases the quantity of line loss incurred in Smart Grid(SG).But,uncertainities in demand and supply of the electricity might lead to instability in P2P market for both prosumer and consumer.In recent times,numerous Machine Learning(ML)-enabled load predictive techniques have been developed,while most of the existing studies did not consider its implicit features,optimal parameter selection,and prediction stability.In order to overcome fulfill this research gap,the current research paper presents a new Multi-Objective Grasshopper Optimisation Algorithm(MOGOA)with Deep Extreme Learning Machine(DELM)-based short-term load predictive technique i.e.,MOGOA-DELM model for P2P Energy Trading(ET)in SGs.The proposed MOGOA-DELM model involves four distinct stages of operations namely,data cleaning,Feature Selection(FS),prediction,and parameter optimization.In addition,MOGOA-based FS technique is utilized in the selection of optimum subset of features.Besides,DELM-based predictive model is also applied in forecasting the load requirements.The proposed MOGOA model is also applied in FS and the selection of optimalDELM parameters to improve the predictive outcome.To inspect the effectual outcome of the proposed MOGOA-DELM model,a series of simulations was performed using UK Smart Meter dataset.In the experimentation procedure,the proposed model achieved the highest accuracy of 85.80%and the results established the superiority of the proposed model in predicting the testing data.

Repaglinide (RPG) regulates the amount of glucose by stimulating the pancreas to release insulin in the blood. In view of its biological importance, we have examined the interaction between RPG and a model protein, bovine serum albumin (BSA) employing various spectroscopic, electrochemical and molecular docking methods. Fluorescence spectra of BSA were recorded in the presence and absence of RPG in phosphate buffer of pH 7.4. Fluorescence intensity of BSA was decreased upon the addition of increased concentrations of RPG, indicating the interaction between RPG and BSA. Stern-Volmer quenching analysis results revealed that RPG quenched the intensity of BSA through dynamic quenching mechanism. This was further confirmed from the time-resolved fluorescence measurements. The binding constant as calculated from the spectroscopic and voltammetric results was observed to be in the order of 104M-1 at 298 K, suggesting the moderate binding affinity between RPG and BSA. Competitive experimental results revealed that the primary binding site for RPG on BSA was site II. Absorption and circular dichroism studies indicated the changes in the secondary structure of BSA upon its interaction with RPG. Molecular simulation studies pointed out that RPG was bound to BSA in the hydrophobic pocket of site II.

Strategic Planning for Equitable RWIS Implementation:A Comprehensive Study Incorporating a Multi-variable Semivariogram Model

This paper extends the previously developed method of optimizing Road Weather Information Systems(RWIS)station placement by unveiling a sophisticated multi-variable semivariogram model that concurrently considers multiple vital road weather variables.Previous research primarily centered on single-variable analysis focusing on road surface temperature(RST).The study bridges this oversight by introducing a framework that integrates multiple critical weather variables into the RWIS location allocation framework.This novel approach ensures balanced and equitable RWIS distribution across zones and aligns the network with areas both prone to traffic accidents and areas of high uncertainty.To demonstrate the effectiveness of this refinement,the authors applied the framework to Maine’s existing RWIS network,conducted a gap analysis through varying planning scenarios and generated optimal solutions using a heuristic optimization algorithm.The analysis identified areas that would benefit most from additional RWIS stations and guided optimal resource utilization across different road types and priority locations.A sensitivity analysis was also performed to evaluate the effect of different weightings for weather and traffic factors on the selection of optimal locations.The location solutions generated have been adopted by MaineDOT for future implementations,attesting to the model’s practicality and signifying an important advancement for more effective management of road weather conditions.

Wireless sensor network(WSN)is an emerging technology which find useful in several application areas such as healthcare,environmentalmonitoring,border surveillance,etc.Several issues that exist in the designing of WSN are node localization,coverage,energy efficiency,security,and so on.In spite of the issues,node localization is considered an important issue,which intends to calculate the coordinate points of unknown nodes with the assistance of anchors.The efficiency of the WSN can be considerably influenced by the node localization accuracy.Therefore,this paper presents a modified search and rescue optimization based node localization technique(MSRONLT)forWSN.The major aim of theMSRO-NLT technique is to determine the positioning of the unknown nodes in theWSN.Since the traditional search and rescue optimization(SRO)algorithm suffers from the local optima problemwith an increase in number of iterations,MSRO algorithm is developed by the incorporation of chaotic maps to improvise the diversity of the technique.The application of the concept of chaotic map to the characteristics of the traditional SRO algorithm helps to achieve better exploration ability of the MSRO algorithm.In order to validate the effective node localization performance of the MSRO-NLT algorithm,a set of simulations were performed to highlight the supremacy of the presented model.A detailed comparative results analysis showcased the betterment of the MSRO-NLT technique over the other compared methods in terms of different measures.

Factors affecting residual exotropia after two muscle surgery for intermittent exotropia

AIM： To study the factors affecting residual exotropia（〉10 PD） at 4-6wk postoperative visit following two rectus muscle surgery for intermittent exotropia [bilateral lateral rectus（LR） recession or unilateral recess resect procedure].METHODS： A retrospective chart review of patients with intermittent exotropia ≤50 PD who underwent two rectus muscle surgery in between Jan. 2011 to Dec. 2013 was performed. Possible factors were compared between patients with residual exotropia（〉10 PD） and successful outcome（within 10 PD of orthotropia） at the 4-6wk postoperative visit. Effect/dose ratio was calculated by dividing the effect of surgery by the total amount（mm） of muscle surgery done.RESULTS： One hundred and fifty-seven patients with mean age of 14y（range 3-53y） were included. Twentyseven patients（17.2%） had residual exotropia at 4-6wk postoperative follow up. Age at surgery（P=0.009） and preoperative deviation for distance（P≤0.001） and near（P=0.001） were identified as important predictors of unsuccessful outcome. The occurrence of residual exotropia was not affected by amblyopia, anisometropia, lateral incomitance, pattern deviation, vertical deviation, type of exotropia or type of surgery done（recess-resect or bilateral LR recession）. The effect/dose ratio was more in deviations 〉40 PD in the both recess-resect and bilateral LR recession type of surgery. The effect/dose ratio was less in patients with residual exotropia as compared to the successful outcome group（1.36 PD/mm vs 2.05 PD/mm in the bilateral LR recession surgery and 1.93 PD/mm vs 2.63 PD/mm in the unilateral recess-resect surgery）.CONCLUSION： Residual exotropia is seen in 17% of patients after two muscle surgery for intermittent exotropia. Patients with older age and larger preoperative deviation have greater chances of developing failure of two muscle strabismus surgery for intermittent exotropia.

One of the significant health issues affecting women that impacts their fertility and results in serious health concerns is Polycystic ovarian syndrome(PCOS).Consequently,timely screening of polycystic ovarian syndrome can help in the process of recovery.Finding a method to aid doctors in this procedure was crucial due to the difficulties in detecting this condition.This research aimed to determine whether it is possible to optimize the detection of PCOS utilizing Deep Learning algorithms and methodologies.Additionally,feature selection methods that produce the most important subset of features can speed up calculation and enhance the effectiveness of classifiers.In this research,the tri-stage wrapper method is used because it reduces the computation time.The proposed study for the Automatic diagnosis of PCOS contains preprocessing,data normalization,feature selection,and classification.A dataset with 39 characteristics,including metabolism,neuroimaging,hormones,and biochemical information for 541 subjects,was employed in this scenario.To start,this research pre-processed the information.Next for feature selection,a tri-stage wrapper method such as Mutual Information,ReliefF,Chi-Square,and Xvariance is used.Then,various classification methods are tested and trained.Deep learning techniques including convolutional neural network(CNN),multi-layer perceptron(MLP),Recurrent neural network(RNN),and Bi long short-term memory(Bi-LSTM)are utilized for categorization.The experimental finding demonstrates that with effective feature extraction process using tri stage wrapper method+CNN delivers the highest precision(97%),high accuracy(98.67%),and recall(89%)when compared with other machine learning algorithms.

OBJECTIVES Syncope at age 65+is associated with increased mortality,irrespective of cause.Syncope rules were designed to aid in risk-stratification but were only validated in the general adult population.Our objective was to determine if they can be applied to a geriatric population in predicting short-term adverse outcomes.METHODS In this single-center retrospective study,we evaluated 350 patients aged 65+presenting with syncope.Exclusion criteria included confirmed non-syncope,active medical condition,drug or alcohol-related syncope.Patients were stratified into high or low risk based on Canadian Syncope Risk Score(CSRS),Evaluation of Guidelines in Syncope Study(EGSYS),San Francisco Syncope Rule(SFSR),and Risk Stratification of Syncope in the Emergency Department(ROSE).Composite adverse outcomes at 48-hour and 30-day included all-cause mortality,major adverse cardiac and cerebrovascular events(MACCE),return emergency department visit,hospitalization,or medical intervention.We assessed each score's ability to predict the outcomes using logistic-regression and compared performances using receiver-operator curves.Multivariate analyses were performed to study the associations between recorded parameters and outcomes.RESULTS CSRS outperformed with AUC of 0.732(95%CI:0.653-0.812)and 0.749(95%CI:0.688-0.809)for 48-h and 30-day outcomes,respectively.Sensitivities for CSRS,EGSYS,SFSR,and ROSE for 48-hour outcomes were 48%,65%,42% and 19%;and for 30-day outcomes were 72%,65%,30% and 55%,respectively.Atrial fibrillation/flutter on EKG,congestive heart failure,antiarrhythmics,systolic blood-pressure300,vasovagal predisposition,and antidepressants highly correlated with 30-day outcomes.CONCLUSIONS Performance and accuracy of four prominent syncope rules were suboptimal in identifying high-risk geriatric patients with short-term adverse outcomes.We identified some significant
