Background:High-intensity interval training(HIIT)induces similar or even superior adaptations compared to continuous endurance ***,just 6 HIIT sessions over 2 weeks significantly improves maximal oxygen uptake(VO_(2max)),submaximal exercise fat oxidation,and endurance *** even faster adaptations can be achieved with HIIT is not ***,we aimed to determine whether 2 sessions of HIIT per day,separated by 3 h,every other day for 5 days(double HIIT(HIIT-D),n=15)could increase VO_(2max),submaximal exercise fat oxidation,and endurance capacity as effectively as 6 sessions of HIIT over 2 weeks(single HIIT(HIIT-S),n=13).Methods:Each training session consisted of 10×60 s of cycling at 100%of VO_(2max)interspersed with 75 s of low-intensity cycling at 60 watt(W).Pre-and post-training assessments included VO_(2max),time to exhaustion at-80%of VO_(2max),and 60-min cycling trials at-67%of VO_(2max).Results:Similar increases(p0.05)and endurance capacity(HIIT-D:80.1%***-S:79.2%,p>0.05)were *** exercise carbohydrate oxidation was reduced in the 2 groups after exercise training(HIIT-D:9.2%,p=0.014 ***-S:18.8%,p=0.012)while submaximal exercise fat oxidation was significantly increased in HIIT-D(15.5%,p=0.048)but not in HIIT-S(9.3%,p=0.290).Conclusion:Six HIIT sessions over 5 days was as effective in increasing VO_(2max)and endurance capacity and was more effective in improving submaximal exercise fat oxidation than 6 HIIT sessions over 2 weeks.

BACKGROUND Polymorphisms of human leukocyte antigen(HLA)genes are suggested to increase the risk of gastric cancer(GC).AIM To investigate the HLA allele frequencies of patients with GC relative to a control group in terms of CagA+multiple(≥2)EPIYA-C *** The patient group comprised 94 patients[44 GC and 50 duodenal ulcer(DU)patients],and the control group comprised 86 individuals[(50 non-ulcer dyspepsia patients and 36 people with asymptomatic Helicobacter pylori(***)].Polymerase chain reaction was performed for the amplification of the *** cagA gene and typing of EPIYA *** sequence-specific oligonucleotide(SSO)typing was performed using Lifecodes SSO typing kits(HLA-A,HLA-B HLA-C,HLA-DRB1,and HLA-DQA1-B1 kits).RESULTS The comparison of GC cases in terms of CagA+multiple(≥2)EPIYA-C repeats showed that only the HLA-DQB1*06 allele[odds ratio(OR):0.37,P=0.036]was significantly lower,but significance was lost after correction(Pc=0.1845).The HLA-DQA1*01 allele had a high ratio in GC cases with multiple EPIYA-C repeats,but this was not significant in the univariate *** compared allele frequencies in the DU cases alone and in GC and DU cases together using the same criterion,and none of the HLA alleles were significantly associated with GC or ***,none of the alleles were detected as independent risk factors after the multivariate *** the other hand,in a multivariate logistic regression with no discriminative criterion,HLA-DQA1*01(OR=1.848),HLA-DQB1*06(OR=1.821)and HLA-A*02(OR=1.579)alleles were detected as independent risk factors for GC and *** None of the HLA alleles were detected as independent risk factors in terms of CagA+multiple EPIYA-C ***,HLA-DQA1*01,HLA-DQB1*0601,and HLA-A*2 were independent risk factors with no criterion in the multivariate *** suggest that the association of these alleles with gastric malignancies is not specifically related to cagA and multiple EPIYA C repeats.

Left Atrial Mechanical Function and Aortic Stiffness in Middle-aged Patients with the First Episode of Atrial Fibrillation

Background： In the early stages of atrial remodeling, aortic stiffness might be an indication of an atrial myopathy, in particular, atrial fibrosis. This study aimed to investigate the association between left atrial （LA） mechanical function, assessed by two-dimensional speckle tracking echocardiography, and aortic stiffness in middle-aged patients with tile first episode of nonvalvular atrial fibrillation （AF）. Methods： This prospective study included 34 consecutive patients with the first episode of AF, who were admitted to Kartal Kos.;uyolu Research and Training Hospital between May 2013 and October 2015, and 31 age- and gender-matched healthy controls. During the 1^st month （mostly in the first 2 weeks） following their first admission, 34 patients underwent the first pulse wave measurements. Then, 21 patients were recalled for their second pulse wave measurement at 11.8 ± 6.0 months following their initial admission. Echocardiographic and pulse wave findings were compared between these 34 patients and 31 healthy controls. We also compared the pulse wave and echocardiographic findings between the first and second measurements in 21 patients. Results： Pulse wave analysis showed no significant differences between tile AF patients and healthy controls with respect to PWV （ 10.2 ± 2.5 m/s vs. 9.7 ± 2.1 m/s： P = 0.370）, augmentation pressure （9.6 ± 7.4 mmHg vs. 9. 1± 5.7 mmHg; P = 0.740）, and aortic pulse pressure （AoPP; 40.4 ± 14.0 mmHg vs. 42. 1 ±7.6 mmHg, P = 0.550）. The first LA positive peak of strain was inversely related to the augmentation pressure （r 0.30; P = 0.02） and aortic systolic pressure （r - 0.26, P - 0.04）. Comparison between the two consecutive pulse wave measurements in 21 patients showed similar results, except for AoPP. In 21 patients, the AoPP at the second measurement （45.1± 14.1 mmHg） showed a significant increase compared with AoPP at the first measurement （39.0 ±10.6 mmHg, P = 0.028）, which was also higher than that of healthy controls （42.1

The present study aimed to investigate the correlation of MPV and splenomegaly as inflammation activity of FMF patients at the attacks free period. We retrospectively reviewed the medical records of 43 patients with FMF. This study was performed at the attack free period as clinical and laboratory. For this study, patients were divided into two groups. Patients with splenomegaly is called group 1 (n = 12) and patients with no splenomegaly is called group 2 (n = 31). Groups were compared respect to age, gender, platelet counts, acute phase reactants and MPV. The mean MPV (fl) were significantly higher in group 1 (8.9 ±0.8) than in group 2 (8.4 ±0.5, p <0.05). This study suggested that increased MPV and splenomegaly without amyloidosis could be a sign of chronic inflammation in children with FMF even in attack free period.

Background: Intraoperative hemostasis during intracranial surgery is one of the most important aspects of the surgical procedure. One of the most widely practiced methods to prevent postoperative hemorrhage is to cover the cerebral wound bed with cellulose. Objective: We report on the use of a new powder form of regenerated oxidized cellulose enriched with calcium (GelitaCel Ca PowderTM, Gelita Medical, Eberbach, Germany) for hemostasis purposes in intracranial surgery. Methods: In 107 patients operated for intracranial mass lesions, the resection cavity was covered with a small layer of cellulose powder for hemostatic purposes. All patients had a postoperative CT or MRI scanning within 24 hours after surgery to detect any surgical complications and to detect the presence of blood within the surgical cavity. Results: Of the 107 operated patients, 96 (90%) had no or minimal blood in the intracranial resection field on postoperative imaging with CT or MRI. Of those 96, 69 patients had no blood at all and 27 patients had a small trace. In the remaining 11 patients, 8 patients (7.5%) had blood filling the resection cavity. In three patients (2.8%), the blood accumulation resulted in mass effect. All these three patients were re-operated due to neurological deterioration. Conclusion: We suggest that cellulose powder is an easy and safe product for hemostasis and prevention of postoperative hemorrhage in intracranial surgery. The main advantage of the powder form over regular oxidized cellulose is the no-touch technique of application and leaves no excess hemostatic material behind which could possibly cause compression.

External Validation of SENIC and NNIS Scores for Predicting Wound Infection in Colorectal Surgery

Objective: We aimed to identify the ratio of Surgical Site Infection (SSI) and also the validity of the National Nosocomial Infection Survellance (NNIS) and Study on the Efficacy of Nosocomial Infection Control (SENIC) risk indexes in colorectal surgery, among Turkish population. Background: Some problems have been reported with the power of NNIS risk index to predict the risk of surgical site infection. We aimed to validate theNNIS and SENIC risk indexes in colorectal surgery. Methods: Between January 2003 and December 2006, surgical site ?nfection survellance was performed to 107 patient who undergo colorectal surgery with NNIS and SENIC risk scales. The mean patient age was 48 years (range, 17 to 86), and 61.7% of the group (66) was female. For this patient cohort, 6 (5.6%) were diagnosed with incisional SSI. While the mean Body Mass Index (BMI) of all patients was 26.6;mean value of BMI among the patiens with SSI was ***: 6 insicional surgical site infection were observed during the study. According to Receiver Operating Characteric (ROC) curve analyze neither NNIS with avalue of 0.70, nor SENIC with a value of 0.67 are perfect risk indexes. Conclusion: As a result both NNIS and SENIC ?s a good risk indexes but not perfect. Scarcely when NNIS and SENIC is used together to predict the SSI they forecast the development of infection better. But there is a lot of other factors that effect the development of SSI, so for excellent surveillance risk index those factors known by everyone must be added to risk index scales.

目的：评价运用Pentacam Scheimpflug成像测量晶状体核密度与torsinal超声乳化动力学参数（如：运用于老年核性白内障患者的超声能量水平、液体留存时间及液体量）间的关系。方法：此研究为前瞻性双盲研究。瞳孔扩大后运用Pentacam Scheimpflug 成像进行测量。白内障的等级由pentacam核密度测定法[ pentacam nucleus densitometry, PND；又名Pentacam核分段（ Pentacam nucleus staging, PNS）软件]自主分为1到5个等级。超声乳化术后,自动在Infiniti Ozil IP超声乳化系统监视器上计算并显示总超声（ U/S）时间、累计耗散能、Torsional U/S时间以及估算液体的使用情况。组间差异运用单因素方差分析（ ANOVA ）进行评定,多组分析运用Tamhane测试法,PND测量的晶状体密度与torsinal超声乳化动力学参数间的关系运用Spearman相关分析进行评定。界定P〈0.05时具有统计学意义。结果：此项研究包括125例患者（125眼）,平均年龄为69.7±9.4岁（范围：48~88岁）,其中61例男性、64例女性。 U/S总时间、torsional U/S时间、CDE及估算液体使用情况的范围分别为0.70~158.9s,0.70~158.5s,0.11~42.65和21~98mL. PND组间数据有统计学差异。评估超声乳化动力学参数与PND数据间关系后得出PND数据与总超声时间（ r=0.767； P〈0.01）、torsional超声时间（r=0.767；P〈0.01）、CDE（r=0.758； P〈0.01）及液体使用量（r=0.602；P〈0.01）均有显著相关性。结论：白内障手术前运用PND评分法获得核密度的客观程度可为超声乳化术中的参数提供可靠的预见性。因此,对每一位患者进行每一单项超声乳化参数的评估,有助于降低超声乳化能量的使用量,减少内皮细胞丢失和类固醇的使用,加速视力的恢复。

Production of Natural Coagulant from Moringa Oleifera Seed for Application in Treatment of Low Turbidity Water

This study focused on developing an efficient and cost effective processing technique for Moringa oleifera seeds to produce natural coagulant for use in drinking water treatment. The produced natural coagulant can be used as an alternative to aluminum sulphate and other coagulants and used worldwide for water treatment. This study investigates processing Moringa oleifera seeds to concentrate the bio-active constituents which have coagulation activity. Moringa oleifera seeds were processed for oil extraction using electro thermal soxhlet. Isolation and purification of bio-active constituents using chromatography technique were used to determine the molecular weight of the bio-active constituents. The molecular weight of bio-active constitu-ents found to be in a low molecular weight range of between 1000 – 6500 Dalton. The proposed method to isolate and purify the bio-active constituents was the cross flow filtration method, which produced the natu-ral coagulant with very simple technique (oil extraction;salt extraction;and microfiltration through 0.45 μm). The turbidity removal was up to 96.23 % using 0.4 mg/L of processed Moringa oleifera seeds to treat low initial turbidity river water between 34-36 Nephelometric Turbidity Units (NTU) without any additives. The microfiltration method is considered to be a practical method which needs no chemicals to be added com-pared to other researchers proposed methods. The natural coagulant produced was used with low dosages to get high turbidity removal which considered to be a breakthrough in this study and recommended to be scaled up for industry level. The product is commercially valuable at the same time it is minimizing the cost of water treatment.
