Determinants of Male Coital Difficulties among Attendees of the Gynae Clinic at a Tertiary Health Center in North-Central Nigeria

Background: Male sexual or coital difficulties are among the factors contributing to infertility in couples seeking fertility as this may result in low coital frequency. Aims/Objectives: To evaluate the causes of male sexual or coital difficulties among couples attending the infertility clinic at BSUTH, Makurdi over a 2 year period. Methodology: This was a cross-sectional study involving all males attending the infertility clinic at Benue State University Teaching Hospital (BSUTH) who consent to participate in the study. A pretested structured questionnaire was administered and analyzed with statistical package for social sciences (version 23.0) and the results were presented in simple proportions. Results: There were several factors responsible for male coital difficulties. The most common was male erectile dysfunction 33 (32.4%), vaginismus 11 (10.8%), penile pain 14 (13.7%), poor response from the female partner 8 (7.8%) and severe dyspareunia 18 (17.1%). Conclusion: Coital difficulties resulting to low frequency of sexual exposure constitutes a major cause of infertility among males attending the infertility clinic at BSUTH, Makurdi. Therefore efforts should be made to evaluate these factors while assessing infertility couples in order to mitigate the effect.

Huge Broad Ligament Leiomyoma: A Case Report

The broad ligament is the commonest extra uterine site for the occurrence of leiomyoma but with a very low incidence rate. Because of its overall low incidence rate, it poses both clinical and radiological difficulty in differentiating with an ovarian tumour. We report a case of a 31-year-old unmarried nullipara (para 0 + 0) who presented with 12-month history of progressive abdominal swelling, three-month history of pelvic pain and one-month history of prolonged heavy menstrual flow. Examination of the abdomen showed a 36-week-size mass which was multi-lobulated, firm, non tender and slightly mobile extending to the xyphisternum. On abdominal ultrasonography scan, the uterus was deviated to the contra lateral side with a huge, irregular mass measuring 24 × 18 cm posterior to the uterus. She had laparotomy and four fibroid seedlings were removed. The largest was a broad ligament myoma which measured 24 × 17 × 12 cm and weighed 3883 g.

Term Pregnancy in a Patient with Septate Uterus: Incidental Finding at Caesarean Section

Mullerian duct anomalies are the most common congenital anomalies of the reproductive system and septate uterus is the most frequently diagnosed Müllerian anomaly. The true incidence of Mullerian duct anomalies is difficult to state because some cases may be asymptomatic and there are pitfalls associated with various diagnostic methods. This is a case report of a 17-year-old primigravida with septate uterus diagnosed at caesarean section performed at term for cephalo-pelvic disproportion. Baby was delivered with bilateral talipes equinovarus.

Term Gravid Uterus as a Content of Congenital Umbilical Hernia—A Complication in a Multigravida That Presented in Labour

Complications arising from neglected umbilical hernia are not uncommon in pregnancy, more especially in developing countries with characteristically poor health seeking behaviour and less encouraging attitude towards surgical intervention for un-complicated medical conditions. The report is on a 34-year-old para 8 who presented with prolonged labour and an irreducible herniated gravid uterus complicating a neglected congenital umbilical hernia. The patient was resuscitated. She had spontaneous vaginal delivery while preparation was being made for an elective caesarean section.
