De-novo hepatocellular carcinoma after pediatric living donor liver transplantation

De-novo malignancies carry an incidence ranging between 3%-26% after transplant and account for the second highest cause of post-transplant mortality behind cardiovascular disease. While the majority of de-novo malignancies after transplant usually consist of skin cancers, there has been an increasing rate of solid tumor cancers over the last 15 years. Although, recurrence of hepatocellular carcinoma(hCC) is well understood among patients transplanted for hCC, there are increasing reports of de-novo hCC in those transplanted for a non-hCC indication. The proposed pathophysiology for these cases has been mainly connected to the presence of advanced graft fibrosis or cirrhosis and always associated with the presence of hepatitis B or C virus. We report the first known case of de-novo hCC in a recipient, 14 years after a pediatric living related donor liver transplantation for end-stage liver disease due to biliary atresia without the presence of hepatitis B or C virus before and after transplant. We present this case report to increase the awareness of this phenomenon and address on the utility for screening and surveillance of hepatocellular carcinoma among these individuals. One recommendation is to use similar guidelines for screening, diagnosis, and treatment for hCC as those used for primary hCC in the pre-transplant patient, focusing on those recipients who have advanced fibrosis in the allograft, regardless of etiology.

Non-alcoholic fatty liver disease(NAFLD) is a recognized problem in patients after orthotopic liver transplantation and may lead to recurrent graft injury. As the increased demand for liver allografts fail to match the available supply of donor organs, split liver transplantation(SLT) has emerged as an important technique to increase the supply of liver grafts. SLT allows two transplants to occur from one donor organ, and provides a unique model for observing the pathogenesis of NAFLD with respect to the role of recipient environmental and genetic factors. here we report on two recipients of a SLT from the same deceased donor where only one developed non-alcoholic steatohepatitis(NASh), suggesting that host factors are critical for the development of NASh.

The liver is the front line organ of the immune system.The liver contains the largest collection of phagocytic cells in the body that detect both pathogens that enter through the gut and endogenously produced antigens.This is possible by the highly developed differentiation capacity of the liver immune system between self-antigens or non-self-antigens,such as food antigens or pathogens.As an immune active organ,the liver functions as a gatekeeping barrier from the outside world,and it can create a rapid and strong immune response,under unfavorable conditions.however,the liver's assumed immune status is anti-inflammatory or immuno-tolerant.Dynamic interactions between the numerous populations of immune cells in the liver are key for maintaining the delicate balance between immune screening and immune tolerance.The anatomical structure of the liver can facilitate the preparation of lymphocytes,modulate the immune response against hepatotropic pathogens,and contribute to some of its unique immunological properties,particularly its capacity to induce antigen-specific tolerance.Since liver sinusoidal endothelial cell is fenestrated and lacks a basement membrane,circulating lymphocytes can closely contact with antigens,displayed by endothelial cells,Kupffer cells,and dendritic cells while passing through the sinusoids.Loss of immune tolerance,leading to an autoaggressive immune response in the liver,if not controlled,can lead to the induction of autoimmune or autoinflammatory diseases.This review mentions the unique features of liver immunity,and dysregulated immune responses in patients with autoimmune liver diseases who have a close association with inborn errors of immunity have also been the emphases.

Until recently, many computational materials scientists have shown little interest in materials databases. This is now changing be-cause the amount of computational data is rapidly increasing and the potential for data mining provides unique opportunities for discovery and optimization. here, a few examples of such opportunities are discussed relating to structural analysis and classification, discovery of correlations between materials properties, and discovery of unsuspected compounds.

Is there an association between helicobacter pylori in the inlet patch and globus sensation?

AIM:To determine the association between helicobacter pylori(h.pylori)and globus sensation(GS)in the patients with cervical inlet patch. METhODS:Sixty-eight patients with esophageal inlet patches were identified from 6760 consecutive patients undergoing upper gastrointestinal endoscopy prospectively.In these 68 patients with cervical inlet patches, symptoms of globus sensation(lump in the throat), hoarseness,sore throat,frequent clearing of the throat,cough,dysphagia,odynophagia of at least 3 mo duration was questioned prior to endoscopy. RESULTS:Cervical heterotopic gastric mucosa(ChGM) was found in 68 of 6760 patients.The endoscopic prevalence of ChGM was determined to be 1%.h.pylori was identified in 16(23.5%)of 68 patients with inlet patch.53 patients were classified as ChGMⅡ.This group included 48 patients with globus sensation,4 patients with chronic cough and 1 patient with hoarseness.All the patients who were h.pylori(+)in cervical inlet patches had globus sensation.CONCLUSION:Often patients with ChGM have a long history of troublesome throat symptoms.We speculate that disturbances in globus sensation are like non-ulcer dyspepsia.

Evolution of Microstructure, Residual Stress, and Tensile Properties of Mg–Zn–Y–La–Zr Magnesium Alloy Processed by Extrusion

The microstructure, texture, residual stress, and tensile properties of Mg–6 Zn–2 Y–1 La–0.5 Zr(wt%) magnesium alloy were investigated before and after extrusion process, which performed at 300 °C and 400 °C. The microstructural characterizations indicated that the as-cast alloy was comprised of α-Mg, Mg–Zn, Mg–Zn–La, and Mg–Zn–Y phases. During homogenization at 400 °C for 24 h, most of the secondary phases exhibited partial dissolution. Extrusion process led to a remarkable grain refi nement due to dynamic recrystallization(DRX). The degree of DRX and the DRXed grain size increased with increasing extrusion temperature. The homogenized alloy did not show a preferential crystallographic orientation, whereas the extruded alloys showed strong basal texture. The extrusion process led to a signifi cant improvement on the compressive residual stress and mechanical properties. The alloy extruded at 300 °C exhibited the highest basal texture intensity, the compressive residual stress and hardness, and yield and tensile strengths among the studied alloys.

Development of an Amperometric hydrogen Peroxide Biosensor based on the Immobilization of horseradish Peroxidase onto Nickel Ferrite Nanoparticle-Chitosan Composite

Nickel ferrite(NiFe_2O_4) nanoparticles have been dispersed in chitosan solution in order to fab ricate nanocomposite films.horseradish peroxidase(hRP) has been immobilized onto this chitosan NiFe_2O_4 nanocomposite film via physical adsorption.The size of the NiFe_2O_4 nanoparticles has been estimated us ing X ray diffraction pattern and scanning electron microscopy(SEM) to be 40±9 nm.The chitosan NiFe_2O_4 nanocomposite film and hRP/chitosan NiFe_2O_4 bioelectrode have been characterized using SEM technique.The hRP/chitosan NiFe_2O_4 nanocomposite bioelectrode has a response time of 4 s,linearity as 0.3 to 12 m M of h2O2,sensitivity as 22 n A/m M.The effects of p h and the temperature of the immobilized hRP electrode have also been studied.

Larvicidal activity of Cyclamen(Myrsinaceae) extracts against the larvae of West Nile virus vector Culex pipiens L.(Diptera:Culicidae)

Objeclive:To evaluate the larvicidal activity of the tuber extracts isolated from two species of Cyclamen（Cyxltimen mimbile hildebr.and Cyclamen alpinum Dammann ex.Sprcnger） against Culex pipiens L.（Diptera:Culicidae）.Methods:Twenty young（first-second） or older（third- fourth） instars of tile Culex pipiens were exposed to various concentrations（100 to I 000 ppm） of the extracts.Mortality was recorded after 12-,24-,48-.72- and 96-h of exposure.Results:We determined that the young larval stages were more susceptible to the extracts in comparison with the older larval stages.A comparison of LC90 and LC90 values has shown that Cyclamen mimbile was more active than Cyclamen alpinum.Conclusions:This study is the first to report on the larvicidal activity of the extracts of Cyclamen species against Culex pipiens.More studies are needed to isolate and identify the active components involved,their mode of action,and effects on other mosquito and pest species.

Shape Memory Behavior of [111]-Oriented NiTi Single Crystals After Stress-Assisted Aging

The shape memory behavior of [111]-oriented Ni_（51）Ti_（49）（at.%） single crystals was investigated after stressassisted aging at 500 °C for 1.5 h under a compressive stress of-150 MPa.It was found that a single family of Ni_4Ti_3 precipitates with two crystallographically equivalent variants was formed after aging under compressive stress.Stressassisted aging resulted in tensile two-way shape memory effect strain of 1.56% under-5 MPa.Thermal cycling under-600 MPa resulted in a transformation strain of-2.15%,while the subsequent thermal cycling under-5 MPa resulted in a tensile two-way shape memory effect strain of 2.2%.
