Neural plasticity secondary to carpal tunnel syndrome: a pseudo-continuous arterial spin labeling study

Conventional neuroimaging techniques cannot truly reflect the change of regional cerebral blood flow in patients with carpal tunnel ***-continuous arterial spinning labeling(pCASL)as an efficient non-invasive neuroimaging technique can be applied to directly quantify the neuronal activities of individual brain regions that show the persistent symptoms owing to its better spatial resolution and increased signal-to-noise ***,this prospective observational study was conducted in 27 eligible female carpal tunnel syndrome,aged 57.7±6.51 *** tests,nerve conduction studies and pCASL neuroimaging assessment were *** results showed that the relevant activated brain regions in the cortical,subcrotical,and cerebral regions were correlated with numbness,pain,functionality,median nerve status and motor amplitude of median nerve(K=21–2849,r=–0.77–0.76,P<0.05).There was a tendency of pain processing which shifted from the nociceptive circuitry to the emotional and cognitive one during the process of chronic pain caused by carpal tunnel *** suggests the necessity of addressing the ignored cognitive or emotional state when managing patients with carpal tunnel *** for this study was obtained from the Institutional Review Board of The University of Hong Kong/Hospital Authority Hong Kong West,China(HKU/HA HKW IRB,approval ***17-129)on April 11,*** study was registered in Clinical Trial Registry of The University of Hong Kong,China(registration number:HKUCTR-2220)on April 24,2017.

Exploratory use of ultrasound to determine whether demyelination following carpal tunnel syndrome co-exists with axonal degeneration

Carpal tunnel syndrome （CTS） accompanied by secondary axonal degeneration cannot be clearly dis- criminated using the current cross-validated ultrasound severity classification system. This study aimed at exploring cut-off values of ultrasound parameters, including wrist cross-sectional area （W-CSA）, wrist perimeter （W-P）, ratio of cross-sectional area （R-CSA） and perimeter （R-P）, changes of CSA and P from wrist to one third distal forearm （△CSA＆AP） for differentiation. Seventy-three patients （13 male and 60 female） were assigned into group A （demyelination only, n = 40） and group B （demyelination with secondary axonal degeneration, n = 33） based on the outcomes of nerve conduction studies （NCS）. Receiver Operative Characteristics （ROC） curves were plotted to obtain sensitivity, specificity, and accuracy of cut- off values for all the ultrasound parameters. The overall identified cut-off values （W-CSA 12.0 mm2, W-P 16.27 mm, R-CSA 1.85, R-P 1.48, △CSA 6.98 mm2, △P 5.77 mm） had good sensitivity （77.1-88.6%）, fair specificity （40-62.2%） and fair-to-good accuracy （0.676-0.758）. There were also significant differences in demographics （age and severity gradation, P 〈 0.001）, NCS findings （wrist motor latency and conduction velocity, P 〈 0.0001; wrist motor amplitude, P 〈 0.05; distal sensory latency, P 〈 0.05; sensory amplitude, P 〈 0.001） and ultrasound measurements （W-CSA, W-P, R-CSA, R-P, △CSA＆△P, P 〈 0.05） between groups. These findings suggest that ultrasound can be potentially used to differentiate demyelinating CTS with sec- ondary axonal degeneration and provide better treatment guidance.

A prospective cohort of men with localized prostate cancer on active surveillance protocol in Hong Kong,China:what did we learn?

This study aimed to report the outcomes of active surveillance(AS)in the management of low-risk prostate cancer(PCa).It recruited87 men who were prospectively followed up according to the Prostate Cancer Research International Active Surveillance(PRIAS)protocol with local adaptation at SH Ho Urology Centre,Prince of Wales Hospital,Hong Kong,*** investigated the predictorsof disease progression and found that baseline prostate-specific antigen density(PSAD)and the presence of the highest ProstateImaging-Reporting and Data System(PI-RADS)score 5 lesion on magnetic resonance imaging(MRI)are significantly correlatedwith disease ***,men with PSAD>0.2 ng ml^(−2)or PI-RADS 4 or 5 lesions had significantly worse upgradingfree survival compared to those with PSAD≤0.2 ng ml−2 and PI-RADS 2 or 3 *** study concludes that AS is a safe andeffective management strategy for selected patients to defer radical treatment and that most disease progression can be detectedafter the first repeated *** combination of PSAD>0.2 ng ml^(−2)and PI-RADS 4 or 5 lesions may serve as a useful predictorof early disease progression and provide a guide to optimize follow-up protocols for men in different risk groups.

WAVE INDUCED OSCILLATORY AND STEADY FLOWS IN THE ANNULUS OF A CATHETERIZED VISCOELASTIC TUBE

A perturbation analysis based on equations of motion in Lagrangian form is presented for the oscillatory and time-mean viscous flows induced by a propagating wave of small amplitude in an annulus with a viscoelastic outer wall. Owing to the steady streaming effect, the existence of a catheter in a blood vessel brings in an additional steady pressure gradient, a correction to that predicted by the linear theory, and an additional steady shear stress, which may increase the possibility of hemolysis of red blood cells.

Lu Xun＇s View of the Awakening of the Chinese People--Was There Really an ＂Epistemological Break＂？

It has been argued that, starting in the late 1920s, Lu Xun＇s intellectual development underwent a significant transformation constituting what the French Marxist philosopher Louis Althusser has termed an ＂epistemological break.＂ Some of the explicitly more positive comments about the masses that Lu Xun made in his later years have been used to demonstrate this point. However, the existence of such a ＂break＂ is still debatable, and a detailed examination of Lu Xun＇s apparently optimistic comments reveals that Lu Xun possessed a more sophisticated understanding of the masses and the Chinese people. His understanding was informed by the concept of ＂national character.＂ This paper attempts to demonstrate the consistency of Lu Xun＇s view of the masses and the Chinese people and to resolve an apparent self-contradiction in Lu Xun＇s arguments.

Who Invited Lu Xun to Hong Kong？： An Examination of Two Accounts and Some New Materials

Lu Xun visited Hong Kong in 1927 for two days and gave two very important lectures, but for several decades little was known about who made the invitation and who organized the trip. In 1981 Liu Sui claimed that it was Dr. Wong San-yin who invited Lu Xun, and Liu Sui himself was part of a reception team headed by Dr. Wong. In 1993 Zhao Jinsheng revealed that he was the person who had invited Lu Xun and organized his trip and lectures. Recently materials supporting both claims have been discovered and this paper examines these two claims, concluding that Zhao Jinsheng＇s account is more reliable.

Using prostate-specific antigen(PSA)for prostate cancer(PCa)screening led to overinvestigation and overdiagnosis of indolent *** aimed to investigate the value of prostate health index(PHI)and magnetic resonance imaging(MRI)prostate in an Asian PCa screening *** aged 50–75 years were prospectively recruited from a community-based PSA screening *** with PSA 4.0–10.0 ng ml^(−1) had PHI result *** prostate was offered to men with PSA 4.0–50.0 ng ml−1.A systematic prostate biopsy was offered to men with PSA 4.0–9.9 ng ml^(−1) and PHI≥35,or PSA 10.0–50.0 ng ml^(−1).Additional targeted prostate biopsy was offered if they had PI-RADS score≥*** significant PCa(csPCa)was defined as the International Society of Urological Pathology(ISUP)grade group(GG)≥2 or ISUP GG 1 with involvement of≥30%of total systematic *** total,12.8%(196/1536)men had PSA≥4.0 ng ml^(−1).Among 194 men with PSA 4.0–50.0 ng ml^(−1),187(96.4%)received MRI *** them,28.3%(53/187)had PI-RADS≥3 ***,7.0%(107/1536)men were indicated for biopsy and 94.4%(101/107)men received *** the men received biopsy,PCa,ISUP GG≥2 PCa,and csPCa was diagnosed in 42(41.6%),24(23.8%),and 34(33.7%)men,*** with PSA/PHI pathway in men with PSA 4.0–50.0 ng ml^(−1),additional MRI increased diagnoses of PCa,ISUP GG≥2 PCa,and csPCa by 21.2%(from 33 to 40),22.2%(from 18 to 22),and 18.5%(from 27 to 32),*** benefit of additional MRI was only observed in PSA 4.0–10.0 ng ml^(−1),and the number of MRI needed to diagnose one additional ISUP GG≥2 PCa was 20 in PHI≥35 and 94 in PHI<*** them,45.4%(89/196)men with PSA≥4.0 ng ml^(−1) avoided unnecessary biopsy with the use of PHI and MRI.A screening algorithm with PSA,PHI,and MRI could effectively diagnose csPCa while reducing unnecessary *** benefit of MRI prostate was mainly observed in PSA 4.0–9.9 ng ml^(−1) and PHI≥35 *** was an important risk stratification step for PCa

Effect of Rorrico, extracted from group of Chinese medicines, on influenza A and H1N1 infections

OBJECTIVE: To investigate the therapeutic and preventive effects of Rorrico on influenza, especially influenza A viral infection, including swine flu(H1N1)in ***: Eighty-nine subjects were recruited in Hong Kong and Macao, and divided into treatment group(TG) and prevention group(PG) based on their influenza A and swine flu symptoms. All subjects were prescribed Rorrico or placebo, and monitored by a Chinese medicine practitioner. Blood samples were collected before and after 7-day Rorrico or placebo treatment for laboratory ***: After treatment, there were some full recoveries and obvious relief of onset symptoms in the TG. Blood test results showed that Rorrico produced(a) no adverse effects on subjects' renal andliver functions, muscle enzyme and hematological status,(b) no up-regulation of pro-inflammatory cytokines tumour necrosis factor-a and interleukin-18 in both TG and PG,(c) mild yet statistically significant elevation of plasma mannose-binding lectin(MBL) in ***: Rorrico has no up-regulating effect on the participants' immune response, or, equally likely, the immuno-modulatory effects of Rorrico do not non-specifically or unnecessarily promote inflammation when not required. It is possible that oral administration of Rorrico can promote hepatic synthesis of MBL in healthy PG subjects, thereby conferring increased protection against infection.

Dear Editor,Plant tissue culture involves callus formation and de novo shoot ***,explants from differentiated tissues are used to generate a pluripotent cell mass,called callus,on auxin-rich callus-inducing medium(CIM),followed by shoot regeneration on cytokinin-rich shoot-inducing medium(SiM).Callus results from division of pericycle-like cells(Atta et al.,2009;Sugimoto et al.,2010);its cellular identity resembles that of lateral root primordia(Atta et al.,2009;Sugimoto et al.,2010).Callus acquires cellular pluripotency by forming root stem cell niches on CIM(Sugimoto et al.,2010)。

ntestinal Duplication Cyst Misdiagnosed as Meckel＇s Diverticulum

An intestinal duplication cyst is a very rare congenital anomaly, with symptoms including abdominal pain, abdominal distention, intestinal bleeding, and hematochezia. Hematochezia can be caused by ectopic gastric mucosa, although there are other causes of infantile hematochezia; hence, an intestinal duplication cyst can be misdiagnosed as bowel intussusception or Meckel＇s diverticulum. Here, a case of an intestinal duplication cyst that was misdiagnosed as Meckel＇s diverticulum is reported.
