Intervention at an early threshold for post-hemorrhagic ventricular dilatation in preterm infants: a systematic review and meta-analysis

Background Very few adequately powered studies exploring early thresholds for intervention in the management of post-hemorrhagic ventricular dilatation(PHVD)in preterm infants have identified consistent neurodevelopmental advantages at 12–30 *** aimed to conduct a meta-analysis on the efficacy and safety of early versus conservative thresholds for intervention,primarily aimed at normalizing cerebrospinal fluid(CSF)pressure,in the management of PHVD in preterm *** Multiple databases were searched for eligible papers,and prospective randomized trials involving preterm infants were *** results are expressed as relative risks(RRs)with 95%confidence intervals(CIs).The main outcome was survival without moderate-to-severe neurodevelopmental impairment at 12–30 *** Ten articles representing seven randomized trials comparing early versus conservative thresholds for interventions were *** trials(n=545 infants)reported no difference in the main outcome between early and conservative groups[RR 0.99(0.71,1.37)].Sensitivity analysis excluding data from a medication trial did not alter the main outcome[RR 1.15(0.95,1.39)].Infants in the early threshold group received significantly more interventions[RR 1.48(1.05,2.09)].Deaths before discharge/during the initial study period[RR 1.04(0.70,1.54)]or a composite of death or shunt insertion[RR 1.04(0.86,1.27)]were comparable between the two *** Early intervention for PHVD,before a clinical or ultrasound threshold is met,leads to additional clinical procedures but does not improve survival without moderate–severe neurodevelopmental impairment at 12–30 *** should be exercised in interpreting these results due to significant variation between the studies.

Development and validation of multicentre study on novel Artificial Intelligence-based Cardiovascular Risk Score(AICVD)

Objective Cardiovascular diseases(CVD)are one of the most prevalent diseases in India amounting for nearly 30%of total deaths.A dearth of research on CVD risk scores in Indian population,limited performance of conventional risk scores and inability to reproduce the initial accuracies in randomised clinical trials has led to this study on large-scale patient *** objective is to develop an Artificial Intelligence-based Risk Score(AICVD)to predict CVD event(eg,acute myocardial infarction/acute coronary syndrome)in the next 10 years and compare the model with the Framingham Heart Risk Score(FHRS)and *** Our study included 31599 participants aged 18–91 years from 2009 to 2018 in six Apollo Hospitals in India.A multistep risk factors selection process using Spearman correlation coefficient and propensity score matching yielded 21 risk factors.A deep learning hazards model was built on risk factors to predict event occurrence(classification)and time to event(hazards model)using multilayered neural ***,the model was validated with independent retrospective cohorts of participants from India and the Netherlands and compared with FHRS and *** The deep learning hazards model had a good performance(area under the curve(AUC)0.853).Validation and comparative results showed AUCs between 0.84 and 0.92 with better positive likelihood ratio(AICVD−6.16 to FHRS−2.24 and QRisk3−1.16)and accuracy(AICVD−80.15%to FHRS 59.71%and QRisk351.57%).In the Netherlands cohort,AICVD also outperformed the Framingham Heart Risk Model(AUC−0.737 vs 0.707).Conclusions This study concludes that the novel AI-based CVD Risk Score has a higher predictive performance for cardiac events than conventional risk scores in Indian population.

The International Study Group of Pancreatic Surgery definition of delayed gastric emptying and the effects of various surgical modifications on the occurrence of delayed gastric emptying after pancreatoduodenectomy

BACKGROUND:A number of definitions have been used for delayed gastric emptying(DGE) after pancreatoduodenectomy and the reported rates varied *** International Study Group of Pancreatic Surgery(ISGPS) definition is the current standard but it is not used *** this comprehensive review,we aimed to determine the acceptance rate of ISGPS definition of DGE,the incidence of DGE after pancreatoduodenectomy and the effect of various technical modifications on its *** SOURCE:We searched PubM ed for studies regarding DGE after pancreatoduodenectomy that were published from 1 January 1980 to 1 July 2015 and extracted data on DGE definition,DGE rates and comparison of DGE rates among different technical modifications from all of the relevant ***:Out of 435 search results,178 were selected for data *** ISGPS definition was used in 80% of the studies published since 2010 and the average rates of DGE and clinically relevant DGE were 27.7%(range:0-100%;median:18.7%) and 14.3%(range:1.8%-58.2%;median:13.6%),*** preservation or retrocolic reconstruction were not associated with increased DGE *** pyloric dilatation,Braun’s entero-enterostomy and Billroth Ⅱ reconstruction were associated with significantly lower DGE rates,pyloric ring resection appears to be most promising with favorable results in 7 out of 10 ***:ISGPS definition of DGE has been used in majority of studies published after *** relevant DGE rates remain high at 14.3% despite a number of proposed surgical *** ring resection seems to offer the most promising solution to reduce the occurrence of DGE.

The neural network has attracted researchers immensely in the last couple of years due to its wide applications in various areas such as Data mining,Natural language processing,Image processing,and Information retrieval *** embedding has been applied by many researchers for Information retrieval *** this paper word embedding-based skip-gram model has been developed for the query expansion *** terms are obtained from the top“k”initially retrieved documents using the Pseudo relevance feedback model and then they are trained using the skip-gram model to find the expansion terms for the user *** performance of the model based on mean average precision is *** proposed model compares with other existing *** improvement of 6.61%,6.93%,and 9.07%on MAP value is observed compare to the Original query,BM25 model,and query expansion with the Chi-Square model *** proposed model also retrieves 84,25,and 81 additional relevant documents compare to the original query,query expansion with Chi-Square model,and BM25 model respectively and thus improves the recall value *** per query analysis reveals that the proposed model performs well in 30,36,and 30 queries compare to the original query,query expansion with Chi-square model,and BM25 model respectively.

Nonlinear dynamic analysis of dragonfly-inspired piezoelectric unimorph actuated flapping and twisting wing

Non-invasive reversal of intraluminal vas deferens polymer injection-induced azoospermia——technology

Aim: The rationale and technique underlying a novel concept of non-invasive removal of an intravasal vas deferens poly-meric contraceptive drug to reverse drag injection-induced azoospermia are explained. Thus the conventional methods ofsurgical exploration to remove vas deferens plugs and intravasal injection of solvents to flush out contraceptive drugs are tobe replaced by steps which will be readily accepted by subjects. Methods: The approach is based upon the non-invasiveapplication of specific forces to various segments of the vas deferens so that non-sclerosing and non-tissue-adherent com-pounds, in particular styrene maleic anhydride (SMA) can be expelled. Forces are generated by palpation; percuta-neous electrical stimulation; vibration application; and percussion. The forces help to propel the intravasal polymer to-wards the ejaculatory duct for expulsion during ejaculation. All aspects of the total technique are clinically acceptable,simple, atraumatic, unlikely to cause pain and discomfort even without tranquilizers, local or general anaesthetics. Theprocedure may be repeated several times in different sittings spaced apart by about one week to achieve adequate plug ex-pulsion. Results: Model experiments demonstrated the feasibility of the concept. The polymer was nonadherent andcould be moved within the vas deferens by the application of specific forces. Sufficient removal was possible to enablespermatic fluid to be transported along a region previously occupied by the polymer. A corroborating subhuman primatestudy by an independent investigator has shown that the semen profile becomes normal following the reversal. Conclu-sion: Adoption of the new technique may provide a means of non surgical restoration of normal semen profile after a pe-riod of fertility control obtained by intravasal drag injection. Role of CYP2E1 gene polymorphisms association with hepatitis risk in Northeast India

AIM:To investigate hepatitis virus, genetic and environmental factors, and their interactions in predisposing patients to liver diseases in Northeast India. METHODS:A total of 104 jaundice patients and 124 community controls were included. Serological analysis was performed by routine enzyme-linked immunosorbent assay, and nucleic acid testing for hepatitis viruses was done by polymerase chain reaction (PCR), followed by PCR direct sequencing for viral genotyping. Cytochrome P450 2E1 (CYP2E1) polymorphism was studied by PCR-restriction fragment length polymorphism. Nitrite and volatile nitrosamines in indigenous foods consumed routinely by the Northeast Indian ethnic population were estimated by Griess’s reagent and GC-MS, ***: Hepatitis A virus (HAV) infection was predominantly prevalent (36.5%) in our cohort, followed by hepatitis B virus (HBV), hepatitis E virus (HEV) andhepatitis C virus. HBV genotype D and HEV genotype 1 were the most dominant. CYP2E1 c1/c2 genotype frequency was comparatively higher in alcoholic (P<0.0001,OR =30.5) and cryptogenic (P=0.014, OR=8.714) patients, and was associated with significantly higher hepatitis risk (P=0.0.007,OR=6.489). Mutant C allele of Cyp2E1 DraⅠ frequency was comparatively higher in HAV (P=0.006), alcoholic (P =0.003) and cryptogenic (P=0.014) cases, and was associated with overall hepatitis risk (P=0.026, OR=5.083). Indigenous foods, Gundruk, Kharoli, betel leaf and nuts were found to have the highest nitrite content. CONCLUSION: Apart from viral factors, CYP2E1 polymorphism might be associated with increased risk of liver diseases in Northeast India. Indigenous foods that contain nitrite and nitrosamine might be an associated risk factor.
