E-cadherin and beta-catenin expression in Epstein-Barr virus-associated gastric carcinoma and their prognostic significance

AIM： To examine the role of E-cadherin and betacatenin in carcinogenesis and to assess their prognostic implication in Epstein-Barr virus-associated gastric carcinomas （EBV-GCs）. METHODS： We compared the frequency of E-cadherin and beta-catenin expression in 59 EBV-GCs and 120 non-EBV-GCs, and examined the association between patients＇ prognosis and the expressions of these proteins. RESULTS： Neither the cellular-membranous nor the cytoplasmic E-cadherin expression showed any difference between EBV-GCs and non-EBV-GCs. On the other hand, loss of membranous expression of beta- catenin occurred more frequently in non-EBV-GCs than EBV-GCs [odds ratio = 0.41; 950 confidence interval （CI）, 0.19-0.90]. Furthermore, the nuclear and/or cytoplosmic expression of beta-catenin was seen more frequently in EBV-GCs than non-EBV-GCs （odds ratio = 2.23; 95% CI, 0.97-5.09）, and was observed in a larger proportion of carcinoma cells of EBV-GCs than non-EBV-GCs （P = 0.024）. Survival analysis for non-EBV-GC revealed that lymph node metastasis was significantly associated with poor prognosis （P 〈 0.001）. Among EBV- GCs, the depth of invasion （P = 0.005）, lymph node metastasis （P = 0.004） and an intestinal type by Lauren classification （hazard ratio = 9.47; 95% CI, 2.67-33.6） were significantly associated with poor prognosis. On the other hand, nuclear and/or cytoplasmic expression of beta-catenin was associated with a better prognosis in patients with EBV-GC （hazard ratio = 0.32; 95% CI, 0.11-0.93）. CONCLUSION： We observed more frequent preservation of beta-catenin in cell membrane and accumulation in nuclei and/or cytoplasm in EBV-GCs than in non-EBV- GCs. Factors involved in the prognosis of EBV-GCs and non-EBV-GCs are different in the two conditions.

A 66-year-old woman presented with a 2-year history of asthma has progression of dyspnea within a week, and a computed tomography (CT) revealed an endotracheal tumor at level of aortic arch. The resected tumor, measured by 18 × 12 × 10 mm, has histologically malignant features including frank invasion with desmoplastic stroma and foci of necrosis. Presence of glycogen and mucin was confirmed by periodic acid-Schiff (PAS) and diastase PAS staining. Immunohistochemically, the tumor cells were diffusely positive for cytokeratin (CK) 7 and AE1/3, focally positive for CK5/6 and CK14, but negative for CK20. Thyroid transcription factor-1 (TTF-1), Napsin A, and SP-A, indicating lung parenchymal origin were all negative. Other muscular markers, neuroendocrine markers, renal cell carcinoma markers, and markers related to perivascular epithelioid cell tumors were all negative. Postoperative systemic examination suggests no primary tumor outside the lung. The follow up was excellent being recurrence free for 14 months in spite of the positive surgical margin by the tumor cells. This is a first report ?of rare neoplasm, clear cell carcinoma of salivary gland type, presenting as an endotracheal mass.

Hydrogen adsorption characteristics of woodceramics,made from radiata pine wood fiber boards carbonized at 1 473 K after impregnation with phenolic resin,with and without chemical treatment in an alkaline(KOH) or an acid(H 2 SO 4) solution were investigated The hydrogen adsorption capacity is improved by chemical *** chemical treatment by KOH improves the adsorption capacity more than by H 2 SO *** improvement of the capacity level,in any solution,is dependent on the type of chemical in the solution and its *** maximum hydrogen capacity measured at 93 K is mass fraction about w(H) = 2% for the woodceramics after chemical treatment in a volume fraction = 50% KOH solution,although that is about w(H) = 0.7% for one without chemical *** microscopic condition of pores in the woodceramics is thought to vary after chemical treatment.

Tissue pad degradation of ultrasonic device may enhance thermal injury and impair its sealing performance in liver surgery

BACKGROUND Ultrasonic devices are widely used in many surgical fields,including hepatectomy;however,the negative effects of tissue pad degradation of ultrasonic devices,including those in liver surgery,remain *** Harmonic®1100(H-1100)scalpel has advanced heat control technology than previous models,such as the Harmonic®HD1000i(H-HD1000i).We hypothesized that,because of its advanced temperature-control technology,the H-1100 scalpel would show less tissue pad degradation,resulting in superior sealing performance,compared to that with the H-HD1000i *** To elucidate ultrasonic device tissue pad degradation effects on instrument temperature and sealing performance using ex vivo porcine liver/vessel *** Two different harmonic scalpels were used and compared:A newer model,the H-1100 scalpel,and an older model,the H-HD1000i *** ex vivo porcine livers,each instrument was activated until the liver parenchyma was *** device temperature(passive jaw temperature)was measured after every 10 consecutive activations,until 300 transections of the porcine liver were *** pad degradation was evaluated after 300 *** performance was evaluated using excised porcine carotid vessels;vessel sealing speed and frequency of vessel burst pressure below 700 mmHg were determined after 300 transections of porcine liver *** The temperature of the H-HD1000i scalpel was approximately 10℃higher than that of the H-1100 scalpel,and gradually increased as the number of activations *** median passive jaw temperature of the H-HD1000i scalpel was significantly higher than that of the H-1100 scalpel(73.4℃vs 65.1℃;P<0.001).After 300 transections of porcine liver parenchyma,less tissue pad degradation was observed with the H-1100 scalpel than with the H-HD1000i scalpel(0.08 mm vs 0.51 mm).The H-1100 scalpel demonstrated faster vessel-sealing speed(4.9 *** 5.1 sec.)and less frequent vessel burst pressure<700 mm

Damped oscillation of Newtonian liquid in a vertical U-tube is one of the well known phenomena and the solution of this liquid motion for the laminar flow regime in the circular pipe was solved, however, generally speaking, even if the period of the oscillational motion by this solution is nearly coincided with that of the experimental result, the estimation of the damped oscillational process with lapse of time by the solved equation is not in agreement with that of the experimental result. Therefore basing upon the experimental results of the velocity distributions of the oscillational motion in the circular U-tube for the Newtonian and non-Newtonian liquids, the velocity distribution of the Bingham plastic flow is assumed. The solutions of the damped oscillation and also of the vertical falling and rising velocities of the free surface in the vertical U-tube of the diameters D= 10mm, 20mm and 40mm are compared with water and water-glycerine solution for the Newtonian liquids and the acrylic co-polymer solutions for the non-Newtorlian liquid. The comparisons of these solved equations by the new flow model are shown in good agreement with the experimental results. The above stated results are described in detail.

A novel screening strategy for clinically significant prostate cancer in elderly men over 75 years of age

A standard modality for prostate cancer detection in men 75 years and older has not been established.A simple screening method for elderly patients is needed to avoid unnecessary biopsies and to effectively diagnose prostate cancer.A retrospective study was conducted on elderly patients who had prostate biopsy at Kanazawa University Hospital(Kanazawa,Japan)between 2000 and *** the 2251 patients who underwent prostate biopsy,254 had clinically significant prostate cancer(CSPC)with a Gleason score(GS)of≥7 and 273 had a GS of24,PSA density≥0.24 ng ml^(-2),positive findings on digital rectal examination,and transrectal with 90.0%sensitivity and 67.4%*** this study,we found that raising the PSA cutoff to 12 ng ml^(-1)for CSPC in elderly individuals can significantly reduce unnecessary prostate ***,CSPC could be efficiently discovered by combining the four supplementary markers in patients with a PSA level of 4-12 ng ml^(-1).By performing this screening for elderly men over 75 years of age,unnecessary biopsies may be reduced and CSPC may be detected efficiently.

A pancreatic tumor was suspected on the abdominal ultrasound of a 72-year-old man. Abdominal computed tomography showed pancreatic enlargement as well as a diffuse, poorly enhanced area in the pancreas; endoscopic ultrasound-guided fine needle aspiration biopsy and endoscopic retrograde cholangiopancreatography failed to provide a definitive diagnosis. Based on the trend of improvement of the pancreatic enlargement, the treatment plan involved follow-up examinations. Later, he was hospitalized with an alveolar hemorrhage and rapidly progressive glomerulonephritis; he tested positive for myeloperoxidase-anti-neutrophil cytoplasmic antibody(ANCA) and was diagnosed with ANCArelated vasculitis, specifically microscopic polyangiitis. It appears that factors such as thrombus formation caused by the vasculitis in the early stages of ANCArelated vasculitis cause abnormal distribution of the pancreatic blood flow, resulting in non-uniform pancreatitis. Pancreatic lesions in ANCA-related vasculitis are very rare. Only a few cases have been reported previously. Therefore, we report our case and a review of the literature.
