AIM： To evaluate the combination of bevacizumab with infusional 5-fluorouracil （5-FU）, leucovorin （LV） and irinotecan （FOLFIRI） in patients with advanced colorectal cancer （CRC） pretreated with combination regimens including irinotecan and oxaliplatin. METHODS： Fourteen patients （median age 56 years） with advanced CRC, all having progressed after oxaliplatin- and irinotecan-based combination chemotherapy, were enrolled in this study. Patients were treated with 2 h infusion of irinotecan 150 mg/m2 on d 1, plus bevacizumab 5 mg/kg iv infusion for 90 min on d 2, and iv injection of LV 20 mg/m2 followed by a bolus of 5-FU 400 mg/m2 and then 22 h continuous infusion of 600 mg/m2 given on two consecutive days every 14 d. RESULTS： The median number of cycles of chemotherapy was six （range 3-12）. The response rate was 28.5%, one patient had a complete response, and three patients had a partial response. Eight patients had stable disease. The median time to progression was 3.9 mo （95% CI 2.0-8.7）, and the median overall survival was 10.9 mo （95% CI 9.6-12.1）. Grade 3/4 neutropenia occurred in five patients, and two of these developed neutropenic fever. Grade 3 hematuria and hematochezia occurred in one. Grade 2 proteinuria occurred in two patients. However, hypertension, bowel perforation or thromboembolic events did not occur in a total of 90 cycles. CONCLUSION： Bevacizumab with FOLFIRI is well tolerated and a feasible treatment in patients with heavily treated advanced CRC.

AIM:To assess the prognostic significance of nuclear factor-kB (NF-kB) and its target genes in gastric cancer. METHODS:The tumor tissues of 115 patients with gastric cancer were immunohistochemically evaluated using monoclonal antibodies against NF-kB RelA. Preoperative serum levels of vascular endothelial growth factor (VEGF), interleukin-6 (IL-6) were assessed via enzyme-linked immuno-sorbent assay. C-reactive protein (CRP) and serum amyloid A (SAA) were measured via immunotrubidimetry. RESULTS:Positive rate of NF-kB RelA was 42.6%. NF-kB RelA expression in tumor tissues was also related to serum levels of IL-6 (P = 0.044) and CRP (P = 0.010). IL-6, SAA, CRP were related to depth of invasion, VEGF and SAA were correlated with lymph node metastasis. IL-6, VEGF, SAA and CRP were related to the stage. Univariate analysis demonstrated that immunostaining of NF-kB RelA, levels of IL-6, VEGF, SAA were significantly related with both disease free survival and over-all survival (OS). Multivariate analysis verified that NF-kB RelA [hazard ratio (HR): 3.40, P = 0.024] and SAA (HR: 3.39, P = 0.045) were independently associated with OS. CONCLUSION: Increased expression of NF-kB RelA and high levels of serum SAA were associated with poor OS in gastric cancer patients.

AIM： To identify the clinical features and outcomes of infrequently reported leptomeningeal carcinomatosis （LMC） of gastric ***： We analyzed 54 cases of cytologically confirmed gastric LMC at four institutions from 1994 to ***： The male-to-female ratio was 32：22, and the patients ranged in age from 28 to 78 years （median,48.5 years）. The majority of patients had advanced disease at initial diagnosis of gastric cancer. The clini-cal or pathologic tumor, node and metastasis stage ofthe primary gastric cancer wasin 38 patients （70%）.The median interval from diagnosis of the primarymalignancy to the diagnosis of LMC was 6.3 mo, rang-ing between 0 and 73.1 mo. Of the initial endoscopic f indings for the 45 available patients, 23 （51%） of the patients were Bormann typeand 15 （33%） patientswere Bormann type. Pathologically, 94% of cases proved to be poorly differentiated adenocarcinomas. Signet ring cell component was also observed in 40% of patients. Headache （85%） and nausea/vomiting （58%） were the most common presenting symptoms of LMC. A gadolinium-enhanced magnetic resonance imaging was conducted in 51 patients. Leptomeningeal enhancement was noted in 45 cases （82%）. Intrathecal （IT） chemotherapy was administered to 36 patients-primarily methotrexate alone （61%）, but also in combi-nation with hydrocortisone/± Ara-C （39%）. The median number of IT treatments was 7 （range, 1-18）. Concomitant radiotherapy was administered to 18 patients, and concomitant chemotherapy to seven patients. Sev-enteen patients （46%） achieved cytological negative conversion. Median overall survival duration from the diagnosis of LMC was 6.7 wk （95% CI： 4.3-9.1 wk）. In the univariate analysis of survival duration, hemoglobin, IT chemotherapy, and cytological negative conversion showed superior survival duration （P = 0.038, P = 0.010, and P = 0.002, respectively）. Background:The response rate and survival improvement for rituximab,a CD20-targeting monoclonal antibody,have been demonstrated in marginal zone lymphoma(MZL)as monotherapy and in combination with chemotherapeutic regimens,yet relapses still occur despite treatment ***,extending the period of remission in MZL patients remains an essential *** multicenter,single-arm,open-label phase II study evaluated the survival efficacy of 2 years of rituximab-maintenance therapy in patients with stage III-IV CD20-positive MZL who had responded to first-line R-CVP(rituximab,cyclophosphamide,vincristine,and prednisolone).The objective of this study was to determine whether rituximab maintenance following R-CVP warrants further ***:Prior to rituximab-maintenance therapy,patients received 6-8 cycles of first-line R-CVP therapy for stage III-IV ***(375 mg/m^(2)),cyclophosphamide(750 mg/m^(2)),and vincristine(1.4 mg/m^(2);maximum 2 mg)were administered via an intravenous infusion on day 1 of each 3-week cycle,while oral prednisolone(100 mg)was given on days 1-5 of each 3-week *** patients who achieved complete response(CR),partial response(PR),or stable disease(SD)to R-CVP treatment,were prescribed rituximab-maintenance therapy which was administered intravenously at a dose of 375 mg/m^(2) every 8 weeks for up to 12 *** primary endpoint was progression-free survival(PFS).Secondary endpoints were overall survival(OS)and treatment ***:47 patients were enrolled,of whom,45(96%)received rituximab-maintenance ***(33%)patients had nodal *** R-CVP first-line therapy,20(44%),22(49%),and 3(7%)patients achieved CR,PR,and SD,*** a median follow-up of 38.2 months,their observed 3-year PFS rate was 81%.During the rituximab-maintenance,6 PR and 1 SD patients achieved CR following the administration of *** LDH and the presence of B symptoms were found to be significant prognostic fac

背景与目的利妥昔单抗是一种靶向CD20的单克隆抗体,在治疗边缘区淋巴瘤(marginal zone lymphoma,MZL)中可以单药或联合化疗使用,可提高缓解率和生存率,但治疗后仍可能会复发。因此,仍需探索如何延长MZL患者的缓解期的治疗方案。这项多中心、单臂、开放标签的Ⅱ期研究评估了对一线R-CVP(rituximab,cyclophosphamide,vincristine,and prednisolone;利妥昔单抗、环磷酰胺、长春新碱和泼尼松龙)治疗有反应的Ⅲ–Ⅳ期CD20阳性MZL患者进行利妥昔单抗维持治疗2年的生存率。本研究旨在明确R-CVP后利妥昔单抗维持治疗的策略是否有进一步研究的价值。方法在利妥昔单抗维持治疗之前,Ⅲ–Ⅳ期MZL患者应接受6–8周期的R-CVP一线治疗。每个周期为3周,第1 d静脉输注利妥昔单抗(375 mg/m^2)、环磷酰胺(750 mg/m^2)和长春新碱(1.4 mg/m^2;最大2 mg),第1–5 d口服泼尼松龙(100 mg)。对R-CVP治疗达到完全缓解(complete response,CR)、部分缓解(part response,PR)或疾病稳定(stable response,SD)的患者进行利妥昔单抗维持治疗,每8周静脉注射利妥昔单抗375mg/m^2,直到12个周期。主要终点为无进展生存期(progression-free surviva,PFS),次要终点为总生存期(overall survival,OS)和治疗安全性。结果共纳入47例患者,其中45(96%)例接受了利妥昔单抗维持治疗。15(33%)例患者有淋巴结MZL。在进行R-CVP一线治疗后,分别有20(44%)例、22(49%)例和3(7%)例的患者达到了CR、PR和SD。中位随访38.2个月后,患者的3年PFS率为81%。在利妥昔单抗维持治疗期间,R-CVP治疗后有6例PR和1例SD患者获得CR。LDH升高和B症状是PFS的重要预后因素(P=0.003),3年OS率为90%。利妥昔单抗维持治疗的耐受性良好,常见的治疗性不良事件为感觉性神经病(18%)、肌痛(13%)、疲劳(9%)和中性粒细胞减少(9%)。结论一线R-CVP治疗后利妥昔单抗维持治疗在Ⅲ–Ⅳ期MZL患者中不仅表现出较好的PFS,而且药物毒性反应评价良好。

目的探讨不同人种痴呆照料者心理健康与其照料过程中的应对策略之间的关系。方法121名痴呆照料者,包括29名美籍华人(CN)、29名美籍非洲人(BK)、32名高加索人(WT)和31名拉丁族裔(LT),所有被试者接受“老年人照料者的预后”问卷调查,内容包括受照料者的认知状况问卷、受照料者的简短生活能力量表和工具性生活能力量表、照料者抑郁量表、照料者健康状况问卷,及照料应对方式问卷。结果BK、WT和LT组照料者以女儿居多;CN组配偶、子女所承担的角色较均衡。照料者抑郁量表评分和健康状况评分组间差异不具显著性。不同种族照料者采取的应对方式不同;接纳现状条目和寻找积极因素条目评分均为BK>WT>LT>CN(P<0.05),增加疾病知识条目评分组间差异无显著性。多元逐步回归分析发现,照料者抑郁体验与照料者和受照料者的基本背景及照料应对方式有关(P<0.01),不同种族照料者抑郁体验相关的应对方式不同。结论不同种族痴呆照料者心理状况与其应对策略有关,抑郁体验相关应对方式具有跨人种差异。有必要探讨适合于不同文化背景的照料者应对策略培训和保健模式,以提高痴呆照料质量和生活质量。
