Simultaneous rectal neuroendocrine tumors and pituitary adenoma:A case report and review of literature

BACKGROUND Neuroendocrine tumors(NET)are rare heterogeneous tumors that arise from neuroendocrine cells throughout the body.Acromegaly,a rare and slowly progressive disorder,usually results from a growth hormone(GH)-secreting pituitary adenoma.CASE suMMARY We herein describe a 38-year-old patient who was initially diagnosed with diabetes.During colonoscopy,two bulges were identified and subsequently removed through endoscopic submucosal dissection.Following the surgical intervention,the excised tissue samples were examined and confirmed to be grade 2 NET.^(18)F-ALF-NOTATATE positron emission tomography-computed tomography(PET/CT)and 68Ga-DOTANOC PET/CT revealed metastases in the peri-intestinal lymph nodes,prompting laparoscopic low anterior resection with total mesorectal excision.The patient later returned to the hospital because of hyperglycemia and was found to have facial changes,namely a larger nose,thicker lips,and mandibular prognathism.Laboratory tests and magnetic resonance imaging(MRI)suggested a GH-secreting pituitary adenoma.The pituitary adenoma shrunk after treatment with octreotide and was neuroendoscopically resected via a trans-sphenoidal approach.Whole-exome sequencing analysis revealed no genetic abnormalities.The patient recovered well with no evidence of recurrence during follow-up.CONCLUSION ^(18)F-ALF-NOTATE PET/CT and MRI with pathological analysis can effectively diagnose rare cases of pituitary adenomas complicated with rectal NET.

Chiral dihydrobenzofuran is a valuable synthetic fragment in both medicinal and organic chemistry.Efficient rhodium(Ⅲ)-catalyzed chemoselective and enantioselective C-H functionalization/[3+2]annulation of N-phenoxyacetamides with 1,3-dienes is reported.In the presence of 5 mol%Cp Rh complex,various chiral dihydrobenzofurans were synthesized in up to 79%yield and 98%ee.This reaction proceeds under mild conditions with excellent functional group compatibility.

Multiresponsive,easy-reversible,and dual-visual Pt(Ⅱ)salt nanostructures for advanced anti-counterfeiting application

Smart materials that integrate multi-stimuli response,full reversibility,and dual-visual read-out channel are highly desired for anticounterfeiting and information encryption applications.Herein,we developed a multiresponsive perchlorate terpyridyl Pt(Ⅱ)nano complex which could undergo fully reversible conversion between three forms stimulated by water or formaldehyde molecule due to the extent of Pt–Pt interaction.Meanwhile,a dual-visual channel,i.e.,the colorimetric channel changed from yellow to orange or red and the corresponding luminescent channel from orange to orange-red or red,has also been found.The weak and equivalent strength of ion-dipole interaction and hydrogen bond that generated between formaldehyde/water and Pt(II)salt result in the easy-control reversibility between the three forms.Furthermore,by introducing different polymer matrices,1Cl·ClO_(4)@PMMA(1Cl·ClO_(4):[Pt(tpy)Cl]·ClO4,tpy:2,2':6',2''-terpyridine),PMMA:poly(methyl methacrylate))and 1Cl·ClO4@PVA(PVA:polyvinyl alcohol)are successfully constructed,which exhibit different reversible behaviors since the PMMA and PVA matrix exert different influences on the strength of hydrogen-bond.Those smart Pt(II)salt nanostructures present great potential for high-security-level anticounterfeiting application.

Carrimycin in the treatment of acute promyelocytic leukemia combined with pulmonary tuberculosis: A case report

BACKGROUND Pulmonary tuberculosis(PTB)is prevalent in immunocompromised populations,including patients with hematologic malignancies,human immunodeficiency virus infections,and chronic diseases.Effective treatment for acute promyelocytic leukemia(APL)combined with PTB is lacking.These patients show an extremely poor prognosis.Therefore,studies should establish efficient treatment options to improve patient survival and prognosis.CASE suMMARY A 60-year-old male with pain in the right side of his chest and a fever for 4 d visited the outpatient department of our hospital.Peripheral blood smear revealed 54%blasts.Following bone marrow examinations,variant APL with TNRC18-RARA fusion gene was diagnosed.Chest computed tomography scan showed bilateral pneumonitis with bilateral pleural effusions,partial atelectasis in the lower lobes of both lungs,and the bronchoalveolar lavage fluid gene X-Pert test was positive,indicative of PTB.Carrimycin,ethambutol(EMB),and isoniazid(INH)were administered since he could not receive chemotherapy as the WBC count decreased continuously.After one week of treatment with carrimycin,the patient recovered from fever and received chemotherapy.Chemotherapy was very effective and his white blood cells counts got back to normal.After being given five months with rifampin,EMB and INH and chemotherapy,the patient showed complete remission from pneumonia and APL.CONCLUSION We report a case of PTB treated successfully with carrimycin with APL that requires chemotherapy.

Multi-carrier Tb/s silicon photonic coherent receiver

We propose a silicon polarization-diversity coherent receiver for wavelength-multiplexing transmission without using the large-footprint arrayed waveguide grating(AWG).We have integrated our proposed coherent receiver on the silicon-on-insulator(SOI) platform for high-capacity transmission.In the proposed coherent receiver,high-frequency photocurrent signals from other wavelengths are suppressed by electrical low-pass filters.Moreover,the signal-signal beat interference(SSBI) generated from each wavelength is eliminated by the balanced detection.These two features lend to the proposed coherent receiver being free of the mm-scale AWG.We have demonstrated our proposed coherent receiver to detect a 1.12-Tb/s wavelength-division-multiplexed and polarization-division-multiplexed 16-ary quadrature amplitude modulation(PDM-16-QAM) signal.The compact footprint of the silicon chip promises small-form-factor receivers for future ultra-high-capacity coherent communication systems that require a high integration level and low fabrication cost.

Molecular characterization and phylogenetic analysis of three odorant binding protein gene transcripts in Dendrolimus species （Lepidoptera： Lasiocampidae）

Pine caterpillar moths, Dendrolimus spp. （Lepidoptera： Lasiocampidae）, are serious economic pest of pines. Previously, phylogenetic analyses of Dendrolimus using different methods yielded inconsistent results. The chemosensory systems of insects may play fundamental roles in promoting speciation. Odorant-binding proteins （OBPs） participate in the first step of odor detection. Studying the evolution of OBPs in closely related species may help us to identify their role in speciation. We identified three OBPs - one pheromone-binding protein and two general odorant-binding proteins - from male antennae of four Dendrolimus species, D. superans （Butler）, D. punctatus （Walker）, D. kikuchii Matsumura, and D. houi Lajonquiere, the olfactory recognition systems of which had not been previously investigated. We analyzed their molecular characteristics and compared their sequences to those of OBPs in D. tabulaeformis Tsai et Liu. Ka/Ks ratio analyses among the five Dendrolimus species indicate that PBP1 genes experienced more evolutionary pressure than the GOBPs. Phylogenetic relationships of PBP1 and GOBP1 both indicated that D. houi was the basal species, then branched D. kikuchii, while D. tabulaeformis, D. punctatus, and D. superans evolved more recently. These relationships are consistent with the changes in sex pheromone components of these five species. Dendrolimus tabulaeformis and D. punctatus are closely related sister species. However, the distances among GOBP2 sequences in the five Dendrolimus were very short, and the relationships of D. houi and D. la＇kuchii could not be resolved. Integrating our results with those of previous studies, we hypothesized that D. kikuchii, D. punctatus and D. superans evolved from the basal ancestor because of sex pheromone mutations and environmental pressure.

Nuclear charge radius predictions by kernel ridge regression with odd-even effects

The extended kernel ridge regression(EKRR)method with odd-even effects was adopted to improve the description of the nuclear charge radius using five commonly used nuclear models.These are:(i)the isospin-dependent A^(1∕3) formula,(ii)relativistic continuum Hartree-Bogoliubov(RCHB)theory,(iii)Hartree-Fock-Bogoliubov(HFB)model HFB25,(iv)the Weizsacker-Skyrme(WS)model WS*,and(v)HFB25*model.In the last two models,the charge radii were calculated using a five-parameter formula with the nuclear shell corrections and deformations obtained from the WS and HFB25 models,respectively.For each model,the resultant root-mean-square deviation for the 1014 nuclei with proton number Z≥8 can be significantly reduced to 0.009-0.013 fm after considering the modification with the EKRR method.The best among them was the RCHB model,with a root-mean-square deviation of 0.0092 fm.The extrapolation abilities of the KRR and EKRR methods for the neutron-rich region were examined,and it was found that after considering the odd-even effects,the extrapolation power was improved compared with that of the original KRR method.The strong odd-even staggering of nuclear charge radii of Ca and Cu isotopes and the abrupt kinks across the neutron N=126 and 82 shell closures were also calculated and could be reproduced quite well by calculations using the EKRR method.

Design Research of a Novel Aftercool-Humidifier Concept for Humid Air Turbine Cycle

Humid air turbine cycle(HAT)has potential of electrical efficiencies comparable to combined cycle,with lower investment cost and NO_(x) emission.The typical heat exchanger network of HAT consists of intercooler(if there is),aftercooler,recuperator,economizer and humidifier,which brings higher efficiency but makes the system more complex.To simplify HAT layout,a novel humidifier concept is proposed by integrating the aftercooler into traditional counter-current humidifier.Based on this concept,a one-dimensional model including pressure drop and exergy calculation is established to distinguish the thermodynamic and hydrodynamic characteristics,and then the structural parameters,such as the number of rows and columns,tube diameter,pitch and type for a micro HAT are identified.The results show that the aftercool-humidifier plays the same role as original aftercooler and humidifier,and can match the in-tube air,out-tube air and water stream well with lower volume.In the case of micro HAT cycle,the volume of heat and mass transfer area can be reduced by 47%compared with traditional design.The major thermal resistance occurred in the convection heat transfer process inside the tube;however,using enhanced tube cannot effectively improve the compactness of device.

Performance of the Multilayer Film for Infrared Stealth based on VO2 Thermochromism

With the development of detection and identification technology,infrared stealth is of great value to realize anti-reconnaissance detection of military targets.At present,infrared stealth materials generally have deficiency,such as complex structure,inconvenient radiation regulation and cumbersome preparation steps,which greatly limit the practical application of infrared stealth materials.In view of the above deficiency of infrared stealth materials,we proposed a kind of multilayer film for infrared stealth using VO2thermochromism based on the temperature response mechanism of tuna to adjust its color,and through the intelligent reversible radiation regulation mechanism to meet the infrared stealth requirements of tanks in different actual scenes.The results show that when the temperature increases from 300 K to 373 K,the peak emissivity of the film decreases from 94% to 20% in the 8-14 μm band after structural optimization,which can realize the infrared stealth of the high temperature target in the 8-14 μm band.The average emissivity of the multilayer film for infrared stealth at3-5μm and 8-14μm band can be reduced to 34% and 27% at 373 K,and the peak emissivity at 5-8 μm band can reach 65%,realizing dual-band infrared stealth in the 3-5 μm and 8-14 μm bands and heat dissipation in the 5-8μm band.The multilayer film for infrared stealth based on VO2thermochromism designed by the authors can meet the characteristics of simple film structure,convenient radiation regulation and simple preparation.

Strategies for improving extraction capacity through preorganization structure:A novel 5,6-bicyclicmalonamide extractant(THPPD)

Rational design of extractant structure to improve the extraction and separation capacity of rare earth elements(REEs)is a long-standing challenge.Herein,a new strategy was proposed to improve the stability of the chelating structure formed by malonamide and REEs.The stability of the chelating structure is greatly improved by using a double-ring framework structure which makes the two carbonyl groups of malonamide unable to rotate freely and both of them point to the metal ion position.Three benzyl groups were used to construct the hydrophobicity of the extractant and the organic shell of the extracted species.Tribenzylhexahydro-pyrrolo-pyridine-dione(THPPD)was designed and synthesized.The structure reduces the energy consumed by rotating carbonyl group in the coordination with metal ions and then improves the extraction ability of extractant.The crystal structure was preorganized as expected,with the two carbonyls pointing in a favorable coordination direction to the structural complement of the metal ion.The extraction behaviors of REEs with THPPD in a nitric acid medium were studied.Compared with N,N’-dibenzyl-N,N’-dimethylmalonamide(DBDM-MA)with a chain structure,the extraction capacity of THPPD is 360 times higher than that of DBDM-MA at 5.0 mol/L sodium nitrate.Furthermore,the binding energy and Gibbs free energy were investigated by density functional theory(DFT)in conjunction with the B3LYP.The theoretical results show that THPPD has more effective interaction with Pr(NO_(3))3 than DBDM-MA.The construction of chelating groups conformation is a worthy direction to improve the coordination ability and even selectivity of extractant.
